1.The Impact of Pcdh15 Deficiency on Cellular Energy Metabolism and Oxidative Stress, and Its Role and Mechanism in Hearing Loss

Objectives:
. This study aimed to explore the role of the Pcdh15 gene in hearing maintenance and to examine the effects of its deficiency on cochlear structure, cochlear function, and hearing loss in mice.
Methods:
. CRISPR/Cas9 technology was used to generate Pcdh15 knockout (KO) mice. Auditory function was evaluated using hearing tests, while histological approaches were employed to assess morphological changes in cochlear hair cells and spiral ganglion neurons (SGNs). RNA sequencing (RNA-seq) was performed on cochlear tissue from postnatal day 18 (P18) wild-type (WT) and Pcdh15 KO mice to identify differentially expressed genes (DEGs). These DEGs were subjected to functional annotation and pathway analysis, with particular emphasis on oxidative phosphorylation (OXPHOS). Reactive oxygen species (ROS) levels were quantified using flow cytometry and DCFH-DA assays.
Results:
. Pcdh15 KO mice exhibited progressive sensorineural hearing loss, which worsened to profound deafness by P18. Notably, cochlear hair cells and SGNs showed substantial loss and pronounced morphological abnormalities, particularly within the basal high-frequency region. RNA-seq analysis identified 1,359 DEGs, including 1,024 downregulated and 335 upregulated genes. Pathway analysis indicated that Pcdh15 deficiency was associated with inhibition of the OXPHOS pathway, potentially disrupting mitochondrial respiratory chain complexes I, III, IV, and V and thereby impairing energy production and ATP metabolism. In addition, early-stage cochleae from KO mice demonstrated elevated ROS levels and increased oxidative stress, suggesting that redox imbalance may contribute to hearing damage.
Conclusion
. Pcdh15 plays a critical role in hearing maintenance. Its deficiency is associated with severe hearing loss and marked cochlear abnormalities, which are linked to inhibition of OXPHOS, disruption of energy metabolism, and exacerbation of oxidative stress. Together, these findings provide new insights into the mechanisms underlying hearing loss and suggest potential targets for therapeutic intervention.
2.Efficacy and safety of Chinese herbal compounds for pulmonary nodules: A systematic review and meta-analysis

Objective To systematically evaluate the efficacy and safety of traditional Chinese medicine (TCM) compound in treating pulmonary nodules, providing evidence-based medical evidence for TCM intervention in pulmonary nodules. Methods Computer search of PubMed, CNKI, Wanfang, VIP, and SinoMed was conducted to select randomized controlled trials (RCTs) of TCM compound intervention in pulmonary nodules, with the retrieval time from the inception to November 29, 2023. The Cochrane bias risk assessment tool was used to evaluate the quality of the included studies, and Review Manager 5.4 was used for Meta-analysis. Results A total of 18 RCTs were included, covering 8 provinces across the country, with a total sample size of 1301 patients. The TCM compounds used in the included studies all incorporated the method of dissolving phlegm and dissipating nodules. There was a high risk of bias uncertainty in the included studies. Meta-analysis results suggested that TCM compound could significantly reduce the diameter of pulmonary nodules [MD=−1.41, 95%CI (−1.70, −1.13), P<0.001], decrease the number of nodules [MD=−0.37, 95%CI (−0.73, −0.01), P=0.05], alleviate clinical symptoms [MD=−4.84, 95%CI (−6.04, −3.64), P<0.001], and improve lung function [forced expiratory volume in one second (FEV1), MD=0.55, 95%CI (0.09, 1.01), P=0.02; FEV1/forced vital capacity, MD=6.12, 95%CI (4.47, 7.78), P<0.001]. However, there was no statistically significant difference in the probability of malignancy between the experimental group and the control group [MD=−0.01, 95%CI (−0.01, 0.00), P=0.09]. Conclusion TCM compound can significantly reduce the diameter of pulmonary nodules, decrease the number of nodules, alleviate clinical symptoms, and improve lung function, but future multicenter, large-sample, high-quality RCTs are still needed to further explore and verify this conclusion.
3.Investigation of neural developmental abnormalities in Nde1 knockout zebrafish using HuC:RFP labeling technology

Objective To investigate the effect of Nde1 gene knockout on early neural development of zebrafish by visualizing the development of neural networks in zebrafish.Methods Transgenic zebrafish Tg(Nde1-/-;HuC:RFP+/-)was constructed by crossing Nde1-/-with Tg(HuC:RFP+/-).The HuC promoter was employed to drive the expression of red fluorescent protein in neurons,which allowing the visualization of zebrafish neural networks and tracking of neural development.Furthermore,by using Image J and Prism to compare the average fluorescence intensity of neurons and the expression level of fluorescent reporter proteins between Nde1 deficiency zebrafish and wild type zebrafish,the effect of Nde1 gene knockout on zebrafish neural development was analyzed.Results From 48 hours post-fertilization(hpf)to 7 days dpf,the average fluorescence intensity of red fluorescence expressed by trigeminal sensory neurons and spinal cord neurons in Nde1 deficiency zebrafish was lower than that in wild type zebrafish.RT-qPCR results also showed that the mRNA expression level of red fluorescent reporter protein in Nde1 deficiency zebrafish was significantly lower than that in the wild type zebrafish.Conclusion Nde1 gene deletion may lead to abnormal development of trigeminal sensory neurons and spinal cordneurons by affecting neural progenitor cell differentiation and increasing apoptosis.
4.Feasibility of Radiomics Combined with Brisque for Automated Assessment of Hepatic Gadolinium Ethoxybenzyl Diethylenetriamine Pentaacetic Acid Dynamic Enhanced Arterial Phase Image Quality

