1.column:Serum short-chain fatty acid levels and their association with atopic dermatitis in pediatric patients

ObjectiveTo investigate the metabolic alterations of serum short chain fatty acids (SCFAs) in pediatric patients with atopic dermatitis (AD) and their correlation with different clinical phenotypes using targeted metabolomics. MethodsThis study enrolled 87 AD patients and 67 healthy controls (HC). Serum levels of eight SCFAs were quantified by ultra-high-performance liquid chromatography-mass spectrometry. The associations between SCFAs and AD were assessed using various statistical methods. ResultsCompared with the HC group, levels of acetic acid (AA), propionic acid (PA), and caproic acid (CA) (P=0.002,P=0.002,P=0.043) decreased in the AD group. Logistic regression analysis identified AA (OR=0.449, 95% CI: 0.289–0.698) and PA (OR = 0.487, 95% CI: 0.324–0.732) as protective factors against AD. The combination of AA and PA yielded an area under the curve (AUC) greater than 0.7, indicating good diagnostic efficacy. Age-stratified analysis revealed that AA reduction was predominant in childhood, whereas PA reduction was predominant in adolescence. Pathway enrichment analysis showed significant enrichment of fatty acid biosynthesis (FDR=0.341, P=0.003) and vitamin K metabolism (FDR=1, P=0.039) pathways. Furthermore, subgroup analyses based on disease severity, personal/family history of atopy, and sex revealed no significant differences in SCFAs levels among the groups. ConclusionDifferential serum SCFAs and their enriched metabolic pathways may be implicated in the pathogenesis of AD.
2.Clinicopathological analysis of conjunctival lymphangiectasia

AIM: To analyze the pathological features, immunophenotype, and imaging findings of conjunctival lymphangiectasia(CL), and to explore the etiological mechanisms and provide a theoretical basis for clinical diagnosis and treatment. METHODS:This single-center descriptive cross-sectional study enrolled postoperative specimens from patients with CL who underwent surgical treatment in the hospital between Feb. 2023 and Sept. 2025. Routine hematoxylin and eosin(HE)staining and immunohistochemical staining(D2-40, CD31, CD34, CK)were performed. Anterior segment optical coherence tomography(AS-OCT)was used to observe the lesion morphology. The pathological results were comprehensively analyzed combined with clinical data. RESULTS: A total of postoperative specimens from 32 eyes of 32 patients with CL were enrolled, including 23 females(72%)and 9 males(28%), with a mean age of 53.03±12.47 y. All patients presented with single or multiple transparent cystic elevations beneath the bulbar conjunctiva. The postoperative pathological manifestations were characterized by dilation of conjunctival lymphatic vessels lined with a single layer of flattened endothelial cells, accompanied by edema and inflammatory infiltration in the surrounding stroma. All cases were positive for D2-40, confirming a lymphatic origin; some cases also expressed CD31 and CD34. AS-OCT revealed the lesions as unilocular or multilocular cystic spaces with low reflectivity. After complete surgical resection, the mean follow-up period was 16.2 mo with no recurrence.CONCLUSION:CL is a benign ocular surface lesion characterized by lymphatic vessel dilation. The endothelium co-expresses lymphatic and some vascular markers, suggesting that CL may belong to the spectrum of vascular malformations. AS-OCT has adjunctive diagnostic value, and surgical resection has definitive therapeutic efficacy.
3.Mediating role of accompanied childbirth experience between the dynamic adjustment after marriage and the paternal role adaptation in spouses of primipara

Objective:To explore the mediating effect of accompanied childbirth experience among spouses of primipara on dynamic adjustment after marriage and paternal role adaptation, with the aim of providing reference for further improving accompanying childbirth services.Methods:Spouses of primipara who participated in accompanying childbirth at Nanjing Drum Tower Hospital Group Suqian Hospital from January 2023 to June 2024 were selected as the research subjects through convenience sampling. The general information questionnaire, the Chinese version of Dynamic Adjustment Scale, the Parents' Experiences of Childbirth and the Newborn Father's Role Adaptation Questionnaire were used to conduct a cross-sectional survey, and establish the intermediary model between the dynamic adjustment after marriage and the paternal role adaptation.Results:A total of 244 spouses of primipara were included, with an age of (28.86 ± 5.77) years. The score for dynamic adjustment in spouses of primipara was (110.82 ± 16.33) points, the score for experiences of childbirth was (124.27 ± 13.89) points, and the score for paternal role adaptation was (71.66 ± 9.22) points. The accompanied childbirth experience among spouses of primipara was positively correlated with the dynamic adjustment after marriage and the paternal role adaptation ( r=0.197, 0.511, both P<0.01). The dynamic adjustment after marriage was positively correlated with the paternal role adaptation ( r=0.403, P<0.01). The accompanied childbirth experience among spouses had a partial mediating effect between the dynamic adjustment after marriage and the parental role adaptation, which the mediating effect was 0.182, accounting for 32.79% of the total effect. Conclusions:The dynamic adjustment after marriage can directly predict the paternal role adaptation, and the experience of accompanying childbirth has a partial mediating effect between the two. The role adaptation level of the spouses of the primiparas should be improved by further improving the delivery service to meet the needs of the primiparas and their spouses.
4.Qingre sanzhuo decoction treats hyperuricemia complicated with gouty arthritis via TLR4/Myd88/NF-κB signaling pathway

