1.Study on the diagnostic value of ultrasound-guided puncture biopsy of intra rectal cavity and colonoscopy biopsy for rectal tumors

Objective:To investigate the diagnostic values of ultrasound-guided puncture biopsy of intra rectal cavity and colonoscopy biopsy for rectal and anal canal tumors.Methods:A total of 100 patients with rectal tumors,who admitted to Beijing Rectum Hospital from March 2021 to March 2023,were selected.All patients completed three-dimensional(3D)ultrasound examination of intra rectal cavity,and all of them adopted both colonoscopy biopsy and ultrasound-guided puncture biopsy of intra rectal cavity to obtain tissue samples.Postoperative pathological results were used as the"gold standard"to compare the diagnostic accuracy,pain index,amount of blood loss and examination time of two kinds of modes in sampling.The Kappa test was adopted to analyze the consistency between two kinds of examination methods and postoperatively pathological staging.Results:The sensitivity,specificity and accuracy rate of pathological diagnosis with colonoscopy biopsy were respectively 90.74%,82.61%and 87.00%in diagnosing the benign and malignant rectal tumor.The sensitivity,specificity and accuracy rate of ultrasound-guided puncture biopsy of intra rectal cavity were respectively 98.25%,97.67%and 98.00%in diagnosing the benign and malignant rectal tumor.The diagnostic accuracy rate of ultrasound-guided puncture biopsy of intra rectal cavity was higher than that of colonoscopy biopsy,and the difference was statistically significant(x2=4.672,P<0.05).Kappa test analysis indicated that there was moderate consistency between colonoscopy biopsy and pathological staging in diagnosing rectal cancer(Kappa=0.66),and there was high consistency between ultrasound-guided puncture biopsy of intra rectal cavity and pathological staging in diagnosing that(Kappa=0.77).Conclusion:Both colonoscopy biopsy and ultrasound-guided puncture biopsy of intra rectal cavity have highly diagnostic accuracy rate for rectal tumors.However,ultrasound-guided puncture biopsy of intra rectal cavity has obvious advantages for the biopsy of submucosal rectal tumors and anal canal tumors,and it has minimally invasive and cost-effective advantages,which has higher clinical application value.

3.Research progress of Parkinson syndrome associated with trimetazidine

Trimetazidine (TMZ) is a drug that could optimize myocardial energy metabolism and is widely used in the prophylactic treatment of angina pectoris. TMZ-associated Parkinson syndrome accounted for 1.9% (3/155) of drug-induced Parkinson syndrome and 8.4% (161/1 916) of TMZ-related adverse events. The estimated incidence was 0.36/100 000 patient-years. TMZ-associated Parkinson syndrome can occur from 1 month to 7 years after taking the medicine and mainly in 9 to 15 months; its clinical manifestation is similar to that of primary Parkinson disease, but the treatments for them are quite different. Therefore, attention should be paid to the differentiation. Age ≥65 years and combination with stroke are risk factors for TMZ-associated Parkinson syndrome. Patients diagnosed with TMZ-associated Parkinson syndrome should discontinue TMZ immediately and should be forbidden from using the drug for life. The symptoms in most patients can completely disappear or significantly be improved after discontinuation of TMZ and for a few patients with persistent symptoms, dopamine supplements such as levodopa and benserazide hydrochloride tablets could be given appropriately. TMZ is contraindicated in patients who have been diagnosed with Parkinson disease. The mechanism of TMZ-induced Parkinson syndrome may be related to the piperazine core structure, which has the effect of blocking dopamine receptors.

OBJECTIVE:
To explore the role of inhibitory KIR (iKIR) and its cognate HLA ligand in the occurrence and development of cervical cancer among ethnic Han Chinese and its potential mechanism.
METHODS:
Peripheral blood samples from 265 Han Chinese patients with cervical intraepithelial neoplasia (CIN)/cervical cancer and 200 ethnically matched healthy controls were collected. The results of KIR PCR-SSP, HLA PCR-rSSO and KIR3DL1 PCR-SBT, together with cervical cancer data from the TCGA database, were used to assess the association of iKIR genes, receptor-ligand gene combinations, iKIR transcription level in the tumor tissue and the KIR3DL1 alleles with the occurrence and development of cervical cancer.
RESULTS:
Among the four iKIR genes (KIR2DL1, 2DL2/3, 3DL1 and 3DL2), the frequencies of KIR3DL1 and KIR3DL1-HLA-Bw4 genes among controls were significantly higher than those of the cervical cancer group (96.5% vs. 87.0%, P = 0.018; 81.5% vs. 64.8%, P=0.009). The survival rate of cervical cancer patients with a high transcription level of KIR3DL1 in tumor tissues was significantly higher than those with a low/medium transcription level (P=0.028). The frequency of strong-inhibitory and high-expression KIR3DL1*01502 allele among the healthy population was significantly higher than that of the cervical cancer group (76.0% vs. 59.3%, P =0.015).
CONCLUSION
Combined KIR3DL1 and KIR3DL1-HLA-Bw4 can confer a protective effect against the development of cervical cancer, which may be attributed to the strong-inhibitory and high-expression allele of KIR3DL1*01502.
