1.Upgrade and practice of the drug traceability code management system in children’s hospital under the “payment by code”background

OBJECTIVE To upgrade the drug traceability code management system for a pediatric hospital under the “payment by code” background, aiming to comprehensively enhance traceability integrity, efficiency, and compliance. METHODS Taking Xiamen Children’s Hospital as the implementation setting, a before-and-after control design was adopted to construct an intelligent drug traceability code management system through systematic upgrades involving the technology platform, core mechanisms, and coordination with medical insurance. Key interventions included: upgrading a traceability code management platform and designing a dynamic code pool; innovating differentiated traceability mechanisms for routine, split-dose, and special drugs; establishing a tiered early-warning and emergency response system; and constructing a data coordination and quality control system. The drug traceability code upload rate served as the primary outcome. Process indicators such as the root causes distribution of failed uploads and the duration of medication returns, and a comprehensive outcome (the number of insurance-flagged abnormal prescriptions) were also analyzed. The data between the baseline period (April 2025) and the observation period (June-August 2025) were compared and evaluated. RESULTS After the upgrade, the overall upload rate of drug traceability codes increased from 9.21% (baseline) to 99.86% (August 2025). The upload rate of traceability codes in previously unmanaged areas, such as the inpatient pharmacy and pharmacy intravenous admixture services, soared from 0 to nearly 100%. The proportion of non-uploads due to system issues fell from 66.44% (June 2025) to 2.62% (August Additionally, the number of insurance-flagged) abnormal prescriptions dropped sharply from 2 275.00 in the first “payment by code” policy month (July 2025) to 212.00 by the end of the observation period (August 2025), a 90.70% decrease. CONCLUSIONS The developed management system effectively addresses complex scenario challenges such as high-frequency drug splitting. It significantly enhances traceability code upload performance and ensures a high degree of compliance with medical insurance data requirements. These outcomes contribute to proactive risk mitigation against insurance claim denials and demonstrate a concurrent optimization of pharmacy operations.
2.Treadmill exercise reshaped mitochondrial dynamics and alleviated de-pression-like behaviors in mice

AIM:To explore the molecular mechanism by which treadmill exercise reshapes the homeostasis of mitochondrial dynamics and alleviates depression-like behavior.METHODS:The mouse model of depression-like be-havior was established by electronic foot shock plus restraint stress.The depression-like mice were intervened with tread-mill exercise.The neurobehavior of mice was evaluated using the open field test and elevated plus-maze test,neuronal in-jury was detected by Nissl staining and TUNEL,and the expression of mitochondrial dynamics-related proteins was detect-ed by Western blot and immunofluorescence.RESULTS:Five weeks of electronic foot shock plus restraint stress success-fully established the depression-like mouse model,and neurobehavioral tests showed that the mice exhibited depression-like behaviors.In depression-like mice,neuronal synapses were degraded and neuronal apoptosis increased,while tread-mill exercise reduced neuronal damage and alleviated abnormal neurobehavior in depression-like mice.The mechanism study shows that the expression of mitochondrial fission-related protein 1(Fis1)and dynamin-related protein 1(Drp1)in-volved in mitochondrial division is up-regulated,and the expression of mitofusin 2(MFN2)and optic atrophy 1 protein(OPA1)involved in mitochondrial fusion is down-regulated in the cerebral cortex of depression-like mice.Notably,tread-mill exercise reverses the expression of these mitochondrial dynamics-related proteins and reshapes the homeostasis of mi-tochondrial dynamics.CONCLUSION:Mitochondrial dynamics disorder is involved in the pathological process of neu-robehavioral changes in mice with depression-like behavior.Treadmill exercise reshapes the homeostasis of mitochondrial dynamics and alleviates depression-like behavior.This study provides a scientific reference for the clinical application of exercise therapy as an adjuvant therapy in the intervention of diseases associated with depression-like behavior.
3.Research on Dynamic Evolution and Vulnerability Characteristic Profile of Multidimensional Health Pover-ty in China

