1.A retrospective cohort study on the risk of pulmonary tuberculosis incidence among individuals with latent tuberculosis infection in schools

Objective:To evaluate the risk of developing pulmonary tuberculosis (PTB) among individuals with latent tuberculosis infection (LTBI) in schools and the protective effect of tuberculosis preventive treatment (TPT).Methods:A retrospective cohort study was conducted to collect data on 15 school outbreaks that occurred in Guangdong Province from 2017 to 2021. Baseline information on tuberculin skin test (TST) or interferon-gamma release test (IGRA) was obtained during contact surveys, as well as baseline information such as TPT. The incidence of PTB between 2017 and 2022 was queried using the Chinese Center for Disease Control and Prevention Information System. Poisson regression analysis was used to compare the incidence risk of PTB in the LTBI population under different TST states at baseline. Current cases, new cases and all cases (the sum of the two) were used as dependent variables. Cox regression models were used to analyze various risk factors affecting the risk of PTB in the LTBI population and evaluate the protective effect of TPT.Results:A total of 6 550 contacts were included in this study, of which 409 received TPT. Within 0-3 months after baseline survey, 119 cases were diagnosed as current cases [19.4‰, 119/(6 550-409)]. A total of 17 221.65 person-years of follow-up were conducted, during which 71 new cases were diagnosed (4.1/1 000 person-years, 71/17 221.65). The incidence density of PTB was 47.7/1 000 person-years, 6.6/1 000 person-years, 1.4/1 000 person-years, and 0.9/1 000 person-years, respectively, in TST strong/IGRA positive, TST moderate positive, TST generally positive, and TST and IGRA negative populations. The difference in PTB incidence density was statistically significant [likelihood ratio test LRT=153.16, P<0.001]. TPT was performed for individuals with strong TST or IGRA positivity, and the protection rate could reach 93% ( HR=0.07, 95% CI: 0.02-0.23). Conclusion:After the outbreak of the school epidemic, individuals with strong TST/IGRA positivity have a higher risk of developing PTB in the future. Targeted implementation of TPT can achieve better protection effects. In addition, the risk of developing PTB in individuals with moderate TST positivity is also worth noting.
2.A qualitative study of the care needs of caregivers of children with acute lymphoblastic leukemia developed chemotherapy-induced oral mucositis

Objective:To explore the care needs of primary caregivers of children with acute lymphoblastic leukemia (ALL) developed chemotherapy-induced oral mucositis (CTOM) during hospitalization and to provide a basis for improving the caregivers′ ability to care for the children and promoting their recovery.Methods:This study adopted the phenomenological design of a qualitative approach, using purposive sampling to select 10 primary caregivers of children with ALL who developed CTOM in Zhongshan People′s Hospital from September 2022 to August 2023. A semi-structured, in-depth interview was conducted to collect the data. Colaizzi analysis was used to analyze the qualitative data.Results:Among the 10 children, 5 were males and 5 were females, aged 3-13 years old; among the 10 primary caregivers, 3 were males and 7 were females, aged 31-44 years old. The primary caregivers′ care needs were categorized into four themes: wound care needs (active oral cleaning and hygiene maintenance, lack of ability to observe wounds, and anticipation of new and more effective medications); the need for knowledge of diseases (lack of knowledge related to the causes of CTOM, lack of knowledge about care and medication, seeking information from multiple sources); nutritional knowledge and dietary care needs (active supplementation awareness, limited dietary choices, lack of specific nutritional knowledge);and social network and psychological support needs (experience sharing in primary caregiver networks, psychosocial support in primary caregiver networks).Conclusions:Primary caregivers have particular problems in caring for children with ALL developed CTOM, including the need for nursing knowledge and practical skills, social support, etc. Healthcare professionals should provide the necessary support and assistance in terms of professional resources, hospital management resources and social resources.

