1.Astragali Radix Polysaccharides Promote M2 Polarization of OGD/R-induced BV2 Microglia by Inhibiting TLR4/NF-κB Signaling Pathway

ObjectiveTo investigate the effects of Astragali Radix polysaccharides (APS) on the polarization of BV2 microglial cells in an oxygen-glucose deprivation/reoxygenation (OGD/R) model through regulation of the Toll-like receptor 4 (TLR4)/nuclear factor-κB (NF-κB) signaling pathway. MethodsThe OGD/R injury model of BV2 microglia was established and divided into blank group, OGD/R group and APS group (0.4 g·L-1 APS). Neuroinflammatory injury was induced by lipopolysaccharide (LPS) and treated with APS. The cells were divided into blank group, LPS group (1 mg·L-1 LPS) and APS group (0.4 g·L-1 APS+1 mg·L-1 LPS). Cell viability was detected using the cell counting kit-8 (CCK-8) assay. Cell morphology was observed under an inverted microscope. Nitric oxide (NO) content in the cell supernatant was determined by the Griess assay. The secretion levels of tumor necrosis factor-α (TNF-α), interleukin (IL)-6, IL-10, and IL-4 were measured by enzyme-linked immunosorbent assay (ELISA). Immunofluorescence (IF) was used to detect the double-positive rates of ionized calcium-binding adapter molecule-1/inducible nitric oxide synthase (Iba-1+/iNOS+) and ionized calcium-binding adapter molecule-1/arginase 1 (Iba-1+/Arg1+), as well as the nuclear translocation rate of nuclear factor-κB p65 (NF-κB p65). Protein expression levels of Iba-1, iNOS, Arg1, TLR4, and NF-κB p65 were detected by Western blot. ResultsIn the OGD/R injury model, compared with the blank control group, BV2 microglial cells in the OGD/R group were activated and exhibited amoeboid morphological changes. The secretion levels of NO, TNF-α, and IL-6 were significantly increased (P<0.01). The double-positive expression rate of Iba-1+/iNOS+ and the protein expression of Iba-1 and iNOS were significantly increased (P<0.01). The nuclear translocation rate of NF-κB p65 and the protein expression levels of TLR4 and NF-κB p65 were significantly increased (P<0.01). The levels of IL-10 and IL-4 were significantly decreased (P<0.01), and the double-positive expression rate of Iba-1+/Arg1+ and Arg1 protein expression were significantly decreased (P<0.01). Compared with the OGD/R group, the APS group (0.4 g·L-1) showed reduced cell activation, significantly decreased secretion levels of NO, TNF-α, and IL-6 (P<0.01), significantly decreased double-positive expression rate of Iba-1+/iNOS+ and relative protein expression of Iba-1 and iNOS (P<0.01), significantly decreased nuclear translocation rate of NF-κB p65 and protein expression levels of TLR4 and NF-κB p65 (P<0.01), significantly increased levels of IL-10 and IL-4 (P<0.01), and significantly increased double-positive expression rate of Iba-1+/Arg1+ and Arg1 protein expression (P<0.01). In the LPS-induced neuroinflammation model, compared with the blank control group, the LPS group showed increased cell activation, significantly increased levels of NO, TNF-α, and IL-6, significantly increased Iba-1+/iNOS+ double-positive expression rate, NF-κB p65 nuclear translocation rate, and protein expression levels of Iba-1, iNOS, TLR4, and NF-κB p65 (P<0.01), while IL-10 and IL-4 levels, Iba-1+/Arg1+ double-positive expression rate, and Arg1 protein expression were significantly decreased (P<0.01). Compared with the LPS group, the APS group showed reduced cell activation, significantly decreased levels of NO, TNF-α, and IL-6, Iba-1+/iNOS+ double-positive expression rate, NF-κB p65 nuclear translocation rate, and protein expression levels of Iba-1, iNOS, TLR4, and NF-κB p65 (P<0.01), while IL-10 and IL-4 levels, Iba-1+/Arg1+ double-positive expression rate, and Arg1 protein expression were significantly increased (P<0.01). ConclusionAPS may reduce microglial activation and promote their polarization toward the M2 phenotype by inhibiting activation of the TLR4/NF-κB signaling pathway, thereby alleviating the neuroinflammatory response induced by OGD/R.
2.Academic Characteristics of Contemporary Chinese Medicine Masters in Treating Diabetic Kidney Disease Based on SrTO