AIM:To investigate the therapeutic effect of Qingre sanzhuo decoction on rats with hyperuri-caemia combined with gouty arthritis and its effect on TLR4/Myd88/NF-κB signalling pathway.METHODS:Forty-eight SD male rats were randomly divided into blank,model,and colchicine groups(0.3 mg·kg-1·d-1),and Origre sanzhou decotion low,medium and high-dosage groups(7.42,14.85,29.70 g·kg-1·d-1),which were treated with the modified Coderre method for hyperuricemia combined with acute gouty arthritis via gavage of yeast paste combined with potassium oxa-late,which was used for the treatment of acute gouty arthritis combined with hyperuricemia.A composite rat model of acute gouty arthritis was constructed by combining yeast paste with potassium oxalate gavage to cause hyperuricaemia,combined with the modified Coderre method.After 7 days of intervention,the circumference of the right ankle joint of rats was measured and the swelling of the ankle joint was calculated,the blood uric acid(HUA)level of rats was determined by biochemical method,the histopatho-logical and morphological changes of the synovial membrane of the ankle joint of rats were examined by HE staining,and the serum levels of inflammatory factors,tumour necrosis factor-alpha(TNF-α),inter-leukin-6(IL-6),and IL-1β were determined by enzyme-linked immunosorbent assay(ELISA),and Western blotting was performed to determine the levels of inflammatory factors,TNF-α,and IL-1β.The protein expression levels of Toll-like receptor 4(TLR4),myeloid differentiation factor 88(Myd88),and nuclear factor-κB(NF-κB)in the synovial tissues of the ankle joints of the rats were determined by Western blot method,and the mRNA expression of TLR4,Myd88,and NF-κB in the rat was determined by real-time fluorescence quantitative polymerase chain reaction(Real-time PCR).RESULTS:Compared with the blank group,rats in the model group showed significantly lower ankle joint swelling(P<0.01),increased levels of HUA,dis-organised synovial tissue structure,large number of inflammatory cells infiltration,and significantly higher serum levels of TNF-α,IL-6,and IL-1β(P<0.01),and the protein and mRNA expression levels of TLR4,Myd88,and NF-κB in the synovial membrane of the ankle joints of the model group were significantly increased(P<0.01).levels were significantly increased(P<0.01);compared with the model group,joint swelling was significantly reduced in the colchicine group,and the medium-and high-dose groups of Qingre sanzhuo decoction(P<0.05);synovial hyperplasia and inflam-matory cell infiltration were improved in the colchicine group and the medium-and high-dose groups of Qingre sanzhuo decoction,and the HUA and the levels of TNF-α,IL-6,and IL-1β were significantly decreased in the dosing groups(P<0.05,P<0.01),and compared with the model group,the medium-and high-dose groups of Qingre sanzhuo decoction could significantly reduce the expression of TLR4,Myd88,and NF-κB protein and mRNA(P<0.01).CONCLUSION:Qingre sanzhuo decoction reduces the release of inflamma-tory factors by inhibiting the TLR4/Myd88/NF-κB pathway,and plays a role in the treatment of hyper-uricaemia combined with gouty arthritis.
5.Research progress of nucleic acid aptamer AS1411 in tumor molecular imaging

Nucleic acid aptamers are a type of single-stranded oligonucleotides screened through in vitro exponentially enriched ligand phylogenetic technology, and they can bind to various targets with high specificity and high affinity. AS1411 is a 26-base guanine-rich DNA aptamer, and its target nucleolin is widely distributed in multiple locations within the cell, including the nucleolus, nucleoplasm, cytoplasm and cell membrane. AS1411 demonstrates multiple advantages, such as weak immunogenicity, low toxicity, easy structural modification, and strong tissue penetration ability. Despite numerous challenges in the clinical transformation process, with the continuous advancement of molecular imaging technology, AS1411 has demonstrated great potential in the fields of targeted imaging and targeted delivery of cancer drugs. This article mainly focuses on the research progress of AS1411 in the field of molecular imaging, covering its applications in magnetic resonance imaging, fluorescence imaging and nuclear medicine imaging, etc.
6.Analysis of the clinical characteristics of Zoletil 50 poisoning