Purpose To explore the feasibility and validity of radiomics combined with blind/referenceless image spatial quality evaluator(Brisque)for automated assessment of arterial-phase image quality in hepatic gadolinium ethoxybenzyl diethylenetriamine pentaacetic acid(Gd-EOB-DTPA)dynamic enhanced MRI examinations.Materials and Methods A total of 326 arterial phase images from January 2020 to March 2024 in the First Affiliated Hospital of Henan University of Chinese Medicine for liver Gd-EOB-DTPA dynamic enhancement MRI examination were retrospectively collected.A 4-level(1-4 points)Likert scale was used for subjective image quality scoring,and then binary classification was used to divide the images into clinically acceptable group(166 images with a score of 2-4)and clinically unacceptable group(160 images with a score of 1).All images were randomly divided into a training set(227 images)and a test set(99 images)in a 7∶3 ratio.Brisque images were created based on arterial-phase images,and subsequently Brisque image features,image histologic features of the original arterial-phase images and combined features of the two were extracted.The extracted data were reduced in dimension and the optimal features of radiomics were selected.Based on the Logistic regression algorithm,radiomics models,Brisque models and combined models of the two were constructed.The receiver operating characteristic curve was drawn for each subject,and the area under the curve(AUC),threshold,sensitivity,specificity,accuracy,negative predictive value and positive predictive value were calculated to analyze the diagnostic performance of each model,and the net benefit of the models was evaluated by decision curve analysis.Results The AUC values of the radiomics model,Brisque model and combined model in the training set were 0.942(95%CI 0.914-0.969),0.722(95%CI 0.655-0.788)and 0.967(95%CI 0.947-0.987),respectively.In the test set,the AUC values were 0.904(95%CI 0.848-0.960),0.684(95%CI 0.579-0.789)and 0.920(95%CI 0.866-0.972),respectively.There was statistically significant difference between two of the three models in the training set(Z=6.493,3.397,7.427;all P<0.05);in the test set,there was statistically significant difference between the combined model and the Brisque model,and between the radiomics model and the Brisque model(Z=4.137,3.740,both P<0.05),but no statistically significant difference between the combined model and the radiomics model(P>0.05).The diagnostic performance was higher in the combined model in both the training set and the test set.The decision curve analysis curve suggested that the net benefit of the combined model was greater than that of the other two models.Conclusion Radiomics combined with Brisque can be applied in the assessment of the quality of arterial-phase images in Gd-EOB-DTPA dynamic-enhanced MRI examinations of the liver to automatically detect clinically unacceptable arterial-phase images of the liver with high diagnostic efficacy,which contributes to the improvement of the quality of the images at the time of acquisition,leading to a reduction in the number of duplicate scans and an increase in the accuracy of the diagnosis.
5.Benefit prediction and management of large medical equipment of daytime surgery center under DIP payment mode

Objective:To explore the effect of benefit prediction and management method of large medical equipment of daytime surgery center under the diagnosis-intervention packet(DIP)payment mode on improving the efficiency of resource utilization and the medical quality of hospital.Methods:According to the DIP payment mode,the benefit prediction and management of large medical equipment were carried out at different stages included pre-hospital,in-hospital and post-hospital.From April 2022 to April 2024,a total of 240 patients who received treatment in the daytime surgery center of Deyang People's Hospital were selected.According to the application of benefit prediction and management of large medical equipment of daytime surgery center under the DIP payment mode,120 patients were divided into control group and observation group,with 120 cases in each group.The medical quality,the efficiency of equipment utilization and the financial benefits of the patients of two groups,who received daytime surgery,under DIP payment mode were compared.A self-made questionnaire was adopted to investigate the overall satisfaction of the patients of two groups,who received daytime surgery,for daytime surgery by using the method of telephone communication.Results:The incidence of postoperative complications,and the rate of readmitted to hospital in the observation group were respectively 5.00%and 4.17%,which were lower than those(13.33 and 11.67%)in the control group,the success rate of surgery in the observation group was 95%,which was higher than that in the control group(86.67%),and the differences were statistically significant(x2=5.004,4.630,0.641,P<0.05).The average inpatient cost,total cost of the operation of medical equipment and DIP settlement fee of the patients of undergoing daytime surgery in the observation group were all lower than those in the control group,and differences were statistically significant(t=2.168,8.512,14.474,P<0.05).The frequency of using equipment and the average scheduling efficiency of the patients of undergoing day surgery in the observation group were higher than those in the control group,while the average failure rate of equipment and the frequency of maintenance for equipment were lower than those in the control group,and the differences were statistically significant(t=23.211,21.585,5.643,23.944,P<0.05).The average satisfaction of the patients of undergoing daytime surgery in the observation group was higher than that in the control group,and the difference was statistically significant(t=8.46,P<0.05).Conclusion:The benefit prediction and management method of large medical equipment in daytime surgery center under the DIP payment mode can improve the operation efficiency and medical quality of the daytime surgery center,and reduce the operation cost of hospital,and promote the optimization allocation of medical resources of hospital.
6.A Case of Sarcoidosis Characterized by Hypercalcemic Crisis and Renal Failure:Case Report and Literature Review

