1.Observation on the effect of endoscopic surgery for parotid gland tumors via the postauricular hairline approach

Objective To compare the efficacy of endoscopic parotidectomy through postauricular incision with conventional incision parotidectomy.Methods A total of 60 parotid tumor patients admitted to Zhejiang Cancer Hospital from March 2023 to March 2024 were selected as the study subjects.Thirty patients had the requirement for small scar cosmetic surgery,and the endoscopic parotid tumor resection of the posterior ear were in endoscopic group,and remaining 30 patients underwent traditional open surgery at the same period in open group.Two groups were compared in terms of operation time,intraoperative blood loss,postoperative drainage volume,length of hospital stay,postoperative complications,nursing satisfaction,and incision satisfaction at one month postoperatively.Results Endoscopic group had significantly lower drainage volume,hospital stay,and postoperative complications than open group,there were significant difference between two groups(P<0.05).The nursing satisfaction and incision satisfaction in endoscopic group were significantly higher than those in open group(P<0.05).Conclusion Efficacy of endoscopic parotidectomy through postauricular incision postoperative recovery was faster,the hospitalization time was shorter,and the postoperative complications were less.Perioperative optimization nursing in the endoscopic group improved patient satisfaction,which is worth promoting clinically.
2.Exercise-based pre-rehabilitation before surgery for patients with lung cancer:a scoping review

Objective To review literature on exercise-based pre-rehabilitation before surgery in patients with lung cancer,hence to provide a guidance for development of standardised programs for pre-rehabilitative exercise.Methods Following the Australian JBI Evidence-based Health Care Centre Scope Review guidelines,a comprehensive search was conducted across the databases of PubMed,Embase,Web of Science,CINAHL,CNKI,WanFang Data and SinoMed.The search covered the inception of databases to 29th February,2024.The retrieved literature was then screened and relevant information was extracted.Results A total of 20 articles were included in this review.Exercise-based pre-rehabilitation for lung cancer patients was mainly divided into single-modality pre-rehabilitation and multimodal pre-rehabilitation(exercise combined with interventions in nutrition,psychology and health behaviour).The period of intervention for exercise-based pre-rehabilitation was typically 2-3 weeks before surgery,with 3-5 sessions per week.The outcome indicators mainly included respiratory function,capacity of physical activity,quality of life,psychological status and postoperative complications.Conclusion The preoperative exercise-based pre-rehabilitation before surgery for the patients with lung cancer is feasible and effective in enhancing physical function of the patients.Further studies can focus on the development of patient-centred standardised exercise pre-rehabilitation programs.

5.Summary of best evidence for prevention of hemodialysis catheter thrombosis

Objective:To retrieve, evaluate, and integrate the best evidence for prevention of hemodialysis catheter thrombosis, so as to provide evidence-based basis for catheter thrombosis prevention in hemodialysis patients.Methods:Guidelines, clinical decisions, evidence summaries, systematic reviews, and expert consensus on the prevention of hemodialysis catheter thrombosis were systematically searched on Guidelines International Network, Scottish Intercollegiate Guidelines Network, Agency for Healthcare Research and Quality, Registered Nurses' Association of Ontario, National Institute for Health and Clinical Excellence, UpToDate, British Medical Journal (BMJ) Best Practice, Joanna Briggs Institute Evidence-Based Health Care Center Database, European Renal Association, National Kidney Foundation, UK Kidney Association, Canadian Society of Nephrology, Japanese Society for Dialysis Therapy, Medlive, Cochrane Library, PubMed, Embase, China National Knowledge Infrastructure, VIP, and Wanfang Data and Chinese Biomedical Database. The search period was from database establishment to March 31, 2023. Investigators conducted quality evaluations separately and extracted and summarized evidence based on the theme.Results:A total of 16 articles were included, including five clinical decisions, four guidelines, two evidence summaries, and five expert consensuses. Finally, 15 best pieces of evidence were formed, including four themes of indwelling dialysis catheters, anticoagulation strategies, flushing and sealing techniques, and daily management.Conclusions:Nursing staff should choose the best evidence for preventing thrombosis in hemodialysis catheters based on clinical situations, reduce the occurrence of catheter thrombosis, extend the usage time of catheters, and improve patient outcomes.
6.Scoping review of supportive care intervention schemes for patients with liver cancer

