Objective To analyze the status of willingness to participate in health science popularization (HSP) and its influencing factors among medical workers in Guangdong Province. Methods A total of 462 medical workers in Guangdong Province were recruited as the study subjects using a stratified convenience sampling method. The Questionnaire on Influencing Factors of Willingness to Participate in Health Science Popularization, developed by the research team, was used to investigate their willingness to participate in HSP and related variables, including previous communication experience, attitude, moral belief, descriptive norm, injunctive norm, self-efficacy, and social support. Hierarchical regression analysis was used to identify the influencing factors of willingness to participate in HSP. Results The median and 25th and 75th percentiles score of willingness to participate in HSP was 4.0 (3.8, 4.5), indicating a moderately high level of willingness. The HSP participation willingness score was positively correlated with scores for previous communication experience, attitude, moral belief, descriptive norm, injunctive norm, self-efficacy, and social support (Spearman correlation coefficients were 0.513, 0.384, 0.507, 0.533, 0.463, 0.556, and 0.435, respectively; all P<0.01). Hierarchical regression analysis showed that previous communication experience, moral belief, descriptive norm and self-efficacy positively predicted willingness to participate in HSP (all P<0.05). Medical workers in the non-Pearl River Delta region had lower willingness than those in the Pearl River Delta region for HSP participation (P<0.05). Conclusion There is still room for improving willingness for HSP participation among healthcare professionals in Guangdong Province. It is recommended to enhance self-efficacy through communication skills training, strengthen professional ethics education, foster a positive peer engagement atmosphere, and intensify resource support and capacity building for HSP in the non-Pearl River Delta region, to improve HSP participation willingness among medical workers in Guangdong Province.

Objective:To identify genetic etiology of ischemic stroke(IS)based on pleiotropy of obe-sity related genes. Methods:A discordant sib-pair study was designed based on the Fangshan family co-hort in Beijing.Body mass index(BMI)polygenic risk score(PRS)was first constructed under different P values.Using the polygenic transmission disequilibrium test(pTDT),we then compared the actual BMI genetic risk of siblings with IS to their expected risk,to analyze whether higher BMI was over-trans-mitted to siblings with IS.The single nucleotide polymorphism(SNP)that comprised the PRS over-trans-mitted with IS and that corresponded to the highest heritability of IS were identified as a pleiotropy SNPs set between BMI and IS.This set was then utilized as a candidate set to identify and verify risk SNPs as-so-ciated IS by transmission disequilibrium test. Finally,we identified independent genomic risk loci and mapped to genes,we then explored the biological function of the identified risk loci and genes by func-tional annotation and pathway enrichment. Results:A total of 541 participants were enrolled, with an average age of(58.4±8.1)years,including 326 discordant sib pairs of ischemic stroke.Compared with non-IS participants,IS participants with males, education level below junior high school, hypertension and hyperlipidemia accounted for a higher proportion(P<0.05).For all the BMI PRS,we found that the actual genetic risk of BMI in siblings with IS was higher than their expectation, suggesting that genetic risk associated with high BMI was over-transmitted with IS.Compared with other SNP sets, the set(P<5 × 10-4)corresponded to the best analytical statistics of pTDT and the highest heritability of IS and was identified as the pleiotropy SNP set between BMI and IS.Within this set, there were 45 SNPs having linkage and association with IS,which were located in 43 independent genomic risk loci and mapped to 40 genes. These genes were significantly enriched in the lipid metabolism pathway. The rs2232852 cor-rected by multiple tests was mapped to CYB5R1 and ADIPOR1,which were related to lipid metabolism and the ferroptosis pathway. Conclusion:Pleiotropy between BMI-related genes and IS was observed.Forty-five SNPs were found with linkage and association with IS in the pleiotropy gene set and mapped to 40 genes, which were functionally enriched in lipid metabolic pathways. The rs2232852 corrected by multiple tests during association analysis validation was mapped to CYB5R1 and ADIPOR1,which were related to lipid metabolism and the ferroptosis pathway, suggesting that lipid metabolism and ferroptosis played an important role in the development of IS.

