1.Pathogenic Mechanisms of Spleen Deficiency-Phlegm Dampness in Obesity and Traditional Chinese Medicine Prevention and Treatment Strategies:from the Perspective of Immune Inflammation

Based on spleen deficiency-phlegm dampness as the core pathogenesis of obesity, and integrating recent advances in modern medicine regarding the key role of immune inflammation in obesity, this paper proposes a multidimensional pathogenic network of "obesity-spleen deficiency-phlegm dampness-immune imbalance". Various traditional Chinese medicine (TCM) herbs that strengthen the spleen, regulate qi, and resolve phlegm and dampness can treat obesity by improving spleen-stomach transport and transformation, promoting water-damp metabolism, and regulating immune homeostasis. This highlights immune inflammation as an important entry point to elucidate the TCM concepts of "spleen deficiency-phlegm dampness" and the therapeutic principle of "strengthening the spleen and eliminating dampness to treat obesity". By systematically analyzing the intrinsic connection between "spleen deficiency generating dampness, internal accumulation of phlegm dampness" and immune dysregulation in obesity, this paper aims to provide theoretical support for TCM treatment of obesity based on dampness.
2.Relationship between Peripheral Blood MiR-21 and Very Early Relapse after Chemotherapy in Children with Acute Lymphoblas-tic Leukemia

Objective:To analyze the relationship between microRNA-21(miR-21)expression and the risk of very early relapse post-induction chemotherapy in children with acute lymphoblastic leukemia(ALL).Methods:A total of 110 newly diagnosed children with ALL admitted to Huanggang Central Hospital from March 2020 to September 2022 were included.All patients received induction chemotherapy according to the CCLG-2008 protocol.The patients who achieved complete response(CR)after induction chemotherapy were followed up for 18 months,with very early relapse as the endpoint event.Then the patients were divided into a relapse group and a non-relapse group.Cox regression was used to analyze the influencing factors of very early relapse after induction chemotherapy in children with ALL.ROC curve and decision curve were used to evaluate the predictive value of peripheral blood miR-21 for very early relapse after induction chemotherapy in children with ALL.Restricted cubic splines were used to analyze the dose-response relationship between peripheral blood miR-21 and very early relapse after induction chemotherapy in children with ALL.Results:A total of 102 children with ALL achieved CR after induction chemotherapy,among whom 24 cases(23.53%)experienced very early relapse,with a median relapse time of 14 months.The proportions of patients with high-risk stratification at initial diagnosis,extramedullary infiltration,and minimal residual disease(MRD)positivity were significantly higher in the relapse group than those in the non-relapse group;The absolute lymphocyte count(ALC)in peripheral blood was significantly lower,while the expression levels of miR-21 and lactate dehydrogenase(LDH)were significantly higher in the relapse group compared with the non-relapse group(all P<0.05).Cox regression analysis showed that very early relapse after induction chemotherapy in children with ALL was associated with medium risk and high risk at initial diagnosis,extramedullary infiltration,decreased ALC in peripheral blood,MRD positivity,as well as high expression levels of miR-21 and LDH(all P<0.05).ROC curve analysis indicated that the area under the curve(AUC)of peripheral blood miR-21 for predicting very early relapse after induction chemotherapy in children with ALL was 0.800,with an optimal cutoff value of 4.830.Restricted cubic spline analysis revealed that there was a non-linear dose-response relationship between peripheral blood miR-21 and the risk of very early relapse after induction chemotherapy in children with ALL.When the expression level of peripheral blood miR-21 exceeded 4.830,the risk of very early relapse increased with the elevation of miR-21 expression.Decision curve analysis demonstrated that combining peripheral blood miR-21 with other risk factors enhanced the predictive performance for the risk of very early relapse after induction chemotherapy in children with ALL.Conclusion:Very early relapse after induction chemotherapy in children with ALL is associated with elevated expression of miR-21 in peripheral blood,and high expression of miR-21 may increase the risk of very early relapse.Detecting miR-21 before induction chemotherapy has predictive significance for very early relapse in children with ALL,and combining it with other risk factors can improve the predictive efficacy.
3.Impact of dynamic neuromuscular stabilization technique on clinical efficacy and balance control in elder-ly patients with chronic low back pain

