Heart failure (HF) is a major cause of mortality and morbidity in South Korea, imposing substantial physical, emotional, and financial burdens on patients and society. Despite the high burden of symptom and complex care needs of HF patients, palliative care and hospice services remain underutilized in South Korea due to cultural, institutional, and knowledge-related barriers. This position statement from the Korean Society of Heart Failure emphasizes the need for integrating palliative and hospice care into HF management to improve quality of life and support holistic care for patients and their families. By clarifying the role of palliative care in HF and proposing practical referral criteria, this position statement aims to bridge the gap between HF and palliative care services in South Korea, ultimately improving patient-centered outcomes and aligning treatment with the goals and values of HF patients.
2.The effect of education in obese type 2 diabetes adolescents and young adults based on continuous glucose monitoring and smartwatch-derived lifelog data

Purpose:
The increasing prevalence of type 2 diabetes (T2D) among adolescents and young adults (AYAs) is a major public health concern worldwide. This pilot study evaluated the effectiveness of lifestyle education in managing T2D in obese AYAs using continuous glucose monitoring (CGM) and smartwatch-derived lifelog data.
Methods:
Seven obese AYAs and T2D aged 12–19 years were enrolled in this prospective interventional study. Patients continued to take their previously prescribed T2D medication. CGM data were collected for 10 days, followed by lifestyle education using CGM and smartwatch data. Outcomes, including anthropometrics, glycemic control, dietary intake, physical activity, and self-management skills, were reassessed after an additional 10 days.
Results:
The median time in range increased from 58.1% (53.2%–75%) to 72% (64%–88%) (p=0.043) and time above range (>250 mg/dL) decreased from 10% (2.9%–18.6%) to 3.0% (1.0%–11.0%) (p=0.028). Median peak total caloric intake decreased from 2,854 (2,465–3,040) kcal/day to 2,091 (1,751–2,283) kcal/day and walking calorie expenditure increased from 163.9 (116.7–321.3) kcal/day to 180.2 (165.3–492.4) kcal/day (p=0.018 for both). The Summary of Diabetes Self-Care Activities score improved from 0.29 (0.05–0.43) to 0.33 (0.32–0.68) (p=0.043).
Conclusion
This integrated approach combining CGM and smartwatch-based education exhibited short-term effects on glycemic control, dietary habits, physical activity, and self-management skills in obese AYAs and T2D. Further studies are needed to confirm the long-term effectiveness of this strategy in this challenging population.
3.Effects of a spousal participation health promotion program for middle-aged and old men who have undergone percutaneous coronary intervention based on Cox’s Interaction Model: A quasi-experimental study with nonequivalent control group pre- and post-test design

To examine the effects of a spousal participation health promotion program (health promotion program, HPP) based on Cox’s Interaction Model of supporting husbands’ health behavior compliance following a percutaneous coronary intervention (PCI). Methods: This study was a nonequivalent control group pretest-posttest design. A total of 52 participants (26 in each group) completed the study. The researchers conducted pretest and posttest at 12-week intervals on 26 control groups. Those in the 26 experimental groups attended in the entire 12-week program with pretest and posttest. Also, 10 control groups who wished to receive the intervention were given the opportunity to participate in the HPP for 3 weeks and were provided with the materials used from weeks 4 through 12 after completion of the study. Descriptive statistics, independent t-test, paired t-test, Mann-Whitney U-test, and a Wilcoxon signed rank test were conducted. Results: Spousal participation HPP was effective in promoting participants’ marital compatibility and health behavior compliance. Conclusion: Spousal participation HPP was an effective intervention for reducing body mass index, total cholesterol, triglyceride, and low-density lipoprotein, enhancing marital harmony, and promoting health behavior compliance among middle-aged and older men who underwent PCI. Therefore, it is recommended to actively utilize the spousal participation HPP as an effective intervention strategy for health management of this population.
4.Biocompatibility evaluation of titanium nitride coating for canine patellar groove implant

