Purpose:
Cystic duct cancers (CDCs) have been classified as extrahepatic bile duct cancers or gallbladder cancers (GBCs); however, it is unclear whether their clinical behavior is similar to that of distal extrahepatic bile duct cancers (DBDCs) or GBCs.
Materials and Methods:
T category of the CDCs was classified using current T category scheme of the GBCs and DBDCs, and clinicopathological factors were compared among 38 CDCs, 345 GBCs, and 349 DBDCs. We modified Nakata’s classifications (type 1, confined within cystic duct [CD]; combined types 2-4, extension beyond CD) and compared them.
Results:
No significant overall survival (OS) difference was observed between the patients with CDC, GBC, and DBDC. The T category of GBC staging was more accurate at distinguishing OS in patients with CDC than the DBDC staging. Patients with T3 CDC and GBC showed a significant OS difference when using the T category for GBC staging, while those with T1-T2 CDC and GBC showed no significant difference. In contrast, the T category of DBDC staging did not show any significant OS difference between patients with T1-T2 CDC and DBDC or T3 CDC and DBDC. Patients with type 1 CDC had significantly better OS than those with combined types.
Conclusion
Unlike GBCs and DBDCs, CDCs exhibit distinct clinicopathological characteristics. The OS is better when the CDC confines within the CD, compared to when it extends beyond it. Therefore, we propose a new T category scheme (T1, confined to CD; T2, invaded beyond CD) for better classifying CDCs.

4.Factors-related to the severity of delirium among older adults in neurologic intensive care units: A retrospective study using electronic medical record data

The purpose of this study was to identify predisposing, precipitating, and environmental factors related to the severity of delirium among older adults in a neurologic intensive care unit (ICU) based on the clinical practice guideline. Methods: A retrospective study was conducted using electronic medical record data from 945 older adults admitted to the neurological ICU at a tertiary hospital in Seoul, South Korea from January 2020 to July 2023. Delirium and subsyndromal delirium were evaluated using the Intensive Care Delirium Screening Checklist. Statistical analyses were performed using one-way ANOVA, χ2 tests, and multinominal logistic regression using the SPSS/WIN 26.0 program. Results: Compared with the normal group, a multinominal analysis revealed factors significantly associated with both delirium and subsyndromal delirium, including physical function, the length of ICU stay, dexmedetomidine use, anticonvulsants use, and physical restraints. However, compared to the normal group, ICU stay emerged as the most strongly related factor to delirium, whereas physical function was identified as the most significant factor related to subsyndromal delirium. These findings suggest that factors associated with delirium vary by its severity. Conclusion: Understanding the factors associated with delirium and subsyndromal delirium is crucial for clinicians. Pre-classification of high-risk groups based on the delirium severity among critically ill older adults is essential for providing high-quality delirium prevention.
5.Surface roughness of flowable bulk fill composite resin by different polishing protocols

Purpose:
This study aimed to evaluate the surface characteristics of full body bulk fill flowable composite resins when polished using the same protocols applied to conventional flowable composite resins.
Materials and Methods:
One conventional composite resin (Filtek Supreme Flowable) and three full body bulk fill flowable composite resins (Metafil Bulk Fill, Charisma Bulk Flow ONE, G-aenial BULK Injectable) were tested. Specimens were polished using three methods: Soflex™ Extra-thin, One Gloss midi, and One Gloss midi combined with Jiffy brush. Surface roughness (Sa) was measured using a confocal laser scanning microscope, and surface morphology was examined under a scanning electron microscope. Statistical analysis was performed using two-way ANOVA and Scheffé’s test.
Results:
No interaction was found between the composite resin type and polishing method. Metafil Bulk Fill showed the highest surface roughness, while specimens polished with Soflex™ Extra-thin showed the smoothest surfaces (P < 0.05).
Conclusion
The surface roughness of full body bulk fill composite resins varies depending on the resin type and polishing method. Therefore, understanding each product’s characteristics and selecting an appropriate polishing technique is essential for achieving a clinically acceptable surface finish and long-term success of the restoration.

