1.Effects of transverse cervical artery related skin flaps in repairing neck and shoulder wounds

Objective To explore the effects of transverse cervical artery related skin flaps in repairing skin soft tissue defects in the neck and shoulder.Methods A total of 13 patients with soft tissue defects in neck and shoulder admitted to Linyi People's Hospital from January 2018 to January 2022 were selected.The wounds of patients were repaired with transverse cervical artery related skin flaps,and the donor area was repaired with direct suturing and/or full-thick skin graft from the inguinal area.The survival,appearance and texture of the flap after surgery were recorded.The wound healing in the donor area and the survival of the grafted skin were observed.The follow-up was conducted for 5 to 12 months,and the appearance of the grafted flap and the recovery of function in the surgical area were observed.Results The flaps had good survival in the first stage after surgery,the color and texture of the flap matched well with the surrounding tissue,and the wound healing in the donor area was good.During the follow-up,the function and appearance of neck and shoulder for patients recovered well.Conclusion The advantages of transverse cervical artery related skin flaps in repairing skin soft tissue defects in the neck and shoulder is that the color and texture of the flap matched well with the surrounding tissue,with reliable blood circulation,and the appearance and postoperative function recover well,which is worthy of promotion and application in clinic.

Traditional Chinese medicine(TCM), a treasure of the Chinese nation, contains abundant chemical components and demonstrates unique pharmacological activities, showing important values in clinical applications. With profound connotations and broad application prospects, TCM urgently needs us to further explore and conduct systematic research. Puerarin is a small-molecule natural isoflavonoid carbon glycoside extracted from plants of Pueraria. It is also the main active ingredient of Puerariae Lobata Radix, a Chinese herbal medicine with both medicinal and edible values. Puerarin has a variety of pharmacological effects such as blood pressure-lowering, anti-atherosclerosis, anti-ischemia-reperfusion injury, antithrombotic, anti-tumor, anti-inflammatory, liver-protecting, nerve cell-protecting, and intestinal microbiota-regulating effects. It is also an active ingredient that has been widely studied. This article comprehensively reviews the research progress in the pharmacological effects and molecular mechanisms of puerarin over the years, aiming to provide references and theoretical support for the in-depth research and development as well as clinical application of puerarin.

6.Analysis of Delayed Hemolytic Transfusion Reaction in Children with Repeated Blood Transfusion

Objective:To summarize and analyze the characteristics of delayed hemolytic transfusion reaction in children,in order to provide a scientific basis for clinical prevention,and ensure the safety of children's blood transfusion.Methods:The basic situation,clinical symptoms and signs,diagnosis time and disappearance time of alloantibody of delayed hemolytic transfusion reaction in children were retrospectively analyzed.The serological test,routine blood test,biochemical detection and urine analysis results were compared pre-and post-transfusion.Results:Among 15 164 children with repeated blood transfusion,23 cases occurred delayed hemolytic transfusion reactions,with an incidence rate of 0.15%,and mainly children with thalassemia and acute leukemia.39.13%of delayed hemolytic reactions occurred in children with more than 20 times of blood transfusions.Anemia was the main clinical symptom in 86.96%of children.4.35%of children had hypotension and dyspnea.Serological test results showed that the positive rate of direct antiglobulin test was 91.30%,and that of erythrocyte homologous antibody test was 100%.Erythrocyte alloantibodies were common in Rh and Kidd blood group systems,accounting for 73.91%and 13.04%,respectively.Laboratory test results showed that hemoglobin,reticulocyte,spherocyte,total bilirubin,indirect bilirubin,lactate dehydrogenase,serum ferritin and urine color were significantly different after transfusion compared with those before transfusion(all P<0.05).The average diagnosis time of delayed hemolytic transfusion reactions was 18.56 days,and the average disappearance time of erythrocyte alloantibodies was 118.43 days.Conclusion:The incidence of delayed hemolytic transfusion reaction is high in children with repeated blood transfusion,and the disappearance time of erythrocyte homologous antibody is long.Blood matched ABO,Rh and Kidd blood group antigens should be transfused prophylactically.Once diagnosed,erythrocyte alloantibody corresponding to antigen-negative blood should be used throughout the whole process.

