1.The study on biological exposure index of occupational exposure to arsenic and its inorganic compounds

Objective:To establish biological exposure index (BEI) of occupational exposure to arsenic and its inorganic compounds through occupational epidemiology and the regression analysis of internal and external exposure of workers.Methods:In November 2021, 125 workers with occupational exposure to arsenic and its inorganic compounds and 49 office administrators in a non-ferrous metal smelter in Yunnan Province were selected as the exposure group and control group, respectively. Air samples from the workplace of the study subjects on weekdays were collected and arsenic concentrations were determined. Urine samples were collected in end-of-work weekend and high performance liquid chromatography-inductively coupled plasma mass spectrometry (HPLC-ICP-MS) was used to detect the levels of trivalent inorganic arsenic (iAs 3+) , pentavalent inorganic arsenic (iAs 5+) , monomethyl arsenic (MMA) and dimethyl arsenic (DMA) in urine. The correlations between arsenic concentration in the workplace air and arsenic species in urine of workers were analyzed. Arsenic exposure concentration and the level of urinary arsenic (∑iAs+MMA+DMA) of workers was analyzed by linear regression and the BEI of arsenic and its inorganic compounds in the workplace was proposed based on the results of micronucleus test. Results:The median of time-weighted average concentration ( CTWA) of arsenic in the workplace air of the exposure group was 0.0116 mg/m 3, and the over-standard rate was 71.2% (89/125) . The concentrations of iAs 3+, iAs 5+, inorganic arsenic (iAs=∑iAs 3++iAs 5+) 、MMA、DMA and urinary arsenic in the exposure group were higher than those in the control group at the end of shift, and the differences were statistically significant ( P<0.05) . The concentration of arsenic in the workplace air had the strongest correlation with the concentration of urinary arsenic at the end of the shift ( rs=0.909, P<0.001) . The regression equation was lg ( y) =7.662+2.968lg ( x) ( r=0.821, P<0.05) . According to the occupational exposure limit (OEL) of arsenic in China, the concentration of urinary arsenic in the end-of-work weekend was calculated to be 53.2 μg/L. Combined with the results of micronucleus test, the BEI of occupational exposure to arsenic and its inorganic compounds in the workplace was proposed to be 50 μg/L. Conclusion:The urinary arsenic in the end-of-work weekend can be used as a biomarker of occupational exposure to arsenic, and its BEI is recommended to be 50 μg/L.
2.Correlation analysis of urinary arsenic species and health effect indicators of occupational arsenic exposure workers

Objective:To explore the correlation between urinary arsenic and health effects through the determination and analysis of urinary arsenic levels in occupational arsenic exposed workers.Methods:In November 2021, 95 workers exposed to arsenic and its inorganic compounds and 31 administrative personnel from a non-ferrous metal smelter in Yunnan Province were selected as the contact group and control group, respectively. Urine forms of arsenic, blood tumor markers, liver function were detected, and micronucleus test was used to analyze the chromosome damage. The correlation between urine forms of arsenic and health effects were analyzed.Results:Compared with the control group, the concentrations of urinary trivalent inorganic arsenic (iAs 3+) , pentavalent inorganic arsenic (iAs 5+) , inorganic arsenic (iAs=∑iAs 3++iAs 5+) , monomethyl arsenic (MMA) , dimethyl arsenic (DMA) and urinary arsenic (∑iAs+MMA+DMA) at the end of class in contact group were higher ( P<0.05) . There was no statistically significant difference in blood tumor markers and liver function indicators between the two groups ( P>0.05) . Compared with the control group, the peripheral blood micronucleus rate and cell micronucleus rate in the contact group were significantly increased ( P<0.05) . The urinary arsenic, iAs 5+, inorganic arsenic and DMA were positively correlated with peripheral blood micronucleus rate in contact group ( rs=0.48, 0.34, 0.37, 0.23, P<0.05) , and the urinary arsenic, iAs 5+, DMA were positively correlated with peripheral blood micronucleus rate ( rs=0.48, 0.34, 0.26, P<0.05) . Conclusion:There is a significant correlation between different valence states of arsenic in the urine and abnormal health effects of occupational arsenic exposed workers. It is necessary to strengthen the detection of arsenic species in the urine of occupational arsenic exposed workers to better protect their health.

