1.Effects of early rehabilitation based on quantitative evaluation on knee function and quality of life of patients undergoing total knee arthroplasty by kinematic alignment technology

Objective To explore the effects of early rehabilitation based on quantitative evaluation on knee function and quality of life of patients undergoing total knee arthroplasty(TKA).Methods A total of 82 patients undergoing TKA from February 2020 to November 2022 in No.904 Hospital of Joint Logistics Support Force of PLA were selected as study subjects.They were divided into control and observation groups according to their admission time,with 41 cases in each group.One patient in the control group was excluded due to loss of follow-up.Kinematic alignment was adopted in all the patients.Routine rehabilitation training was carried out in the control group,and early rehabilitation based on quantitative evaluation was applied in the observation group.The knee function and quality of life of the two groups were observed and compared on the next day and 3 months after operation.Results There were significant differences in the inter-group,time,and interaction of active flexion angle of knee joint between the two groups(P<0.05).The active flexion angle of knee joint at 1,2 and 3 months after operation was larger than that on the next day after operation in both groups.There was no statistically significant difference in the active flexion angle of knee joint between the two groups on the next day and 1 month after operation(P>0.05).The active flexion angle of knee joint in the observation group was significantly larger than that in the control group at 2 months and 3 months after operation(P<0.05).There was no statistically significant difference in the scores of each dimension or the total score of the short-form arthritis impact measurement scales 2(AIMS2-SF)on the next day after operation between the two groups(P>0.05).The scores of each dimension and total scores of AIMS2-SF at 3 months after operation were significantly lower than those on the next day after operation in both groups(P<0.05).And theses scores in the observation group were significantly lower than those in the control group at 3 months after operation(P<0.05).Conclusion Early rehabilitation based on quantitative evaluation is helpful to improve the knee function of patients undergoing TKA by kinematic alignment technology and has a positive impact on the improvement of the quality of life.

3.Analysis of genetic evolution of parechovirus in neonates from Hunan Provincial People’s Hospital in 2021

Objective:To understand the prevalence of human parechovirus (HPeV) in neonates of Hunan Provincial People’s Hospital, and analyze genetic evolutionary characteristics.Methods:From June to September 2021, fecal samples of inpatient neonates were collected in Hunan Provincial People′s Hospital. TaqMan real-time qPCR and RT-PCR were used for HPeV screening and genotyping. High-throughput sequencing and PCR were used to obtain whole genomes. Phylogenetic analysis was performed after sequencing.Results:A total of 123 fecal samples of neonates were collected, of which 22 were HPeV positive, with 17.89% positive rate. All the strains belonged to the HPeV-1 genotype. One full-length genomic sequence of 7 269 bp were obtained, and provisionally named Hunan/HPeV/2021, which has the highest nucleotide identity with known HPeV-1 genotype, with 86.6%-91.9% nucleotide identity. The nucleotide and amino acid identity of open reading frame (ORF) with known similar sequences were 90.3%-92.6% and 97.3%-98.3%, respectively. The phylogenetic analysis showed that Hunan/HPeV/2021 belongs to the HPeV-1 genotype, which is clustered into the same clade as the popular HPeV-1 strains in China.Conclusions:HPeV has a high prevalence in inpatient neonates of Hunan Provincial People’s Hospital and belong to the HPeV-1 genotype.
4.Clinical features of liver injury in children with adenovirus pneumonia with normal immune function

Objective:To study the clinical characteristics of children with adenovirus pneumonia complicated with liver injury and the understanding of clinicians.Methods:The children diagnosed with community-acquired adenovirus pneumonia were enrolled from January 1, 2013 to December 31, 2020. The clinical characteristics of the normal liver function group and the liver injured group were compared, and multivariate regression was applied.Results:Among the 3 294 enrolled children, 1 704 children had elevated alanine aminotransferase and/or aspartate aminotransferase on admission, and the liver function impairment rate was 51.7% (1 704/3 294), of which 1 671 cases were mildly injured (98.06%, 1 671/1 704), 28 cases were moderately injured (1.64%, 28/1 704), and 5 cases were severely injured (0.29%, 5/1 704). Compared with the normal liver function group, the proportion of male children in the liver injured group was higher (68.1% vs. 64.2%, P=0.017), and the age was younger[16(9, 30) months vs. 38(19, 53) months, P<0.001]. Children with liver injury were more likely to have wheezing (32.9% vs. 18.7%, P<0.001), shortness of breath (13.7% vs. 9.1%, P<0.001), and diarrhea (11.0% vs. 4.7%, P<0.001). The white blood cell counts, neutrophil counts and hemoglobin in the liver injured group were lower than those in the normal group, while the lymphocyte counts were higher than those in the normal group. The levels of creatine kinase, cardiac creatine kinase and lactate dehydrogenase were higher. The proportion of severe pneumonia in the liver injured group (26.9% vs. 11.2%, P<0.001) was higher than that in the normal liver function group, and the case fatality rate was significantly higher (1.47% vs. 0.25%, P<0.001). Comparing the probability of liver injury in children with mild and severe adenovirus pneumonia, it was found that the severe pneumonia group had more frequent liver injury (73.0% vs 46.6%, P<0.001), and both moderate and severe liver injury groups were higher than in the mild group. Further multivariate logistic regression analysis did not find a significant factor associated with the incidence of liver injury. Conclusions:The incidence of adenovirus pneumonia in children with liver injury was high, and mild injury is more common. Children with liver injury were younger, had more severe clinical symptoms such as wheezing and shortness of breath, and higher proportion of severe pneumonia and mortality. Among them, children with severe pneumonia had a higher proportion of liver injury, and moderate to severe injury was more common. The monitoring of children with different degrees of liver injury should be strengthened.

