Portal vein embolization (PVE) can induce atrophy of the embolized lobe and compensatory regeneration of the non-embolized lobe. However, due to inadequate regeneration of future liver remnant (FLR) after PVE, some patients remain unsuitable for hepatectomy after PVE. In recent years, liver venous deprivation (LVD), which combines PVE with hepatic vein embolization (HVE), has induced enhanced FLR regeneration. Compared with associating liver partition and portal vein ligation for staged hepatectomy (ALPPS), LVD triggers faster and more robust FLR regeneration, with lower incidence rate of postoperative complications and mortality rate. By reviewing related articles on LVD, this article introduces the effectiveness of LVD and analyzes the differences and safety of various technical paths, and it is believed that LVD is a safe and effective preoperative pretreatment method.
3.Early lactate/albumin ratio combined with quick sequential organ failure assessment for predicting the prognosis of sepsis caused by community-acquired pneumonia in the emergency department.

OBJECTIVE:
To investigate the predictive value of early lactate/albumin ratio (LAR) combined with quick sequential organ failure assessment (qSOFA) for the 28-day prognosis of patients with sepsis caused by emergency community-acquired pneumonia (CAP).
METHODS:
The clinical data of patients with sepsis caused by CAP admitted to the department of emergency of Beijing Haidian Hospital from June 2021 to August 2022 were retrospectively analyzed, including gender, age, comorbidities, lactic acid (Lac), serum albumin (Alb), LAR, procalcitonin (PCT) within 1 hour, and 28-day prognosis. Patients were divided into two groups based on 28-day prognosis, and risk factors affecting patients' prognosis were analyzed using univariate and multivariate Cox regression methods. Patients were divided into two groups according to the best cut-off value of LAR, and Kaplan-Meier survival curves were used to analyze the 28-day cumulative survival of patients in each group. Time-dependent receiver operator characteristic curve (ROC curve) were plotted to analyze the predictive value of sequential organ failure assessment (SOFA), acute physiology and chronic health evaluation II (APACHE II), and qSOFA+LAR score on the prognosis of patients with sepsis caused by CAP at 28 days. The area under the curve (AUC) was calculated and compared.
RESULTS:
A total of 116 patients with sepsis caused by CAP were included, of whom 80 survived at 28 days and 36 died, 28-day mortality of 31.0%. There were no statistically significant differences in age, gender, comorbidities, pH, platelet count, and fibrinogen between the survival and death groups, and there were significantly differences in blood urea nitrogen (BUN), white blood cell count (WBC), hemoglobin, Lac, Alb, PCT, D-dimer, LAR, as well as qSOFA score, SOFA score, and APACHE II score. Univariate Cox regression analyses showed that BUN, WBC, pH, Lac, Alb, PCT, LAR, qSOFA score, SOFA score, and APACHE II score were associated with mortality outcome. Multifactorial Cox regression analysis of the above variables showed that BUN, WBC, PCT, and APACHE II score were independent risk factors for 28-day death in the emergency department in patients with sepsis caused by CAP [hazard ratio (HR) were 1.081, 0.892, 1.034, and 1.135, respectively, all P < 0.05]. The best cut-off value of early LAR for predicting the 28-day prognosis of sepsis patients was 0.088, the Kaplan-Meier survival curve showed that the 28-day cumulative survival rate of sepsis patients in the LAR ≤ 0.088 group was significantly higher than that in the LAR > 0.088 group [82.9% (63/76) vs. 42.5% (17/40), Log-Rank test: χ2 = 22.51, P < 0.001]. The qSOFA+LAR score was calculated based on the LAR cut-off value and qSOFA score, and ROC curve analysis showed that the AUCs of SOFA score, APACHE II score, and qSOFA+LAR score for predicting 28-day death of patients with sepsis caued by CAP were 0.741, 0.774, and 0.709, respectively, with the AUC of qSOFA+LAR score slightly lower than those of SOFA score and APACHE II score, but there were no significantly differences. When the best cut-off value of qSOFA+LAR score was 1, the sensitivity was 63.9% and the specificity was 80.0%.
CONCLUSION
The qSOFA+LAR score has predictive value for the 28-day prognosis of patients with sepsis caused by CAP in the emergency department, its predictive value is comparable to the SOFA score and the APACHE II score, and it is more convenient for early use in the emergency department.