Medial patellar luxation is a common orthopedic disorder in dogs, and advanced cases with severe skeletal deformities or femoropatellar osteoarthritis are often unresponsive to conventional techniques. Patellar groove replacement (PGR) has been proposed as an alternative surgical option; however, systematic comparisons of coating technologies for veterinary PGR implants remain limited. This study aimed to evaluate the physicochemical properties, biological compatibility, and functional performance of a newly developed titanium nitride (TiN)–coated PGR system compared with a clinically available amorphous diamond-like carbon (ADLC)–coated device. TiN-coated prototypes were fabricated using Ti-6Al-4V alloy by injection molding combined with arc ion plating, which requires simpler equipment and lower production costs than the vacuum plasma deposition used for ADLC. Physicochemical evaluations, including corrosion resistance, hardness, surface roughness, and coating thickness, were conducted following International Organization for Standardization (ISO) and Korean Industrial Standards (KS) guidelines. In vitro biocompatibility was assessed using MTT andcell adhesion assays with L-929 fibroblasts, while inflammatory cytokine profiling (interleukin [IL]-1β and IL-6) in a rat subcutaneous model was used to evaluate local tissue responses.Functional feasibility was examined in a canine femoral model bilaterally implanted with TiNor ADLC-coated PGR systems and monitored for one year through clinical, radiographic, computed tomography (CT), magnetic resonance imaging , and micro-CT assessments. Both coatings demonstrated excellent corrosion resistance and absence of cytotoxicity. TiN-coated implants showed slightly greater hardness and coating thickness, with comparable surfaceroughness and biocompatibility. All implants maintained stable fixation, proper patellar tracking, and satisfactory bone–implant integration. These findings indicate that TiN-coated PGRimplants achieve biological and mechanical performance equivalent to ADLC devices while offering advantages in manufacturing simplicity, scalability, and cost-efficiency, supporting their clinical applicability in veterinary orthopedics.
5.Current Status and Future Perspective of Seoul National University Hospital-Dementia Brain Bank with Concordance of Clinical and Neuropathological Diagnosis

This paper introduces the current status of Seoul National University Hospital Dementia Brain Bank (SNUH-DBB), focusing on the concordance rate between clinical diagnoses and postmortem neuropathological diagnoses. We detail SNUH-DBB operations, including protocols for specimen handling, induced pluripotent stem cells (iPSC) and cerebral organoids establishment from postmortem dural fibroblasts, and adult neural progenitor cell cultures. We assessed clinical-neuropathological diagnostic concordance rate. Between 2015 and September 2024, 162 brain specimens were collected via brain donation and autopsy. The median donor age was 73 years (1-94) with a male-to -female ratio of 2:1. The median postmortem interval was 9.5 hours (range: 2.5-65). Common neuropathological diagnoses included pure Lewy body disease (10.6%), Lewy body disease (LBD) with other brain diseases (10.6%), pure Alzheimer's disease-neuropathological change (ADNC) (6.0%), ADNC with other brain diseases (10.7%), vascular brain injury (15.2%), and primary age-related tauopathy (7.3%). APOE genotype distribution was following: ε3/ε3: 62.3%, ε2/ε3:9.6%, ε2/ε4: 3.4%, ε3/ε4: 24.0%, and ε4/ε4: 0.7%. Concordance rates between pathological and clinical diagnoses were: ADNC/AD at 42.4%; LBD at 59.0%; PSP at 100%; ALS at 85.7%; Huntington’s disease 100%. The varying concordance rates across different diseases emphasize the need for improved diagnostic criteria and biomarkers, particularly for AD and LBD. Tissues have been distributed to over 40 national studies. SNUH-DBB provides high-quality brain tissues and cell models for neuroscience research, operating under standardized procedures and international guidelines. It supports translational research in dementia and neurodegenerative diseases, potentially advancing diagnostic and therapeutic strategies.

