1.Study on evaluation of LYM,IL-6 and D-dimer on severity of sepsis

Objective To investigate the evaluation value of lymphocyte count(LYM),interleukin(IL)-6 and D-dimer on the severity in the patients with sepsis.Methods The clinical data in 107 patients with sepsis admitted and treated in the Emergency ICU(EICU)of this hospital from January 2023 to November 2023 were retrospectively analyzed.The basic clinical data,laboratory routine inflammatory indicators,im-mune indicators,coagulation indicators and the related disease condition score within 24 h after entering EICU were collected.The patients were divided into the sepsis group(n=54)and septic shock group(n=53)ac-cording to whether or not septic shock occurred.The multivariate logistic regression model was used to ana-lyze the influencing factors of septic shock occurrence,the receiver operating characteristic(ROC)curve was used to analyze the diagnostic efficiency,and Spearman correlation method was used to analyze the correlation between LYM,IL-6 and D-dimer with septic shock occurrence in the patients with sepsis.Results The se-quential organ failure assessment(SOFA)score,acute physiology and chronic health evaluation Ⅱ(APACHEⅡ)score and the levels of LYM,CRP,IL-6 and D-dimer had statistically significantly differences between the two groups(P<0.05).The multivariate logistic regression analysis results showed that LYM,IL-6 and D-di-mer were the influencing factors of septic shock occurrence(P<0.05).The ROC curve analysis results showed that LYM,IL-6 and D-dimer alone or combination detection all had certain predictive value for septic shock occurrence,but the area under the curve(AUC),sensitivity and specificity of the three combined detec-tion were higher than those of detection alone.The Spearman correlation analysis results showed that IL-6(r=0.407)and D-dimer(r=0.482)levels were positively correlated with the severity of septic shock,while the LYM level(r=-0.513)was negatively correlated with the severity of septic shock(P<0.01).Conclusion The combination application of LYM,IL-6 and D-dimer could effectively evaluate the severity of the patients with sepsis.
2.Prevalence of menopausal syndrome among postmenopausal women in Pan'an County

Objective:
To investigate the prevalence and influencing factors of menopausal syndrome among postmenopausal women in Pan'an County, Zhejiang Province, so as to provide the basis for guiding the health management of postmenopausal women.
Methods:
From May 2023 to April 2024, the postmenopausal women aged 40 to 69 years in Pan'an County were selected using the random cluster sampling method. Demographic information, lifestyle and prevalence of gynecological diseases were collected through questionnaire surveys. The prevalence of menopausal syndrome was assessed by modified Kupperman Score Scale. Factors affecting menopausal syndrome were analyzed by a multivariable logistic regression model.
Results:
A total of 816 postmenopausal women were surveyed, with an mean age of (57.63±2.92) years and a mean natural menopause age of (49.85±2.13) years. There were 574 cases with menopausal syndrome, with a prevalence of 70.34%. Flashes and sweating, insomnia and irritability were common symptoms, accounting for 62.87%, 47.43% and 41.18%, respectively. Multivariable logistic regression analysis showed that monthly personal income of ≤5 000 yuan (<3 000 yuan, OR=3.124, 95%CI: 1.829-5.335; 3 000-5 000 yuan, OR=2.399, 95%CI: 1.370-4.201) and having gynecological diseases (OR=1.970, 95%CI: 1.292-3.004) were associated with a higher risk of menopausal syndrome, while average (OR=0.141, 95%CI: 0.072-0.276) or sufficient sleep quality (OR=0.095, 95%CI: 0.049-0.185) were associated with a lower risk of menopausal syndrome.
Conclusion
The prevalence of menopausal syndrome among postmenopausal women in Pan'an County is relatively high, and is mainly influenced by personal economic status, sleep quality and the presence of gynecological diseases.
3.Development strategy of teaching in sanitary microbiology guided by the One Health concept

Outbreaks of zoonotic emerging infectious diseases have posed serious threats to human health and social development. The experience of epidemic prevention and control highlights the importance of the One Health concept in the prevention and control of emerging infectious diseases. Sanitary microbiology studies the ecological relationship between environmental microorganisms and humans, detection techniques, and preventive strategies. Its research scope overlaps and is closely related to that of emerging infectious disease prevention and control guided by the One Health concept. Vigorously promoting the research and teaching reform of sanitary microbiology in universities can provide key technologies and high-tech talents for One Health practices, promote development of new strategies for the prevention and control of emerging infectious diseases, and enhance the ability to respond to biosafety crises.
4.Expert consensus on humanistic care for patients in hospice care

