2.Analysis of the incidence trend of inflammatory bowel disease in the elderly in China from 2002 to 2021 based on the Bayesian age-period-cohort model

Objective:To describe the incidence trends of inflammatory bowel disease (IBD) among the elderly in China from 2002 to 2021, estimate age-period-cohort (APC) effects, and predict future trends.Methods:Data on elderly IBD incidence and age-standardized incidence rates (ASIR) from 2002 to 2021 were extracted from the global burden of disease study 2021 database. Estimated annual percentage change (EAPC) evaluated ASIR trends. APC and Bayesian APC (BAPC) models were used to analyze effects and predict incidence from 2022 to 2031. A sensitivity analysis was conducted by applying alternative hyperprior values (a=1, b=0.0005) to assess model stability.Results:From 2002 to 2021, ASIR increased from 0.186 per 100, 000 to 0.272 per 100, 000 (EAPC = 2.234%, 95% CI: 1.715%-2.756%). Age effects showed a downward-then-upward trend in males and a decline in females, with peak incidence at 60-<64 years. Period effects increased steadily in males, while females showed an initial rise followed by a decline. Cohort effects rose and then declined with successive birth cohorts. Projections suggest ASIR will continue to rise through 2031. Sensitivity analysis showed ASIR trends closely matched the main model, indicating robustness. Conclusion:The burden of IBD among older adults in China is increasing. Strengthened prevention, health education, and lifestyle promotion for those aged 60-<70 are recommended.

6.Benefit prediction and management of large medical equipment of daytime surgery center under DIP payment mode

Objective:To explore the effect of benefit prediction and management method of large medical equipment of daytime surgery center under the diagnosis-intervention packet(DIP)payment mode on improving the efficiency of resource utilization and the medical quality of hospital.Methods:According to the DIP payment mode,the benefit prediction and management of large medical equipment were carried out at different stages included pre-hospital,in-hospital and post-hospital.From April 2022 to April 2024,a total of 240 patients who received treatment in the daytime surgery center of Deyang People's Hospital were selected.According to the application of benefit prediction and management of large medical equipment of daytime surgery center under the DIP payment mode,120 patients were divided into control group and observation group,with 120 cases in each group.The medical quality,the efficiency of equipment utilization and the financial benefits of the patients of two groups,who received daytime surgery,under DIP payment mode were compared.A self-made questionnaire was adopted to investigate the overall satisfaction of the patients of two groups,who received daytime surgery,for daytime surgery by using the method of telephone communication.Results:The incidence of postoperative complications,and the rate of readmitted to hospital in the observation group were respectively 5.00%and 4.17%,which were lower than those(13.33 and 11.67%)in the control group,the success rate of surgery in the observation group was 95%,which was higher than that in the control group(86.67%),and the differences were statistically significant(x2=5.004,4.630,0.641,P<0.05).The average inpatient cost,total cost of the operation of medical equipment and DIP settlement fee of the patients of undergoing daytime surgery in the observation group were all lower than those in the control group,and differences were statistically significant(t=2.168,8.512,14.474,P<0.05).The frequency of using equipment and the average scheduling efficiency of the patients of undergoing day surgery in the observation group were higher than those in the control group,while the average failure rate of equipment and the frequency of maintenance for equipment were lower than those in the control group,and the differences were statistically significant(t=23.211,21.585,5.643,23.944,P<0.05).The average satisfaction of the patients of undergoing daytime surgery in the observation group was higher than that in the control group,and the difference was statistically significant(t=8.46,P<0.05).Conclusion:The benefit prediction and management method of large medical equipment in daytime surgery center under the DIP payment mode can improve the operation efficiency and medical quality of the daytime surgery center,and reduce the operation cost of hospital,and promote the optimization allocation of medical resources of hospital.

Patients with severe trauma require an extremely timely treatment and transfusion plays an irreplaceable role in the emergency treatment of such patients. An increasing number of evidence-based medicinal evidences and clinical practices suggest that patients with severe traumatic bleeding benefit from early transfusion of low-titer group O whole blood or hemostatic resuscitation with red blood cells, plasma and platelet of a balanced ratio. However, the current domestic mode of blood supply cannot fully meet the requirements of timely and effective blood transfusion for emergency treatment of patients with severe trauma in clinical practice. In order to solve the key problems in blood supply and blood transfusion strategies for emergency treatment of severe trauma, Branch of Clinical Transfusion Medicine of Chinese Medical Association, Group for Trauma Emergency Care and Multiple Injuries of Trauma Branch of Chinese Medical Association, Young Scholar Group of Disaster Medicine Branch of Chinese Medical Association organized domestic experts of blood transfusion medicine and trauma treatment to jointly formulate Chinese expert consensus on blood support mode and blood transfusion strategies for emergency treatment of severe trauma patients ( version 2024). Based on the evidence-based medical evidence and Delphi method of expert consultation and voting, 10 recommendations were put forward from two aspects of blood support mode and transfusion strategies, aiming to provide a reference for transfusion resuscitation in the emergency treatment of severe trauma and further improve the success rate of treatment of patients with severe trauma.