Allergen immunotherapy (AIT) has been used for over a century and has been demonstrated to be effective in treating patients with various allergic diseases. AIT allergens can be administered through various routes, including subcutaneous, sublingual, intralymphatic, oral, or epicutaneous routes. Sublingual immunotherapy (SLIT) has recently gained clinical interest, and it is considered an alternative treatment for allergic rhinitis (AR) and asthma. This review provides an overview of the current evidence-based studies that address the use of SLIT for treating AR, including (1) mechanisms of action, (2) appropriate patient selection for SLIT, (3) the current available SLIT products in Korea, and (4) updated information on its efficacy and safety. Finally, this guideline aims to provide the clinician with practical considerations for SLIT.

Allergen immunotherapy (AIT) is a causative treatment of allergic diseases in which allergen extracts are regularly administered in a gradually escalated doses, leading to immune tolerance and consequent alleviation of allergic diseases. The need for uniform practice guidelines in AIT is continuously growing as the number of potential candidates for AIT increases and new therapeutic approaches are tried. This updated version of the Korean Academy of Asthma Allergy and Clinical Immunology recommendations for AIT, published in 2010, proposes an expert opinion by specialists in allergy, pediatrics, and otorhinolaryngology. This guideline deals with the basic knowledge of AIT, including mechanisms, clinical efficacy, allergen standardization, important allergens in Korea, and special consideration in pediatrics. The article also covers the methodological aspects of AIT, including patient selection, allergen selection, schedule and doses, follow-up care, efficacy measurements, and management of adverse reactions. Although this guideline suggests the optimal dosing schedule, an individualized approach and modifications are recommended considering the situation for each patient and clinic.
8.Immunoglobulin G4-Related Disease Affecting the Tonsil:Case Report and Literature Review

Immunoglobulin G4-related disease (IgG4-RD) can involve multiple sites throughout the body. The most commonly affected sites in the head and neck are the salivary and lacrimal glands, respectively. Very few cases of IgG4-RD manifesting in the tonsils have been reported. Herein, we report the imaging findings of a 49-year-old female with pathologically confirmed IgG4-RD after a right-side tonsillectomy. The lesion showed diffuse homogeneous enhancement and appeared isointense and slightly hyperintense on T1- and T2-weighted magnetic resonance imaging, respectively. Diffusion restriction was also observed, which was considered to be due to the large number of IgG4-rich plasmacytes, and could vary depending on the degree of fibrosis. As distinguishing IgG4-RD from other diseases based solely on imaging is challenging, it is important to consider differential diagnosis, and clinical considerations are essential.
9.The role of PD-1/PD-L1 pathway in ulcerative colitis and changes following tonsil-derived mesenchymal stem cells treatment

Background/Aims:
The programmed death 1 (PD-1)/programmed death-ligand 1 (PD-L1) pathway has not been fully evaluated in inflammatory bowel disease. We evaluated PD-1/PD-L1 levels in patients with ulcerative colitis (UC) and their significance in tonsil-derived mesenchymal stem cells (TMSCs) treatment.
Methods:
Using acute and chronic murine colitis model, we measured the PD-1 and PD-L1 levels in inflamed colonic tissues pre- and post-treatment with TMSCs. We also measured PD-1 and PD-L1 levels in colonic tissues from UC patients, compared to normal controls.
Results:
In the analysis using human colonic tissues, a significant increase in the levels of PD-1 and PD-L1 was observed in the colonic mucosa of patients with UC compared with normal controls (p < 0.001 and p = 0.005, respectively). When comparing the maximal disease extent, PD-L1 levels were highest in patients with proctitis (38.5 ± 46.7), followed by left-side colitis (17.5 ± 23.1) and extensive colitis (5.2 ± 8.2) (p < 0.001). In the chronic colitis model, the level of PD-L1 was decreased (p = 0.040) and the level of PD-1 increased more than in normal controls (p = 0.047). After treatment with TMSC, significant improvements were observed in body weight, disease activity index, and colon length recovery. Additionally, the levels of PD-1 and PD-L1 were recovered; PD-L1 significantly increased (p = 0.031), while the level of PD-1 decreased (p = 0.310).
Conclusions
The altered expression of PD-1 and PD-L1 in colonic mucosa may be a possible mechanism of UC, and T-MSC-derived PD-L1 could help suppress colitis.
