1.Efficacy and Safety of Ifosfamide and Mesna in Metastatic Castration-Resistant Prostate Cancer after Taxane-Based Chemotherapy and Novel Hormonal Therapy Failure

Purpose:
Limited treatment options exist for patients with metastatic castration-resistant prostate cancer (mCRPC) after the failure of taxane-based chemotherapy and novel hormonal therapy. Here, we report the safety and efficacy of ifosfamide and mesna in patients with mCRPC after the failure of taxane-based chemotherapy and novel hormonal therapy (NCT06236789).
Materials and Methods:
Patients with histologically confirmed prostate cancer who had failed taxane-based chemotherapy and novel hormonal therapy received ifosfamide 2,500 mg/m2 and mesna 1,500 mg/m2 on days 1–3, repeated every 21 days. Safety, objective response rate, disease control rate, reduction in serum prostate-specific antigen (PSA) concentration by >50% (PSA50) or >90% (PSA90), radiographic progression-free survival (rPFS), and overall survival (OS) were analyzed.
Results:
A total of 47 patients with mCRPC were included in the study. The median number of lines of treatment was 5 (range, 3 to 7). All patients were previously administered docetaxel and novel hormonal therapies including abiraterone (51.1%) and/or enzalutamide (61.7%). Thirty-eight patients (80.9%) were administered cabazitaxel. The objective response and disease control rates were 21.3% and 80.9%, respectively. PSA50 and PSA90 were achieved in 31.9% and 10.6%, respectively. During a median follow-up duration of 54.3 months, rPFS and OS were 5.0 and 9.0 months, respectively. All the patients experienced treatment-related adverse events of any grades; however, no new safety signs were detected. Genomic biomarker analysis revealed that alterations in the TP53 pathway were associated with inferior rPFS and OS.
Conclusion
Ifosfamide and mesna showed appreciable efficacy and manageable safety profiles in heavily treated patients with mCRPC.

Background:
Barriers to treatment with intravenous thrombolysis (IVT) for patients with acute ischemic stroke (AIS) in South Korea remain incompletely characterized. We analyze a nationwide prospective cohort to determine patient-level features associated with delayed presentation and non-treatment of potential IVT-eligible patients.
Methods:
We identified consecutive patients with AIS from 01/2011 to 08/2023 from a multicenter and prospective acute stroke registry in Korea. Patients were defined as IVT candidates if they presented within 4.5 hours from the last known well, had no lab evidence of coagulopathy, and had National Institute of Health Stroke Scale (NIHSS) ≥ 4. Multivariable generalized linear mixed regression models were used to investigate the associations between their characteristics and the IVT candidates or the use of IVT among the candidates.
Results:
Among 84,103 AIS patients, 41.0% were female, with a mean age of 69 ± 13 years and presentation NIHSS of 4 [interquartile range, 1–8]. Out of these patients, 13,757 (16.4%) were eligible for IVT, of whom 8,179 (59.5%) received IVT. Female sex (adjusted risk ratio [RR], 0.90; 95% confidence interval [CI], 0.86–0.94) and lower years of education (adjusted RR, 0.90; 95% CI, 0.84–0.97 for 0–3 years, compared to ≥ 13 years) were associated with a decreased likelihood of presenting as eligible for IVT after AIS; meanwhile, young age (adjusted RR, 1.12; 95% CI, 1.01–1.24 for ≤ 44 years, compared to 75–84 years) was associated with an increased likelihood of being an IVT candidate. Among those who were eligible for IVT, only age was significantly associated with the use of IVT (adjusted RR, 1.09; 95% CI, 1.03–1.16 for age 65–74 and adjusted RR, 0.83; 95% CI, 0.76–0.90 for ≥ 85 years, respectively).
Conclusion
Most patients with AIS present outside IVT eligibility in South Korea, and only 60% of eligible patients were ultimately treated. We identified increased age, female sex and lower education as key features on which to focus interventions for improving IVT utilization.

