1.Value of ovarian-adnexal reporting and data system MRI score combined with tumor markers in ovarian tumors

Objective To explore the value of ovarian-adnexal reporting and data system(O-RADS)MRI score combined with tumor markers(CA125+HE4)in ovarian tumors.Methods Data from 223 patients with ovarian tumors confirmed by pathology were analyzed retrospectively,including 260 lesions.The Kappa test was used to assess the consistency of O-RADS MRI score between low and high seniority physician groups.The receiver operating characteristic(ROC)curve was drawn to analyze the diagnostic effi-cacy of O-RADS MRI score combined with tumor markers(CA125+HE4)in ovarian tumors.Results The Kappa value of the O-RADS MRI score between low and high seniority physician groups was 0.803[95%confidence interval(CI)0.746-0.860].The sensitivity based on O-RADS MRI score for distinguishing benign and malignant ovarian tumors was 0.957 and 0.989,the specificity was 0.784 and 0.820,the accuracy was 0.846 and 0.881,the positive predictive value was 0.712 and 0.754,and the negative pre-dictive value was 0.970 and 0.993,and the area under the curve(AUC)was 0.871 and 0.905,respectively in the low and high senior-ity physician groups.Combined with tumor markers(CA125+HE4),the sensitivity and negative predictive value of both low and high seniority physician groups were 1.000.Conclusion The O-RADS MRI score has high diagnostic efficacy and good repeatability in ovarian tumors.Combined with tumor markers CA125 and HE4,the O-RADS MRI score can further improve the diagnostic sen-sitivity.

AIM:To explore the role and mechanism of calcium-sensing receptor(CaSR)in regulating macro-phage polarization in hypertensive rats.METHODS:Male spontaneously hypertensive rats(SHR)and Wistar-Kyoto(WKY)rats were categorized into WKY group,SHR group,SHR+R568(CaSR agonist)group,and SHR+NPS2143(CaSR inhibitor)group.The thoracic aorta was isolated,and the expression of CaSR and macrophage polarization markers in the aorta was observed through immunofluorescence staining.The primary peritoneal macrophages of SHR and WKY rats were aseptically extracted following anesthesia.After intervention with R568 and NPS2143,the expression levels of M1 and M2 markers of peritoneal macrophages were observed by Western blot and immunofluorescence staining.The levels of interleukin(IL)-1β and IL-10 were measured by ELISA.The concentration of Ca2+in peritoneal macrophages was mea-sured by immunofluorescence.Western blot was employed to identify the expression of CaSR and nucleotide-binding oligo-merization domain-like receptor protein 3(NLRP3)inflammasome components.Following anesthesia,vascular smooth muscle cells(VSMCs)were isolated from SHR using an adherent method.Subsequently,a co-culture system was estab-lished with macrophage supernatant.The optimal action time for this co-culture system was determined through CCK-8 as-say.RESULTS:Compared with SHR group,activation of CaSR resulted in a significant decrease in the protein expres-sion of M1 polarization markers(P<0.05)and a concomitant increase in the protein expression of M2 polarization markers in the aorta(P<0.05).Compared with SHR group,administration of R568 led to a significant decrease in the protein ex-pression of M1 polarization markers(P<0.05)and a concomitant increase in the protein expression of M2 polarization markers(P<0.05)in peritoneal macrophages.Additionally,there was a notable reduction in the protein levels of NLRP3 inflammasome components(P<0.05).Furthermore,the fluorescence intensity of intracellular Ca2+was significantly en-hanced following R568 treatment(P<0.05).After administration of MCC950,an NLRP3 inflammasome inhibitor,the re-sults were consistent with those observed following R568 treatment,demonstrating statistical significance(P<0.05).This effect was reversed by the combined intervention of U73122,a phospholipase C(PLC)inhibitor(P<0.05).Compared with the control(0 h),the 24-h peritoneal macrophage supernatant exhibited the strongest capacity to enhance the viabili-ty of VSMCs after 24 h of culture(P<0.05).CONCLUSION:In hypertensive rats,the CaSR inhibits NLRP3 inflamma-some activation via the PLC-Ca2+signaling pathway,thereby mediating an increase in macrophage polarization towards the M2 phenotype and a decrease towards the M1 phenotype.

5.Value of transvaginal ultrasound combined with 3.0T magnetic resonance imaging for the diagnosis of ectopic pregnancy

Objective To compare the value of transvaginal ultrasound, 3.0T magnetic resonance imaging (MRI) scanning alone and in combination for diagnosis of ectopic pregnancy, so as to provide insights into early screening of ectopic pregnancy. Methods This study enrolled a total of 130 patients with suspected ectopic pregnancy admitted to Dachuan People’s Hospital in Dazhou City, Sichuan Province, China between February 2019 and December 2022. All patients underwent transvaginal ultrasound examination and 3.0T MRI scanning. The consistency of transvaginal ultrasound and 3.0T MRI with clinical diagnostic results was evaluated with surgical pathology or clinical follow-up results as the golden standards. The sensitivity, specificity, and accuracy of transvaginal ultrasound and 3.0T MRI, alone and in combination, were compared for diagnosis of ectopic pregnancy. Results Of the 130 patients with suspected ectopic pregnancy, 108 cases were confirmed with ectopic pregnancy by surgical pathology, and 22 cases were confirmed without ectopic pregnancy by clinical follow-up. The sensitivity, specificity, and accuracy of transvaginal ultrasound were 85.19% (92/108), 54.55% (12/22), and 80.00% (104/130), respectively, with 0.358 consistency with clinical diagnostic results. The sensitivity, specificity, and accuracy of 3.0T MRI were 92.59% (100/108), 81.81% (18/22), and 90.77% (118/130), respectively, with 0.694 consistency with clinical diagnostic results. The sensitivity, specificity, and accuracy of transvaginal ultrasound combined with 3.0T MRI were 98.15% (106/108), 72.73% (16/22), and 93.85% (122/130), respectively, with 0.764 consistency with clinical diagnostic results. In addition, the sensitivity and accuracy of transvaginal ultrasound combined with 3.0T MRI were significantly higher than transvaginal ultrasound alone for diagnosis of ectopic pregnancy (χ2 = 11.88 and 10.96, both P < 0.01). Conclusion Transvaginal ultrasound combined with 3.0T MRI may provide more diagnostic information for ectopic pregnancy, and is highly consistent with the clinical diagnostic results. In addition, transvaginal ultrasound combined with 3.0T MRI improves the diagnostic sensitivity and accuracy for ectopic pregnancy than transvaginal ultrasound alone.
6.Correlation between post-traumatic growth and meaning in life among family caregivers of the disabled elderly in the community