Objective To investigate the clinical characteristics and prognosis of pneumococcal meningitis(PM),and drug sensitivity of Streptococcus pneumoniae(SP)isolates in Chinese children.Methods A retrospective analysis was conducted on clinical information,laboratory data,and microbiological data of 160 hospitalized children under 15 years old with PM from January 2019 to December 2020 in 33 tertiary hospitals across the country.Results Among the 160 children with PM,there were 103 males and 57 females.The age ranged from 15 days to 15 years,with 109 cases(68.1% )aged 3 months to under 3 years.SP strains were isolated from 95 cases(59.4% )in cerebrospinal fluid cultures and from 57 cases(35.6% )in blood cultures.The positive rates of SP detection by cerebrospinal fluid metagenomic next-generation sequencing and cerebrospinal fluid SP antigen testing were 40% (35/87)and 27% (21/78),respectively.Fifty-five cases(34.4% )had one or more risk factors for purulent meningitis,113 cases(70.6% )had one or more extra-cranial infectious foci,and 18 cases(11.3% )had underlying diseases.The most common clinical symptoms were fever(147 cases,91.9% ),followed by lethargy(98 cases,61.3% )and vomiting(61 cases,38.1% ).Sixty-nine cases(43.1% )experienced intracranial complications during hospitalization,with subdural effusion and/or empyema being the most common complication[43 cases(26.9% )],followed by hydrocephalus in 24 cases(15.0% ),brain abscess in 23 cases(14.4% ),and cerebral hemorrhage in 8 cases(5.0% ).Subdural effusion and/or empyema and hydrocephalus mainly occurred in children under 1 year old,with rates of 91% (39/43)and 83% (20/24),respectively.SP strains exhibited complete sensitivity to vancomycin(100% ,75/75),linezolid(100% ,56/56),and meropenem(100% ,6/6).High sensitivity rates were also observed for levofloxacin(81% ,22/27),moxifloxacin(82% ,14/17),rifampicin(96% ,25/26),and chloramphenicol(91% ,21/23).However,low sensitivity rates were found for penicillin(16% ,11/68)and clindamycin(6% ,1/17),and SP strains were completely resistant to erythromycin(100% ,31/31).The rates of discharge with cure and improvement were 22.5% (36/160)and 66.2% (106/160),respectively,while 18 cases(11.3% )had adverse outcomes.Conclusions Pediatric PM is more common in children aged 3 months to under 3 years.Intracranial complications are more frequently observed in children under 1 year old.Fever is the most common clinical manifestation of PM,and subdural effusion/emphysema and hydrocephalus are the most frequent complications.Non-culture detection methods for cerebrospinal fluid can improve pathogen detection rates.Adverse outcomes can be noted in more than 10% of PM cases.SP strains are high sensitivity to vancomycin,linezolid,meropenem,levofloxacin,moxifloxacin,rifampicin,and chloramphenicol.[Chinese Journal of Contemporary Pediatrics,2024,26(2):131-138]
9.A survey on the management status and indicators of pathogen detection rate before antimicrobial treatment of inpatients in 265 medical institu-tions in Guangdong Province

