1.Stratified mucin-producing epithelial neoplastic lesions of the cervix:clinicohistologic and molecular pathological characteristics

Objective:To investigate the clinicohistologic and molecular pathological characteristics of invasive stratified mucin-producing carcinoma(ISMC).Methods:The clinicopathological data,immunohistochemistry,alcian blue/periodic acid-Schiff(AB/PAS)staining,molecular detection and PD-L1 expressions of 11 cases of ISMC and 4 cases of stratified mucin-producing intraepithelial lesion(SMILE)in the pathological database of Anqing Medical Center of Anhui Medical University/Anqing Municipal Hospital and the first Affiliated hospital of Wannan Medical School/Yijishan Hostital between January of 2018 and March of 2024 were retrospectively analyzed.Results:ISMC patients often presented with irregular vaginal bleeding.Morphologically,the cells containing mucus in the cytoplasm were arranged in stratified layers,surrounded by a palisade,and the tumor cells might be signet ring-shaped or have clear cytoplasm.ISMC might occur in both pure and mixed forms.ISMC had highly invasive biological characteristics.CK7,p16,p40 and(or)p63 were expressed focally or positively in the form of palisades around the cancer nests.AB-PAS staining was positive.HPV test results:HPV16/18 was positive in ISMC(1/4);HPV16/18 was positive before surgery(4/6);no HPV was detected in SMILE;all ISMC cases expressed PD-L1.Eight ISMC patients were successfully followed up for 4-39 months(average 20.50 months),and four SMILE patients for 1-25 months(average 8.25 months).All followed-up patients survived;however,one ISMC patient developed multi-organ metastases post-surgery.Conclusion:ISMC has unique morphological characteristics and immunophenotypes,indicated by high invasiveness and poor prognosis.All ISMC are PD-L1 positive,suggesting that these patients may benefit from PD-L1 immunotherapy.
2.Application of group guidance based on intelligent communication matrix in patients with type 2 diabetes mellitus

Objective:To explore the application effect of group guidance based on intelligent communication matrix in patients with type 2 diabetes mellitus (T2DM) .Methods:Using the convenient sampling method, a total of 60 T2DM patients with poor self-management ability who scored less than 19.6 on Summary of Diabetes Self Care Activities (SDSCA) in the Affiliated Chuzhou Hospital of Anhui Medical University were selected as the research objects from May to June 2020. They were randomly divide into the observation group ( n=30) and the control group ( n=30). The observation group was given continuous group guidance based on intelligent communication matrix, while the control group was given routine health guidance. The SDSCA score, body mass index and glycosylated hemoglobin (HbA1c) before and after intervention were compared between the two groups. Results:After intervention, the SDSCA score of the observation group was higher than that of the control group, HbA1c was lower than that of the control group and body mass index was lower than that of the control group, and the differences were statistically significant ( P<0.05) . Conclusions:The group guidance based on intelligent communication matrix can improve the self-management level of T2DM patients and reduce their body mass index and HbA1c levels, which has clinical application value.
3.Chinese Medicine Regulates Hepatocellular Carcinoma-related Signaling Pathways: A Review

Hepatocellular carcinoma (HCC) is a common tumor in the digestive tract, the formation mechanism of which remains to be fully elucidated. Although surgery, radiation, chemotherapy, targeted therapy, and immunotherapy have achieved significant results in the treatment of HCC, these methods are accompanied by a considerable number of adverse reactions and complications. In recent years, Chinese medicine has shown remarkable efficacy in the treatment of HCC, and both basic experiments and clinical studies have confirmed the effectiveness of Chinese medicine, which exerts therapeutic effects via multiple components and multiple targets. However, the pathogenesis of HCC is exceptionally complex and not fully understood, which means that studies remain to be carried out regarding the specific mechanism of Chinese medicine in preventing and treating HCC. Network pharmacology and molecular biology can be employed to decipher the mechanism of Chinese medicine in the treatment of diseases. Studies have shown that Chinese medicine can regulate various pathways such as the mitogen-activated protein kinase (MAPK), phosphatidylinositol 3-kinase/protein kinase B (PI3K/Akt), Hedgehog, Wnt/β-catenin, nuclear factor-κB (NF-κB), Janus kinase 2/signal transducer and activator of transcription 3 (JAK2/STAT3), and transforming growth factor-β (TGF-β)/Smad signaling pathways. Chinese medicine can exhibit its anti-HCC effects by inducing cell apoptosis, inhibiting cell proliferation and migration, and blocking the cell cycle via the above pathways. However, the specific mechanisms remain to be systematically studied. This study comprehensively reviews the regulatory effects of Chinese medicine on HCC-related signaling pathways to reveal the molecular mechanisms of Chinese medicine in the treatment of HCC. This view holds the promise of providing new targets, new perspectives, and new therapies for HCC treatment and advancing the modernization and development of Chinese medicine.
4.Effects of Trimetazidine on Myocardial Remodeling and Oxidative Stress in Patients with Hypertensive Heart Disease

OBJECTIVE:To prospectively study the effects of trimetazidine on myocardial remodeling and oxidative stress in patients with hypertensive heart disease (HHD),in order to provide reference for clinical treatment of HHD.METHODS:Eighty-two HHD patients were selected from our hospital during Jan.2014-Jul.2016,and they were divided into control group and observation group by sortition randomization method,with 41 cases in each group.Control group received routine HHD chemical drug (antihypertensive drugs,lipid-lowering drugs,hypoglycemic agents and antiplatelet drugs,etc.) therapy.Observation group was additionally given trimetazidine 20 mg,tid,on the basis of control group.Both groups were treated for 3 months.Before treatment and after 3 months of treatment,SBP,DBP,New York Heart Association (NYHA) cardiac function grade,LVEF,LVESD,LVEDD,LVMI and the levels of GSH-Px,SOD,MDA and ROS were compared between 2 group.The occurrence of ADR was observed in 2 groups.RESULTS:Before treatment,there was no statistical significance in each index between 2 groups (P>0.05).After 3 months of treatment,SBP and DBP of 2 groups were decreased significantly compared to before treatment (P<0.01);there was no statistical significance between 2 groups (P>0.05).There was no statistical significance in NYHA cardiac function grade compared to before treatment or between 2 groups (P>0.05).LVESD,LVEDD and LVMI of observation group were decreased significantly compared to before treatment (P<0.05);LVEF was increased significantly compared to before treatment (P<0.05);LVEDD and LVMI of observation group were significantly lower than control group (P<0.05).Compared to before treatment,SOD level of control group was decreased significantly,while the levels of GSH-Px and SOD in observation group were increased significantly;MDA and ROS of observation group were significantly lower than those of control group,with statistical significance (P<0.05).No obvious ADR was found in 2 groups.CONCLUSIONS:Trimetazidine can improve cayocardial remodeling and reduce oxidative stress level of HHD patients with good safety.