Objective The purpose of writing the"expert consensus on humanistic care for patients in hospice care"(hereinafter referred to as the"consensus")aims to standardize the practice of humanistic care in the field of hospice care,ensuring that humanistic care is integrated throughout the entire service process for hospice care patients and their families.Methods A systematic search was conducted in domestic and foreign databases for literature related to hospice care and humanistic care,including guidelines,expert consensuses,systematic reviews or Meta-analyses,and evidence summaries.High-quality evidence was evaluated,extracted,and summarized to form the initial draft of the"consensus".From June to October 2024,20 experts from the fields of hospice care,nursing humanities,and evidence-based nursing were invited to participate in 1 round of expert consultation.Among them,13 experts were selected for 2 rounds of expert demonstration meetings.After collating and analyzing the experts' opinions,the initial draft was revised and refined,ultimately resulting in the final version of the"consensus".Results The effective response rate of the consultation questionnaire was 100%,with expert authority coefficient of 0.880,judgment coefficient of 0.935,and familiarity level of 0.825.The Kendall harmony coefficient of the expert consultation was 0.134(P<0.05).The"consensus"consisted of 13 aspects,including the targets and objectives,principles,institutional guarantees,environmental requirements,etc.Conclusion This"consensus"possesses strong scientific rigor and practicality,which can provide guidance and references for the practice of humanistic care in the field of hospice care,promoting the standardization and humanization of hospice care services.
5.Construction of nomogram model for predicting adverse pregnancy outcomes of placenta accreta spectrum disorders by multimodal MRI

Objective To explore the predictive value of the nomogram model based on multimodal MRI signs for adverse pregnancy outcomes in placenta accreta spectrum disorders(PAS).Methods The clinical and MRI data of 60 patients with PAS diagnosed by surgery and/or pathology were collected.Multivariate logistic regression was used to analyze the independent risk factors of adverse pregnancy outcomes in PAS.According to the results of multivariate logistic regression analysis,the nomogram prediction model of adverse pregnancy outcomes in PAS was constructed.Results Placenta/uterine protrusion[odds ratio(OR)=6.717,P=0.015],abnormal blood vessels in the placenta(OR=7.929,P=0.009),and diffusion weighted imaging(DWI)placental spike/nodular protrusion into the muscular layer(OR=12.134,P=0.003)were independent risk factors for adverse pregnancy outcomes in PAS.Based on the results,a nomogram prediction model was constructed.The area under the curve(AUC)of the model for predicting adverse pregnancy outcomes of PAS was 0.907,with a sensitivity of 0.906 and a specificity of 0.821.Conclusion The nomogram model constructed based on multimodal MRI signs has certain value in predicting adverse pregnancy outcomes of PAS.
6.Differential expression and prognostic significance of exosomal miRNA derived from bone marrow stromal cells in the bone marrow supernatants of patients with AML