ObjectiveTo explore the academic characteristics of contemporary renowned Chinese medicine masters in treating diabetic kidney disease (DKD) from the perspectives of principles, methods, formulas, and medications. MethodsIn strict accordance with the Systematic Review of Text and Opinion (SrTO) process developed by the Joanna Briggs Institute (JBI), an Australian evidence-based healthcare center, the databases including China National Knowledge Infrastructure (CNKI), VIP Database, Wanfang Data, and China Biomedical Literature Service System (SinoMed) were searched. Based on predefined inclusion and exclusion criteria, text information extraction, quality evaluation, and text information synthesis were conducted sequentially. The data were analyzed and presented in the form of text and figures. ResultsA total of 215 articles related to 43 contemporary renowned experts in the fields of Chinese medicine nephrology and endocrinology were included. The study found that the academic thoughts of these masters in the treatment of DKD are extensive, involving multiple levels such as disease understanding, therapeutic strategies, formula application, and medication use. In terms of disease understanding, the primary pathogenesis is characterized by deficiency in the root and excess in the manifestation. It is emphasized that internal factors, such as congenital endowment deficiency, interact with external factors such as improper diet, emotional disturbances, invasion of exogenous pathogens, and delayed or inappropriate treatment, to jointly induce the disease. This further gives rise to various pathogenetic theories, including obstruction of renal collaterals by blood stasis, toxin-induced damage to renal collaterals, latent wind disturbing the kidney, and internal heat leading to mass formation. In terms of therapeutic strategies and medication use, the principal treatment method is to replenish Qi and nourish Yin. Stage-based and syndrome-differentiated treatments are advocated. Flexible use of insect-derived drugs and wind-dispelling drugs is emphasized, along with proficiency in applying classical formulas and drug pairs. Integrated internal and external treatments, as well as the combined application of multiple therapeutic approaches, are commonly employed for comprehensive management. Meanwhile, the concept of "preventive treatment of disease" is upheld, and individualized long-term management of patients is advocated. ConclusionThrough the SrTO process, the academic thoughts of contemporary renowned Chinese medicine masters in the treatment of DKD have been systematically and standardly synthesized, providing a scientific and standardized basis for future theoretical exploration.

4.Relationship between skin failure and nutritional status in elderly critically ill patients and its predictive efficiency

Objective To investigate the relationship of skin failure (SF) with nutritional status in elderly critically ill patients and analyze the predictive efficiency of nutritional status on SF. Methods A total of 340 elderly critically ill patients admitted to the hospital from January 2020 to January 2025 were selected as research subjects. According to whether skin failure occurred, the above patients were classified into skin failure group and non-skin failure group. The nutritional status indicators [serum albumin (ALB), prealbumin (PA), total protein (TP), hemoglobin (Hb), body mass index (BMI), nutritional risk score (NRS 2002)] were compared between both groups. Multivariate analysis was performed on statistically significant indicators, and ROC curve was applied to analyze the predictive value. Results Among the 340 elderly critically ill patients, 142 cases (41.76%) developed skin failure. The ALB, PA, TP, Hb and BMI in the skin failure group were lower than those in the non-skin failure group (P<0.05) while the NRS2002 score was higher (P<0.05). After logistic multivariate analysis, ALB, PA and Hb were independent influencing factors of skin failure (P<0.05). ROC curve analysis revealed that the predictive value of ALB (AUC=0.850, 95%CI: 0.808-0.888, Z=-3.707, P<0.001) was better than that of PA (AUC=0.770, 95%CI: 0.717-0.816, Z=-3.100, P=0.002) or Hb (AUC=0.773, 95%CI: 0.722-0.819, Z=-2.556, P=0.011). Conclusion The occurrence of skin failure in elderly critically ill patients is closely related to nutritional status. ALB, PA and Hb are independent risk factors of SF, and ALB has the best predictive efficiency on SF.
5.Impact of prenatal triclosan exposure on ADHD-like symptoms in school-aged children

