Background/Aims:
We investigated sex- and stage-specific associations of body mass index (BMI), fasting glucose, and high-density lipoprotein cholesterol (HDL-C) with gastric cancer.
Methods:
In total, 3,382 patients with gastric cancer and 19,609 healthy controls were enrolled. BMI was categorized into five groups. HDL-C was classified as low (< 40 and < 50 mg/ dl in males and females, respectively) and normal (≥ 40 and ≥ 50 mg/dl in males and females, respectively). Logistic regression analysis was performed to calculate odd ratios (ORs) and 95% confidence intervals (CIs).
Results:
After adjustment, low BMI (OR, 1.44; 95% CI, 1.13–1.84), low HDL levels (OR, 2.28;95% CI, 2.07–2.50), and high fasting glucose levels (OR, 2.94; 95% CI, 2.22–2.99) were associated with gastric cancer, whereas high BMI (OR, 0.61–0.81) was inversely associated with gastric cancer. In sex-specific analysis, BMI was inversely associated with gastric cancer only in males (trend: p < 0.001). Low serum HDL and high fasting glucose levels were strongly associated with gastric cancer in both males and females. The effect of high glucose content was more pronounced in females (OR, 4.02) than in males (OR, 2.58). BMI was inversely associated with both AGC (trend: p < 0.001) and EGC (trend: p = 0.001). Low serum HDL and high fasting glucose levels were strongly associated with gastric cancer in EGC and AGC.
Conclusions
The effect of BMI on gastric cancer varies by sex and stage, whereas low HDL levels are associated with gastric cancer regardless of these factors.
2.The Improvement of Intraoperative Motor Evoked Potential after Decompression in Cervical Compressive Myelopathy: Its Significance and Related Factors

Objective:
This study investigated the relationship between intraoperative motor evoked potential (MEP) improvement after decompression surgery for cervical compressive myelopathy (CCM) and postoperative neurological outcomes, and preoperative factors influencing MEP improvement.
Methods:
MEP amplitudes were measured prospectively before and after decompression in 38 patients with CCM. The patients were categorized into three groups according to whether the intraoperative MEP slightly decreased, slightly increased, or significantly increased. Functional outcomes were assessed using the recovery rate (RR) and absolute improvement (AI) of the modified Japanese Orthopaedic Association score on postoperative days (PODs) 7 and 28. The preoperative characteristics and intraoperative MEP changes among the three groups were compared. Additionally, the correlation between the increase in MEP amplitude during surgery and the extent of improvement in functional outcomes was investigated.
Results:
The significantly increased MEP group had a lower baseline MEP amplitude (152.46 µV; p=0.009). In the slightly decreased MEP group, the RR was 27.98 ± 32.29% at POD 7 (p=0.010) and 11.61 ± 69.84% at POD 28 (p=0.200); the AI was 0.79 ± 0.80 at POD 7 (p=0.010) and 0.79 ± 1.42 at POD 28 (p=0.100). In the slightly increased MEP group, the RR was 23.75 ± 28.36% at POD 7 (p=0.040) and 28.47 ± 43.38% at POD 28 (p=0.070); the AI was 1.00 ± 1.21 at POD 7 (p=0.030) and 1.08±1.88 at POD 28 (p=0.100). In the significantly increased MEP group, the RR was 41.06 ± 32.01% at POD 7 (p=0.009) and 59.78 ± 34.52% at POD 28 (p=0.006); the AI was 3.08 ± 2.07 at POD 7 (p=0.009) and 4.33 ± 2.54 at POD 28 (p=0.006). Greater intraoperative MEP improvement correlated with better postoperative recovery at 1 month (RR, p=0.010; AI, p<0.001).
Conclusion
Intraoperative MEP monitoring is valuable for predicting postoperative neurological outcomes in CCM patients, particularly those with lower baseline MEP amplitudes. Significant intraoperative MEP improvements are associated with better functional recovery. These findings underscore the importance of MEP monitoring in optimizing surgical strategies and predicting neurological recovery.

