1.Clinical effect of gemcitabine combined with high-intensity focused ultrasound in treatment of advanced pancreatic cancer: A Meta-analysis

ObjectiveTo evaluate the clinical effect and safety of high-intensity focused ultrasound (HIFU) combined with gemcitabine in the treatment of advanced pancreatic cancer. MethodsSCI, Cochrane Library, Embase, PubMed, Wanfang Data, CNKI, CBM, and VIP were searched for randomized controlled trials (RCTs) of HIFU combined with gemcitabine in the treatment of advanced pancreatic cancer, with the assistance of expanded search, and these RCTs were screened according to the inclusion criteria. Review Manager 5.3 was used to perform the Meta-analysis. A fixed effects model was used for non-heterogeneous data; heterogeneity was explained by subgroup analysis based on intervention methods, and if it could not be explained by subgroup analysis, a random effects model was used. Relative risk (RR) and 95% confidence interval (CI) were used as evaluation indices, and funnel plots were generated based on the outcome measure involved in the highest number of studies. ResultsA total of 8 RCTs with 474 patients were included. The patients in the experimental group received gemcitabine-based chemotherapy and HIFU, and those in the control group received gemcitabine-based chemotherapy alone. Compared with the control group, the experimental group had significantly better results in 3-, 6-, and 12-month survival rates (3-month: RR=1.07, 95%CI: 1.00-1.14, P<0.05; 6-month: RR=2.19, 95%CI: 1.75-2.75, P<0.05; 12-month: RR=235, 95%CI: 1.07-5.14, P<0.05), tumor control (RR=1.64, 95%CI: 1.21-2.24, P=0.002), and pain control (RR=3.15, 95%CI: 2.45-4.05, P<0.05). There were no significant differences between the two groups in the incidence rates of leukopenia (RR=1.05, 95%CI: 0.85-1.30, P>0.05), gastrointestinal reactions (RR=0.89, 95%CI: 0.56-1.42, P>0.05), and liver injury (RR=1.29, 95%CI: 0.95-1.75, P>0.05). Since the outcome measure of pain control was involved in the highest number of studies, funnel plots were generated and showed no significant risk of bias. ConclusionCompared with gemcitabine alone, HIFU combined with gemcitabine can increase patients’ survival rate and improve their symptoms, with a similar incidence rate of adverse effects. Further studies are needed for this combined therapy.
2.Effect of red-green duochrome test in subjective refraction

AIM: To evaluate the effect of red-green duochrome test in subjective refraction.
METHODS: A randomly-selected sample size of 60 new patients with myopia were measured twice using subjective refraction with and without the red-green duochrome test. Assessments on the effectiveness were made based on comprehensive optometry analysis, accommodation response, accommodation sensitivity, positive and negative relative accommodation.
RESULTS: Application of red-green duochrome test on the targeted population did not show any statistical significance to the subjective refraction results. No difference was observed on patients with dilated pupils(P=0.686)and non-dilated pupils(P=0.655). Bland-Altman analysis of displayed good consistency between both subjective refraction measurements on each patient. Multivariate linear regression analysis shows that the results of subjective refraction were independent of age, obliquity, regulation lag, positive relative adjustment, and negative relative adjustment effect(P>0.05).
CONCLUSION: The red-green duochrome test does not affect optometric results.
3.Research on the precision of virtual surgical planning in two⁃jaw orthognathic surgery

Objective:
To provide a clinical reference by evaluating the precision of virtual surgical planning in two⁃
jaw orthognathic surgery.
Methods :
Thirty consecutive patients who required two⁃jaw orthognathic surgery were includ⁃
ed. A composite skull model was reconstructed using data from spiral computed tomography scan and surface scanning
of the dental arch. LeFort I osteotomy of the maxilla and bilateral sagittal split ramus osteotomy of the mandible were
simulated using Dolphin Imaging 11.7 Premium. Genioplasty was performed if indicated. Virtual plan was then trans⁃
ferred to operation room using 3D⁃printed surgical templates. Frankfort horizontal plane (FHP), midfacial plane (perpen⁃
dicular to the FHP through the nasion), and coronal plane (perpendicular to the FHP through the sella point) were the
selected three symmetry planes.Midpoint of the contact of the maxillary and mandibular central incisors (UI, LI), and
the mesio⁃buccal cusp of the first maxillary and mandibular molars (U6⁃R,U6⁃L, L6⁃R, L6⁃L) were the six chosen volu⁃metric landmarks. To calculate the linear difference and overall mean linear difference (mean difference of the distance
between UI, LI, U6⁃R, U6⁃L, L6⁃R, L6⁃L to FHP, midfacial and coronal plane) between simulated and postoperative
models, the distance between selected landmarks and symmetry planes was measured. To calculate the angular differ⁃
ence and overall mean angular difference, values of the angles constructed by the occlusal, palatal, and mandibular
plane to FHP and midfacial plane respectively were determined on simulated and postoperative models
Results:
The
virtual surgical planning was successfully transferred to actual surgery with the help of 3D⁃printed surgical templates.
All patients were satisfied with the postoperative facial profile and occlusion. The overall mean linear difference was
0.81 mm (0.71 mm for maxilla and 0.91 mm for mandible); and the overall mean angular difference was 0.95° (the mean
angular difference relative to FHP was 1.10°, and that relative to midfacial plane was 0.83°)
Conclusion
Virtual sur⁃
gical planning facilitated the diagnosis, treatment planning, and precise bony segments repositioning in two⁃jaw orthog⁃
nathic surgery.
4.Predictive value of inflammation-based Glasgow prognostic score for the prognosis in patients with ST-segment elevation myocardial infarction