Objective To explore the clinical characteristics of Zoletil 50 poisoning.Methods The clinical data of 33 patients with Zoletil 50 poisoning admitted to our hospital recently were retrospectively analyzed.Results Thirty-two cases were male and 1 case was female in all the 33 patients.Five cases had previous exposure to psychoactive substances such as nitrous oxide,methamphetamine and ketamine,and 19 cases had a history of smoking or drinking alcohol.Limb shaking was in 33 cases(100%),blurred and yellowed vision was in 18 cases(54.55%),unresponsiveness and hypomnesia were in 17 cases(51.52%),hallucination was in 17 cases(51.52%),irritability and panic were in 16 cases(48.48%),insomnia was in 13 cases(39.39%),unfavorable speech was in 9 cases(27.27%),dizziness and lightheadedness were in 8 cases(24.24%),delusion was in 6 cases(18.18%),nausea and vomiting were in 6 cases(18.18%),frequent urination and difficulty in urination were in 5 cases(15.15%),diarrhea and bloating were in 4 cases(12.12%),itchy skin and the abnormal sensation were in 3 cases(9.09%).There was 1 case(3.03%)of epileptic seizure and 1 case(3.03%)of chills.Ataxia was in 29 cases(96.99%),cognitive dysfunction was in 17 cases(51.52%),abnormal tendon reflex was in 13 cases(39.39%),dysarthria was in 9 cases(27.27%),nystagmus was in 6 cases(18.18%),delirium was in 3 cases(9.09%),and vision loss was in 2 cases(6.06%).Auxiliary examinations suggested neutrophil abnormalities in 19 cases(57.58%),elevated transaminases in 21 cases(63.64%),elevated uric acid in 13 cases(39.39%),increased urinary crystals in 8 cases(24.24%),elevated lipids in 11 cases(33.33%),elevated homocysteine in 5 cases(15.15%),and abnormal thyroid hormone abnormalities in 11 cases(33.33%).EEG showed sinus bradycardia in 4 cases(12.12%).Among the 16 patients who underwent EEG examination,there were 2 cases of mild and moderate abnormalities.Among the 6 patients with evoked potentials,there were 5 cases of optic nerve and auditory nerve abnormalities.There were 3 cases of peripheral nerve abnormalities in 7 cases who underwent EMG.Among the 16 patients who underwent cranial MRI examination,there were 3 cases of cortical atrophy,2 cases of white matter degeneration,and 1 case of cerebellar atrophy.Spinal cord MRI showed spinal cord abnormal signal in 1 case.In the treatment,B vitamins were supplemented to control limb shaking,improve sleep,relieve anxiety,control mental symptoms,improve cognition and other symptomatic and supportive therapy.Symptoms were relieved in 29 cases;1 case was discharged automatically;1 case relapsed during treatment,and no significant improvement was observed;2 cases died.Conclusions The specific mechanism of Zoletil 50 poisoning has not been fully clarified,and the main clinical manifestations are significant limb shaking,blurred vision,slow reaction,hallucinations,insomnia,etc.,which can be fatal in severe cases.At present,symptomatic supportive treatment can only mildly relieve the symptoms.

Diseases caused by animal virus infection seriously restricts the healthy development of animal husbandry.In-depth study of the molecular mechanism of viral replication and pathogenesis will provide theoretical basis for screening vaccine and drug targets.N6-methyladenosine(m6 A)modification occurs extensively in viral and host transcriptomes and affects viral replication and pathogenicity by regulating gene expression,which acts as a novel regulator of gene expression in addition to DNA and protein modifications.Insight into the regulatory molecular mechanism of m6 A modification in virus infection is the research hotspots and frontiers.In recent years,there are re-ports of alternation of the m6 A modification-associated epitranscriptomes and related function a-nalysis during virus infection.Here,we summarize the alternation of the epitranscriptomes induced by African swine fever virus(ASFV),porcine reproductive and respiratory syndrome virus(PRRSV),porcine epidemic diarrhoea virus(PEDV),cestode virus(CSFV),porcine pseudorabies virus(PRV),Marek's disease virus(MDV),Newcastle disease virus(NDV),avian leukaemia virus(ALV)and duck hepatitis A virus(DHAV)infection,and the subsequent effects on viral replica-tion and pathogenicity.We also discuss the potential role and molecular mechanism of m6 A modification in animal virus replication and pathogenesis,which will contributes to the prevention and control for animal disease.
8.Effectiveness of Transcatheter Closure Treatment of Ventricular Septal Defect Without Distance From the Aortic Valve With Eccentric Occluder

