Background: Several studies have investigated the morphological risk factors contributing to posterior cruciate ligament (PCL) injury. However, no study has focused on the injury patterns of intrasubstance tears or tibial avulsion fractures in relation to morphologies and regional bone quality. This study aimed to investigate the impact of the knee morphology and regional bone quality of the proximal tibia on the PCL injury pattern. Methods: This retrospective study compared the radiological features of 76 patients with PCL injuries (PI group) and 76 matched patients with normal PCL (control group). The PI group was divided into 2 subgroups: PCL avulsion fracture (PAF subgroup; n = 43) and PCL intrasubstance tear (PIT subgroup; n = 33). Measurements included the coronal notch width index, coronal tibial slope, medial and lateral posterior tibial slopes, and medial tibial depth. Three Hounsfield unit average measurements were taken from the proximal tibia around the PCL insertion site to create a summative measure of overall bone quality. Results: The medial tibial depth and coronal notch width index were significantly lower in the PI group than those in the control group (medial tibial depth: 2.04 ± 0.77 vs. 3.02 ± 1.04, p = 0.017; coronal notch width index: 0.21 ± 0.03 vs. 0.24 ± 0.03, p = 0.041). In the subgroup analysis, medial tibial depth and Hounsfield unit were significantly lower in the PAF subgroup than in the PIT subgroup (medial tibial depth: 1.74 ± 0.66 vs. 2.43 ± 0.77, p = 0.008, Hounsfield unit: 89.48 ± 31.73 vs. 120.15 ± 29.24, p = 0.004). Binary logistic regression analysis showed that medial tibial depth (odds ratio [OR], 0.161; p < 0.001) and Hounsfield unit (OR, 0.950;p < 0.001) were independent risk factors for PCL tibial avulsion fractures. Conclusions Lower notch width index and medial tibial depth are associated with PCL injuries. Shallower medial tibial depth and lower Hounsfield unit measurements around the PCL insertion site were associated with an increased incidence of PCL avulsion fractures. These findings may assist clinicians in identifying patients at risk of distinct PCL injury patterns.

Background/Aims: Hepatocellular carcinoma (HCC) exhibits significant sex disparities in incidence, yet its molecular mechanisms remain unclear. We explored the role of telomerase reverse transcriptase (TERT) genetic alterations and hepatitis B virus (HBV) integration, both known major contributors to HCC, in sex-specific risk for HBV-related HCC. Methods: We examined 310 HBV-related HCC tissues to investigate sex-specific TERT promoter (TERT-pro) mutations and HBV integration profiles, stratified by sex and age, and validated with single-cell RNA sequencing (scRNA-seq) data. Results: Tumors predominantly exhibited TERT-pro mutations (26.0% vs. 0%) and HBV-TERT integration (37.0% vs. 3.0%) compared to non-tumorous tissues. While TERT-pro mutations increased with age in both sexes, younger males (≤60 years) showed marked predominance compared to younger females. Males had significantly more HBV integrations at younger ages, while females initially had fewer integrations that gradually increased with age. Younger males' integrations showed significantly greater enrichment in the TERT locus compared to younger females, alongside a preference for promoters, PreS/S regions, and CpG islands. Overall, TERT genetic alterations were significantly sex-differential in younger individuals (75.3% in males vs. 23.1% in females) but not in older individuals (76.9% vs. 83.3%, respectively). These alterations were associated with increased TERT expression. The skewed TERT abnormalities in younger males were further corroborated by independent scRNA-seq data. Conclusions Our findings highlight the critical role of TERT alterations and HBV integration patterns in the male predominance of HCC incidence among younger HBV carriers, offering insights for future exploration to optimize sex-specific patient care and HCC surveillance strategies.

Background/Aims: Peptic ulcer bleeding (PUB) is a major cause of nonvariceal gastrointestinal bleeding. As the older population increases, the prevalence of sarcopenia is also growing. Although sarcopenia influences the prognosis of various diseases, its association with clinical outcomes of PUB remains unknown. Therefore, this study aimed to explore this correlation. Methods: We retrospectively analyzed 2,050 patients who underwent esophagogastroduodenoscopy for suspected gastrointestinal bleeding between January 2014 and December 2021. Patients who underwent computed tomography scans were included for sarcopenia evaluation based on the psoas muscle index, defined as the total psoas area normalized by the square of the height of the patient. Sarcopenia was defined using specific cutoffs: ≤7.3 cm2/m2 and ≤5.1 cm2/m2 for men and women, respectively. The primary outcome measured was the 30-day mortality rate. Results: Out of 358 patients, 149 were diagnosed with sarcopenia. The 30-day mortality rate was significantly higher in patients with sarcopenia than in those without. Multivariate regression analysis revealed significant associations between sarcopenia, a high age, blood tests, and comorbidity score, and administration of inotropic agents with 30-day mortality. Conclusions Our study showed that the presence of sarcopenia, elevated comorbidity scores, and use of inotropes were associated with higher 30-day mortality rates. Considering that sarcopenia may influence the clinical outcomes in patients with PUB, it is crucial to manage patients with sarcopenia with particular care.

PURPOSE: The purpose of this study was to compare the surface characteristics and healing patterns after implantation of implants treated with SLA and those treated with both SLA and femtosecond laser. MATERIALS AND METHODS: A total of 10 male New Zealand white rabbits were used to compare recovery levels between implants treated with SLA (SLA group) and those treated with both SLA and femtosecond laser (SF group). The implants’ surface characteristics were determined through topographic evaluation, element analysis, surface roughness, and wettability evaluation. In total, 4 implants were placed in each rabbit (2 in each tibia), with 20 implants per treatment group. Using the implant stability quotient (ISQ), marginal bone volume, and histological analysis (bone-to-implant contact (BIC), bone volume/tissue volume (BV/TV)), and post implantation outcomes were assessed. Outcome data were analyzed using independent t-tests, Mann-Whitney U tests, Wilcoxon signed-rank tests, and one-way ANOVA (α = 0.05). RESULTS: No significant differences were noted between SLA and SF groups in terms of ISQ, marginal bone volume, BIC, and BV/TV (P > .05). However, significant differences in ISQ were observed within each group over time (P < .05). Furthermore, significant differences were noted in the marginal bone volume of the SF group (P < .05) and the BV/TV of the SLA group between weeks 4 and 6 (P < .05). CONCLUSION Surface treatment via SLA and femtosecond laser is feasible compared with SLA treatment alone in terms of ISQ, marginal bone volume, BIC, and BV/TV. However, further clinical research is warranted.