Objective To describe the evolution of multidimensional health poverty among Chinese residents over a decade and to depict the characteristics of vulnerable groups.Methods We collected data from 76 999 households in the China Family Panel Studies across six periods from 2010 to 2020.Using tools developed by the research team,it measured the multidimensional health poverty index and decomposed its contributing factors.A comprehensive multi-level analysis was conducted based on household head,family,and regional characteristics.Results From 2010 to 2020,the multidimensional health poverty index and incidence significantly decreased from 0.338 and 68.5%to 0.163 and 37.9%respectively.The contribution of the health rights dimension initially decreased and then increased,the health capability dimension's contribution decreased from 0.256 to 0.158,and the health risk dimension's contribution steadily increased from 0.375 to 0.500.If the head of the household is male,older,less educated,has chronic disease comorbidities,participates in the new rural cooperative medical system,and the household income is low and lives in rural areas and western regions,the multidimensional health poverty index of the household is higher.Conclusion From 2010 to 2020,the multidimensional health poverty index and incidence rate of Chinese families decreased,in which health risk gradually became the dominant factor,and the distribution of health poverty index showed distinct demographic,household and regional characteristics.
4.Design and application of a special bra for vest-type breast dressing

Objective:To design a vest-type breast dressing bra and explore its application effect in the external application of traditional Chinese medicine therapy for patients with granulomatous lobular mastitis(GLM).Methods:A randomized controlled study was conducted, utilizing convenience sampling to select female patients with 122 GLM who underwent their first topical medication treatment in the Breast Department of Guangdong Provincial Hospital of Traditional Chinese Medicine between January and May 2023. The patients were divided into a control group and an observation group, with 61 patients in each, using a random number table. Following medication application, patients in the control group used a regular sports bra to secure the medication, while patients in the observation group used a custom-designed vest-style breast medication bra for the same purpose. Three hours after the intervention, comparisons were made between the two groups in terms of the incidence of medication detachment, medication displacement, clothing contamination, patient satisfaction, and safety.Results:Both groups of patients completed the study. The age of the observation group was (32.18 ± 5.50) years old, while the age of the control group was (33.66 ± 5.20) years old. Three hours after the intervention, the incidence of medication detachment, medication displacement, clothing contamination in the observation group were 3.28% (2/61), 6.56% (4/61), and 8.20% (5/61), which were lower than those in the control group with 18.03% (11/61), 26.23% (16/61), and 67.21% (41/61), respectively. The differences were statistically significant ( χ2=6.97, 8.61, 45.23; all P<0.05). The satisfaction level of patients towards the bra in the observation group was (4.43 ± 0.85) points, higher than that of the control group (3.85 ± 1.08) points, with a statistically significant difference ( t=3.30, P<0.05). The incidence rates of skin allergic reactions were 3.28%(2/61) and 4.92%(3/61) in the observation group and the control group, respectively, with no statistically significant difference ( P>0.05). Conclusions:The vest-type breast dressing bra designed in this study can properly secure the medicinal paste, reduce the rate of medication detachment, medication displacement, clothing contamination, and enhance patient satisfaction. It facilitates the external application of traditional Chinese medicine for patients with GLM and is worthy of clinical promotion.
5.Burden of Attention Deficit Hyperactivity Disorder in Children and Adolescents in China and Globally from 1990 to 2021