This paper reports 3 cases of acute hepatitis of unknown cause in children who met the case definition of WHO. Human adenovirus group C was detected in case 3, and the quantity of viral DNA was relatively high, which may be related to the liver function damage in the patient, but its role in pathogenesis needs further study to confirm.

7. Epidemiological investigation of adenovirus pneumonia in children in Hunan province

Objective:
To investigate the detection results and epidemiological characteristics of human adenovirus (HAdV) among children with community acquired pneumonia (CAP) in Hunan province.
Methods:
A descriptive retrospective survey was conducted to investigate the clinical epidemiological data of 33 056 children with CAP admitted to the Children′s Medical Center of Hunan People′s Hospital from May 1st, 2014 to April 30th, 2019.The seasonal distribution and demographic characteristics of HAdV infection were analyzed.
Results:
(1) A total of 33 056 children with CAP were included in this study.A total of 3 770 children with HAdV positive were detected, with a total detection rate of 11.40% (3 770/33 056). From May 2014 to April 2019, the detection rates of adenovirus in different years were 13.76%, 8.57%, 7.94%, 9.53%, 12.36% and 24.75% respectively.Adenovirus detection rate reached its peak in 2019.(2) The detection rate of HAdV among male children were higher than that among female children (
8. Analysis of eight fatal cases of severe adenovirus pneumonia in children

Objective:
To analyze the clinical characteristics of fatal cases with confirmed severe adenovirus pneumonia in children in order to improve the diagnosis and treatment.
Methods:
The fatal cases of severe adenovirus pneumonia admitted to Pediatric Intensive Care Unit of Hunan Provincial People′s Hospital from January 2019 to July 2019 were collected, whose clinical features, diagnosis, treatment, and the causes of death were analyzed retrospectively.
Results:
A total of eight children were enrolled, and the age ranged from 3 months to 3 years old, and five cases were between 6 months to 2 years old.Three cases had underlying diseases.Adenovirus genotype identification was performed on six patients, and the results showed that all patients were infected with adenovirus type 7.All patients presented persistent high fever, with a peak temperature between 39.8℃ to 41.0℃, which persisted 10 to 37 days.Blood routine test before admission PICU showed that four cases had the decrease in white blood count and hemoglobin concentration, accompanied byincreased serum ferritin levels.Seven cases complicated with infection.Four cases had parainfluenza virus type 3 infection.Six cases had bacterial infection, and Gram-negative bacilli were predominant.One had fungal septicemia.Conventional mechanical ventilation were performed in all patients in this group.Four cases in this group died of severe acute respiratory distress syndrome.The other four cases died of disseminated intravascular coagulation associated with massive gastrointestinal bleeding, pulmonary hemorrhage, heart failure and septic shock combined with multiple organ failure.
Conclusion
Fatal adenoviruspneumonia mostly apppears in children between 6 months to 2 years old or with underlying diseases.Adenovirus type 7 was the main serotype.The occurrence of reactive hemophagocytic phenomenon should be worsen progression of the disease.Co-infection may be an important cause of death.

(3) The detection rate of adenovirus was the lowest in autumn,and the other three seasons were all high﹣incidence seasons (χ2 = 268. 744,P<0. 001). (4) The hospitalization days of adenovirus pneumonia were longer than those of non﹣adenovirus pneumonia (Z= -10. 419,P<0. 001). Conclusion The preva﹣lence of HAdV infection is related to age,season and sex. The susceptibility of adenovirus varies with severity of pneumonia. We should be vigilant against the outbreak of adenovirus infection characterized by years.