Objective To analyze prognostic ability of inflammation-based Glasgow prognostic score (GPS) in patients with ST-segment elevation myocardial infarction (STEMI). Methods We retrospectively analyzed the clinical data of 289 patients with STEMI admitted to the Department of Emergency in West China Hospital from April 2015 to January 2016. All study subjects were divided into three groups: a group of GPS 0 (190 patients including 150 males and 40 females aged 62.63±12.98 years), a group of GPS 1 (78 patients including 58 males and 20 females aged 66.57±15.25 years), and a group of GPS 2 (21 patients including 16 males and 5 females aged 70.95±9.58 years). Cox regression analysis was conducted to analyze the independent risk factors of predicting long-term mortality of patients with STEMI. Results There was a statistical difference in long-term mortality (9.5% vs. 23.1% vs. 61.9%, P<0.001) and in-hospital mortality (3.7% vs. 7.7% vs. 23.8%, P<0.001) among the three groups. The Global Registry of Acute Coronary Events (GRACE) scores and Gensini scores increased in patients with higher GPS scores, and the differences were statistically different (P<0.001). Multivariable Cox regression analysis showed that the GPS was independently associated with STEMI long-term all-cause mortality (1 vs. 0, HR: 2.212, P=0.037; 2 vs. 0, HR: 8.286, P<0.001). Conclusion GPS score is helpful in predicting the long-term and in-hospital prognosis of STEMI patients, and thus may guide clinical precise intervention by early risk stratification.

Three dimensionally printed composite porous bone tissue engineering scaffolds have become a research focus. Composite polyvinyl alcohol (PVA) has good biocompatibilityand degradability, but it cannot be prepared indepen⁃dently because it cannot resist highmechanical resistance. This material shows many advantages, such as good biocom⁃patibility, degradability and mechanical properties, when compounded with other materials with good mechanical proper⁃ties and good biocompatibility. Therefore, 3D printed composite PVA scaffold material can optimize the performance of PVA scaffolds. This article reviews 3D printing bone scaffold technology, polyvinyl alcohol (PVA), and composite PVA scaffolds for in vivo and in vitro bone formation.
6.Mechanistic evaluation of gastro-protective effects of KangFuXinYe on indomethacin-induced gastric damage in rats.

KangFuXinYe (KFX), the ethanol extract of the dried whole body of Periplaneta americana, is a well-known important Chinese medicine preparation that has been used to treat digestive diseases such as gastric ulcers for many years in China. However, its therapeutic effect and mechanism are not yet well understood. Thus, the aim of this study was to investigate the gastro-protective effects of KangFuXinYe (KFX) in indomethacin-induced gastric damage. Rats were randomly divided into six groups as follows: control, treated with indomethacin (35 mg·kg), different dosages of KFX (2.57, 5.14 and 10.28 mL·kg, respectively) plus indomethacin, and sucralfate (1.71 mL·kg) plus indomethacin. After treatment, rat serum, stomach and gastric homogenates were collected for biochemical tests and examination of histopathology firstly. Rat serum was further used for metabolomics analysis to research possible mechanisms. Our results showed that KFX treatment alleviated indomethacin-induced histopathologic damage in rat gastric mucosa. Meanwhile, its treatment significantly increased cyclooxygenase-1 (COX-1), prostaglandin E (PGE) and epidermal growth factor (EGF) levels in rat serum and gastric mucosa. Moreover, KFX decreased cyclooxygenase-2 (COX-2) and interleukin-6 (IL-6) levels. Nine metabolites were identified which intensities significantly changed in gastric damage rats, including 5-hydroxyindoleacetic acid, indoxylsulfuric acid, indolelactic acid, 4-hydroxyindole, pantothenic acid, isobutyryl carnitine, 3-methyl-2-oxovaleric acid, sphingosine 1-phosphate, and indometacin. These metabolic deviations came to closer to normal levels after KFX intervention. The results indicate that KFX (10.28 mL·kg) exerts protective effects on indomethacin-induced gastric damage by possible mechanisms of action (regulating tryptophan metabolism, protecting the mitochondria, and adjusting lipid metabolism, and reducing excessive indomethacin).
7.Chinese Medicine Sequential Therapy Improves Pregnancy Outcomes after Surgery for Endometriosis-Associated Infertility: A Multicenter Randomized Double-blind Placebo Parallel Controlled Clinical Trial.