4.Thymol improves the core symptoms of autism through regulating scinderin expression

Objective:To explore the effect of thymol on improving the core symptoms of autism through regulating scinderin expres-sion in autism spectrum disorder(ASD)rats by constructing valproic acid-induced ASD rat model and intervening with thymol.Methods:The ASD-like behaviors of rats were observed using three box social experiment,open field experiment,and youth social ex-periment.RNA-seq was used to detect the expression of RNA in the prefrontal cortex of rats.The protein level of scinderin in the pre-frontal cortex of rats was measured by Western blot.Results:Compared with the rats in the control group,the rats in the model group showed ASD-like behaviors,including decreased social ability,impaired social preference,decreased exploration ability,and repeated stereotyped behavior.Compared with the control group,the model group showed reduced standing times(P<0.001)and increased self combing time(P=0.003).Moreover,the expression level of scinderin in the prefrontal cortex increased in model rats.After thymol treat-ment,the ASD-like behaviors in the model group were significantly improved.Compared with the model group,the number of standing times in the intervention group increased(P<0.001)and the self grooming time decreased(P=0.004).The expression of scinderin basi-cally returned to the normal level.Conclusion:The abnormal expression of scinderin in the prefrontal cortex of ASD model rats can lead to ASD-like behaviors.Thymol intervention can reduce the expression of scinderin and improve the core symptoms of ASD.
5.Efficacy of transarterial chemoembolization combined with targeted therapy and immunotherapy in treating advanced hepatocellular carcinoma

Objective To discuss the efficacy of transcatheter arterial chemoembolization(TACE)in combination with targeted therapy and immune checkpoint inhibitors for advanced hepatocellular carcinoma(HCC),and to identify the influencing factors.Methods A total of 60 patients with advanced HCC,who were admitted to the Air Force Medical Center of China from January 2016 to December 2022,were enrolled in this study.Thirty patients received TACE combined with targeted therapy and immune checkpoint inhibitors(TACE-L-P group),and the other 30 patients received TACE combined with targeted therapy(TACE-L group).The progression-free survival(PFS),overall survival(OS),disease control rate(DCR),and objective response rate(ORR)were compared between the two groups.Results In the TACE-L group and TACE-L-P group,the median PFS(mPFS)was 7 months and 10 months respectively(P=0.011),the median OS(mOS)was 15.5 months and 29 months respectively(P=0.014).Child-Pugh class B(HR=3.89,95%CI:1.27-11.94,P=0.018)and Barcelona Clinic Liver Cancer(BCLC)stage C(HR=2.83,95%CI:1.32-6.03,P=0.007)were the independent risk factors for OS,while micro wave ablation(HR=0.21,95%CI:0.07-0.63,P=0.005)and TACE-L-P(HR=0.09,95%CI:0.03-0.3,P=0.001)were the independent protection factors for OS.Besides,elevated bilirubin level(HR=1.03,95%CI:1-1.06,P=0.032)and elevated gamma-glutamyl transferase(GGT)level(HR=1.01,95%CI:1-1.01,P=0.002)were the independent risk factors for disease progression,and TACE-L-P(HR=0.27,95%CI:0.09-0.79,P=0.017)was the independent protection factor for disease progression.The ORR and DCR in TACE-L-P group were remarkably higher than those in TACE-L group,which were 43.4%vs 13.3%and 63.4%vs 23.3%respectively,the differences between the two groups were statistically significant(both P<0.05).Conclusion In treating advanced HCC,TACE combined with targeted therapy and immune checkpoint inhibitors is superior to TACE combined with targeted therapy in therapeutic efficacy.
6.Cross-sectional study on working mode and human resource setting of PIVAS in national medical institutions

OBJECTIVE To know about the working mode and human resource status of pharmacy intravenous admixture services (PIVAS) in national medical institutions. METHODS Through questionnaire survey, the national PIVAS was invited to fill out questionnaire and statistical analysis was performed on the effective sample data related to PIVAS working mode and human resources in the questionnaire. RESULTS In this study, 761 PIVAS from 722 medical institutions of 29 provinces were involved in the questionnaire survey, with 471 valid questionnaires for working mode and 441 valid questionnaires for human resources survey. In terms of working mode, among 471 PIVAS, 292 PIVAS (62.0%) were in pharmacist-alone mode, and 176 PIVAS (37.4%) were in pharmacist-nurse cooperative mode; there was no significant difference in the types of medical orders received by PIVAS between these two working modes except for the other medical orders (P>0.05). In terms of human resource setting, among 441 PIVAS, the average number of total staff of single PIVAS was 24(16,33), including 11(6,19) pharmacists, 7(2,13) nursing staff, and 3(1,5) workers; there was a statistically significant difference in the number of personnel among three groups (P< 0.01). The per capita income of PIVAS respondents in 2019 was [7.9(4.8,10.7)]×104 yuan, and in 2021 it was [8.8(5.8,11.7)]× 104 yuan, with an increase of 9.0% compared to 2019. The difference between the two groups was statistically significant (P< 0.01). CONCLUSIONS Medical institutions’ PIVAS in China had not fully implemented the pharmacist-alone work model, and some medical institutions had chosen a pharmacist-nurse cooperative mode. It is suggested that relevant departments formulate corresponding qualification requirements and training standards for nursing personnel as soon as possible based on sufficient research on PIVAS’s demand for nursing professionals.
7.Cross-sectional study on balance of payments in PIVAS of medical institutions nationwide