Objective:To explore the surgical treatment strategies for Mason type III radial head fractures complicated with adult Bado type II Monteggia fractures. Methods:A retrospective analysis was performed on the clinical data of 25 adult patients with Mason type III radial head fractures complicated with adult Bado type II Monteggia fractures, admitted to the Upper Extremity Orthopaedics Department of Zhengzhou Orthopaedic Hospital from June 2013 to June 2023. There were 15 males and 10 females, with an average age of 43.5±14.7 years (range: 20-67 years). Among them, 5 cases were complicated with humeroulnar joint dislocation. The patients were divided into two groups: 17 cases were treated with open reduction and internal fixation (ORIF) of radial head fractures combined with ORIF of proximal ulnar fractures (open reduction group), and 8 cases were treated with radial head replacement combined with ORIF of proximal ulnar fractures (radial head replacement group). At the last follow-up, elbow joint range of motion was recorded, and pain, elbow function, and subjective upper limb function were evaluated using the Visual Analogue Scale (VAS), Mayo Elbow Performance Score (MEPS), and Disabilities of the Arm, Shoulder and Hand (DASH) scale. The incidence of complications was also recorded. Results:All 25 patients were followed up for an average of 25.6±9.0 months (range: 12-45 months). At the last follow-up, the affected elbows in the open reduction group had a flexion of 124.47°±12.59° (range, 90°-140°), extension of 21.12°±10.07° (range, 10°-50°), pronation of 48.59°±11.62° (range, 20°-61°), and supination of 48.53°±8.43° (range, 30°-60°). In the radial head replacement group, the affected elbows showed flexion of 128.75°±13.17° (range, 100°-140°), extension of 14.00°±7.71° (range, 0°-25°), pronation of 61.25°±10.26° (range, 60°-80°), and supination of 71.88°±10.33° (range, 60°-80°). The MEPS score in the open reduction group was 82(75, 85) points (range, 55-90 points), the VAS pain score was 1(1, 2) points (range, 0-3 points), and the DASH score was 9(8, 14) points. In the radial head replacement group, the MEPS score was 90(85, 90) points (range, 85-90 points), the VAS pain score was 1(0, 1) points (range, 0-1 points), and the DASH score was 5(5, 6) points. Complications included 5 cases of heterotopic ossification, 1 case of incision infection, 1 case of nonunion, 1 case of ulnar nerve injury combined with traumatic arthritis, and 1 case of proximal radioulnar bone bridge formation. Conclusions:Both radial head replacement and open reduction internal fixation combined with proximal ulnar fracture fixation can effectively treat Mason type III radial head fractures complicated with adult Bado type II Monteggia fractures. There was no significant difference in postoperative flexion and extension, but the radial head replacement group demonstrated better forearm rotation and DASH scores postoperatively.

Objective:To investigate the risk factors for positive surgical margins(PSM)after robot-assisted radical prostatectomy(RARP),and to develop and validate a predictive nomogram. Methods:We retrospectively analyzed the clinicopathological data of 874 prostate cancer patients who underwent RARP performed by a single surgeon at the First Medical Center of Chinese PLA General Hospital between January 2012 and December 2018. Patients were divided into positive surgical margin(n=327)and negative surgical margin(n=547)groups based on postoperative margin status. The PSM group had significantly higher preoperative median tPSA[31.200(19.050,54.400)ng/ml vs. 15.050(9.840,27.590)ng/ml, P<0.01],higher proportion of patients with PSAD>1 ng/ml 2[49.5%(162/327)vs. 21.2%(116/547), P<0.01],biopsy Gleason score ≥8[33.3%(109/327)vs. 21.2%(116/547), P<0.01],ISUP grade 4-5[33.3%(109/327)vs. 21.2%(116/547), P<0.01],clinical T stage ≥cT 3[11.3%(37/327)vs. 4.2%(23/547), P<0.01],and high-risk classification[82.3%(269/327)vs. 55.9%(306/547), P<0.01]compared to the negative surgical margin group. Conversely, the PSM group had a lower prevalence of hypertension[29.7%(97/327)vs. 40.2%(220/547), P=0.002].Patients were randomly split into a training cohort(n=656,75%)and an internal validation cohort(n=218,25%). An external validation cohort included 71 patients who underwent RARP by different surgeons between January 2014 and December 2016. No significant differences in baseline characteristics were observed between cohorts( P>0.05).Univariate and multivariate logistic regression analyses identified independent predictors of PSM,which were incorporated into a nomogram. Predictive performance was assessed using receiver operating characteristic(ROC)curves, decision curve analysis(DCA),and calibration curve. Internal and external validations were performed. Results:The PSM group had longer postoperative hospitalization[6(5,8)vs. 6(5,7)days, P=0.028],higher rates of pathologic Gleason score ≥8[41.5%(115/277)vs. 24.9%(111/446), P<0.01],ISUP grade 4-5[41.5%(115/277)vs. 24.9%(111/446), P<0.01],pT 3 stage[52.3%(171/327)vs. 17.4%(95/547), P<0.01],pN 1 stage[12.8%(42/327)vs. 3.8%(21/547), P<0.01],extracapsular extension[52.3%(171/327)vs. 17.4%(95/547), P<0.01],and seminal vesicle invasion[34.6%(113/327)vs. 9.1%(50/547), P<0.01].Multivariate analysis identified elevated tPSA( OR=1.014,95% CI 1.004—1.024,P=0.006)and PSAD ≥0.15 ng/(ml/g)( OR=11.638,95% CI 1.450—93.396,P=0.021)as independent risk factors for PSM. The area under the ROC curve(AUC)of the nomogram constructed based on the above variables was 0.770(95% CI 0.735—0.805). The AUC values for the internal and external validation sets were 0.698(95% CI 0.630—0.767)and 0.643(95% CI 0.513—0.774),respectively. The calibration curve demonstrated good agreement between the predicted and observed outcomes, and the DCA indicated that the predictive model has potential clinical utility in decision-making. Conclusion:tPSA and PSAD were identified as independent risk factors for PSM. The nomogram constructed based on these two independent predictive variables effectively predicted PSM after RARP.