OBJECTIVE:
To evaluate the efficacy and safety of Chinese medicine (CM) improving pregnancy outcomes after surgery for endometriosis-associated infertility.
METHODS:
A multicenter, randomized, double-blind placebo parallel controlled clinical trial was designed. A total of 202 patients who had laparoscopy for endometriosis-associated infertility with qi stagnation and blood stasis syndrome were included and randomly divided into the CM treatment group and placebo control group at a ratio of 1:1 using a central block randomization from May 2014 to September 2017, 101 patients in each group. The two groups received continuous intervention at 1-5 days after surgery, for 6 menstrual cycles. Before ovulation, the CM group was treated Huoxue Xiaoyi Granule (); after ovulation, Bushen Zhuyun Granule ( was involved. The control group was treated with placebo. Transvaginal ultrasonography was performed every menstrual cycle during the treatment, and female hormone levels in the follicular and luteal phases were measured during the 1st, 3rd and 6th menstrual cycles. The analysis was continued until pregnancy. The primary outcomes were clinical pregnancy rate and pregnancy outcome, and the secondary outcomes were follicular development and endometrial receptivity. Safety evaluations were performed before and after treatment.
RESULTS:
(1) Clinical pregnancy and live birth rates: the clinical pregnancy and live birth rates of the CM group were significantly higher than those of the placebo group [44.6% (45/101) vs. 29.7% (30/101), 34.7% (35/101) vs. 20.8% (21/101), both P<0.05]. (2) Follicle development: the incidence of dominant follicles, rate of cumulative cycle ovulation, and rate of cumulative cycle mature follicle ovulation were significantly higher in the CM group than those in the placebo group [93.8% (350/373) vs. 89.5% (341/381), 80.4% (275/342) vs. 69.1% (253/366), 65.8% (181/275) vs 56.1% (142/253), P<0.05 or P<0.01]). The incidence of cumulative cycle luteinized unruptured follicle syndrome was significantly lower in the CM group than in the placebo group [11.7% (40/342) vs. 17.8% (65/366), P<0.05). (3) Endometrial receptivity: after treatment, both endometrial types and endometrial blood flow types in the CM group were mainly types A and B, while those in the placebo group were mainly types B and C, with a significant difference between the two groups (both P<0.05). (4) Adverse events: the incidence of adverse events between the two groups was not significantly different (P>0.05).
CONCLUSION
Strategies for activating blood circulation-regulating Gan (Liver)-tonifying Shen (Kidney) sequential therapy can effectively improve the clinical pregnancy rate and live birth rate of endometriosis-associated infertility with qi stagnation and blood stasis after laparoscopy, improve follicular development, promote ovulation, improve endometrial receptivity, while being a safe treatment option. This article reports a case of an orthodontic adolescent patient without a right inferior incisor. The right lower canine was used as the abutment. The single-retainer all-ceramic resin-bonded fixed partial denture was used to restore the complete dentition. Thus, the missing space was filled, and the function and aesthetics were restored.

10.Seroprevalence of and Identification of Risk Factors in China.

Serum samples were tested for IgG antibodies using indirect immunofluorescence assays. We then analyzed associated risk factors. Serum samples were considered positive when reactive at a dilution of more than 1:320. Differences between groups and risk factors associated with exposure were statistically analyzed using Chi-square tests and the generalized linear model. 122 of 1,260 samples (9.68%) were positive for infection. The infection rate ranged from 0% to 30.43% and differed significantly among age groups ( < 0.01); infection rate in the 50-59 years group was significantly higher than that in other age groups. The seroprevalence of varied significantly among sites within the four provinces, and the infection rate of field workers was significantly higher than that of urban workers.
