1.Clinical comprehensive evaluation of 16 commonly used kinds of enteral nutrition preparations in Hebei province

OBJECTIVE To comprehensively evaluate the 16 commonly used kinds of enteral nutrition preparations in Hebei province, aiming to provide a reference for the selection of drugs in medical institutions and clinical drug decision-making. METHODS Based on the Quick Guide for Drug Evaluation and Selection in Chinese Medical Institutions (the Second Edition), evaluation evidence was collected, and the included drugs were scored and evaluated from four dimensions of pharmaceutical characteristics, clinical characteristics, economy and other attributes. RESULTS & CONCLUSIONS The scores for Enteral nutritional emulsion (TPF-T), Enteral nutritional emulsion (TPF-D), Enteral nutritional emulsion (TPF), Enteral nutritional emulsion (TPF-HE), Enteral nutritional emulsion (TP), Enteral nutritional emulsion (SP), Enteral nutritional suspension (TPF) (1.5 kcal/mL, 1 kcal=4.184 kJ), Enteral nutritional suspension (TPF) (1.0 kcal/mL), Intact protein enteral nutrition (powder), Enteral nutritional suspension (TPF-DM), Enteral nutritional suspension (TPF-MCT), Enteral nutritional suspension (SP), Short- peptide enteral nutrition, Enteral nutritional powder (TP), Enteral nutritional suspension (TPF-D) and Enteral nutritional suspension (TPF-FOS) were 82.9, 84.1, 84.1, 86.1, 78.4, 79.1, 82.6, 82.3, 82.4, 80.2, 83.0, 82.4, 82.1, 85.7, 76.0, 82.4 points, respectively. All medications scored above 70 points. In practice, appropriate drugs can be selected according to clinical requirements and patient needs.
2.Value of ovarian-adnexal reporting and data system MRI score combined with tumor markers in ovarian tumors

Objective To explore the value of ovarian-adnexal reporting and data system(O-RADS)MRI score combined with tumor markers(CA125+HE4)in ovarian tumors.Methods Data from 223 patients with ovarian tumors confirmed by pathology were analyzed retrospectively,including 260 lesions.The Kappa test was used to assess the consistency of O-RADS MRI score between low and high seniority physician groups.The receiver operating characteristic(ROC)curve was drawn to analyze the diagnostic effi-cacy of O-RADS MRI score combined with tumor markers(CA125+HE4)in ovarian tumors.Results The Kappa value of the O-RADS MRI score between low and high seniority physician groups was 0.803[95%confidence interval(CI)0.746-0.860].The sensitivity based on O-RADS MRI score for distinguishing benign and malignant ovarian tumors was 0.957 and 0.989,the specificity was 0.784 and 0.820,the accuracy was 0.846 and 0.881,the positive predictive value was 0.712 and 0.754,and the negative pre-dictive value was 0.970 and 0.993,and the area under the curve(AUC)was 0.871 and 0.905,respectively in the low and high senior-ity physician groups.Combined with tumor markers(CA125+HE4),the sensitivity and negative predictive value of both low and high seniority physician groups were 1.000.Conclusion The O-RADS MRI score has high diagnostic efficacy and good repeatability in ovarian tumors.Combined with tumor markers CA125 and HE4,the O-RADS MRI score can further improve the diagnostic sen-sitivity.
3.Mechanism of calcium-sensing receptor regulating macrophage polariza-tion in hypertensive rats