Objective To evaluate the management and indicators of pathogen detection before antimicrobial treat-ment for inpatients in second level and above medical institutions(MIs)in Guangdong Province,and provide direc-tion and decision-making basis for the improvement of pathogen detection quality in the region.Methods The ma-nagement status,information system functions,and pathogen detection rate indicators of secondary and above MIs in 21 cities in Guangdong Province was surveyed through online questionnaire surveys and system submission.A baseline survey on sentinel monitoring MIs was conducted from July 15th to August 8th,2023.From November 7th to 30th,a baseline survey on non-sentinel monitoring MIs was launched.Surveys on indicator information of all MIs were completed from January 15th to 30th,2024.Results A total of 265 MIs were surveyed,and the proportions of establishing special working groups(83.98%),developing special action improvement plans(79.01%),estab-lishing pathogen detection rate management systems(91.71%),and developing management assessment plans(76.80%)of tertiary MIs were all higher than that of secondary MIs,differences were all statistically significant(all P<0.05).The proportion of tertiary MIs with various information system functions was higher than that of secondary MIs(all P<0.05).The pathogen detection rate(61.07%)before antimicrobial treatment and health-care-associated infection(HAI)diagnosis-related pathogen detection rate(88.00%)of inpatients in tertiary MIs were both higher than those in secondary MIs(both P<0.05).Among different types of MIs,pathogen detection rate before antimicrobial treatment of inpatients in maternal and child health MIs was higher than that in other types of MIs.HAI diagnosis-related pathogen detection rate in other specialized hospitals was the highest,and pathogen detection rate before combined use of key antimicrobial treatment in traditional Chinese medicine hospitals was the lowest,differences were all statistically significant(all P<0.05).Conclusion Tertiary MIs have more advantages in management strategies and information technology construction than secondary MIs,secondary MIs need more guidance and support.Monitoring and analysis of pathogen detection rate indicators in MIs of different levels and types should be strengthened through special actions.

Objective:To explore the application effect of the P-SAFE (Protection-Soothe, Antithrombotic,Foodstuff,Eudipsia) nursing model based on enhanced recovery after surgery (ERAS) in patients with hemifacial spasm after microvascular decompression surgery, and provide a protocol for the rapid recovery of patients.Methods:A non-randomized quasi-experimental study was used to conveniently select patients with hemifacial spasm who underwent microvascular decompression in Union Hospital Affiliated to Tongji Medical College of Huazhong University of Science and Technology from January 2022 to December 2023. Among them, 80 patients treated with the traditional nursing management model from January to December 2022 were selected as the control group, while 94 patients treated with the P-SAFE nursing model under the ERAS concept from January to December 2023 were selected as the experimental group. The two groups were compared in terms of incidence of perioperative hunger and thirst, nausea and vomiting, thrombus, pain, adverse events as well as length of hospital stay, hospital costs, and pre-discharge nursing satisfaction.Results:Among the 80 patients in the control group, there were 19 males and 61 females, with an age of (54.98 ± 7.44) years. Among 94 patients in the experimental group, there were 15 males and 79 females, aged (54.93 ± 8.71) years. The incidences of perioperative hunger, thirst, nausea and vomiting, thrombosis and adverse events in the experimental group were 29.79%(28/94), 29.79%(28/94), 7.45%(7/94), 0 and 0, respectively, all of which were better than those in the control group, which were 45.00%(36/80), 47.50%(38/80), 23.75%(19/80), 8.75%(7/80), and 5.00%(4/80), respectively, pain score in the experimental group was 0(0, 1.25) points, lower than 0(0, 3.00) points in the control group, the differences were statistically significant ( χ2 values were 4.30 to 9.04, Z=-2.96, all P<0.05). The hospital stays, hospital costs and nursing satisfaction in the experimental group were 9(7, 10) days, 3.97(3.91, 4.31) ten thousand yuan, and 98.00(92.75, 100.00) points, respectively, compared to the control group′s 11 (10, 12) days, 4.90(4.36, 5.08) ten thousand yuan, and 80.00(79.00, 100.00) points. These differences were also statistically significant ( Z=-7.84, -8.57, -5.79, all P<0.05). Conclusions:The P-SAFE nursing mode for patients with hemifacial spasm undergoing microvascular decompression under the concept of ERAS can reduce their perioperative discomfort, reduce the incidence of postoperative complications, improve the prognosis of patients, and help accelerate the recovery of patients.
