Objective To investigate the factors influencing poor prognosis in patients with persistent atrial fibrillation combined with heart failure undergoing catheter ablation. Methods The clinical data of 166 patients with persistent atrial fibrillation who underwent elective radiofrequency ablation in the Department of Cardiology of our hospital from January 2023 to June 2024 were retrospectively analyzed.Patients who under-went radiofrequency ablation and developed persistent atrial fibrillation were placed in the poor prognosis group(n=89),and those who did not develop heart failure were placed in the control group(n=77).Logistic regression analysis was used to identify factors influencing poor prognosis in patients with persistent atrial fi-brillation after catheter ablation;latent class analysis(LCA)was used to compare differences in the distribu-tion characteristics of influencing factors between the high-risk group for poor prognosis[B-type natriuretic peptide(BNP)>200 pg/mL]and the low-risk group(BNP≤200 pg/mL).Results There were statistically significant differences between the two groups of patients in terms of age,use of ACEI/ARB medications,dis-ease course, postoperative NYHA cardiac function classification, use of ivabradine, preoperative TG,preopera-tive TC,preoperative HDL-C,preoperative serum cholinesterase, preoperative galectin-3,preoperative LVEF,preoperative NT-proBNP,preoperative cTnI,and preoperative BNP(P<0.05).A disease course≥3.2 years, postoperative NYHA cardiac function classification score≥2 points, preoperative HDL-C<1.05 mmol/L,preoperative NT-proBNP≥12.5 μg/L,and preoperative BNP≥200.00 pg/mL were independent factors in-fluencing adverse outcomes in patients with persistent atrial fibrillation(P<0.05).LCA results showed that the"high-risk type distribution"accounted for a higher proportion in the high-risk group, while the"low-risk type distribution"accounted for a higher proportion in the low-risk group. The incidence of clinical endpoint e-vents in the poor prognosis group was 38.20%,higher than 9.09%in the control group(P<0.05).Conclu-sion Heart failure has an adverse effect on the prognosis of patients with persistent atrial fibrillation after catheter ablation, and there are differences in the incidence of adverse outcomes as well as the distribution characteristics of influencing factors between high-and low-risk groups.

In clinical dentistry, addressing unique conditions such as tilted, elongated, and torsion teeth during prepa-ration can be effectively managed through digital tooth morphology design. The production of a multi-stage 3D-printed guide offered a more efficient and accurate solution. This article presented a case of significant inclination, elongation, and torsion in the maxillary and mandibular canines that were successfully treated using crown restoration modification.A crown preparation guide was fabricated based on the final form design of the restoration using the target restorative space analysis technique to ensure precise tooth preparation.A tooth preparation guide was also designed and utilized fur-ther to enhance accuracy and efficiency during complex tooth preparation. The combined application of these multi-stage guides demonstrated promising clinical prospects.

Objective To evaluate the accuracy in placement of cervical anterior transpedicular screws (ATPS) under the guidance of our modified drilling template. Methods Eight wet cadaveric specimens of adult cervical spine,4 male and 4 female ones, were used for this experiment.Data of CT thin layer scanning of the cervical specimens in DICOM format were imported into software Mimics 17.0.After 3D models of the cervical vertebrae were reconstructed, the optimal trajectory was designed for ATPS placement in the transparent mode of the 3D models.Normal and our modified drilling templates for ATPS placement were designed and manufactured by 3D printing. The 2 kinds of drilling templates were used to assist ATPS placement onto one cervical vertebra, with laterality randomized.Altogether 64 pieces of ATPS were placed, with 32 ones for each drilling template.Postoperative CT scanning was conducted to evaluate the accuracy in ATPS placement. The 2 kinds of ATPS template were compared in terms of the deviations between actual and ideal trajectories in direction (caudal-angle and extroversive-angle) and position (X and Y axes) of the entry point. Results Our modified drilling template led to one screw penetrating the pedicle cortical bone while the normal drilling template led to 4 screws penetrating the pedicle cortical bone, yielding an accuracy rate of 96.9% and of 87.5% respectively. The extroversive-angle deviation (0.72°±0.69°),caudal-angle deviation (0.91°±0.70°),X axis shift (0.53 ±0.40 mm) and Y axis shift (0.54 ±0.42 mm) related to the modified template were all significantly smaller than those related to the normal template (1.16° ± 0.70°,1.49°±0.68°,0.88±0.40mm,1.22±0.42mm,respectively) (P <0.05).Conclusion Compared with a normal ATPS template, our modified drilling template for ATPS may be advantageous in better fitness, greater stability, increased accuracy in screw placement, and protection of adjacent discs.
