1.The implication and association between post-traumatic growth and health-related quality of life in German cancer survivors

Objective To analyze the relationship between post-traumatic growth(PTG)and health-related quality of life(HRQOL)among long-term cancer survivors(time since diagnosis≥5 years)and to explore the trends and reasons for changes over time.Methods A total sampling method was used to conduct baseline and follow-up surveys on long-term cancer survivors from 6 cancer registries in Germany during 2008-2011 and 2018-2019.General information questionnaires,Post-Traumatic Growth Inventory,and European Organization for Research and Treatment of Cancer-Quality of Life Questionnaire were applied.The collected data were analyzed using a linear mixed-effects regression model for longitudinal relationships.Results A total of 2704 long-term cancer survivors completed the baseline and follow-up surveys and the effective sample size is 2 686.The high PTG group had a significantly higher overall health status/QOL score than the low PTG group,with a statistically significant difference(P<0.05).However,the difference in scores between the 2 groups at follow-up was not statistically significant(P>0.05).The interaction between PTG groups and time was statistically significant for overall health status and insomnia(all P<0.05).There was a stable positive relationship between PTG and economic difficulty scores with a statistically significant difference(P<0.001).Conclusion There is a positive correlation between PTG and HRQOL among long-term cancer survivors that weakens over time,and the perceived economic burden related to health is a continuous source of stress.
2.Fixation with modified double-pulley anchor suture for Rockwood type Ⅱ patella fractures

Objective:To investigate the clinical efficacy of fixation with a modified double-pulley anchor suture in the treatment of Rockwood type Ⅱ patella fractures.Methods:A retrospective study was conducted to analyze the 60 patients with Rockwood type Ⅱ patellar fracture who had been admitted to Department of Joint Surgery, The Hospital Affiliated to Qingdao University between November 2021 and November 2023. The cohort included 31 males and 29 females, with an age of (53.5±10.4) years. According to the treatment methods, the patients were divided into a study group (28 cases) which was treated by traction reinforcement of the terminal anchor suture in addition to fixation with a double-pulley anchor suture, and a control group (32 cases) which was treated by the traditional Kirschner wire tension band technique. The operation time, intraoperative blood loss, incision length, reoperation rate, complication rate, fracture healing, and visual analog scale (VAS) pain scores and Bostman scores at postoperative 1 month, 3 months, and 6 months and at the last follow-up were compared between the 2 groups.Results:No statistically significant differences were observed in the preoperative general data between the 2 groups, indicating that the 2 groups were comparable ( P>0.05). All patients were followed up for (12.8±1.4) months postoperatively. The study group demonstrated significantly lower intraoperative blood loss [(25.9±6.8) mL] than the control group [(40.5±11.6) mL], and a significantly lower reoperation rate (0) than the control group [53.1% (17/32)] ( P<0.05). There was no statistically significant difference between the 2 groups in operation time, incision length, incidence of complications, as well as in VAS pain scores or Bostman scores at postoperative 1 month, 3 months, or 6 months or at the last follow-up (all P>0.05). Fractures healed in both groups at 6 months postoperatively. Conclusions:For patients with Rockwood type Ⅱ patellar fracture, fixation with modified double-pulley suture anchor can achieve good clinical efficacy. Compared to the traditional Kirschner wire tension band technique, the modified mothed significantly reduces intraoperative blood loss and avoids secondary trauma caused by a second surgery for implant removal.
3.Clinicalpathological characteristics of 21 cases of breast carcinoma within fibroad-enoma and literature review

Purpose To investigate the clinicopathological features,differential diagnosis,and prognosis of breast carcinoma within fibroadenoma(FA).Methods A retrospective analysis was performed on the clinical and imaging data of 21 cases of FA-associated carcinoma.HE staining,immunohistochemistry using the EnVision method,and next-generation sequencing were employed to assess the clinicopathological characteristics and genetic alterations,with a review of the relevant literature.Results All patients were female,aged 21-67 years,with a median age of 48 years.Clinically,all lesions presented with well-circumscribed mass lesions.On imaging,12 cases demonstrated punctate or coarse calcifications,while 3 cases exhibited locally indistinct margins.Pathological examination revealed that the tumors had an average maximum diameter of 2.25 cm,with a gray-white nodular cut surface resembling FA.Microscopically,there was proliferation of either epithelial or spindle cells.The pathological diagnosis was FA-associat-ed in situ carcinoma in 16 cases(15 cases of low-grade ductal carcinoma in situ and 1 case of lobular carcinoma in si-tu),and invasive carcinoma in 5 cases(comprising 2 cases of invasive carcinoma-no special type,1 case of invasive lobular carcinoma,1 case of low-grade adenosquamous carcinoma and 1 case of fibromatosis-like metaplastic carcino-ma).In 18 cases,the carcinoma was confined within the FA,whereas in 3 cases the tumor locally extended beyond the FA,and 1 case exhibited ipsilateral axillary lymph node metastasis.Immunophenotypically,tumor cells in in situ carcinomas were negative for CK5/6,while the myoepithelial cells were p63-positive,and both estrogen receptor(ER)and progesterone receptor(PR)were diffusely and consistently positive.In invasive carcinoma,both ductal and lobular subtypes were ER and PR positive and HER2 negative.Conversely,in the low-grade adenosquamous carcinoma and fi-bromatosis-like metaplastic carcinoma,ER,PR,and HER2 were all negative,with p63 and CK5/6 positivity.Two ca-ses were negative for mismatch repair protein PMS2.Six cases had a family or personal history of malignant tumors;a-mong the 5 cases that underwent next-generation sequencing,2 cases harbored germline BRCA2 mutations,2 cases had germline PMS2 mutations,and 1 case exhibited no definitive genetic alteration.Conclusion Carcinoma arising within FA exhibits atypical clinical and imaging features.In patients with high-risk factors for breast cancer or family history of malignancy,or when calcifications are observed on imaging,prompt excision and biopsy are recommended.Recogniz-ing the abnormal morphology within FA and the use of immunohistochemistry are essential for accurate diagnosis,with careful differentiation from spindle cell metaplastic carcinoma and low-grade adenosquamous carcinoma.Overall,the prognosis is relatively favorable,and treatment should be tailored according to tumor type,molecular subtype,and stage.