5.Transjugular intrahepatic portosystemic shunt for portal hypertension with portal vein thrombosis:analysis of its efficacy and safety

Objective To discuss the efficacy and safety of transjugular intrahepatic portosystemic shunt(TIPS)for portal hypertension with portal vein thrombosis(PVT).Methods A total of 31 patients with portal hypertension complicated by PVT,who underwent TIPS at the Chinese PLA General Hospital between December 2017 and October 2022,were enrolled in this study.The clinical data,including preoperative laboratory tests,operation pattern,intraoperative portal vein pressure(PVP),postoperative follow-up ultrasound or enhanced CT examination findings,and the presence or absence of hepatic encephalopathy(HE),were collected.Paired t-test was used to compare the differences in PVP before and after stenting,Kaplan-Meier curve was used to analyze the postoperative shunt patency rate,rebleeding rate,incidence of HE,and survival rate.Log-rank test was used to analyze the differences in follow-up results between the patients having cavernous transformation of portal vein(CTPV)and the patients having no CTPV.Results The technical success rate of TIPS procedure was 93.55%(29/31).In the 29 patients with successful surgery,the mean PVP value decreased from preoperative(30.15±4.61)mmHg to postoperative(20.84±5.57)mmHg,the difference was statistically significant(t=8.975,P<0.05).The postoperative median follow-up time was 22.90 months(range of 4.50-61.80 months).During the follow-up period,24.14%of patients(7/29)developed shunt tract dysfunction,17.24%of patients(5/29)developed rebleeding,17.24%of patients(5/29)developed HE,and 5 patients(5/29,17.24%)died.Ten patients(10/29,34.48%)with PVT were complicated by CTPV,among them 5 patients had shunt dysfunction,3 patients had rebleeding,one patient developed HE,and 3 patients died.Of the 19 patients who having no coexisting CTPV,shunt dysfunction was seen in 2,rebleeding in 2 and HE in 4,and 2 patients died.The incidences of shunt tract dysfunction and rebleeding in the patients having CTPV were obviously higher than those in the patients having no CTPV(both P<0.05),while no statistically significant differences in the incidences of HE and death existed between the two groups(both P>0.05).Conclusion TIPS can effectively reduce the PVP in patients with PVT.The incidences of shunt tract dysfunction and rebleeding in the PVP with coexisting CTPV are remarkably higher than those in the PVT patients with no coexisting CTPV.(J Intervent Radiol,2024,33:537-542)
6.Comparison of the effects of CR and PS prostheses in the treatment of knee osteoarthritis

Objective To compare the effect of posterior cruciate ligament retaining(CR)and posterior cruciate substituting(PS)prosthesis in knee arthroplasty.Methods 64 patients with knee osteoarthritis were admitted to our department from June 2021 to June 2022 and whom met the selection criteria were randomly selected and divided into observation group and control group(n = 32).CR prosthesis was used in the observation group,and PS prosthesis was used in the control group.Operation time,postoperative drainage volume,difference of hemoglobin(HGB)before and after operation,postoperative hospital stay,postoperative range of motion at 7 d,14 d,45 d,3 months and 1 year were recorded.ROM,visual analog pain Scale(VAS),Hospital for special surgery(HSS)score,and maximum forward and backward displacement of knee joint and the regular radiographs were also recorded,too.Results The two groups were similar in terms of operation time,postoperative hospital stay,postoperative VAS score and maximum anterior and posterior displacement of knee joint,with P values greater than 0.05,which had no statistical significance.The postoperative drainage volume in the observation group was significantly less than that in the control group,with statistical significance(P<0.05).The HGB difference of observation group was significantly lower than that of control group,with statistical significance(P<0.05).There was no statistically significant differ-ence in ROM and HSS scores between the two groups before surgery;the ROM and HSS scores of the control group at 7,14 and 45 days after surgery was better than that of the observation group,with statistical significance(P<0.05);the ROM and HSS scores of the two groups at 3 months after surgery was not statistically significant.The ROM and HSS scores in the observation group was better than that in the control group at 6 months and 1 year after operation,and the difference was statistically significant(P<0.05).Conclusions CR prosthesis is superior to PS prosthesis in terms of intraoperative blood loss and post-operative drainage volume,which can reduce surgical risk.Both kinds of prosthesis can achieve good knee stability after surgery,while PS prosthesis has better early clinical effect,while CR prosthesis has better long-term clinical effect.
7.Relationship between peripheral blood TLR4,IL-1β and NLR and the progression and prognosis of acute pancreatitis

