Gastrointestinal stromal tumors (GISTs) are rare mesenchymal neoplasms that mainly occur in the stomach and small intestine; those arising in the esophagus are rarer. A 54-year-old woman was referred to our hospital with a one-month history of dysphagia. Esophagogastroduodenoscopy (EGD), performed approximately five months earlier, had not revealed any specific findings. However, an EGD performed in our hospital showed the presence of a round, protruding lesion (approximately 40×30 mm in size), with a normal overlying mucosal surface, 35–39 cm from the upper incisor. Chest computed tomography (CT) revealed a large esophageal mass. Enucleation was performed on the esophageal mass, and a GIST was diagnosed using immunochemical staining. Imatinib mesylate administration was initiated two months postoperatively. The patient was stable, without any evident recurrence in the 8-month postoperative follow-up EGD and chest CT examinations. Therefore, physicians should consider that patients with worsening dysphagia may have an underlying organic condition, such as an acute increase in size of an esophageal GIST, even if recent examinations were unremarkable.

6.The characteristics and costs of traumatic extremity amputation versus replantation at a single center in the Republic of Korea: a retrospective observational study

Purpose:
This study investigated the epidemiology and treatment outcomes of patients with traumatic limb amputation who visited a regional trauma center.
Methods:
From November 2015 to December 2021, patients with traumatic limb amputation who visited the Regional Trauma Center at Pusan University Hospital were retrospectively studied. The injury mechanism, number of operations for accompanying injuries, hospitalization period, injury severity score, insurance classification, and medical costs were investigated using medical records. We analyzed medical costs according to the injury site and treatment method.
Results:
We enrolled 57 patients who visited the hospital for traumatic limb amputation. The median patient age was 55 years, and there were 48 males and nine females. Seventeen patients underwent replantation, and 40 patients underwent amputation. Replantations were performed in 43.7% of cases of upper extremity injuries and in 12.0% of cases of lower extremity injuries. Six operations were performed per patient for replantation and three for amputation (p=0.027). In an analysis of the total medical costs submitted to the national health insurance system, replantation surgery was over twice as expensive as amputation surgery for the upper extremities (p=0.029). However, no significant cost difference was observed for lower limb amputations.
Conclusion
As a treatment for limb amputation patients, replantation requires a higher number of operations and a longer hospital stay than amputation. There was no difference in patients’ contributions to medical costs between replantation and amputation, but from the standpoint of national health insurance coverage, upper extremity injuries cost more when treated by replantation than by amputation.
7.Development and Validation of a Screening Questionnaire for Dementia With Lewy Bodies (DLB): the DLB Screening Questionnaire (DLBSQ)

Background:
and Purpose: Although dementia with Lewy bodies (DLB) is the second most common cause of neurodegenerative dementia, its clinical prevalence is low. We developed a short and easy-to-complete DLB screening questionnaire (DLBSQ) to raise diagnostic sensitivity in routine clinical settings.
Methods:
A total of 501 participants were retrospectively enrolled, including 71 controls, 184 patients without DLB, and 246 patients with probable DLB. All patients underwent clinical evaluation, including core features of DLB, the DLBSQ, brain magnetic resonance imaging, and detailed neuropsychological assessments. The diagnostic performance of the DLBSQ for probable DLB was investigated using a receiver operating characteristic curve analysis.
Results:
Total DLBSQ score was associated with visuospatial and frontal/executive dysfunction and the diagnosis of probable DLB. The area under the receiver operating characteristic curve for total DLBSQ score was 0.727. Youden’s method revealed an optimal cutoff value of 3. The sensitivity and specificity of the DLBSQ were 68.7% and 62.4%, respectively. Its discriminating performance improved when cognitive test profiles were additionally considered (area under the curve: 0.822, sensitivity: 80.6%, and specificity: 70.4%).
Conclusions
The DLBSQ might be a useful screening tool for DLB in routine clinical practice with good sensitivity and specificity.
8.Immunotherapy for Advanced/Metastatic Esophageal Squamous Cell Carcinoma