5.Education Concept of Four Famous Traditional Chinese Medicine Doctors in Beijing During Republic of China

The education concepts of four famous traditional Chinese medicine(TCM) doctors(XIAO Longyou, SHI Jinmo, KONG Bohua and WANG Fengchun) in Beijing during the Republic of China were compared, the commonalities in their education concepts of TCM were sorted out, and the root system of the cultivation and growth of talents in TCM, as well as the direction and way of further development were searched, so as to improve the quality of the inheritance and development of TCM. Based on the systematic review of text and opinion method(SrTO) developed by the centre for evidence-based health care at the Joanna Briggs Institute(JBI) in Australia, this study was conducted to collate and analyze the relevant information, including 14 books, 115 journal papers and 7 documents. Related theory construction and practice of early tutor and academic education have been summarized from college, continuing and tutor education, academic research, examination, and so on. And then the connections and commonalities between the different forms of education in the period were summarized to form a SrTO-based narrative, opinion, text assessment and review instrument(NOTARI) summary table. The findings revealed that these four esteemed TCM doctors and educators, through their substantial influence on TCM education in Beijing, emphasized the importance of classics in teaching and clinical practice, underscored the cultivation of virtue, preserved the traditional teaching model, and championed the establishment of TCM schools. They also put forward novel requisites for the compilation of curricula, teaching materials, and books. Moreover, they advocated for a unified perspective on TCM and western medicine, fostering talent capable of bridging the gap between the two. They encouraged the standardization of TCM teaching examination system, and actively participated in scientific research and book writing. The four TCM doctors transcended the traditional boundaries of TCM practice, fostering a new TCM model of education-clinical-research, and profoundly influencing the contemporary TCM colleges and teacher education.

This paper puts forward several suggestions, aiming to standardize the preoperative screening and evaluation during the COVID-19 period, strictly grasp the indications and timing of surgery, optimize the medical management process, individualize surgical decision-making, and minimize the risk of COVID-19 infection to surgery.

BACKGROUND:
Perioperative treatment has become an increasingly important aspect of the management of patients with non-small cell lung cancer (NSCLC). Small-scale clinical studies performed in recent years have shown improvements in the major pathological remission rate after neoadjuvant therapy, suggesting that it will soon become an important part of NSCLC treatment. Nevertheless, neoadjuvant immunotherapy may be accompanied by serious adverse reactions that lead to delay or cancelation of surgery, additional illness, and even death, and have therefore attracted much attention. The purpose of the clinical recommendations is to form a diagnosis and treatment plan suitable for the current domestic medical situation for the immune-related adverse event (irAE).
METHODS:
This recommendation is composed of experts in thoracic surgery, oncologists, thoracic medicine and irAE related departments (gastroenterology, respirology, cardiology, infectious medicine, hematology, endocrinology, rheumatology, neurology, dermatology, emergency section) to jointly complete the formulation. Experts make full reference to the irAE guidelines, large-scale clinical research data published by thoracic surgery, and the clinical experience of domestic doctors and publicly published cases, and repeated discussions in multiple disciplines to form this recommendation for perioperative irAE.
RESULTS:
This clinical recommendation covers the whole process of prevention, evaluation, examination, treatment and monitoring related to irAE, so as to guide the clinical work comprehensively and effectively.
CONCLUSIONS
Perioperative irAE management is an important part of immune perioperative treatment of lung cancer. With the continuous development of immune perioperative treatment, more research is needed in the future to optimize the diagnosis and treatment of perioperative irAE.
10.Effects of empowerment management model based on Myers-Briggs Type Indicator Scale on the quality of orthopedic nursing management

Objective:To explore the effect of implementing group empowerment management model based on Myers-Briggs Type Indicator (MBTI) Scale on the quality of orthopedic nursing management.Methods:From January 2018 to December 2019, MBTI Scale was used to test the personality types of 46 nurses in Department of Orthopedics in Zhejiang Hospital. Test results were analyzed and nurses were grouped and given empowerment management measures. Score of orthopedic nurses on the leaders, score of psychological empowerment of nurses and score of nursing quality were compared before and after the improvement of management.Results:MBTI Scale was used to classify personality types of 46 orthopedic nurses, among which ISFJ, ISTJ, ENFP and ESFJ were the most common personality types. After the implementation of empowerment management, score of nurses on the leaders, score of psychological empowerment of nurses and score of nursing quality were all improved than before, and the differences were statistically significant ( P<0.05) . Conclusions:Based on MBTI Scale, the group empowerment management of orthopedic nurses can give full play to the core advantages of nurses, improve self-management ability of nurses and nursing quality.