Objective To study the relationship between peripheral blood Toll like receptor 4(TLR4),interleukin-1β(IL-1β)and neutrophil-to-lymphocyte ratio(NLR)and the progression and prognosis of acute pancreatitis(AP).Methods A total of 250 patients with AP were divided into the mild group(121 cases),the moderately severe group(89 cases)and the severe group(40 cases)according to the disease severity.Patients were divided into the poor prognosis group(33 cases)and the good prognosis group(217 cases)based on the prognosis after 5 days of treatment.Peripheral blood levels of TLR4,IL-1β and NLR were compared at different time points,different conditions and different prognosis in patients.The correlation of peripheral blood levels of TLR4,IL-1β and NLR,Ranson score and amylase were analyzed by Pearson correlation analysis.The value of each indicator in the diagnosis of severe AP was analyzed using ROC curve,and multivariate Logistic regression analysis was conducted to identify the prognostic risk factors.Results TLR4,IL-1β and NLR levels in AP patients were higher 24 h after admission than 48 h and 72 h after admission(P<0.05).Peripheral blood TLR4 and IL-1β levels in AP patients at 24 h after admission increased with the condition became worse.NLR level was higher in the severe group than that in the moderately group or the mild group(P<0.05).Peripheral blood TLR4,IL-1β and NLR levels in AP patients 24 h after admission were positively correlated with Ranson score at 48 h after admission and amylase level at 24 h after admission(P<0.05).The area under ROC curve,sensitivity and specificity of the combination of TLR4,IL-1β and NLR at 24 h after admission for predicting severe AP were 0.895,84.53%and 81.69%.Peripheral blood TLR4,IL-1β and NLR levels 24 h after admission were higher in the poor prognosis group than those in the good prognosis group(P<0.05).Multivariate Logistic regression analysis showed that elevated TLR4,IL-1β and NLR at 24 h after admission were independent risk factors for poor prognosis of patients with AP(P<0.05).Conclusion Peripheral blood TLR4,IL-1β and NLR in patients with AP are closely related to disease progression and prognosis.Therefore,they can be taken as important monitoring indicators.
8.Research on the method of setting depth thresholds for second generation sequencing of forensic genetic markers-Taking microhaplotypes as an example

Objective Sequencing depth is a key parameter in next generation sequencing,which is closely related to the accuracy of sequencing results.Forensic biological evidence examination requires extremely high accuracy.It is crucial to scientifically and reasonably set the sequencing depth analysis threshold for forensic next generation sequencing testing.Methods This study used targeted sequencing data of microhaplotypes from 50 samples with known genotypes.By calculating the accuracy,precision,recall,and F1 score of each locus under various threshold conditions,two types of analysis threshold setting methods,which were based on fixed read count and fixed sequencing depth ratio,were studied extensively.Results The results showed that false positives were observed when the analysis threshold was set at 50×or 100×.When the analysis threshold was set at 200×,false negatives were observed.When the analysis threshold was set at 1.5%,3.0%,or 4.5%,false positives were observed.This study further proposed a third type of analysis threshold setting method,which was based on sequencing depth ratio scatter plots.With this method,no false positive or false negative was observed in the results.This article then explored four factors that lead to significant differences in the sequencing depth of forensic next generation sequencing experiments,compared with the analysis threshold setting method for capillary electrophoresis technology,and discussed the correlation between analysis thresholds and the ability to distinguish mixed DNA.Conclusion Employing the sequencing depth ratio scatter plot method to set analysis threshold has significant application value in next generation sequencing-based forensic genetic marker genotyping.

10.Management of perioperative coagulation dysfunction in patients with massive blood transfusion during retroperitoneal tumor resection

Objective:To investigate the perioperative alterations and management of coagulation function in patients of massive blood transfusion during retroperitoneal tumor (RT)resection.Methods:Fourty-seven RT patients at Peking University International Hospital from Jan 2016 to Dec 2021 undergoing resection with massive blood transfusion more than 20 U within 24 h were reviewed for coagulation function before and after surgery.Results:Intraoperative bleeding was 3 000-25 800 ml, 10 patients had blood loss ≥10 000 ml. During the operation, (25.3±9.9) U of red blood cells were transfused, (2 720±1 369) ml plasma transfused, and (2.4±3.3) U platelets were transfused in 6 patients. Fourty-five patients received intraoperative albumin of (79.5±46.5) g; All 47 patients received fibrinogen of (2.3±1.3) g; Prothrombin complex was given in 45 patients (1 205±807) U. Preoperative hemoglobin was statistically different compared to postoperatively and days 1, 3 and 5 ( W=1 790, P<0.001; W=1 672, P<0.001; W=1 704, P<0.001; W=1 486, P=0.004);As with platelets, the difference was also statistically significant compared to postoperative days 1, 3, and 5 ( W=2 153, P<0.001; W=2 092, P<0.001; W=1 732, P<0.001); Preoperative albumin was different compared to postoperative days 1 and 3 ( W=1 568, P<0.001; W=1 578, P<0.001,); Preoperative fibrinogen was different compared to postoperative day 1 ( W=1 964, P<0.001). PT and APTT were prolonged on postoperative days 1 and 3 ( W=628, P<0.001, W=804, P=0.023) ( W=661, P<0.001, W=796, P=0.02). Patient's preoperative fibrin degradation products and D-dimer were above the normal value and were higher on postoperative days 3 and 5 ( W=498, P<0.001, W=345, P<0.001). Conclusions:Coagulation disorders occur perioperatively in patients with massive transfusion while undergoing surgery for RT.The implementation of ratiional transfusion strategy and close postoperative survey and management of coagulation dysfunction help avoid the coagulation related morbidities.