Esophageal squamous cell carcinomas constitute a major proportion of esophageal cancers in Korea. Conventional chemotherapy and radiation therapy are the options for treating advanced/ metastatic esophageal cancer, but the prognosis remains poor. Immunotherapy has significantly improved the prognosis of several advanced/metastatic cancers including esophageal squamous cell carcinoma. In Korea, immunotherapy is used to treat advanced/metastatic esophageal squamous cell carcinoma, and treatment results are expected to further improve. Immunotherapy is a term used to describe a treatment modality involving a biological/targeted agent that aims to enhance and restore the ability of the immune system to detect and destroy cancer cells by modifying or blocking co-stimulating signals. Immune checkpoint inhibitors have revolutionized cancer treatment with the administration of a single agent (monotherapy) or combinations of multiple agents, with the three approved agents being anti-PD-1 (programmed death 1), antiPD-L1 (programmed cell death ligand 1), and anti-CTLA4 (cytotoxic T-lymphocyte–associated protein 4) monoclonal antibodies. Anti-PD-1 drugs including nivolumab and pembrolizumab have been mainly investigated for treating advanced/metastatic esophageal squamous cell cancer. Studies on their effectiveness in a neoadjuvant setting, a curative adjuvant setting, or as the first-line treatment for advanced or metastatic setting are ongoing. This review describes the principle of action, summary of existing clinical studies, and prospects for immune checkpoint inhibitors used in the treatment of advanced/metastatic esophageal squamous cell cancer.
9.Ocular Vestibular-Evoked Myogenic Potential Assists in the Differentiation of Multiple System Atrophy From Parkinson’s Disease

Objective:
Vestibular-evoked myogenic potentials (VEMPs) can help in assessing otolithic neural pathway in the brainstem, which may also contribute to the cardiovascular autonomic function. Parkinson’s disease (PD) is associated with altered VEMP responses; however, the associations between VEMP abnormalities and multiple system atrophy (MSA) remain unknown. Therefore, we compared the extent of otolith dysfunction using ocular (oVEMP) and cervical VEMPs between patients with MSA and PD.
Methods:
We analyzed the clinical features, VEMP, and head-up tilt table test (HUT) findings using the Finometer in 24 patients with MSA and 52 with de novo PD who had undergone neurotologic evaluation at a referral-based university hospital in South Korea from January 2021 to March 2023.
Results:
MSA was associated with bilateral oVEMP abnormalities (odds ratio [95% confidence interval] = 9.19 [1.77–47.76], p = 0.008). The n1–p1 amplitude was negatively correlated with the Unified Multiple System Atrophy Rating Scale I-II score in patients with MSA (r = -0.571, p = 0.033), whereas it did not correlate with the Movement Disorder Society-Unified Parkinson’s Disease Rating Scale-III score in patients with PD (r = -0.051, p = 0.687). The n1 latency was negatively correlated with maximum changes in systolic blood pressure within 15 s during HUT in patients with PD (r = -0.335, p = 0.040) but not in those with MSA (r = 0.277, p = 0.299).
Conclusion
Bilaterally abnormal oVEMP responses may indicate the extent of brainstem dysfunction in MSA. oVEMP reflects the integrity of otolith-autonomic interplay, reliably assists in differentiating between MSA and PD, and helps infer clinical decline.
10.A Case of Type II Achalasia Occurring in a Nonagenarian Diagnosed with Acute Food Impaction

Achalasia is an esophageal motility disorder characterized by loss of esophageal peristalsis and impaired relaxation of the lower esophageal sphincter. Patients with achalasia often complain of persistent symptoms for several years before diagnosis. On the other hand, achalasia diagnosed as a sudden esophageal food impaction is uncommon, and no report has been issued on the diagnosis and successful treatment of achalasia in a 95-year-old patient. We report a case of achalasia diagnosed by high-resolution esophageal manometry and timed barium esophagography after food material removal by endoscopy in a 95-year-old woman who visited the hospital due to sudden esophageal food impaction and was successfully treated by endoscopic balloon dilatation.