Accurate occupation classification is essential in various fields, including policy development and epidemiological studies. This study aims to develop an occupation classification model based on DistilKoBERT. This study used data from the 5th and 6th Korean Working Conditions Surveys conducted in 2017 and 2020, respectively. A total of 99,665 survey participants, who were nationally representative of Korean workers, were included. We used natural language responses regarding their job responsibilities and occupational codes based on the Korean Standard Classification of Occupations (7th version, 3-digit codes). The dataset was randomly split into training and test datasets in a ratio of 7:3. The occupation classification model based on DistilKoBERT was fine-tuned using the training dataset, and the model was evaluated using the test dataset. The accuracy, precision, recall, and F1 score were calculated as evaluation metrics. The final model, which classified 28,996 survey participants in the test dataset into 142 occupational codes, exhibited an accuracy of 84.44%. For the evaluation metrics, the precision, recall, and F1 score of the model, calculated by weighting based on the sample size, were 0.83, 0.84, and 0.83, respectively. The model demonstrated high precision in the classification of service and sales workers yet exhibited low precision in the classification of managers. In addition, it displayed high precision in classifying occupations prominently represented in the training dataset. This study developed an occupation classification system based on DistilKoBERT, which demonstrated reasonable performance. Despite further efforts to enhance the classification accuracy, this automated occupation classification model holds promise for advancing epidemiological studies in the fields of occupational safety and health.

Background:
s/Aims: Reported incidence of extrahepatic bile duct cancer is higher in Asians than in Western populations. Korea, in particular, is one of the countries with the highest incidence rates of extrahepatic bile duct cancer in the world. Although research and innovative therapeutic modalities for extrahepatic bile duct cancer are emerging, clinical guidelines are currently unavailable in Korea. The Korean Society of Hepato-Biliary-Pancreatic Surgery in collaboration with related societies (Korean Pancreatic and Biliary Surgery Society, Korean Society of Abdominal Radiology, Korean Society of Medical Oncology, Korean Society of Radiation Oncology, Korean Society of Pathologists, and Korean Society of Nuclear Medicine) decided to establish clinical guideline for extrahepatic bile duct cancer in June 2021.
Methods:
Contents of the guidelines were developed through subgroup meetings for each key question and a preliminary draft was finalized through a Clinical Guidelines Committee workshop.
Results:
In November 2021, the finalized draft was presented for public scrutiny during a formal hearing.
Conclusions
The extrahepatic guideline committee believed that this guideline could be helpful in the treatment of patients.

10.Association of decreased estimated glomerular filtration rate with lung cancer risk in the Korean population

OBJECTIVES:
Inconsistent results are available regarding the association between low estimated glomerular filtration rate (eGFR) and lung cancer risk. We aimed to explore the risk of lung cancer according to eGFR category in the Korean population.
METHODS:
We included 358,293 adults who underwent health checkups between 2009 and 2010, utilizing data from the National Health Insurance Service-National Sample Cohort. Participants were categorized into 3 groups based on their baseline eGFR, as determined using the Chronic Kidney Disease Epidemiology Collaboration equation: group 1 (eGFR ≥90 mL/min/1.73 m2), group 2 (eGFR ≥60 to <90 mL/min/1.73 m2), and group 3 (eGFR <60 mL/min/1.73 m2). Incidences of lung cancer were identified using the corresponding codes from the International Classification of Diseases, 10th revision. Multivariate Cox proportional hazard models were employed to calculate the adjusted hazard ratios (HRs) and 95% confidence intervals (CIs) for lung cancer incidence up to 2019.
RESULTS:
In multivariate analysis, group 2 exhibited a 26% higher risk of developing lung cancer than group 1 (HR, 1.26; 95% CI, 1.19 to 1.35). Furthermore, group 3 demonstrated a 72% elevated risk of lung cancer relative to group 1 (HR, 1.72; 95% CI, 1.58 to 1.89). Among participants with dipstick proteinuria of 2+ or greater, group 3 faced a significantly higher risk of lung cancer than group 1 (HR, 2.93; 95% CI, 1.37 to 6.24).
CONCLUSIONS
Low eGFR was significantly associated with increased lung cancer risk within the Korean population. A particularly robust association was observed in individuals with severe proteinuria, emphasizing the need for further investigation.