7.Automatic epileptic seizure detection algorithm based on dual density dual tree complex wavelet transform.

It is very important for epilepsy treatment to distinguish epileptic seizure and non-seizure. In this study, an automatic seizure detection algorithm based on dual density dual tree complex wavelet transform (DD-DT CWT) for intracranial electroencephalogram (iEEG) was proposed. The experimental data were collected from 15 719 competition data set up by the National Institutes of Health (NINDS) in Kaggle. The processed database consisted of 55 023 seizure epochs and 501 990 non-seizure epochs. Each epoch was 1 second long and contained 174 sampling points. Firstly, the signal was resampled. Then, DD-DT CWT was used for EEG signal processing. Four kinds of features include wavelet entropy, variance, energy and mean value were extracted from the signal. Finally, these features were sent to least squares-support vector machine (LS-SVM) for learning and classification. The appropriate decomposition level was selected by comparing the experimental results under different wavelet decomposition levels. The experimental results showed that the features selected in this paper were different between seizure and non-seizure. Among the eight patients, the average accuracy of three-level decomposition classification was 91.98%, the sensitivity was 90.15%, and the specificity was 93.81%. The work of this paper shows that our algorithm has excellent performance in the two classification of EEG signals of epileptic patients, and can detect the seizure period automatically and efficiently.

8.Status and implications of researches on benefit finding of family caregivers of cancer patients

Family caregivers of cancer patients have a huge burden of care and psychological pressure. At the same time, they also get positive benefits in caring for patients. This article reviews the conceptual analysis of the benefit finding of cancer family caregivers from domestic and foreign scholars, the application of measuring tools, the influencing factors of benefit findings and the interventions. Transferring psychological focusing view, it explores the psychological rehabilitation implication of benefit findings in cancer family caregivers in China from the aspect of positive psychology.

Objective To explore the clinical significance of early biomarkers in neonatal asphyxia diagnosis with myocardial damage by detection of ischemia modified albumin in neonatal serum(IMA) and glycogen phosphorylase isoenzyme BB(GPBB) for screening sensitive markers with direct myocardial injury.Methods 166 neonates were selected in our hospital as the research object,in which 136 cases with myocardial injury dividend into the experimental group and 30 cases into the control group.The experimental group were divided into mild group and severe group according to the degree of asphyxia.All the children were tested for GPBB and IMA 1 h after admission.Results The levels of GPBB in neonatal asphyxia myocardial injury group and severe asphyxia group were significantly higher than those in the control group,the difference was statistically significant(P<0.05).The sensitivity of GPBB in diagnosis of asphyxia was higher than that of IMA,the difference was statistically significant(P<0.05).Conclusion The degree of asphyxia is closely related to serum IMA,GPBB level in neonatal asphyxia complicated with myocardial injury.The sensitivity and specificity of GPBB in diagnosis asphyxia is better than IMA in children complicated with myocardial damage.
10.The influencing factors of benefit finding in patients with cancer

Objective To understand the status and influencing factors of benefit finding in patients with cancer.Methods 350 patients with cancer were collected with convenience sampling method from two Jiangsu hospitals.All patients received a questionnaire survey.Multiple linear regression analysis was used to analyze the influencing factors of benefit finding in patients with cancer.Results (1) The score of benefit finding in patients with cancer was statistically significant in different frequency of physical exercise (F=8.37,P=0.00),different character (F=3.04,P=0.04) and different educational level (F=2.91,P=0.03).(2) There was a positive correlation between benefit finding(78.53± 10.50) and communication with physicians(7.27±2.19),medical coping(43.48±4.50),optimism(17.33±2.24) and social support(44.81±3.91)(r=0.31,0.35,0.29,0.23,all P<0.05) in patients with cancer.The benefit finding in patients with cancer was negatively correlated with depression(8.59± 1.83) (r=-0.37,P<0.05).(3) Multiple linear regression analysis results showed that medical coping modes-confrontation,objectively social support and utilization,physical exercise,depression,communication with physicians and character were the factors influencing benefit finding in patients with cancer(△R2 =28%,P< 0.05;△R2 =5.5%,P< 0.05;△R2 =3.1%,P< 0.05;△R2 =1.9%,P<0.05;△R2 =1.7%,P<0.05;△R2 =1.0%,P<0.05;△R2 =1.1% P<0.05),which explained 42.0% of the total variance.Conclusion Medical coping modes-confrontation,objectively social support and utilization,physical exercise,depression,communication with physicians and character are the factors influencing benefit finding in patients with cancer.