Objective:To investigate the effects of matrix metalloproteinase 10(MMP10)on the proliferation,migration,invasion,and lymph node metastasis of oral squamous cell carcinoma(OSCC)cells,and to explore the underlying mechanisms.Methods:Gene expression profile data and survival data of head and neck squamous cell carcinoma(HNSCC)tissues and adjacent non-cancerous tissues were downloaded from The Cancer Genome Atlas(TCGA).Protein expression profile data of HNSCC tissues and adjacent non-cancerous tissues were downloaded from the Clinical Proteomic Tumor Analysis Consortium(CPTAC)database,and select the data of OSCC from HNSCC.The data from public database were used to analyze the differences in MMP10 expression at the mRNA and protein level between HNSCC/OSCC tissues and adjacent non-cancerous tissues,and to analyze the impact of MMP10 expression level on the survival rate of HNSCC patients,as well as the impact of human papillomavirus(HPV)on MMP10 gene expression in HNSCC patients.MMP10 expression level in 52 pairs of OSCC tissue samples and adjacent non-cancerous tissue samples(from the First Affiliated Hospital of Fujian Medical University)was measured by real-time quantitative PCR.After silencing MMP10 expression using siRNA(siMMP10-1291 and siMMP10-1336),the effect of MMP10 on the proliferation ability of OSCC cells was detected by CCK-8 assay and colony formation assay.The effects of MMP10 on the migration and invasion abilities of OSCC cells was assessed by wound-healing assay and transwell assay,respectively.To verify the effect of MMP10 knocking down on lymph node metastasis of OSCC cells in vivo,we constructed a nude mouse tongue cancer orthotopic model by shRNA(shMMP10-1291 and shMMP10-1336).Differential expression genes associated with MMP10 were screened from the gene expression profile data of OSCC patients in the TCGA database,and pathway enrichment analysis was performed using the GSEA software to further analyze the correlation between MMP10 and genes from the significantly enriched pathway.Results:Compared with adjacent non-cancerous tissues,the expression level of MMP10 in HNSCC and OSCC tissues was significantly increased(P<0.000 1),and HNSCC patients with high MMP10 expression had a lower survival rate(HR=1.32,P=0.04).HNSCC patients in the HPV-group had higher MMP10 gene expression levels(P<0.000 1),but their survival rate was significantly lower than that of HNSCC patients in the HPV+group(HR=0.48,P=0.004).Downregulating MMP10 expression could inhibit the migration,invasion and lymph node metastasis in vivo of OSCC cells.In OSCC patients,differentially expressed genes associated with MMP10 were significantly enriched in the Wnt signaling pathway(NES=1.505,P=0.029),and multiple genes in the Wnt signaling pathway,including WNT3,WNT3A,WNT5A,and WNT5B,were positively correlated with the expression level of MMP10.Conclusion:MMP10 promotes the invasion,metastasis and lymph node metastasis in vivo of OSCC cells by regulating the Wnt signaling pathway.
10.Moxifloxacin treatment for Mycoplasma hominis meningitis in an extremely preterm infant

The patient,a male newborn,was admitted to the hospital 2 hours after birth due to prematurity(gestational age 27+5 weeks)and respiratory distress occurring 2 hours postnatally.After admission,the infant developed fever and elevated C-reactive protein levels.On the fourth day after birth,metagenomic next-generation sequencing of cerebrospinal fluid indicated a positive result for Mycoplasma hominis(9 898 reads).On the eighth day,a retest of cerebrospinal fluid metagenomics confirmed Mycoplasma hominis(56 806 reads).The diagnosis of purulent meningitis caused by Mycoplasma hominis was established,and the antibiotic treatment was switched to moxifloxacin[5 mg/(kg·day)]administered intravenously for a total of 4 weeks.After treatment,the patient's cerebrospinal fluid tests returned to normal,and he was discharged as cured on the 76th day after birth.This article focuses on the diagnosis and treatment of neonatal Mycoplasma hominis purulent meningitis,introducing the multidisciplinary diagnosis and treatment of the condition in extremely preterm infants.[Chinese Journal of Contemporary Pediatrics,2024,26(4):432-436]
