1.The Role and Effectiveness of Remote Mental Health Interventions in Disaster Response: A Narrative Review

Objective:
Disasters such as pandemics and natural catastrophes cause significant psychological distress, increasing the risk of depression, anxiety, and post-traumatic stress disorder (PTSD). This review aimed to evaluate the effectiveness of remote mental health interventions, such as telepsychiatry, online cognitive behavioral therapy (CBT), mobile applications, and artificial intelligence (AI)-based tools, during disasters.
Methods:
A comprehensive literature search was conducted using international databases (PubMed, EMBASE, PsycINFO, Web of Science, Scopus, CINAHL) and Korean domestic databases (KISS, RISS, DBpia, KMbase, KoreaMed). Studies published between January 2010 and February 2025 were screened following the PRISMA 2020 guidelines. Two board-certified psychiatrists independently reviewed titles, abstracts, and full texts. Peer-reviewed articles and relevant grey literature, including government manuals and expert guidelines, were included.
Results:
A total of 147 studies were synthesized, including randomized controlled trials, meta-analyses, observational studies, and policy documents. Evidence showed that remote interventions effectively reduced symptoms of depression, anxiety, and PTSD. Online CBT was beneficial for mild to moderate distress; mobile apps facilitated psychoeducation and symptom tracking; and telepsychiatry ensured care continuity, especially in underserved areas. AI tools showed promise in screening and triage but require better clinical integration. Effectiveness was highest when interventions were implemented early, integrated within stepped-care models, and guided by trained professionals.
Conclusion
Remote mental health interventions are practical and effective tools for bridging mental health care gaps during disasters. To ensure sustainability, these interventions should be integrated into national disaster mental health frameworks, accompanied by adequate funding, clinician training, legal safeguards, and cultural adaptation.
2.Harnessing Institutionally Developed Clinical Targeted Sequencing to Improve Patient Survival in Breast Cancer: A Seven-Year Experience

Purpose:
Considering the high disease burden and unique features of Asian patients with breast cancer (BC), it is essential to have a comprehensive view of genetic characteristics in this population. An institutional targeted sequencing platform was developed through the Korea Research-Driven Hospitals project and was incorporated into clinical practice. This study explores the use of targeted next-generation sequencing (NGS) and its outcomes in patients with advanced/metastatic BC in the real world.
Materials and Methods:
We reviewed the results of NGS tests administered to BC patients using a customized sequencing platform—FiRST Cancer Panel (FCP)—over 7 years. We systematically described clinical translation of FCP for precise diagnostics, personalized therapeutic strategies, and unraveling disease pathogenesis.
Results:
NGS tests were conducted on 548 samples from 522 patients with BC. Ninety-seven point six percentage of tested samples harbored at least one pathogenic alteration. The common alterations included mutations in TP53 (56.2%), PIK3CA (31.2%), GATA3 (13.8%), BRCA2 (10.2%), and amplifications of CCND1 (10.8%), FGF19 (10.0%), and ERBB2 (9.5%). NGS analysis of ERBB2 amplification correlated well with human epidermal growth factor receptor 2 immunohistochemistry and in situ hybridization. RNA panel analyses found potentially actionable and prognostic fusion genes. FCP effectively screened for potentially germline pathogenic/likely pathogenic mutation. Ten point three percent of BC patients received matched therapy guided by NGS, resulting in a significant overall survival advantage (p=0.022), especially for metastatic BCs.
Conclusion
Clinical NGS provided multifaceted benefits, deepening our understanding of the disease, improving diagnostic precision, and paving the way for targeted therapies. The concrete advantages of FCP highlight the importance of multi-gene testing for BC, especially for metastatic conditions.
3.Serum miR-329-3p as a potential biomarker for poor ovarian response in an in vitro fertilization

Several miRNAs have been identified as differentially expressed in patients with poor ovarian response (POR) compared to those with normal responses. This study aims to assess the potential of serum miR-329-3p as a biomarker for diagnosing POR.
Methods:
We conducted a Kyoto Encyclopedia of Genes and Genomes (KEGG) pathway analysis to confirm the target genes of miR-329-3p. KGN cells were transfected with both miR-329-3p mimic and inhibitor to assess the differential expression of these target genes. In accordance with the Bologna criteria, we enrolled 16 control patients and 16 patients with POR. We collected patient samples, including serum from day 2 and the human chorionic gonadotropin (hCG) day, as well as granulosa and cumulus cells, to validate the expression of miR-329-3p using quantitative real-time polymerase chain reaction.
Results:
KEGG pathway analysis revealed that miR-329-3p targeted adenylyl cyclase 9 (ADCY9) and protein kinase A subunit beta (PRKACB), both of which are involved in ovarian steroidogenesis. In KGN cells treated with a miR-329-3p mimic, ADCY9 and PRKACB expression levels were significantly reduced (p<0.05). Elevated levels of miR-329-3p suppressed aromatase expression and 17β-estradiol production by modulating ADCY9 and PRKACB in KGN cells. These effects were also observed in POR patients. Follicle-stimulating hormone receptor (FSHR) expression was diminished in the granulosa cells of POR patients. On day 2, on hCG day, and in granulosa cells, miR-329-3p exhibited high expression levels in the serum of POR patients.
Conclusion
miR-329-3p exhibited increased expression in granulosa cells and in the sera of POR patients. Consequently, we propose that miR-329-3p may be a potential biomarker for the diagnosis of POR.