Background:
An idiopathic outflow tract premature ventricular complex (OT-PVC) is a common arrhythmia, and the accuracy of site of origin prediction using the 12-lead electrocardiogram (ECG) algorithm is not high. There are no studies about a systematic strategy that can provide practical help to electrophysiologists in OT-PVC mapping and ablation. This study aims to evaluate the efficacy and safety of the proposed ablation protocol and establish an optimal catheter ablation strategy by simultaneously investigating and synthesizing various indicators observed during the mapping procedure.
Methods:
and designThis study (ABOUT-PVC) was designed as a prospective multicenter study to enroll 210 patients from 11 tertiary university hospitals over an estimated 27 months. Patients with idiopathic OT-PVC requiring catheter ablation will receive the procedure through a proposed ablation strategy and will be followed up for at least 12 months. The primary outcome is the acute procedural success rate. The secondary outcomes are clinical success rate, procedure time, complication rate, symptom relief, and changes in echocardiographic parameters.
Conclusions
The ABOUT-PVC study was designed to investigate the efficacy and safety of the proposed ablation strategy and establish an optimal catheter ablation strategy. We expect this study to overcome the limitations of the ECG prediction algorithms and provide a practical guide to electrophysiologists, increasing the procedure’s success rate and reducing complications and procedure time.
6.Effectiveness of the Bivalent mRNA COVID-19 Vaccine for Preventing Critical Infection From the SARS-CoV-2 Omicron Variant in the Republic of Korea

Background:
This retrospective observational matched cohort study assessed the differences in critical infections caused by severe acute respiratory syndrome coronavirus 2 (SARSCoV-2) during the omicron-predominant period of the coronavirus disease 2019 (COVID-19) pandemic. We evaluated the vaccine effectiveness of bivalent mRNA vaccine compared to unvaccinated individuals.
Methods:
We collected COVID-19 case data from the Korean COVID-19 vaccine effectiveness cohort. We calculated the probability of critical COVID-19 cases by comparing the vaccinated and unvaccinated groups.
Results:
The risk of being critically infected due to SAR-CoV-2 infection was 5.96 times higher (95% confidence interval, 5.63–6.38) among older individuals who were unvaccinated compared to those who received the bivalent COVID-19 vaccine.
Conclusion
Our findings indicate that the bivalent vaccine reduces the disease burden of the SARS-CoV-2 omicron variant, particularly among the older population. Further studies are warranted to determine the effectiveness of booster doses of vaccines for SARS-CoV-2 infection.
7.Assessment of diet quality of adults from North Korea: using nutrition quotient (NQ) for Korean adults

Purpose:
The purpose of this study was to assess the diet intake status and quality of adults from North Korea who had lived in Seoul and Gyeonggi-do, using the nutrition quotient (NQ) for Korean adults.
Methods:
A number of 166 adults aged from 20 to 64 years were enrolled as study subjects.A structured questionnaire was used to collect information on their demographic characteristics, food consumption frequency, eating behaviors, and NQ. The NQ score was then used to measure the overall diet quality. The NQ score was composed of four major agebased categories. ‘Balance’, ‘Diversity’, ‘Moderation’, and ‘Dietary behavior’ were utilized as scores for measurements within each of the four categories. The scores for all four categories were summed up, resulting in the total NQ score. Participants’ age groups were stratified by 20’s, 30’s, 40’, and over 50’s. A χ2 test and generalized linear regression (GLM) model were used to assess a significance for difference of subject distribution in categorical and continuous variables in the food consumption frequency, eating behaviors, and NQ scores.
Results:
Participants in younger age groups were more likely to report consumption of ramyeon, fast food, sweet and greasy baked products, processed beverage, delivery food, and night snacks than the older age groups. Most importantly, participants in 20’s age group were less likely to report the total NQ and moderation score than the 50’s age group.
Conclusion
In this study, the 20’s age group displayed a lower NQ characteristic for an unbalanced diet. Therefore, it is imperative for adults from North Korea to be given individualized diet instructions along with nutritional education programs.