AIM:To explore the role and mechanism of calcium-sensing receptor(CaSR)in regulating macro-phage polarization in hypertensive rats.METHODS:Male spontaneously hypertensive rats(SHR)and Wistar-Kyoto(WKY)rats were categorized into WKY group,SHR group,SHR+R568(CaSR agonist)group,and SHR+NPS2143(CaSR inhibitor)group.The thoracic aorta was isolated,and the expression of CaSR and macrophage polarization markers in the aorta was observed through immunofluorescence staining.The primary peritoneal macrophages of SHR and WKY rats were aseptically extracted following anesthesia.After intervention with R568 and NPS2143,the expression levels of M1 and M2 markers of peritoneal macrophages were observed by Western blot and immunofluorescence staining.The levels of interleukin(IL)-1β and IL-10 were measured by ELISA.The concentration of Ca2+in peritoneal macrophages was mea-sured by immunofluorescence.Western blot was employed to identify the expression of CaSR and nucleotide-binding oligo-merization domain-like receptor protein 3(NLRP3)inflammasome components.Following anesthesia,vascular smooth muscle cells(VSMCs)were isolated from SHR using an adherent method.Subsequently,a co-culture system was estab-lished with macrophage supernatant.The optimal action time for this co-culture system was determined through CCK-8 as-say.RESULTS:Compared with SHR group,activation of CaSR resulted in a significant decrease in the protein expres-sion of M1 polarization markers(P<0.05)and a concomitant increase in the protein expression of M2 polarization markers in the aorta(P<0.05).Compared with SHR group,administration of R568 led to a significant decrease in the protein ex-pression of M1 polarization markers(P<0.05)and a concomitant increase in the protein expression of M2 polarization markers(P<0.05)in peritoneal macrophages.Additionally,there was a notable reduction in the protein levels of NLRP3 inflammasome components(P<0.05).Furthermore,the fluorescence intensity of intracellular Ca2+was significantly en-hanced following R568 treatment(P<0.05).After administration of MCC950,an NLRP3 inflammasome inhibitor,the re-sults were consistent with those observed following R568 treatment,demonstrating statistical significance(P<0.05).This effect was reversed by the combined intervention of U73122,a phospholipase C(PLC)inhibitor(P<0.05).Compared with the control(0 h),the 24-h peritoneal macrophage supernatant exhibited the strongest capacity to enhance the viabili-ty of VSMCs after 24 h of culture(P<0.05).CONCLUSION:In hypertensive rats,the CaSR inhibits NLRP3 inflamma-some activation via the PLC-Ca2+signaling pathway,thereby mediating an increase in macrophage polarization towards the M2 phenotype and a decrease towards the M1 phenotype.

Objective To explore the potential categories of dyadic psychological capital and related factors among stroke patients and their spouses,in order to inform the improvement of their mental health.Methods Stroke patients and their spouses were recruited in the Neurology Department of 3 tertiary general hospitals from March,2023 to August,2023 by convenience sampling method.The general information questionnaire,Modified Rankin Scale(mRs),Psychological Capital Questionnaire,The Kessler Psychological Distress Scale(K10)and Perceived Social Support Scale(PSSS)were used for investigation.Latent profile analysis was adopted to explore the categories of dyadic psychological capital,and multiple Logistic regression analysis was used to assess the effect of various factors on different categories.Results A total of 232 returned questionnaires(out of the 235)were valid,resulting an effective response rate of 98.7%.Stroke patients and their spouses were divided into 3 categories,including dyadic low psychological capital-low self-efficacy group(6.0%),dyadic high psychological capital-low resilience group(15.9%)and dyadic medium psychological capital group(78.1%).The stroke patients'education level,mRs scores,occupational status after the illness,level of perceived social support,and spouses'working condition were the influencing factors on potential profiles of dyadic psychological capital(P<0.05).Conclusion There are heterogeneity and interdependence in the level of dyadic psychological capital of stroke patients and their spouses.Healthcare professionals should promote dyadic mental health by developing targeted interventions for stroke patients and their spouses with different psychological capital characteristics from a dyadic holistic perspective.

9.The experiences of family caregivers in dealing with care-resistant behaviors in elderly individuals with dementia:a qualitative study

Objective To explore the experiences of family caregivers in dealing with the care-resistance behaviors in elderly patients with dementia,to improve the quality of care for elderly patients with dementia,and to enhance the ability of family caregivers to cope with resistance behaviors.Methods Utilizing purposeful sampling,we conducted in-depth interviews with 16 family caregivers of elderly patients with dementia between February 2024 and May 2024.The participants were recruited from a tertiary-grade hospital and the surrounding community in Zhengzhou,Henan Province.The interview data were subjected to thematic analysis using the Colaizzi 7-step method.Results A total of 19 interviews were conducted in this study,with each caregiver being interviewed for 30-44 minutes.The total transcription was about 82,000 words,and 4 themes and 11 sub-themes were identified:the considerable physical and psychological burden(persistent negative emotions,gradually increasing physical burden),realization of self-worth(sense of filial responsibility,care benefit feeling),subjective adjustment of coping strategies(actively seeking external support,resorting to restrictions and protection,passively coping out of helplessness),and facing multiple coping challenges(difficulty in establishing effective communication,poor sleep quality,lack of caregiving skills,unmet diverse support needs).Conclusion Family caregivers experience a heavier burden of care and develop various negative emotions when dealing with resistance behaviors.Different caregivers gradually adapt and develop different coping strategies in the care process.However,there are still some coping dilemmas that need to be resolved.Healthcare professionals should pay timely attention to caregivers'experiences in dealing with resistance behaviors,correct their attitudes towards care-resistance behaviors,strengthen health education on managing care-resistance behaviors,and improve their ability to cope with care-resistance behaviors.
