1.Analysis of the incidence and associated factors of cyclosporine-associated acute kidney injury in hospitalized patients based on real-world data

OBJECTIVE To analyze the incidence of cyclosporine (CsA)-associated acute kidney injury (AKI) in hospitalized patients, identify influencing factors, and construct a risk prediction model. METHODS A single-center retrospective study was conducted, enrolling clinical data from hospitalized patients treated with CsA at the First Affiliated Hospital of Shandong First Medical University & Shandong Provincial Qianfoshan Hospital from January 2018 to July 2024. The patients were classified into AKI group and non-AKI group based on the occurrence of CsA-related AKI. Univariate analysis and multivariate Logistic regression analysis were used to identify independent risk factors for CsA-related AKI, and a risk prediction model was constructed and its performance was evaluated. RESULTS A total of 439 patients were included, of whom 54 developed CsA-related AKI, with an incidence rate of 12.30%. The occurrence of CsA-associated AKI was positively correlated with concurrent bacterial pulmonary infection, cytomegalovirus viremia, respiratory failure, renal insufficiency, gastrointestinal bleeding, and peripheral central venous catheterization (odds ratios of 763.750, 16.944, 41.933, 236.806, 17.537 and 212.789, respectively; P <0.05); while uric acid, prealbumin, and calcium levels were negatively associated with it (odds ratios of 0.983, 0.967 and 0.058, respectively; P <0.05). The prediction model constructed based on the above factors yielded a χ 2 value of 10.254 ( P >0.05) in the Hosmer-Lemeshow test. The average area under the curve (AUC) from 10-fold cross-validation was 0.885. The AUC of the receiver operating characteristic curve was 0.883, with a sensitivity of 84.3% and a specificity of 80.4%, respectively, at the optimal cutoff value of 0.1. CONCLUSIONS Six factors, including concurrent bacterial pulmonary infection and cytomegalovirus viremia, are positively associated with the occurrence of CsA-related AKI; while uric acid, prealbumin, and calcium levels are negatively associated. The Logistic regression model constructed based on these factors demonstrates good predictive performance and can assist clinic in conducting early risk assessment and personalized interventions.

It is proposed that yangming (阳明) turbid heat and dysfunction of the chong and ren (冲任) vessels are the key pathogenesis underlying abnormal uterine bleeding caused by polycystic ovary syndrome (PCOS). The general principle of treatment is to clear and drain the yangming channel and resolve turbid heat. During the acute bleeding phase, the approach is to clear heat, cool the blood, consolidate the chong vessel, and stop bleeding. During the chronic phase, the focus shifts to resolving phlegm and removing stasis, unblocking and purging qi movement. And during the late stage, the method of boosting qi and nourishing yin, consolidating the root and regulating menstruation is suggested. In clinical practice, the self-formulated Yishen Quzhuo Formula (益肾祛浊方) serves as the foundation, with flexible modifications and adjustments made according to disease stage and individual clinical presentation.
3.Epidemic and hospitalization characteristics of pertussis in Jingzhou City in 2013–2024

Objective To understand the epidemiological characteristics and current status of pertussis in Jingzhou City from 2013 to 2024. Methods Data on reported pertussis cases in Jingzhou City (2013–2024) and hospitalized pertussis cases from two medical institutions (January–May 2024) were collected for descriptive epidemiological analysis. Results A total of 1 553 pertussis cases were reported in Jingzhou City from 2013 to 2024. The average annual incidence rate from 2013 to 2023 was 0.12/100 000, and that in 2024 was 28.86/100 000. The incidence peaked in spring and summer. High-incidence areas included Jingzhou District, Shashi District, and Jiangling County. There was no difference between men and women. The highest incidence occurred in the 0-year-old group, primarily among students, scattered children, and childcare attendees. Hospitalized cases exhibited milder and atypical symptoms. Age>2 months, and admission for treatment at the first visit were the protective factors for severe pertussis, while having a history of patent foramen ovale was the risk factor for severe pertussis. Conclusion Jingzhou City experienced a rapid and significant rise in pertussis incidence in 2024. Prevention and control measures in childcare facilities and schools should be prioritized during spring and summer. Enhancing laboratory testing in medical institutions can facilitate early diagnosis and treatment. Adjusting vaccination strategies for young infants and older children is critical to reducing both incidence and severe case rates.

5.Thinking and Practice in Treating Gastrointestinal-Gynecological Comorbidities via the Jueyin

In clinical practice,common comorbid states between the gastrointestinal and gynecological systems present significant challenges.The strengths of TCM lie in its"treating different diseases with the same therapy"approach using classical prescriptions.In modern medicine,patients with comorbidities are often treated by different departments,and there are differences in the intervention strategies for digestive system and gynecological system diseases.TCM can treat different diseases with the idea of treating different diseases with the same therapy,grasp the core link of the problem,and simplify the treatment.During the clinical apprenticeship,the authors observed that according to the patients'spleen stomach gynecology comorbidity,the treatment can be carried out from Jueyin,which can achieve good efficacy.According to the physiological functions of Jueyin Meridian and Zang-fu organs,starting from the Jueyin failing to close,this article discussed the changes of pathogenesis caused by too much and too little Jueyin failing to close,and used the classic prescriptions to treat the gastrointestinal-gynecological comorbidities.
6.Significance of high-throughput sequencing combined with lactate dehydrogenase in predicting severe Mycoplasma pneumonia pneumonia in children

