BACKGROUND:Obesity negatively affects dynamic balance during walking, and crossing barriers is a more routine functional activity that requires more stability in controlling body posture. OBJECTIVE:To investigate the differences in dynamic stability between obese and average males, and to assess the balance ability of obese males using a relatively more challenging obstacle crossing. METHODS:A total of 24 male youths(12 each in the obese and normal groups)were recruited to complete the tests of walking on level ground and crossing obstacles of different heights(4 cm,11 cm,15 cm)in random order.Kinematic and dynamic data were collected using the Qualisys motion capture system and Kistler force stage.Statistical analysis was performed using two-factor(2 groups * 4 movement types)repeated measures analysis of variance. RESULTS AND CONCLUSION:The obese group had a lower step speed than the normal group(P<0.05),the proportion of the first single support period decreased and the proportion of the second double support period increased when crossing the 11 cm versus 15 cm hurdles(P<0.05).When walking on level ground, the margin of stability in the internal and external directions in the normal group was greater than that of the obese group(P<0.05).When crossing the 4 cm hurdles, the margin of stability in the obese group was less than that in the normal group(P<0.05).When crossing the 11 cm hurdles, there was no significant difference between the two groups in the anterior-posterior direction(P>0.05),while there was a significant difference in the internal-external direction(P<0.05).When crossing the 15 cm hurdles, the margin of stability in the obese group was lower than that in the normal group(P<0.05).Overall,obesity decreases the body's ability to control the body, reduces dynamic stability during crossing the barrier, and increases the risk of falls compared with the general population. In addition, compared with level ground walking, the decrease in the dynamic stability when crossing barriers is more significant in the obese group than the general population.

Comprehensively improving the new efficiency of high-quality development of public hospitals is the key top-level design for deepening the reform of public hospitals during the"14th Five-Year Plan"period. In view of the changes in the macro environment faced by public hospitals in recent years, such as the cancellation of the addi-tion of pharmaceutical consumables and the deep promotion of the reform of payment methods,as well as the out-standing problems existing at the micro level of public hospital operation, such as the separation of diagnosis and treatment business from economic operation activities, extensive decision-making methods, unreasonable alloca-tion of resources and urgent optimization of processes.By referring to relevant theories such as value management theory,a new"trinity"public hospital lean operation management model based on"integration of industry and fi-nance,lean operation and evidence-based decision-making"is explored and constructed. It breaks the relatively parallel operation barriers of clinical diagnosis and treatment business and economic operation management, innovates a new method of linear staff management with the integration of industry and finance, gives full play to the role of experts in governing hospitals, and integrates the professional committee system driven by"technology+manage-men"into the hospital decision-making staff system to provide data evidence-based basis for assisting the hospital leadership to make scientific decisions.

Objective To measure and comprehensively analyze the operation efficiency of coronary heart disease Center of National Regional Medical Center for Cardiovascular Disease from the dimensions of department service in-come,department service quality, department service efficiency and department service benefit, and put forward targeted operation management optimization strategies based on the analysis results. Methods The operation effective-ness evaluation index of CHD centers in sample hospitals from 2020 to 2022 was measured by the empirical re-search method from the overall level of the center and the level of clinical departments, and the scores were com-pared and analyzed. Results From 2020 to 2022,the operation effectiveness evaluation index of CHD centers in sam-ple hospitals showed a good trend, increasing from 80.57 points to 82.86 points. The 3-year average score was 81.74;Among them, the score rate of department service benefit dimension is higher, the average is 96.64%;The score rate of department service efficiency was lower, with an average of 68.53%.The departments with the lowest operational efficiency scores from 2020 to 2022 are all A2 departments, with 74.39,72.41 and 75.89 scores respec-tively,mainly due to the relatively low scores of A2 departments in the dimensions of department service revenue and department service efficiency. Conclusion The results of clinical department operation effectiveness evaluation can provide the evidence-based basis for hospital operation management, and hospitals can establish benchmarking management departments according to the evaluation results and take targeted measures to improve the comprehen-sive operation efficiency of departments.

Non-specific lower back pain is a common clinical disease whose pathogenesis and causes are still unclear, and the advantages and disadvantages of various therapeutic programs are controversial.Current research on this disease is mostly limited to clinical studies, and there is an urgent need to invest in a large number of animal experiments to analyze its underlying mechanisms. The construction of animal models is an important means to study the pathogenesis of non-specific lower back pain and to explore therapeutic method, but there are certain limitations and delays in the establishment of models for this disease. Therefore, this paper reviews the selection of animals, construction method, and evaluation method of relevant indexes of animal models of non-specific lower back pain, and the advantages and disadvantages of various models,to clarify existing problems in current basic research of this disease. We provide new research ideas and aim to lay a theoretical foundation for studies into the mechanisms of non-specific lower back pain and improved therapeutic strategies.

Objective To investigate the effects of shoes type (barefoot, ordinary running shoes, minimalist shoes) and walking speed (jogging, walking at normal speed) on biomechanical parameters of knee joint, so as to provide theoretical reference for scientific fitness. Methods Vicon three-dimensional (3D) motion capture system and Kistler 3D force plate were used to collect biomechanical parameters of lower limbs from 10 subjects during walking at different speed with different shoes. Two-way (2 walking conditions × 3 shoe conditions) repeated measures analysis of variance was used to statistically analyze each dependent variable. Results Compared with jogging, the lateral excursion of plantar center of pressure (COP) was greater, the moment arm in frontal plane, the adduction moment and peak loading rate of knee joint were smaller, but the angular impulse of knee joint in frontal plane was greater. Compared with ordinary running shoes, the stride length was decreased, the lateral excursion of COP was greater, and the moment arm of knee joint in frontal plane, the knee adduction moment, the peak load rate and the angular impulse of knee joint in frontal plane were smaller. Conclusions In order to reduce the angular impulse and peak loading rate of knee joint in frontal plane, it is recommended to jog with small strides for ordinary people with minimalist footwear.