Objective:To summarize the research status of supportive care intervention schemes for patients with liver cancer, analyze relevant studies and identify the forms, contents, outcome indicators and effects of interventions, in order to provide references for medical staff to carry out relevant research.Methods:The scoping review method was used to search relevant articles in WanFang data, VIP, China National Knowledge Infrastructure, Web of Science, PubMed, Embase, Scopus, CINAHL and Cochrane Library. The search period was from the establishment of the databases to May 2, 2023, and the included articles was analyzed.Results:A total of 14 articles were included. The intervention scheme included information and care skills guidance, palliative care support, psychological support, multidisciplinary team support, family rehabilitation exercise guidance, and stress management. Intervention forms included online, offline and mixed intervention. The intervention duration was usually 3 or 6 months. The intervention frequency was usually 1~5 times a week, with each session lasting 20~60 minutes. The intervention scheme could improve the quality of life of liver cancer patients to a certain extent, alleviate anxiety and depression.Conclusions:At present, supportive care interventions are led by nurses and have the characteristics of comprehensiveness, personalization and unclear intervention effects. In the future, it is necessary to further optimize the content and strategy of intervention, and formulate scientific, applicable and standardized supportive care intervention schemes according to the disease and physical function status of liver cancer patients.
7.Factors influencing cardiopulmonary resuscitation competency among community medical staff in Beijing and countermeasures

Objective:To investigate the status of the knowledge of cardiopulmonary resuscitation (CPR) in community medical staff, and analyze the factors influencing their levels of CPR knowledge and skills, and to provide a basis for improving community CPR training programs.Methods:From January to March 2022, we conducted a survey for the knowledge of CPR among community medical staff in Beijing through WeChat using a self-made questionnaire based on the 2016 National Consensus on Cardiopulmonary Resuscitation in China and the 2020 American Heart Association CPR guidelines. The questionnaire mainly focused on the basic information and CPR knowledge (full score, 17 points) and practice of medical workers. R4.0.3 software was used to perform multiple linear regression analysis.Results:A total of 990 medical personnel from 51 communities effectively responded to the survey. The mean CPR knowledge score was (10.27±2.87) points. The regression analysis showed that the CPR score was significantly lower in groups of male, non-31-40-year-olds, technical degrees, only 0-1 trainings in two years, not ever practicing CPR, and not ever using an automated external defibrillator (AED), suggesting that sex, age, educational attainment, training frequency, whether having the experience of CPR and AED practice or not were independent factors influencing the CPR score of community medical staff.Conclusions:The CPR competency of community medical staff needs to be improved, especially for those with technical degrees and non-31-40-year-olds. The training frequency should be increased based on the Ebbinghaus' forgetting curve, and stratified training is recommended for different populations. Those without practical experience should receive more CPR training with real-time feedback devices and high-fidelity simulation to improve their CPR skills.

Objective To establish a phase Ⅱ remote home-based cardiac rehabilitation (HBCR) program for patients with coronary heart disease (CHD) after percutaneous coronary intervention (PCI) based on social cognitive theory (SCT). Methods A preliminary draft of the phase Ⅱ HBCR program for CHD patients after PCI based on SCT was developed through evidence-based literature review and group discussion, and the final version was formed through Delphi expert consultation. Results A total of 18 experts participated in three rounds of expert consultation. Objective:To design a continuous renal replacement therapy (CRRT) waste liquid bag disposal vehicle and explore its effectiveness.Methods:In February 2024, convenience sampling was used to select 100 waste liquid bags generated during CRRT in the ICU of Zhejiang Provincial Hospital of Chinese Medicine as research subjects. The waste liquid bags were divided into an observational group ( n=50) and a control group ( n=50) using a random number table method. The control group was treated using usual method, while the observational group was treated using a CRRT waste liquid bag disposal vehicle. The emptying time of a single bag of waste liquid, the number of splashes per unit area, and operator satisfaction were compared between two groups. Results:The emptying time of a single waste liquid bag in the observational group was lower than that in the control group [ (38.34±1.98) vs. (315.46±11.23) s, P<0.01]. The number of splashes per unit area of 5 cm × 5 cm in the observational group was less than that in the control group [ (0.28±0.09) vs. (8.62±0.64), P<0.01]. The operator satisfaction score of the observational group was higher than that of the control group [ (4.88±0.05) vs. (1.44±0.10) points, P<0.01] . Conclusions:The CRRT waste liquid bag disposal vehicle can reduce the emptying time of waste liquid bags and the pollution to the surrounding environment during the discharge process, improve operator satisfaction, and is worthy of promotion in clinical practice.