Background Attention-deficit hyperactivity disorder (ADHD) is a common neurodevelopmental and behavioral disorder in children, often diagnosed during school age. The etiology of ADHD remains unclear; however, existing studies suggest that environmental factors, such as exposure to triclosan (TCS), may be associated with the occurrence of ADHD-like symptoms in offspring. Nevertheless, relevant research in China remains limited. Objective To investigate the impact of early pregnancy TCS exposure on ADHD-like symptoms in 7-year-old children. Methods This study was based on the Shanghai Birth Cohort (SBC) and included 662 mother-child pairs. TCS concentrations in early pregnancy urine samples were measured using high-performance liquid chromatography-tandem mass spectrometry (HPLC-MS/MS). Demographic information was collected via questionnaires and medical record abstraction. ADHD-like symptoms in 7-year-old children were first assessed using the Strengths and Difficulties Questionnaire (SDQ). Further differentiation of ADHD-like symptom subtypes (inattentive and hyperactive/impulsive) was conducted using the SNAP-IV, a clinically validated ADHD screening tool. Negative binomial regression models were applied to evaluate the associations between prenatal TCS exposure and hyperactive behavior (SDQ assessment) as well as ADHD-like symptom subtypes (SNAP-IV assessment) in 7-year-old children. Results The positive rate of TCS in early pregnancy urine samples was 91.39%, with median concentrations of 0.69 μg·L−1 and 0.63 μg·g−1 before and after the creatinine adjustment, respectively. The modeling results indicated that prenatal TCS exposure was associated with an increased risk of hyperactive symptoms (SDQ assessment) in 7-year-old children (RR=1.04, 95%CI: 1.02, 1.06); the stratified analyses by children sex revealed similar effects for both boys (RR=1.04, 95%CI: 1.02, 1.07) and girls (RR=1.04, 95%CI: 1.01, 1.07). Further analysis of ADHD-like symptom subtypes showed that prenatal TCS exposure increased the risk of inattentive symptoms (RR=1.03, 95%CI: 1.00, 1.05); the sex-stratified analyses indicated associations between TCS exposure and inattentive symptoms (RR=1.03, 95%CI: 1.00, 1.07) as well as hyperactive/impulsive symptoms (RR=1.04, 95%CI: 1.01, 1.08) in girls. Conclusion Prenatal TCS exposure is associated with an increased risk of ADHD-like symptoms in 7-year-old children, primarily contributing to the risk of the inattention subtype. The impact is more pronounced in girls.
6.Effects of long working hours and shift work on the mental health of community medical workers

Objective To explore the association of working hours and shift work with occupational stress, fatigue accumulation, and depressive symptoms among primary community medical workers. Methods A total of 516 medical workers from five community medical service centers in Pudong New Area, Shanghai City, were selected as the research subjects using the convenience sampling method. The Core Scale of Occupational Stress Measurement, the Workers' Fatigue Accumulation Self-diagnosis Questionnaire, and the Patient Health Questionnaire were used to assess research subjects' occupational stress, fatigue accumulation, and depressive symptoms, respectively. Results Long working hours (>40 hours/week) were reported by 50.4% of workers among the research subjects, while shift works were reported by 16.9% of the workers. The detection rates of occupational stress, fatigue accumulation, and depressive symptoms were 26.6%, 41.7%, and 30.8%, respectively. Multivariate logistic regression analysis result revealed that, after adjusting for confounders such as age, sex, and education level, longer working hours were associated with higher risks of occupational stress, fatigue accumulation, and depressive symptoms (all P<0.05). Shift workers in community medical centers had higher risks of occupational stress, fatigue accumulation, and depressive symptoms compared with non-shift workers (all P<0.05). Conclusion Long working hours and shift work could increase the risks of occupational stress, fatigue accumulation, and depressive symptoms among community medical workers.
7.Exploring mechanism of Lycium barbarum polysaccharides in preventing inflam-matory bowel disease in chicks based on network pharmacology

