In a healthy human, the airway mucus forms a thin, protective liquid layer covering the surface of the respiratory tract. It comprises a complex blend of mucin, multiple antibacterial proteins, metabolic substances, water, and electrolytes. This mucus plays a pivotal role in the lungs' innate immune system by maintaining airway hydration and capturing airborne particles and pathogens. However, heightened mucus secretion in the airway can compromise ciliary clearance, obstruct the respiratory tract, and increase the risk of pathogen colonization and recurrent infections. Consequently, a thorough exploration of the mechanisms driving excessive airway mucus secretion is crucial for establishing a theoretical foundation for the eventual development of targeted drugs designed to reduce mucus production. Across a range of lung diseases, excessive airway mucus secretion manifests with unique characteristics and regulatory mechanisms, all intricately linked to mucin. This article provides a comprehensive overview of the characteristics and regulatory mechanisms associated with excessive airway mucus secretion in several prevalent lung diseases.

ObjectiveTo establish a geographical origin identification model for Foeniculi Fructus from Haiyuan, providing a new technical reference for the protection of Haiyuan's geo-authentic medicinal materials and its designation as a national geographical indication agricultural product. MethodsSamples of Foeniculi Fructus were collected from eight producing areas, including Minqin (Gansu), Bozhou (Anhui), Qingdao (Shandong), Dezhou (Shandong), Urumqi (Xinjiang), Nujiang (Yunnan), Gutuo (Inner Mongolia), and Haiyuan (Ningxia). Gas chromatography-ion mobility spectrometry (GC-IMS) was used to detect the volatile organic compounds (VOCs) in samples from these geographic origins. VOCs were qualitatively analyzed through dual matching with the National Institute of Standards and Technology (NIST) mass spectral database and the IMS drift time database. Using the Reporter module and Gallery Plot visualization tools within the LAV analytical platform, VOC fingerprint profiles characterizing geographic origins were constructed. A non-targeted analytical strategy was adopted, and 97 VOCs detected via GC-IMS were subjected to principal component analysis (PCA) and partial least squares discriminant analysis (PLS-DA) based on their differential distribution patterns to construct an origin identification model for Foeniculi Fructus from Haiyuan region. Key discriminative markers were screened using variable importance in projection (VIP) values greater than 1. ResultsA total of 97 VOCs were identified, including alcohols, aldehydes, ketones, esters, organic acids, terpenoids, ethers, alkenes, and benzenes. The PLS-DA model, based on VOCs data obtained by GC-IMS, effectively distinguished Foeniculi Fructus in Haiyuan region from those of other origins. During cross-validation, the model achieved a prediction parameter (Q2) of 0.976 and a goodness-of-fit parameter (R2) of 0.936, with no overfitting observed in permutation testing. Twelve key flavor markers with VIP > 1 were identified as characteristic indicators of Haiyuan origin. ConclusionA stable and highly predictive origin identification model for Foeniculi Fructus from Haiyuan was successfully established using GC-IMS technology, PLS-DA, and VIP-based marker screening. This model provides a novel technical strategy for accurately distinguishing Foeniculi Fructus in Haiyuan region from other regional varieties and offers new technical support for its protection as a geo-authentic medicinal material and a nationally designated geographical indication agricultural product in China.

Body weight abnormalities, including overweight, obesity, and underweight, have become a dual public health challenge in Chinese adults: overweight and obesity lead to a variety of chronic complications, while underweight increases the risks of malnutrition, sarcopenia, and organ dysfunction. To systematically address these issues, multidisciplinary experts in endocrinology, sports science, nutrition, and psychiatry from various regions have held multiple weight management seminars. Based on the latest epidemiological data and clinical evidence, they expanded the guideline to include assessment and intervention strategies for underweight, in addition to the core content of obesity management. This guideline outlines the etiological mechanisms, evaluation methods, and multidimensional management strategies for overweight and obesity, covering key areas such as diagnosis and assessment, medical nutrition therapy, exercise prescription, pharmacological intervention, and psychological support. It is intended to provide a scientific and standardized approach to weight management across the adult population, aiming to curb the rising prevalence of obesity, mitigate complications associated with abnormal body weight, and improve nutritional status and overall quality of life.

Chinese Journal of Microsurgery 2025;48(3):326-332
Objective:Through anatomical observations, anatomical data on the radial perforating vessels around the neck of first metacarpus were observed and classified, hence to provide an anatomical evidence for clinical application of the flap based on the perforating vessels. Methods:From July 2022 to June 2023, 15 fresh adult upper limb specimens were studied at the Department of Hand Surgery, Northwest University First Hospital. All perforators within the zone for flap design around the neck of radial first metacarpus were observed. The number, origin and outer calibre of the perforators were recorded. Perforators with an outer diameter greater than 0.13 mm were defined as dominant arteries. SPSS 20.0 statistical software was used to count the number and outer diameter of cutaneous branches originating from the radial artery of the first metacarpal bone, the radial palmar proper artery of thumb and other arteries. The results were expressed as mean ± standard deviation ( ± s), with P<0.05 indicating statistical significance. From July 2023 to May 2024, 14 patients (10 males, 4 females and aged 27-53 years) with digital soft tissue defects were treated. The defects were of 3 thumbs, 4 index fingers, 5 middle fingers, 1 ring finger and 1 little finger, and sized from 2.5 cm × 2.0 cm to 4.0 cm × 2.5 cm. The perforator flaps of the neck of radial first metacarpus (3.0 cm × 2.5 cm - 4.5 cm × 3.0 cm) were harvested and donor sites were closed directly in the emergency surgery. Follow-up through the visits of outpatient clinic and via telephone and WeChat interviews to assess the flap survival, digital function and aesthetics of the flap. Results:In the specimens, it was found that about 75% of dominant perforators were originated from the first metacarpal radial artery. There were statistical significance in the number and outer calibre of the perforators between the first metacarpal radial artery, radial proper palmar digital artery of thumb and other arteries ( P<0.05). Postoperative follow-up lasted for 5-12 (mean 10.1) months, all flaps survived after surgery. Sensation regained according to the British Medical Research Council (BMRC), and they were: S 2 in 6 flaps and S 3 in 8 flaps. According to Evaluation Trial Standards of Upper Limb Partial Functional of Hand Surgery of Chinese Medical Association, 11 digits were excellent and 3 in good. Scar evaluation scored 3-4 (mean 3.43) points according to the Vancouver Scar Scale (VSS). Conclusion:The perforator flap around the neck of first radial metacarpus is primarily supplied by the radial artery of the first metacarpus. This flap achieves satisfactory functional and aesthetic outcomes with good sensory recovery, therefore it is an ideal flap for reconstruction of digital soft tissue defect.