The aim of this study was to develop evidence-based recommendations for determining the surgical extent in patients with locally invasive differentiated thyroid cancer (DTC). Locally invasive DTC with gross extrathyroidal extension invading surrounding anatomical structures may lead to several functional deficits and poor oncological outcomes. At present, the optimal extent of surgery in locally invasive DTC remains a matter of debate, and there are no adequate guidelines. On October 8, 2021, four experts searched the PubMed, Embase, and Cochrane Library databases; the identified papers were reviewed by 39 experts in thyroid and head and neck surgery. The Grading of Recommendations Assessment, Development and Evaluation (GRADE) approach was used to assess the quality of evidence, and to develop and report recommendations. The strength of a recommendation reflects the confidence of a guideline panel that the desirable effects of an intervention outweigh any undesirable effects, across all patients for whom the recommendation is applicable. After completing the draft guidelines, Delphi questionnaires were completed by members of the Korean Society of Head and Neck Surgery. Twenty-seven evidence-based recommendations were made for several factors, including the preoperative workup; surgical extent of thyroidectomy; surgery for cancer invading the strap muscles, recurrent laryngeal nerve, laryngeal framework, trachea, or esophagus; and surgery for patients with central and lateral cervical lymph node involvement. Evidence-based guidelines were devised to help clinicians make safer and more efficient clinical decisions for the optimal surgical treatment of patients with locally invasive DTC.
9.Reducing Microbial Contamination in Hematopoietic Stem Cell Products and Quality Improvement Strategy: Retrospective Analysis of 1996-2021 Data

Background:
Sterility and safety assurance of hematopoietic stem cell (HSC) products is critical in transplantation. Microbial contamination can lead to product disposal and increases the risk of unsuccessful clinical outcomes. Therefore, it is important to implement and maintain good practice guidelines and regulations for the HSC collection and processing unit in each hospital. We aimed to share our experiences and suggest strategies to improve the quality assurance of HSC processing.
Methods:
We retrospectively analyzed microbial culture results of 11,743 HSC products processed over a 25-year period (January 1996 to May 2021). Because of reorganization of the HSC management system in 2008, the 25-year period was divided into periods 1 (January 1996 to December 2007) and 2 (January 2008 to May 2021). We reviewed all culture results of the HSC products and stored aliquot samples and collected culture results for peripheral blood and catheter samples.
Results:
Of the 11,743 products in total, 35 (0.3%) were contaminated by microorganisms, including 19 (0.5%) of 3,861 products during period 1 and 16 (0.2%) of 7,882 products during period 2. Penicillium was the most commonly identified microorganism (15.8%) during period 1 and coagulase-negative Staphylococcus was the most commonly identified (31.3%) during period 2. HSC product contamination occurred most often during HSC collection and processing.
Conclusions
The contamination rate decreased significantly during period 2, when the HSC management system was reorganized. Our results imply that handling HSC products by trained personnel and adopting established protocols, including quality assurance programs, aid in decreasing the contamination risk.
10.Safety and efficacy of nilotinib in adult patients with chronic myeloid leukemia: a post-marketing surveillance study in Korea

Background:
Nilotinib is a tyrosine kinase inhibitor approved by the Ministry of Food and Drug Safety for frontline and 2nd line treatment of Philadelphia chromosome-positive chronic myeloid leukemia (Ph+ CML). This study aimed to confirm the safety and efficacy of nilotinib in routine clinical practice within South Korea.
Methods:
An open-label, multicenter, single-arm, 12-week observational post-marketing surveillance (PMS) study was conducted on 669 Korean adult patients with Ph + CML from December 24, 2010, to December 23, 2016. The patients received nilotinib treatment in routine clinical practice settings. Safety was evaluated by all types of adverse events (AEs) during the study period, and efficacy was evaluated by the complete hematological response (CHR) and cytogenetic response.
Results:
During the study period, AEs occurred in 61.3% (410 patients, 973 events), adverse drug reactions (ADRs) in 40.5% (271/669 patients, 559 events), serious AEs in 4.5% (30 patients, 37 events), and serious ADRs in 0.7% (5 patients, 8 events). Furthermore, unexpected AEs occurred at a rate of 6.9% (46 patients, 55 events) and unexpected ADRs at 1.2% (8 patients, 8 events). As for the efficacy results, CHR was achieved in 89.5% (442/494 patients), and minor cytogenetic response or major cytogenetic response was achieved in 85.8% (139/162 patients).
Conclusion
This PMS study shows consistent results in terms of safety and efficacy compared with previous studies. Nilotinib was well tolerated and efficacious in adult Korean patients with Ph + CML in routine clinical practice settings.