Objective:To explore the status of post-traumatic growth and meaning in life among family caregivers of the disabled elderly in the community, and to analyze the correlation between the two.Methods:From August 2019 to August 2020, convenience sampling was used to select 560 family caregivers of the disabled elderly in the community of Guangzhou as the research object. The General Information Questionnaire, Posttraumatic Growth Inventory (PTGI) and the Chinese Meaning in Life Questionnaire (C-MLQ) were used for the investigation. Pearson correlation and hierarchical regression were used to analyze the correlation between post-traumatic growth and meaning in life in family caregivers of the disabled elderly. A total of 560 questionnaires were distributed, and 526 valid questionnaires were recovered after excluding incomplete questionnaires and answering questionnaires according to a certain rule.Results:The total scores of PTGI and C-MLQ of 526 family caregivers of the disabled elderly were (49.43±20.43) and (42.96±7.61) , respectively. The total score of PTGI and the scores of each dimension were positively correlated with the total score of C-MLQ and the two dimensions of meaning experience and meaning pursuit ( P<0.01) . The hierarchical regression showed that, after controlling for general data, both meaning experience and meaning pursuit could positively predict post-traumatic growth. Conclusions:Post-traumatic growth of family caregivers of disabled elderly is at low level. Family caregivers' meaning in life has a positive effect on post-traumatic growth. Community workers should strengthen the meaning in life education, help caregivers to effectively cope with traumatic events, improve the level of post-traumatic growth, and promote mental health.
7.The mediating effect of meaning in life between family care and subjective well-being of family caregivers of community disabled elderly

Objective:To explore the mediating effect of meaning in life between family care and subjective well-being of family caregivers of community disabled elderly, and provide reference for improving caregivers' mental health.Methods:From August 2019 to August 2020, A total of 526 family caregivers of disabled elderly from several communities in Guangzhou were household surveyed by general information questionnaire, family APGRA index, Chinese meaning in life questionnaire and general well-being schedule.SPSS 22.0 software was used for descriptive analysis, correlation analysis and mediating effect analysis.AMOS 22.0 software was used for structural equation model construction, and Bootstrap method was used for mediation effect test.Results:The score of subjective well-being, family care, meaning in life of family caregivers of disabled elderly were (69.41±19.17), (6.42±2.56), (42.96±7.61), respectively.The family care was positively correlated with meaning in life and subjective well-being ( r=0.275, 0.289, both P<0.01) and a positive correlation was found between meaning in life and subjective well-being ( r=0.345, P<0.01). Meaning in life had a partial mediating effect between family care and subjective well-being.The mediating effect value was 4.304, accounting for 30.6% of the total effect value. Conclusion:The family care can directly or indirectly affect caregivers' subjective well-being through meaning in life.

With the appearance of direct-acting antivirals (DAAs), breakthroughs have been achieved in the antiviral therapy for hepatitis C and hepatitis C patients can be cured completely. Due to the insidious onset of hepatitis C, most patients do not know their own conditions, and thus expanding diagnosis and treatment through screening is the key to the elimination of hepatitis C. However, hepatitis C virus is distributed widely and unevenly in the world, which results in difficulties in the screening and diagnosis of hepatitis C. This article introduces the current economic research on hepatitis C screening, analyzes the factors affecting the cost-effectiveness of hepatitis C screening, and shares the strategies and advances for hepatitis C elimination in other countries, so as to provide a reference for eliminating hepatitis C in China.
10.Qualitative research on real experience of clinical nurses using intelligent nursing technology

Objective:To get a deep understanding of true feelings of nurses using intelligent nursing technology in intelligent hospital and to provide basis for optimizing the use, management and promotion of intelligent nursing technology.Methods:By using the phenomenological approach of qualitative research, 15 clinical nurses in a Class Ⅲ Grade A hospital (Intelligent Hospital) in Chongqing from April to June 2019 were selected for face-to-face in-depth interviews. Colaizzi analysis was used to analyze the data.Results:The true experience of nurses in the intelligent hospital on intelligent nursing technology was summarized into four main themes, including the experience of intelligent nursing, lack of adequate equipment and technical support, some time needed for learning and mastering new technologies and full of earnest expectations for intelligent nursing technology.Conclusions:The managers of intelligent hospitals should pay attention to the real feeling of nurses using intelligent nursing technology, solve the problems encountered in use in time, do a good job in communication, feedback and training and optimize the evaluation index of nursing quality in time.