Objective:To investigate the clinical effects and impact on balance control ability in elderly patients with chronic low back pain using dynamic neuromuscular stabilization(DNS)technique.Method:Forty elderly patients with chronic low back pain were randomly divided into two groups:an experi-mental group and a control group,with 20 participants in each group.The experimental group received DNS training,while the control group received conventional core muscle training.Both groups trained three times a week for four consecutive weeks.Before and after the intervention,visual analogue scale(VAS),Oswestry disability index(ODI),and Roland-Morris disability questionnaire(RMDQ)were used to evaluate the pain and functionality in both groups.The balance control performance of the two groups was evaluated using a bal-ance testing system,assessing parameters such as anterior-posterior movement speed,left-right movement speed,and travel distance during open-eyed double-leg stance,closed-eyed double-leg stance,and open-eyed single-leg stance before and after intervention.Repeated measures analysis of variance was used to assess differ-ences in clinical scales and balance performance before and after intervention in two groups.Result:After 4 weeks of intervention,both groups showed significant reductions in VAS(F=63.386,P<0.001),ODI(F=21.204,P<0.001),and RMDQ(F=38.501,P<0.001)scores compared to pre-intervention(P<0.01).There were no statistically significant differences between two groups(P>0.05).During open-eyed double-leg stance,the experimental group had smaller left-right movement speed than the control group(F=4.228,P=0.047).During closed-eyed double-leg stance,the experimental group had smaller left-right move-ment speed(F=6.560,P=0.015)compared to the control group.During open-eyed single-leg stance,the experi-mental group had smaller anterior-posterior movement speed(F=14.252,P=0.002)and travel distance(F=1.014,P=0.036)compared to the control group.Conclusion:DNS training can relieve pain,improve functionality and balance control ability in elderly pa-tients with chronic low back pain.
4.Establishment and application of a method for detecting Toxoplasma gondii based on recombinant polymerase amplification technology

To establish a method for detecting Toxoplasma gondii based on recombinant polymerase amplification(RPA)technology and apply it to clinical sample validation of pet cats.Using the 529 repeat sequence of the Toxoplasma gondii gene as the target gene sequence,primers and probes were designed,and the Rep-529 recombinant plasmid was constructed as the standard.A fluorescent RPA reaction system was established.Dilute the plasmid standard 10 times to different concentrations as the detection template for sensitivity testing;Specific testing was conducted using genomic DNA from several parasitic spe-cies,including Toxoplasma gondii,Cryptosporidium,Neosporidium,Trichinella spiralis,Giardia flagellata,Babesia bo-vis and Theileria annulata as templates;Simultaneously,fluorescence RPA and RT-PCR were used to detect 52 positive and 40 negative cats clinical samples,and the coincidence rate of the detection results of the two methods were compared and ana-lyzed.The RPA reaction system was successfully established using PTRep recombinant plasmid as the standard,ToxD-F/ToxD-R as the primer,and RepD-P as the fluorescent probe.The reaction temperature was constant at 39 ℃,the reaction time was 30 minutes,and the detection sensitivity was 1 copy/μL.There is no significant cross reaction with parasites such as Cryptosporidium,Neosporidium,Trichinella spiralis,Giardia,Babesia bovis and Theileria annulata,and the specificity is good.A total of 92 clinical fecal samples from cats were tested,and the positive coincidence rate of fluorescence RPA detection method was higher than that of conventional RT-PCR method(98.08%vs.82.69%),and the difference of the positive rate was not statistically significant(X2=1.392,P>0.05).The fluorescence RPA detection method for Toxoplasma gondii suc-cessfully established in this study has the characteristics of being fast,sensitive,specific,accurate,and reliable.It can be used as a rapid clinical detection kit for Toxoplasma gondii in cats and other animals,providing new technical support for the subsequent epidemiological monitoring and precise clinical diagnosis of toxoplasmosis in cats,other animals,and humans in the future.
5.Exploration on the Effects of Diet Therapy on Alcohol Induced Chronic Liver Injury in Rats Based on TLR4 Pathway

Objective To explore the effect of diet therapy on alcohol-induced chronic liver injury in rats and its relationship with TLR4 pathway.Methods According to Pueraria:Poria:Amomum villosum:Pericarpium Citri Reticulatae:Codonopsis:Zingiberis Rhizoma=15∶15∶10∶10∶10∶6,the water extract of the therapeutic prescription was prepared.A total of 78 female SD rats were randomly divided into 6 groups:normal group(n=12),model group(n=18),Hugan tablet group(0.35 g·kg-1,n=12),low-,medium-and high-dose Shiliaofang groups(2,6,18 g·kg-1,n=12).Except for the normal group,the rats in each group were induced alcoholic liver disease(ALD)by gradient alcohol gavage.After 10 weeks of intervention,the changes of body weight and water intake of rats were observed.The whole blood routine,serum alanine aminotransferase(ALT),aspartate aminotransferase(AST),triglyceride(TG),total cholesterol(TC)and liver malondialdehyde(MDA)were measured.The levels of serum alcohol dehydrogenase(ADH),aldehyde dehydrogenase(ALDH),lipopolysaccharide(LPS),D-lactic acid(D-LA),tumor necrosis factor-α(TNF-α),interleukin-6(IL-6)and interleukin-10(IL-10)were detected by enzyme-linked immunosorbent assay.HE staining was used to observe the pathological changes of liver and ileum.Oil red staining was used to observe liver fat deposition.Immunohistochemistry was used to detect the expression of ileal tight junction component occludin.Western blot was used to detect the expression of TLR4,MyD88 and NF-κB p65 protein in liver.Results Compared with the model group,the levels of WBC,LYC,TC,ALT,AST,LPS,D-LA,TNF-α and IL-6 in each diet group decreased,IL-10 increased,liver inflammatory cell infiltration and red lipid droplets decreased,intestinal villi were intact and densely arranged,the expression of occludin protein in ileum increased,and the expression of TLR4,MyD88 and NF-κB p65 protein in liver decreased.Conclusion The self-made dietotherapy prescription has a significant protective effect on chronic liver injury induced by alcohol in rats,which may be related to its down-regulation of TLR4-MyD88-NF-κB p65 protein expression and up-regulation of ileal occludin protein expression,improvement of intestinal mucosal barrier function,reduction of endotoxin entry into the liver,activation of TLR4/MyD88/NF-κB pathway,thereby reducing the second hit to the liver.This diet can be developed as a potential anti-alcoholic liver injury drug.