This study analyzes the disease burden of Attention-Deficit Hyperactivity Disorder (ADHD) in children and adolescents in China and globally from 1990 to 2021, providing data support and strategic recommendations for public health policy and disease intervention. Data on the incidence, prevalence, and disability-adjusted life years (DALYs) of ADHD in children and adolescents from 1990 to 2021 were obtained from the Global Burden of Disease (GBD) 2021 database. Joinpoint regression models were used to analyze the annual percentage change in disease burden, and socio-demographic index (SDI) was incorporated to analyze health inequality and frontier trends. From 1990 to 2021, the age-standardized incidence rate (ASIR), age-standardized prevalence rate (ASPR), and age-standardized DALYs rate (ASDR) for ADHD in China showed an upward trend, while they declined globally. China's ASIR increased from 280.61/100 000 to 356.80/100 000, ASPR rose from 3079.72/100 000 to 3653.25/100 000, and ASDR went up from 37.85/100 000 to 45.00/100 000.Globally, ASIR declined from 166.40/100 000 to 159.93/100 000, ASPR decreased from 1900.79/100 000 to 1730.67/100 000, and ASDR dropped from 23.28/100 000 to 21.22/100 000. ADHD disease burden was highest among the 5-14-year-old population, with males significantly higher than females. SDI showed a positive correlation with ADHD disease burden, and inequality among different socio-economic groups has somewhat alleviated, though there remains room for improvement in some countries. The increasing ADHD disease burden in China contrasts with the global trend, highlighting the need for enhanced early screening interventions and age- and gender-sensitive strategies. High SDI countries should also focus on the impact of social and psychological factors, optimize diagnostic and treatment pathways, and promote health equity.

Objective Based on the assumption of spatial heterogeneity,the distribution pattern and risk characteristics of health poverty in middle-aged and elderly people with chronic diseases are described from the perspective of spatial differentiation.In order to providing a theoretical basis for the optimization of subsequent poverty reduction policies and a model policy for other countries.Methods It used factor detector and interaction detector to capture the role of single-factor and multi-factor interactions on the spatial differentiation of health poverty,and risk detectors were utilized to explore the high-risk factors in risky areas Results The single factor explanation of medical assistance and health education activities is prominent,and the factors such as PM2.5,old-age dependency ratio and urban unemployment rate have strong interaction.Furthermore,it identified high-risk factor characteristics in areas at high risk of health poverty.Conclusion The spatial differentiation pattern of health poverty among the middle-aged and elderly chronic disease population in China is the result of the synergistic driving effect of multidimensional factors,and there is variability in the risk characteristics among regions.The government should establish a contextual optimization strategy and pay attention to the joint effect of multiple factors to establish a synergistic management system.
8.Clinical Study on Ultrasound-Guided Ilioinguinal Transversus Abdominis Plane Block in Elderly Parturients Undergoing General Anesthesia for Cesarean Section

Objective:To observe the clinical efficacy on ultrasound-guided ilioinguinal transversus abdominis plane(I-TAP)block in elderly parturients undergoing general anesthesia for cesarean section.Methods:90 elderly parturients who underwent general anesthesia for cesarean section surgery in Xiaoshan District First People's Hospital of Hangzhou from January 2023 to December 2023 were selected,they were divided into control group(general anesthesia only),TAP block group,and I-TAP block group according to random number table method,with 30 cases in each group.Hemodynamics[heart rate(HR),mean arterial pressure(MAP)],visual analogue score(VAS),ramsay scale score,umbilical artery blood gas index[power of hydrogen(pH),partial pressure of oxygen(PO2),partial pressure of carbon dioxide(PCO2)],neonatal behavioral neural score(NBNA)among the three groups were compared.Results:HR and MAP in I-TAP block group and TAP block group were lower than those in control group at 1min after block(T1)~exit room(T4)time points,and I-TAP block group was lower than that in the TAP block group(P<0.05).The VAS scores in I-TAP group and TAP group were lower than those in the control group at 12 h and 24 h after surgery,and the I-TAP group was lower than that in the TAP group(P<0.05).Ramsay scale score in I-TAP group and TAP group were higher than those in control group at 12 h and 24 h after surgery,and the I-TAP group was higher than that in the TAP group(P<0.05).There was no difference in pH,PCO2 and PO2 among the three groups(P>0.05).There was no difference in the NBNA scores at the 3 d,14 d and 28 d after birth among the three groups(P>0.05).Conclusion:Ultrasound-guided I-TAP block applied in elderly parturients undergoing general anesthesia for cesarean section,which can play good sedative and analgesic effect,maintain the hemodynamic stability of the body,and have good safety.