OBJECTIVE To know about the pharmacy intravenous admixture charge and operation balance of pharmacy intravenous admixture services (PIVAS) in national medical institutions. METHODS Using questionnaire survey method, the national PIVAS leaders were invited to fill in the questionnaire, investigation and statistical analysis of the drug dispensing charge standard and the income and expenditure situations of PIVAS nationwide were conducted. RESULTS A total of 761 PIVAS completed the questionnaire, among which 466 PIVAS (61.2%) had already started implementing pharmacy intravenous admixture charge, mainly in tertiary hospitals. The charge standards for chemotherapy drugs and parenteral nutrition solutions were relatively high, while the standards for packaged drugs were the lowest, with differences in charge standards among provinces(P<0.05). Among the 25 provinces that reported annual drug preparation fee revenue, Hubei had the highest revenue in both 2019 and 2021. In 2019, the number of PIVAS with a balance of payments was more than that of PIVAS with an imbalance of payments, but the number of PIVAS with an imbalance of payments in 2021 exceeded the number of PIVAS with a balance of payments (P<0.05); among them, eight provinces were unbalanced in 2019 and 2021, such as Tianjin, Chongqing, Guizhou, etc. CONCLUSIONS PIVAS charge standards of the surveyed medical institutions in all provinces are not unified. It is suggested to improve the charge standard further, formulate the charge adjustment cycle, and promote a sustainable development of PIVAS.
8.Current status and new advancements in molecular imaging of liver cancer

Early-stage diagnosis of liver cancer is challenging, with an overall poor prognosis. The tumor microenvironment of primary liver cancer is complex, exhibiting significant heterogeneity both interpersonally and intratumorally. Therefore, it is of paramount importance to dynamically analyze biological markers in the tumor microenvironment of primary liver cancer in vivo. In recent years, significant progress has been made in the imaging diagnosis and treatment of liver cancer with the development of molecular imaging. Molecular imaging techniques utilize specific nano-imaging probes to evaluate pathological changes of liver cancer at the molecular and cellular levels in real-time. These techniques enable precise imaging to reveal key molecular biomarkers involved in the occurrence and progression of liver cancer, exploring their associations with cancer progression and outcomes. This article focuses on molecular imaging, emphasizing the current research status and latest advancements in the field of liver cancer diagnosis and therapy using techniques such as CT, MRI, optical imaging, PET imaging, and multimodal imaging. It also identifies important future directions and significant challenges for further development.
9.Research on the Path of Integrating the Life View of "Human Life is Most Important" in Traditional Chinese Medicine into Medical Ethics Education for Medical Students

The life view of "human life is the most important" in Traditional Chinese Medicine (TCM) originates from the people-oriented theory of Chinese traditional culture, the benevolence thought of Confucianism, the rebirth thought of Taoism and the compassion thought of Buddhism. Taking the life view of "human life is the most important" as the starting point, it has formed medical ethics thoughts such as the social responsibility of saving the world, the medical practice purpose of treating all patients equally, the meticulous working attitude, the diligent and tireless learning attitude, and the exploration spirit of widely exploring the medical roots, which still has important educational significance. Therefore, the life view of "human life is the most important" should be integrated into the medical ethics education of medical students, and the medical ethics literacy of medical students should be improved through the following ways: strengthen the vocational ideal education of saving the dying and healing the wounded, and enhance the sense of social responsibility of medical students; consolidate the theme of life first classroom education and improve the professional identity of medical students; enrich and innovate the practice forms of protecting life, pay attention to the practicality of medical ethics education.
10.Establishment and Preliminary Application Analysis of A Multiplex Detection Method for Influenza A and B Virus Antigen Based on Quantum Dot-encoded Microsphere Flow Cytometry Technology

Objective To establish a multiplex assay method for the simultaneous detection of FluA and FluB virus(IBV)antigen based on the flow cytometry(FCM)quantum dot-encoded bead technologies,laying the foundation for the assay of multiple respiratory virus biomarkers.Methods Coupling was performed for FluA and FluB nucleoprotein(NP)monoclonal antibodies using self-made quantum dot-encoded beads,separately.FCM was used to detect known concentrations of FluA and FluB antigens separately and simultaneously,optimize the detection conditions,and establish a joint detection method for FluA and FluB antigens.Compared with the quantitative real-time PCR(qPCR)method,clinical samples were used to evaluate the clinical performance of this joint detection method.Results The joint detection method for FluA and FluB antigens was established,with detection limits of 26.1 pg/ml and 10.7 pg/ml,respectively,and measurement ranges of 15.3~250 000 pg/ml.The joint detection method for clinical sample evaluation was well correlated with the qPCR,with a positive coincidence rate of 57.4%,a negative coincidence rate of 100%,and a total coincidence rate of 71.6%.In addition,the joint detection method was superior to colloidal gold immunochromatographic strip assay commonly used in clinical practice(positive coincidence rate of 56.49%,negative coincidence rate of 99.75%).Conclusion The FCM quantum dot-encoded bead multiplex assay can be used for the joint detection of FluA and FluB antigens,which have a high sensitivity,good specificity and wide detection range.It may lay a good foundation for the multiplex detection of common respiratory viruses,and has clinical application prospects.