This study aims to explore protective effects of Lycium barbarum polysaccharides(LBP)on intestinal damage caused by lipopolysaccharide(LPS)-induced inflammatory bowel disease(IBD)in chicks.Network pharmacology was initially employed to determine the target proteins of wolfberry in the prevention and treatment of IBD.Following this,protein-protein interaction analy-sis,GO and KEGG pathway enrichment analysis,and molecular docking studies were conducted.Subsequently,an animal study was conducted:a total of 100 one-day-old male Hy-line brown lay-ing hens were randomly divided into five groups:a blank control group(CON),an LPS treatment group(LPS),a low-dose LBP group(LPS+LBP 0.25 g/L,L-LBP),a medium-dose LBP group(LPS+LBP 0.5 g/L,M-LBP),and a high-dose LBP group(LPS+LBP 1 g/L,H-LBP).Upon reac-hing 21 days old,duodenal,jejunal,ileal,and cecal tissues were collected to determine SOD and GSH-Px levels.Furthermore,the mRNA expression levels of TNF-α,AKT1,IL-6,IL-1β and TP53 in the intestinal tissues were measured using quantitative real-time PCR.The results demonstrated that network pharmacology identified 45 active ingredients in wolfberry that target 116 key protein sites,including TNF,AKT1 and IL6.The primary objectives focus on signaling pathways including AGE-RAGE,IL-17,TNF,HIF-1,and NF-κB.Molecular docking showed excellent ligand-receptor docking scores,with stable binding facilitated by hydrogen bonds and hydrophobic interactions.Compared to the LPS group,the 0.5 g/L LBP exhibited notably higher levels of SOD and T-AOC.In comparison with the LPS group,the medium and high-dose LBP experimental groups showed notably decreased the mRNA expressions of TNF-α,AKT1,IL-6,and IL-1β,while TP53 mRNA expression was significantly upregulated(P<0.01).In summary,wolfberry exerts preventive and therapeutic effects on IBD through a multi-component,multi-target,and multi-pathway mecha-nism.
8.Effect of lumbar sympathetic ganglion block on postoperative ileus after colonic surgery

Objective:To investigate the effect of lumbar sympathetic ganglion block (LSGB) on postoperative ileus (POI) in patients undergoing colonic surgery.Methods:Ninety patients scheduled for elective laparoscopic colonic surgery in the same treatment group at the Nanjing First Hospital from March 2022 to August 2023 were selected and randomly divided into a control group and an observation group using a random number table, with 45 patients in each group. The observation group received ultrasound-guided LSGB before anesthesia, while the control group received no intervention. Both groups received standardized anesthesia protocols and management after entering the operating room. The time to gastrointestinal motility recovery (first defecation time), first flatus time, first feeding time, number of cases requiring gastric tube decompression, postoperative hospital stay, number of cases with nausea and vomiting, intraoperative additional and titrated sufentanil dosage, remifentanil dosage, first patient-controlled intravenous analgesia (PCIA) press time, sufentanil dosage at 4, 8, 12, 16, and 24 hours postoperatively, and the number of analgesic pump presses were recorded.Results:Compared with the control group, the observation group had shorter first defecation and flatus times, earlier first feeding time, shorter postoperative hospital stay, and fewer cases of nausea (all P<0.05). The observation group also had less intraoperative sufentanil supplementation and titration, later first PCIA press time (all P<0.05), and significantly fewer sufentanil dosages and analgesic pump presses at 4, 8, 12, 16, and 24 hours postoperatively compared with the control group (all P<0.05). Conclusions:LSGB reduces the incidence of POI in colonic surgery patients, decreases opioid use, and shortens hospital stay.
9.Design and implementation of online continuing education platform for transfusion medicine