8.Adenosine receptors activate cyclic adenosine monophosphate signaling in salivary gland cells

Sympathetic innervation stimulates β-adrenergic receptors, triggering cyclic adenosine monophosphate (cAMP) production and enhancing protein secretion in salivary gland cells. While cAMP signaling, in conjunction with Ca 2+ signaling, is essential for salivary gland function, the identified cAMP-producing G-protein-coupled receptors (GPCRs) remains limited. Here, we report the presence of cAMP-producing adenosine receptors in salivary gland cells. By reanalyzing publicly available single-cell transcriptome datasets of human and mouse submandibular glands, we identified mRNA expression of adenosine A1, A2A, A2B, and A3 receptors. Additionally, we confirmed that 5’-N-ethylcarboxamidoadenosine (NECA), an adenosine A2B receptor agonist, increases cAMP levels in human salivary gland cells, suggesting a physiological role for adenosine A2B receptors. Our findings enhance understanding of adenosine’s regulatory function in salivary glands and highlight new avenues for research on cAMPproducing adenosine receptors.
9.Suicide Prevention in Hospitals: A Survey of Patient Safety Management Specialists

Objectives:
This study examined the experiences and perceptions of healthcare safety management specialists regarding suicide and self-harm incidents in hospitals. The aim was to assess the current state of in-hospital suicide prevention systems, identify vulnerabilities according to the institution type, and suggest directions for improving the related policies and safety guidelines.
Methods:
A nationwide cross-sectional survey was conducted among 152 healthcare safety management specialists. Data were collected via an online questionnaire between May and June 2022. The survey items included experiences with suicide and self-harm incidents, types of incidents, components of prevention systems, staff training, and follow-up measures. Descriptive and inferential statistics were used for analysis, including chi-square tests for comparisons across hospital types.
Results:
The most frequently reported suicide methods were hanging (40.1%) and jumping (28.3%), while self-harm incidents often involved self-injury (45.4%). Only 55.3% of respondents indicated that institutional guidelines on suicide/self-harm were present and implemented. Multidisciplinary treatment planning and suicide prevention education for all staff were lacking, showing 32.2% and 27.6%, respectively. Significant differences were observed across hospital types regarding the existence and implementation of prevention guidelines (p<0.01).
Conclusion
Despite the increasing awareness, substantial variability remains in suicide prevention preparedness across hospital types. Strengthening institutional guidelines, environmental modifications, and multidisciplinary approaches, particularly in nursing hospitals, is critical. Policy support for standardizing mental health consultations and mandatory staff training is essential for enhancing suicide prevention in hospitals.
10.Factors Affecting Clozapine Adherence in Patients With Schizophrenia: A Qualitative Study

Objectives:
This study examined the psychosocial factors that influence the continuation of clozapine use among individuals with schizophrenia. This examination was conducted through indepth qualitative interviews.
Methods:
Thirty-one adults diagnosed with schizophrenia (Diagnostic and Statistical Manual of Mental Disorders, Fifth Edition) who had been taking Clozapine ® for at least 3 months were interviewed. Data were collected from May to October 2024 and analyzed using Braun and Clarke’s thematic analysis.
Results:
Two overarching themes emerged: 1) transformation of perceptions and engagement with treatment, including initial resistance stemming from severe symptoms and lack of insight, experiences of coercive interventions, rebuilding trust in the mental health system, and evolving attitudes toward psychiatric medications; and 2) personal recovery and social support, encompassing coping with side effects, recognizing symptom improvement and renewed hope, and the pivotal roles of family and therapeutic alliance. Trust in healthcare providers and emotional support from family were central to sustained adherence to clozapine.
Conclusion
Sustained clozapine use in schizophrenia patients is influenced by intertwined psychological, relational, and contextual factors. Acceptance of treatment was supported by symptom improvement, family support, and trust in healthcare providers. These elements contributed to medication adherence and broader functional recovery. A person-centered, integrated approach is essential to promote continued use and long-term outcomes.
