1.Comparison of Curative Effect between Modified Transvaginal Cervical Cer-clage and Classical Cervical Cerclage

Objective:To combine and improve two kinds of classical transvaginal cervical cerclage,and to provide a new reference for the clinical treatment of patients with cervical insufficiency(CIC).Methods:Retro-spective analysis was performed on the clinical data of 117 patients who underwent transvaginal cervical cerclage due to CIC from January 1,2018 to August 31,2023 in the Second Affiliated Hospital of Kunming Medical Univer-sity.According to different surgical methods,the patients were divided into the modified cerclage group(40 ca-ses),the McDonald cerclage group(40 cases)and the Shirodkar cerclage group(37 cases).Intraoperative and postoperative conditions and pregnancy outcomes were compared.Univariate and multivariate Logistic regression analysis was performed to analyze the related factors of preterm birth after modified cervical cerclage and the oth-er two groups of classical cervical cerclage.Results:①Compared with the McDonald cerclage group,the modified cerclage group had longer effective cervical length,longer delivery and gestational weeks,higher term delivery rate,and shorter post-operative contraction inhibitor time,with statistical significance(P<0.05).Compared with the Shirodkar cerclage group,the operation time of patients in the modified cerclage was significantly shortened,the amount of intraoperative blood loss was reduced,the use time of postoperative contraction inhibitors was shortened,and the rate of postoperative wire-cutting was decreased,with statistical significance(P<0.05).②Multivariate Logistic regression analysis of preterm birth showed that the longer the length of the cervix and the later gestational age when the modified cervical cerclage was performed with reference to McDonald's cerclage,the longer the length of the cervix after surgery,the later the gestational age when the suture was removed.The risk of preterm birth was reduced(OR<1,P<0.05),while preoperative BMI≥24 kg/m2 was associated with an increased risk of preterm birth(OR>1,P<0.05).Using Shirodkar's cerclage as reference,the risk of preterm delivery was consistent between the two types of cervix(P>0.05).Longer cervical length and later gestational age after suture removal reduced the risk of preterm delivery(OR<1,P<0.05).Conclusions:The modified transvaginal cervical cerclage is simple and safe in clinical application,it has certain advantages in improving ad-verse pregnancy outcomes of pregnant women,which provides a new reference for the treatment of pregnant women with CIC.Due to the small number of cases included in the study,comprehensive evaluation by clinicians is still needed for clinical application.

9.Application of coaching technique in respiratory function exercise of patients with chronic obstructive pulmonary disease in stable period

Objective:To explore the application effect of coaching technique in respiratory function exercise of patients with chronic obstructive pulmonary disease (COPD) in stable stage.Methods:Using the convenient sampling method, a total of 40 COPD patients in stable stage who were treated in Geriatric and Respiratory Departments of 3 provincial hospitals in Hangzhou were selected as the research objects from August 2018 to June 2019. They were given a six-month breathing intervention based on coaching techniques by trained health coaches. COPD Self-Efficacy Scale (CSES) and COPD Assessment Test (CAT) were used to evaluate self-efficacy and quality of life of patients before and after intervention. The awareness and implementation of respiratory function exercise were compared before and after intervention.Results:After6 months of intervention, compared with those before intervention, the score of awareness and implementation of respiratory function exercise of patients increased, and the differences were statistically significant ( P<0.05) . After the intervention, the score of CSES increased and the scores of CAT decreased, and the differences were statistically significant compared with those before the intervention ( P<0.05) . Conclusions:The application of coaching technique in the respiratory function exercise of COPD patients in stable period has a certain effect, which can improve the awareness and implementation of patients with respiratory function exercise and improve their sense of self-efficacy and quality of life.
10.Path on effects of personal traits and psychological capital on career among new nurses in Chinese medical hospitals

Objective To explore the path of personal traits, psychological capital and career among new nurses in ClassⅢ Grade A Chinese medical hospitals, so as to provide a reference for career planning management for new nurses. Methods In September 2016, we selected 170 new nurses at three ClassⅢGrade A Chinese medical hospitals in Zhejiang Province as subjects by convenience sampling. Career Condition Evaluation Scale, Psychological Capital Questionnaire(PCQ) and Eysenck Personality Questionnaire (EPQ) were used to investigate career, psychological capital and personal traits of new nurses. Pearson correlation was used to analyze the correlation among psychological capital, personal traits and career condition and to establish the path correlation model. Results A total of 170 questionnaires were sent out and 158 valid questionnaires were collected with 92.94% for the valid recovery rate. The total scores of personal traits, psychological capital and career were (39.59±12.89), (84.69±14.57) and (78.93±13.67) respectively. Psychological capital had a positive correlation with the career condition in new nurses (P<0.05). In personal traits, psychoticism was correlated with the career identity and concealment was positively correlated with the willingness to remain (P<0.05). Psychological capital had a positive prediction effect with the career of new nurses with 0.78 for the path coefficient; personal traits had positive effects on psychological capital with 0.17 for the path coefficient;personal traits had positive effects on career condition with 0.15 for the path coefficient. Psychological capital had a partial mediating role between psychological capital and career prompting personal traits had direct and indirect effects on the career condition. Conclusions Nursing managers should provide an education path of career planning for new nurses based on psychological capital intervention model to cultivate hope quality, optimism and anti-frustration ability of new nurses. Besides, nursing managers should assign jobs and collocate work team according to personal traits of new nurses to create harmonious organizational climate for staff and to promote career development of them.