8.Research progress of antigen specific T cell detection technology

T cell response plays an important role in anti-viral infection and anti-tumor immunity,and antigen-specific T cell detection is essential for study of T cell response.This article reviews progress of antigen-specific T cell detection technology,including enzyme-linked immunospot(ELISPOT)assay,intracellular cytokine staining(ICS)assay and activation-induced labeling(AIM)assay,which based on detection of cytokine secretion or activation phenotypes of specific T cells after stimulation and reactivation with antigen in vitro.Another class of methods include Tetramer technology based on known epitopes-human leukocyte antigen(HLA)restriction and recently developed single-cell transcriptomes and T-cell antigen receptor(TCR)sequencing technology.Application of the above methods has advanced our understanding of antigen specific T cell response:Strength and duration of the response,subpop-ulation information,epitopes and their associated HLA-restriction,TCR cloning information and transcriptome characteristic.
9.Construction of a risk nomogram prediction model for symptomatic intracranial hemorrhage after percutaneous intracranial artery thrombolysis in patients with acute cerebral infarction

Objective To construct an acute cerebral infarction risk nomogram prediction model to evaluate the risk of symptomatic intracranial hemorrhage in patients after percutaneous intracranial artery thrombolysis.Methods A total of 272 patients with acute cere-bral infarction who underwent percutaneous intracranial artery thrombolysis in our hospital from January 2021 to February 2024 were selected as the study participants and divided into the training set(n=190)and validation set(n=82)at a ratio of 7∶3.The training set was divided into a bleeding group(n=61)and non-bleeding group(n=129)based on the presence or absence of symptomatic intracranial hemorrhage during the postoperative period,and the general data of the patients in the two groups were compared.Binary logistic regres-sion was used to analyze the factors influencing symptomatic intracranial hemorrhage.The R Language 4.3.3 toolkit was used to construct a predictive model for the risk of symptomatic intracranial hemorrhage.The predictive efficacy,calibration,and clinical applicability of the model were assessed using subject operating characteristic(ROC),calibration,and decision curves.Results Atrial fibrillation,fasting blood glucose level,and preoperative NIHSS scores were identified as factors influencing symptomatic intracranial hemorrhage after percutaneous intracranial artery thrombolysis in patients with acute cerebral infarction.The AUC of the training and validation sets for predicting symptomatic intracranial hemorrhage was 0.986(95%CI:0.974-0.999)and 0.986(95%CI:0.967-1.000),with a sensitivity of 95.08%and 99.98%,and a specificity of 95.35%and 92.98%.Conclusion The risk nomogram prediction model constructed in this study provides an effective tool of clinical value for assessing the risk of symptomatic intracranial hemorrhage after percutaneous intracra-nial artery thrombolysis in patients with acute cerebral infarction.
10.Advances in testicular organoid culture technology

With the increasing incidence of male reproductive system diseases and reproductive dysfunctions, diagnosis and treatment of male infertility and fertility preservation have become clinical challenges to be overcome. Organoids, a kind of cell product cultured in vitro under different microenvironments by using adult stem cells or embryonic stem cell, can possess histological anatomy and physiological function that are similar to natural tissues. In recent years, technological breakthroughs have been made in testicular organoids in terms of sperm proliferation and differentiation induced in vitro, providing a new idea for solving the above problems. This article briefly introduces common testicular organoid techniques (micropore culture, microfluidics culture method, three-layer gradient system, air-liquid interface culture, extracellular matrix hydrogel scaffold, hanging drop cultures, and 3D printing technology, etc.), including technical features, research conditions, technical bottlenecks and summarizes the application characteristics and situations of some ordinary organoid culture solutions in the field of testicular organoids as well.