Objective:
To investigate the aberrant alterations of microRNAs ( miRNAs) in exosomes derived from bone marrow stromal cells ( BMSCs) in the bone marrow supernatants of patients with acute myeloid leukemia (AML) and their impact on the prognosis of AML patients .
Methods:
Bone marrow supernatant samples were col- lected from three AML patients and three healthy donors . Exosomes were isolated using a commercial kit , identif- ying the morphology and marker expression , and subjected to miRNA sequencing to determine differentially ex- pressed miRNAs (DE-miRNAs) . The DE-miRNAs were then intersected with the exosomal miRNA expression pro- files of primary AML cells (GSE64029) to exclude AML cell - derived signals and to identify BMSC-derived DE - miRNAs . Subsequently , candidate miRNAs were identified through Cox regression and Lasso regression analyses based on data from The Cancer Genome Atlas (TCGA) . A prognostic risk model for AML was constructed , and pa- tients were stratified into high-risk and low-risk groups according to the median risk score . The prognostic value and clinical relevance of the model were further validated . Finally , the target genes of the candidate miRNAs were pre- dicted , followed by pathway enrichment analysis , construction of key regulatory networks , and correlation analysis between the expression levels of key miRNAs and their corresponding target genes .
Results:
Isolated exosomes ex- hibited a typical cup-shaped morphology with intact structures with particle size of 30 - 150 nm , and expressed exo- somal markers CD63 , ALIX , and TSG101 . miRNA sequencing identified 103 DE-miRNAs in AML patients com- pared with healthy donors; after intersection with the GSE64029 dataset , 83 BMSC-derived DE-miRNAs were re- tained . Among these , five candidate miRNAs ( miR-25-3p , miR-532-5p , miR-194-5p , miR-10a-5p , and miR- 20a-5p) were used to construct the prognostic model . Kaplan-Meier survival analysis demonstrated significantly lon- ger overall survival in the low-risk group compared with the high-risk group (P < 0. 05) . The areas under the ROC curve for the training/validation cohorts were 0. 80/0. 74 , 0. 80/0. 78 , and 0. 79/0. 64 at 1 , 2 , and 3 years , re- spectively . The prognostic model was significantly associated with risk stratification , patient age , and FAB classifi- cation (P < 0. 05) . KEGG pathway enrichment revealed that target genes of the candidate miRNAs were closely linked to cancer-related signaling pathways , including hepatocellular carcinoma , breast cancer , and non-small cell lung cancer. Correlation analysis indicated that the candidate miRNAs were significantly associated with key genes such as HIF1A , CREB1 , PIK3CA , IGF1R , PIK3R1 , TIAM1 , CRK , and PTEN (P < 0. 05) .
Conclusion
AML patients exhibit distinct miRNA expression profiles in BMSC-derived exosomes . A five-miRNA signature ( miR-25 - 3p , miR-532-5p , miR-194-5p , miR-10a-5p , and miR-20a-5p) demonstrates robust prognostic performance , sup- porting its potential clinical utility in risk stratification and outcome prediction for AML.
7.Genetic analysis of two fetuses with Mosaic variegated aneuploidy syndrome caused by compound heterozygous variants in BUB1B and its upstream regulatory elements and a literature Review.

OBJECTIVE:
To explore the genetic etiology of two fetuses with Mosaic variegated aneuploidy syndrome (MVA) in a pedigree.
METHODS:
A 30-year-old pregnant woman, who presented at the Center for Medical Genetics and Prenatal Diagnosis of Shandong Maternal and Child Health Care Hospital on November 16, 2023, was enrolled. Clinical data of the pedigree were collected, and peripheral blood samples from the parents and amniotic fluid samples from the two fetuses were obtained for genomic DNA extraction. Whole exome sequencing (WES) was performed on both fetuses, followed by Sanger sequencing for familial validation and pathogenicity analysis of candidate variants. Chromosomal karyotyping of the parents was conducted to quantify the proportion of premature chromatid separation (PCS). This study was approved by the Medical Ethics Committee of Shandong Maternal and Child Health Care Hospital (Ethics No. 2024-034).
RESULTS:
Both fetuses exhibited structural brain anomalies and developmental delays during the second trimester. Amniocyte karyotyping revealed low-level mosaic aneuploidy involving multiple chromosomes, while chromosomal microarray analysis (CMA) showed no abnormalities. Pregnancy termination was performed for fetus 1. WES identified compound heterozygous variants in BUB1B, i.e., c.2363_2364del (p.S788Cfs*29) and ss804270619: G>A, in both fetuses. Sanger sequencing confirmed paternal inheritance of c.2363_2364del and maternal inheritance of ss804270619:G>A. According to the American College of Medical Genetics and Genomics (ACMG) and Clinical Genome Resource (ClinGen) Standards and Guidelines for the Interpretation of Sequence Variants, the c.2363_2364del variant was classified as likely pathogenic (PVS1 + PM2_Supporting). Parental karyotyping demonstrated PCS traits, with a higher proportion of abnormal metaphases in the father.
CONCLUSION
The compound heterozygous variants c.2363_2364del (p.S788Cfs*29) and ss804270619: G>A in BUB1B may constitute the genetic etiology of the two MVA fetuses in this pedigree.

8.The value of deep learning models based on ultrafast dynamic contrast-enhanced MRI for diagnosing malignant breast lesions