Objective To investigate the predictive value of serum lactate dehydrogenase(LDH)level com-bined with high-throughput sequencing in children with severe Mycoplasma pneumoniae pneumonia(SMPP).Methods The clinical data of 99 children with Mycoplasma pneumoniae pneumonia(MPP)admitted to Hua-zhong University of Science and Technology Union Hospital from October 2023 to March 2024 were retro-spectively analyzed.According to the severity of the disease,the children were divided into mild group(33 ca-ses)and severe group(66 cases).Spearman correlation analysis was used for correlation analysis,Logistic re-gression was used to analyze the influencing factors of SMPP,and the receiver operating characteristic(ROC)curve was used to evaluate the value of LDH in predicting SMPP.Results The level of LDH in the severe group was higher than that in the mild group(P<0.05).The results of high-throughput sequencing showed that the proportions of virulence positive and drug resistance gene A2063G mutation in the severe group were higher than those in the mild group(P<0.05).Multivariate Logistic regression analysis showed that LDH level and drug resistance gene A2063G mutation were related to the SMPP(P<0.05).ROC curve analysis showed that LDH combined with high-throughput sequencing detection of drug resistance gene A2063G muta-tion had a high value in predicting SMPP,and the area under the curve was 0.724.Conclusion Serum LDH combined with high-throughput sequencing detection of drug resistance gene A2063G mutation can be used as an effective indicator to predict SMPP.

As a physical treatment technique, modified electro-convulsive therapy (MECT) is used to treat mental and certain neurological disorders by causing seizures with short, suitable electrical currents applied to the brain while the patient is under general anesthesia and muscle relaxants. MECT is recognized for its therapeutic efficacy and clinical safety, rendering it one of the most prevalent interventions in psychiatric care. To enhance clinical outcomes and minimize adverse effects, this consensus document delineates the indications, therapeutic parameters, therapeutic procedures, potential adverse effects, and associated management strategies for MECT. These guidelines are informed by the latest clinical research and expert consensus, integrating evidence-based medicine methodologies. The objective is to furnish clinicians with precise operational guidelines and to advance the standardization of MECT practices in clinical settings.

Vascular aging plays a key role in the induction of human aging and serves as the pathological basis for the occurrence and development of atherosclerosis (AS), myocardial fibrosis, and hypertension. Research suggests that vascular aging is the initiating link in the aging process of the body, and cellular senescence is a key mechanism of vascular aging. The process of vascular aging mainly involves the senescence of endothelial cells (ECs) and smooth muscle cells. This review, therefore, focuses on the molecular mechanisms related to telomere dysfunction, oxidative stress, and metabolic disorders involved in senescence of ECs, elaborating on the correlation between the senescence of endothelial cells and the development of vascular dysfunction, AS, myocardial fibrosis, hypertension, and other cardiovascular diseases (CVDs), and at the same time, systematically summarizes the relevant treatment strategies with western medicines, traditional Chinese medicine formulas, and monomers, to provide some theoretical reference for subsequent treatment and research.

ARTYN is a WeChat official account and Mini Program designed for HIV/AIDS health management.By pub-lishing original articles,providing treatment information research,self-assessment tools,and expert consultations,it effectively improves patient treatment adherence and self-management capabilities.To date,ARTYN has published 197 original articles,which have collectively received over 660 000 views,with a notable increase in annual readership.The most-read article has ap-proached 30 000 views,and the content has been shared by various public accounts a total of 1 356 times,establishing ARTYN as a prominent resource in the domain of HIV antiviral treatment.System usability surveys indicate that user satisfaction with AR-TYN is 95.7%,significantly reducing the workload pressure on medical staff.The treatment effectiveness rate(VL<1 000 cop-ies/mL)for patients subscribed to ARTYN is 98.20%,higher than that of unsubscribed patients(97.04%),with a significant difference(P<0.001).This article emphasizes the importance of strengthening content construction,protecting patient privacy,and continuously promoting patient binding to the mini-program.Moving forward,ARTYN aims to leverage information technology to further improve operational efficiency and patient experience,thereby becoming an integral component of the informatization management strategy for HIV/AIDS antiretroviral therapy in Yunnan Province.
10.Analysis of the Correlation between Plasma Fibrinogen and Osteoporosis Defined by Quantitative Computed Tomography

[Objective]To clarify the associations between plasma fibrinogen(Fbg)and volumetric bone mineral density(vBMD)as well as osteoporosis measured by quantitative computed tomography(QCT),and to explore the role of plasma Fbg in early screening and diagnosis of osteoporosis.[Methods]Patients with hypertension who were hospitalized in the Department of Endocrinology of Sun Yat-sen Memorial Hospital of Sun Yat-sen University from January 2018 to June 2022 and underwent QCT examinations were included for cross-sectional analysis.The study analyzed the correlation between plasma Fbg and osteoporosis in patients.The diagnostic efficacy of plasma Fbg for osteoporosis was evaluated by the area under the receiver operating characteristic(ROC)curve(AUC).[Results]Totally 441 subjects were included in the analysis,with an average age of 46.0±14.5 years and a prevalence of osteoporosis of 6.4%(28/441).As the level of plasma fibrinogen increased,the incidence of osteoporosis significantly increased(P<0.000 1)while the average bone mineral density of L1 and L2 were significantly decreased(P<0.05).Compared with the first quartile of plasma Fbg(1.99g/L-2.37g/L),the risk of osteoporosis in the fourth quartile of plasma Fbg(3.67g/L-4.46g/L)increased by 8.85 times after adjusting for related confounding factors.[Conclusion]This study found a negative correlation between plasma fibrinogen levels and bone density in patients with hypertension.Plasma fibrinogen levels may serve as a potential screening indicator for osteoporosis,aiding in early diagnosis and therapeutic monitoring.This discovery offers a new perspective for the study of bone metabolic diseases and warrants further investigation.