Sarcoidosis was a rare granulomatous disease that mainly affects the lungs and superficial lymph nodes,followed by the skin and eyes.This article reported a case of sarcoidosis patient with hypercalcemic crisis and renal failure as the main manifestations,its clinical manifestations were unique,and the diagnostic process was challenging and complex.The final diagnosis was confirmed through bronchoscopy lung tissue biopsy,and after treatment with glucocorticoids,both blood calcium and renal function improved significantly compared to before.At present,there was no relevant literature reports on cases of sarcoidosis characterized by hypercalcitonin crisis and renal failure in clinical practice.This article reported a case of sarcoidosis characterized by hypercalcitonin crisis and renal failure,and conducted literature review to provide reference for clinical doctors to identify and diagnose sarcoidosis,and actively take corresponding treatment measures to avoid further damage to the patient's target organs caused by the disease.
7.Research progress on mechanism of intestinal microbiota in tumor immunotherapy resistance

Previous studies have shown that the diversity and composition of intestinal microbiota are related to the effect of tumor immunotherapy,but the mechanism of intestinal microbiota affecting tumor immunotherapy resistance has rarely been sum-marized.This article not only expounds the current clinical sta-tus of tumor immunotherapy resistance,but also summarizes the correlation and regulatory mechanism between the composition and homeostasis of intestinal microbiota and drug resistance to different types of tumor immunotherapy,so as to provide a refer-ence for the study of potential targets for improving tumor immu-notherapy resistance based on intestinal microbiota.
8.Analysis of risk factors for high-risk colorectal adenoma:focusing on non-alcoholic fatty liver disease and multiple metabolic abnormalities

Objective To retrospectively analyze the association between metabolic factors and high-risk colorectal adenoma(CRA).Methods The medical records of patients aged 18-75 years who underwent their initial colonoscopy at Karamay Central Hospital of Xinjiang Uygur Autonomous Region from Jul 2000 to Mar 2017 were collected.The comparison between normal colonoscopy(NC)and high-risk CRA patients was conducted using an unpaired t-test,while chi-square test was used for categorical variables.Least absolute shrinkage and selection operator(LASSO)regression and Logistic regression were utilized to analyze the association between metabolic factors and high-risk CRA.Results A total of 1 798 patients meeting the inclusion and exclusion criteria were enrolled and divided into normal colonoscopy(NC)findings group(n=972)and high-risk CRA group(n=826).The high-risk CRA group exhibited significantly lower levels of high-density lipoprotein cholesterol(HDL-C)in comparison to the NC group,while uric acid and fibrosis 4(FIB-4)index levels were significantly higher than those observed in the NC group(all P<0.05).Based on LASSO regression analysis,we identified 12 variables that potentially influence the occurrence of high-risk CRA,including age,gender,smoking history,alcohol consumption history,non-alcoholic fatty liver disease(NAFLD),hypertension,coronary artery disease,hyperglycemia,hypercholesterolemia,low levels of HDL-C,elevated alanine aminotransferase,and elevated gamma-glutamyl transferase.Multivariate analysis revealed that individuals aged over 50 years,male gender,cigarette and alcohol consumption,low HDL-C levels,history of NAFLD and hypertension were identified as independent risk factors associated with high-risk CRA(P<0.05).In addition,without or with adjusting for age,sex,smoking,and drinking history,patients with a high TG/HDL-C ratio(the ratio≥2.68)had a significantly higher risk of high-risk CRA than those with a low TG/HDL-C ratio(the ratio<2.68)[odds ratios(ORs)were1.430 and 1.235 respectively,all P<0.05)].Without or with adjusting variables,the ORs for NAFLD patients with FIB-4 index>2.67 were 1.849(P=0.466)and 1.435(P=0.707),respectively.Conclusion A significant association exists between metabolic factors and high-risk CRA.Independent risk factors for high-risk CRA include older age(≥50 years),male,smoking history,alcohol consumption history,low levels of HDL-C,and a history of NAFLD and hypertension.Individuals exhibiting a TG/HDL-C ratio exceeding 2.68 manifest a significantly heightened susceptibility to the development of high-risk CRA.Therefore,elderly males with one or more aforementioned metabolic abnormalities should be considered a priority population for colorectal screening.