Objective:To explore the value of deep learning models based on ultrafast dynamic contrast-enhanced MRI (UF-DCE MRI) in predicting malignant breast lesions.Methods:The study was a cross-sectional study. Clinical and imaging data of 347 patients with breast lesions who received treatment at Tianjin Medical University Cancer Institute and Hospital from March 2023 to January 2024 were analyzed retrospectively. A total of 347 lesions were observed in the 347 patients, including 75 benign and 272 malignant lesions. The random number method was used to divide into the training set with 243 cases and the validation set with 104 cases in a ratio of 7∶3. All patients underwent breast UF-DCE MRI and conventional dynamic-enhanced MRI (DCE-MRI). A 27-channel model (27-phase enhancement images of input UF-DCE MRI), a 3-channel model (3-phase enhancement images of input DCE-MRI), and a 1-channel model (1st-phase enhancement images of DCE-MRI) were built based on the pre-trained ResNet18 deep learning model on ImageNet. The efficacy of each model in predicting breast malignant lesions was analyzed using receiver operating characteristic curves and area under the curve (AUC). The differences of AUC were compared using DeLong test.Results:In the training and validation sets, the 27-channel model had the highest AUC for diagnosing malignant breast lesions, which were 0.848 (95% CI 0.818-0.877) and 0.784 (95% CI 0.752-0.817), respectively. DeLong test showed no statistically significant difference in the AUC values of the three models in the validation set for the diagnosis of malignant lesions of the breast in a two-by-two comparison ( P>0.05). UF-DCE MRI scans were 27 phases totaling 81 s with a temporal resolution of 3 s/phase; DCE-MRI scans were 3 phases totaling 270 s with a temporal resolution of 90 s/phase. Conclusions:The model combining UF-DCE MRI with deep learning demonstrates comparable efficacy to DCE-MRI deep learning model in diagnosing breast malignant lesions. However the UF-DCE MRI has the advantages of high temporal resolution and short scanning time, which makes this model valuable for precise diagnosis and treatment of breast cancer.
9.Upper Limb Function in Post-ischemic Stroke Hemiplegia Patients with Stage Brunnstrom Ⅰ Treated with Auricular Intradermal Acupuncture:A Single-Blinded,Randomized Controlled Clinical Trial

Background Previous studies have demonstrated that the vagus nerve stimulation improves upper limb function in patients with stroke sequelae(six month-two years).This study aimed to investigate whether the auricular intradermal acupuncture(AIA)acted on the auricular branch of the vagus nerve can facilitate the recovery of upper limb motor function and expedite the effects and characteristics of rehabilitation processes in flaccid phase after stroke in Phase Brunnstrom Ⅰ.Methods This study utilized a randomized controlled single-blinded clinical design.Patients with ischemic stroke in Phase Brunnstrom Ⅰ were randomly assigned to the auricular intradermal acupuncture(AIA)group(AIA,standard rehabilitation,conventional acupuncture)or the sham auricular intradermal acupuncture(sham AIA)group(the sham AIA,standard rehabilitation,conventional acupuncture)for 6 days of 1 week(6 times).Primary outcome measures:Brunnstrom(Ueda Assessment),Fugl-Meyer assessment of the upper extremity(FMA-UE)and its subtype score(FMA-UEFS,FMA-UEES),Wolf motor function test scale(WMFT).Secondary outcome measures:Upper limb active range of joint motion(AROM),manual muscle test(MMT)of upper limb muscles,Barthel index(BI).Results Of the 332 patients who were screened,70 patients met the criteria and were recruited.There were 35 patients in the AIA group,1 patient was fall off,34 patients were actually completed;35 patients in the sham AIA group had no falling off case.Primary outcome measures:The upper Ueda assessment of the AIA group were better than those of the sham AIA group after treatment on days 3 and 6(P<0.05,P<0.001,respectively).The FMA-UEFS score on days 6 of the AIA group were significantly better than those of the sham AIA group(P<0.05).FMA-UEES of the AIA group was more associated with FMA-UE than FMA-UEFS with FMA-UE after treatment on day 6(FMA-UEFS:r=0.728,P<0.001;FMA-UEES:r=0.744,P<0.001).After 3 days and 6 days of treatment,the AIA group of the WMFT were significantly improved in the sham AIA group(P<0.05,P<0.001,respectively).Secondary outcome measures:AROM:After 6 days of treatment,the AIA group of shoulder abduction were more improved than in the sham AIA group(P<0.05).MMT:The muscle strength of elbow flexion,and elbow extension muscle improved more in the AIA group than in the sham AIA group(P<0.05).Barthel index(BI):After 6 days of treatment,there was a significant improvement in the group compared with before treatment(P<0.05),but no difference compared with the group(P>0.05).Conclusions AIA combined with conventional rehabilitation and acupuncture,can effectively improve the upper limb motor function of patients with post-ischemic stroke flaccid hemiplegia and accelerates the rehabilitation process of upper limb motor function.Clinical Trial Registration The study protocol is registered withhttp://www.chictr.org.cn(accession number:ChiCTR2200058141).