Excessive lipid peroxidation and renin–angiotensin system dysregulation are central mechanisms linking oxidative stress to hypertension. In this study, a bioactive tetrapeptide, CAAP (Cys-Ala-Ala-Pro), isolated from the muscle hydrolysate of Paralichthys olivaceus (olive flounder), was evaluated for its dual antioxidant and antihypertensive actions. In Vero cells, CAAP significantly suppressed 2,2′-azobis(2-amidinopropane)-induced lipid peroxidation, reduced malondialdehyde accumulation, and preserved cell viability. Fluorescent imaging and flow cytometric analyses revealed that CAAP mitigated nuclear fragmentation and apoptotic sub-G1 cell populations through modulation of the Bcl-xL/Bax/caspase-3 axis, indicating cell protection. Molecular docking analysis demonstrated that CAAP interacts with the S′₂ pocket of angiotensin-converting enzyme (ACE) via hydrogen bonding with Glu281, His513, Lys511, and Tyr520, supporting a strong inhibitory potential (IC50 = 39.3 µg/mL). In spontaneously hypertensive rats, oral administration of CAAP (40 mg/kg) elicited a sustained decrease in systolic blood pressure within 2–6 h post-treatment, comparable to that of captopril. Collectively, these findings highlight CAAP as a multifunctional marine peptide capable of attenuating oxidative and hypertensive injury through cell stabilization and ACE inhibition, suggesting its potential as a natural therapeutic or functional food ingredient for cardiovascular protection.

In addition to cellular damage, ischemia-reperfusion (IR) injury induces substantial damage to the mitochondria and endoplasmic reticulum. In this study, we sought to determine whether impaired mitochondrial function owing to IR could be restored by transplanting mitochondria into the heart under ex vivo IR states. Additionally, we aimed to provide preliminary results to inform therapeutic options for ischemic heart disease (IHD). Healthy mitochondria isolated from autologous gluteus maximus muscle were transplanted into the hearts of Sprague–Dawley rats damaged by IR using the Langendorff system, and the heart rate and oxygen consumption capacity of the mitochondria were measured to confirm whether heart function was restored. In addition, relative expression levels were measured to identify the genes related to IR injury. Mitochondrial oxygen consumption capacity was found to be lower in the IR group than in the group that underwent mitochondrial transplantation after IR injury (p < 0.05), and the control group showed a tendency toward increased oxygen consumption capacity compared with the IR group. Among the genes related to fatty acid metabolism, Cpt1b (p < 0.05) and Fads1 (p < 0.01) showed significant expression in the following order: IR group, IR + transplantation group, and control group. These results suggest that mitochondrial transplantation protects the heart from IR damage and may be feasible as a therapeutic option for IHD.

This study aimed to develop and evaluate machine learning models, specifically CatBoost and extreme gradient boosting (XGBoost), for diagnosing lower urinary tract symptoms (LUTS) in male patients. The objective is to differentiate between bladder outlet obstruction (BOO) and detrusor underactivity (DUA) using a comprehensive dataset that includes patient-reported outcomes, uroflowmetry measurements, and ultrasound-derived features.
Methods:
The dataset used in this study was collected from male patients aged 40 and older who presented with LUTS and sought treatment at the urology department of Samsung Medical Center. We developed and trained CatBoost and XGBoost models using this dataset. These models incorporated features like prostate size, voiding parameters, and responses from questionnaires. Their performance was assessed using standard metrics such as accuracy, precision, recall, F1-score, and area under the receiver operating characteristic curve (AUROC).
Results:
The results indicated that the CatBoost models displayed greater sensitivity, rendering them effective for initial screenings by accurately identifying true positive cases. Conversely, the XGBoost models showed higher specificity and precision, making them more suitable for confirming diagnoses and reducing false positives. In terms of overall performance for both BOO and DUA, XGBoost surpassed CatBoost, achieving an AUROC of 0.826 and 0.819, respectively.
Conclusions
Integrating these machine learning models into the diagnostic workflow for LUTS can significantly enhance clinical decision-making by offering noninvasive, cost-effective, and patient-friendly diagnostic alternatives. The combined application of CatBoost and XGBoost models has the potential to improve diagnostic accuracy and provide customized treatment plans for patients, ultimately leading to better clinical outcomes.