To provide in-service medical technicians and nurses with convenient access to continuing education resources in transfusion medicine, reduce transfusion-related adverse events, and ensure the safety, rationalization, and effectiveness of clinical transfusion, we designed and developed an online transfusion continuing education platform. The platform was based on the new managed code programming model.NET Core and the powerful functions of hypertext preprocessor PHP 7.4, addressing current issues in transfusion online continuing education. Through in-depth analysis of student attributes, learning behaviors, and teaching behaviors, a comprehensive online continuous teaching quality evaluation index system was established. This system not only facilitates the quantitative assessment of teaching quality but also successfully integrates the two core functions of teaching and management, thereby achieving unified online teaching.
10.Protective effect of lycium barbarum polysaccharide on testicular injury induced by perfluorooctanoic acid in offspring mice

To examine how Lycium barbarum polysaccharide(LBP)impacts reproductive harm caused by perfluorooctanoic acid(PFOA)in male offspring mice.Sixty expectant female rats were randomly allocated into 6 groups,with each group consisting of 10 rats.From 1 to 17 d of gesta-tion,the blank control group was given 0.2 mL deionized water;The PFOA group was given 3.5 mg/kg PFOA;the LBP intervention group were given different doses(40,80,120 mg/kg)of LBP by gavage based on the PFOA group;the control group was given 80mg/kg.At 21 d of age,the offspring mice were weaned,and the offspring male mice were fed normally until 35 d of age,and testicular tissue was taken.HE staining was used to observe the pathological changes of testic-ular tissue sections.Body mass and testicular index were measured.The levels of SOD,MDA,LDH,SHD,GSH-PX and AR in testicular tissue were determined.Serum levels of LH,FSH,Gn-RH and T were determined by ELISA.Differential genes and differential metabolites of testis were analyzed in combination with transcriptome and metabolome.Real-time fluorescence quantitative PCR was used to detect the expression of key genes in fatty acid metabolic pathway.The results showed that LBP could alleviate PFOA-induced decline in body mass and increase testicular index(P<0.01).LBP could alleviate the necrosis,edema of testicular cells caused by PFOA.LBP can in-crease the contents of SOD,GSH-Px,LDH,SDH and AR in testicular tissue of offspring male mice induced by PFOA,increase the contents of FSH,LH,GnRH and T in serum of offspring male mice,and decrease the content of MDA.Analysis of the transcriptome revealed that 1 388 genes exhibited differential expression between the groups exposed to PFOA and 80 mg/kg LBP,with 696 genes being up-regulated and 692 genes being down-regulated.The differentially ex-pressed genes were predominantly involved in pathways such as fatty acid metabolism,unsaturated fatty acid biosynthesis,cholesterol metabolism,the PPAR signaling pathway,and protein digestion and absorption,with fatty acid metabolism being the most prominent among them.Examination of the metabolome revealed 34 metabolites that were expressed differently between the PFOA group and the 80 mg/kg LBP group.Key metabolic pathways such as fatty acid biosynthesis,unsaturated fatty acid biosynthesis,and steroid hormone biosynthesis were found to be significant.In a joint transcriptome and metabolome analysis,the Differentially expressed genes(DEGs)and Differential expression of metabolites(DEMs)are highly correlated in signaling pathways such as fatty acid bi-osynthesis and unsaturated fatty acid biosynthesis.This study shows that PFOA can cause testicu-lar injury in offspring male mice,and LBP can effectively alleviate the testicular injury caused by PFOA in offspring male mice.Meanwhile,transcriptomic metabolism shows that LBP can effec-tively improve the testicular injury caused by PFOA in offspring male mice through fatty acid bio-synthesis and unsaturated fatty acid biosynthesis signaling pathways.Among them,80 mg/kg LBP has better effect.