Objectives:To evaluate the feasibility and clinical efficacy of eccentric occluder for the treatment of ventricular septal defect(VSD)without distance from the aortic valve.Methods:This analysis included 16 patients(9 males and 7 females)with VSD without distance from the aortic valve,who were treated at Fuwai Central China Cardiovascular Hospital from February 2022 to June 2024.Ten cases had mild right coronary sinus prolapse,and 2 had mild aortic valve regurgitation.All patients were diagnosed with VSD located zero-distance from aortic valve through transthoracic echocardiography(TTE)and left ventricular angiography.Eccentric occluder was used for interventional closure.TTE and 12-lead electrocardiography(ECG)were performed at 1 month,3 months,and 6 months after surgery to observe the presence of residual shunt,aortic valve regurgitation,and cardiac electrical conduction abnormalities.Results:Transcatheter closure was successful in 15 patients,procedure failed in one patient due to the unstable fixation of the occluder.Average diameter of the defect is(4.37±1.53)mm,and the diameter of the occluder is 6.5(6.0,8.0)mm.Postoperative left ventricular angiography showed minimal residual shunt in 3 cases,and no complications such as pericardial tamponade,hemolysis,thromboembolism,or infection,were observed.At one month after occlusion,TTE results showed that residual shunt disappeared in 3 patients with residual shunt.During 3-month and 6-month follow-up,all the occluders were well-positioned with no new aortic valve regurgitation or worsening of the original regurgitation,and no atrioventricular block or bundle branch block and other electrocardiographic conduction abnormalities.Conclusions:The use of eccentric occluder for intervention of ventricular septal defect patients without distance from the aortic valve is safe and effective.
9.Mechanistic study of combined poisoning of diazepam and ethanol based on metabolomics

Objective To study the plasma metabolomics of mice poisoned by different dosage of the combination of diazepam and ethanol,and to reveal the toxicological mechanisms of combined poisoning of diazepam and ethanol.Methods Female Kunming mice were randomly divided into blank group,single and combined poisoning group(n=6),Based on the LD50 of diazepam co-administered with graded ethanol doses,mice in the single-drug and combined groups received oral gavage at 1/2,1,and 2 × LD50.Retro-orbital blood samples(~500 μL)were collected within 24 hours post-administration and analyzed by UPLC-QE-MS technology.Principal component analysis and orthogonal partial least squares discriminant analysis were used to identify differential metabolites and associated metabolic pathways.Results A total of 387 differential metabolites were identified in the combined poisoning group of diazepam and ethanol implicating the key pathways including tryptophan metabolism,phenylalanine metabolism,arginine and proline metabolism,Glycerophospholipid metabolism,phenylalanine,tyrosine and tryptophan biosynthesis.Conclusion Combined diazepam and ethanol poisoning exerts significant systemic effects by disrupting neurotransmitters conduction,exacerbating oxidative stress response and dysregulating energy metabolism.
10.TACE and apatinib combined with camrelizumab for treating giant hepatocellular carcinoma

Objective To explore the efficacy and safety of TACE and apatinib combined with camrelizumab for treating giant hepatocellular carcinoma(HCC).Methods Totally 78 patients with giant HCC were retrospectively collected,including 22 cases received TACE and apatinib combined with camrelizumab(TACE+AC group)and 56 cases received TACE and apatinib(TACE+A group).Propensity score matching analysis was used to select 44 cases(TACE+A'group)from TACE+A group who were matched to those in TACE+AC group at 1:2 ratio.The overall survival(OS),progression-free survival(PFS)and the adverse events were recorded and compared among groups.Results Patients in TACE+AC group had a median OS of 17.8(95%CI:17.5-18.1)months and a median PFS of 8.8(95%CI:5.4-12.3)months,which in TACE+A'group was 9.8(95%CI:7.6-12.1)months and 5.5(95%CI:2.7-8.3)months,respectively.The overall OS rate and PFS rate in TACE+AC group were significantly higher than those in TACE+A' group(both P<0.05).The incidences of thyroid dysfunction,immune pneumonia and reactive cutaneous capillary endothelial proliferation in TACE+AC group were significantly higher than those in TACE+A' group(all P<0.05).No death associated with adverse events occurred.Conclusion Compared with TACE and apatinib,further combining with camrelizumab could get better survival benefit for giant HCC patients with acceptable adverse events.
