1.Scoping review of medication-related risk factors for falls in older adults

OBJECTIVE To systematically review medication-related risk factors for falls in older adults, to provide references for ensuring medication safety among older adults. METHODS A systematic search was conducted in PubMed, Embase, Web of Science, and CNKI for relevant literature published from database inception to November 1, 2025. Relevant studies on medication-related falls in older adults, both domestic and international, were included. Drug factors influencing falls in older adults were summarized and analyzed. RESULTS A total of 22 studies were included. Four major classes of fall-risk-increasing drugs were identified: psychotropic medications [12 studies, odds ratio (OR) range 1.500-5.790], cardiovascular system drugs (5 studies, OR range 1.236-4.784), analgesics (3 studies, OR range 1.500-4.490), and hypoglycemic agents (3 studies, OR range 2.070-2.751). Additionally, anticholinergic burden (1 study, OR was 2.610) and polypharmacy (7 studies, OR range 2.902-25.897 for the use of ≥4 medications) were identified as significant risk factors for falls. CONCLUSIONS Falls in older adults are significantly associated with psychotropic medications, cardiovascular system drugs, analgesics, and hypoglycemic agents, among which psychotropic medications pose the highest risk. Anticholinergic burden and polypharmacy are also important risk factors. In clinical practice, interventions should be implemented through deprescribing and risk monitoring to effectively reduce the risk of falls in older adults.
2.Analysis of the disease burden of falls and influencing factors among the elderly from 1990 to 2023

OBJECTIVE To analyze trends in the burden of disease associated with falls from 1990 to 2023, identify risk factors for falls, and provide a reference for the development of measures to reduce the burden of disease associated with falls and the formulation of public health policies. METHODS Based on the data of 2023 from the Global Burden of Disease (GBD) database, the changes in fall-related prevalence, mortality, disability-adjusted life years (DALYs) and incidence in China and globally from 1990 to 2023 were analyzed, and the stratified analyses by gender, age and socio-demographic index (SDI) were conducted. The risk factors related to falls in 2023 were systematically analyzed, and the impact of multimorbidity and polypharmacy on the occurrence of falls by combining relevant literature evidence was explored. RESULTS & CONCLUSIONS Globally, the percentage changes in age-standardized rates of fall-related prevalence, mortality, DALYs and incidence from 1990 to 2023 were -2.4% (95%UI: -4.8% to -0.3%), -7.8% (95%UI: -20.5% to 10.9%), -10.6% (95%UI: -16.5% to -4.3%), and -4.6% (95%UI: -8.1% to -0.8%), respectively. However, in some regions, the percentage changes in age-standardized rates for these indicators were greater than 0 (e.g., prevalence, mortality and DALYs in Australasia, and incidence in Andean Latin America). In China, the percentage changes in age-standardized rates of fall-related mortality and DALYs from 1990 to 2023 were -18.3% (95%UI: -56.7% to 32.3%) and 1.9% (95%UI: -10.1% to 12.4%), while the percentage changes in age-standardized rates of prevalence and incidence were 25.9% (95%UI: 20.2% to 32.0%) and 35.7% (95%UI: 28.5% to 44.1%), respectively. Globally, in regions with high and medium-to-high SDI levels, the percentage changes in age-standardized rates of fall-related prevalence and incidence showed an upward trend, while DALYs showed a downward trend; however, the trend in mortality was unclear. In regions with a middle SDI level, the percentage changes in age-standardized rates of fall-related prevalence and incidence both showed an upward trend, while the trends in mortality and DALYs were unclear. In regions with low-to-medium SDI level, the percentage changes in age-standardized rate of fall-related prevalence, DALYs and incidence all showed a downward trend, while the trend in mortality was unclear; in regions with low SDI level, the trends for all above indicators were unclear. Correlation analyses of the burden of fall-related diseases in China and globally indicated that the risk of fall-related diseases was higher among younger and middle-to-older adult male populations, while women were concentrated in older age groups. Meanwhile, as the SDI levels increase, the percentage change in age-standardized rate of DALYs for fall-related diseases showed a clear upward trend, and this trend was more pronounced in high-SDI regions. Tobacco use, alcohol consumption, low bone density, and occupational risks were risk factors for falls, with low bone density and occupational risks contributing significantly (exceeding 20% in some regions); multimorbidity and polypharmacy could also significantly increase the risk of falls. It is recommended that clinical settings establish a list of fall-risk increasing drugs and a warning system, improve medication reviews, strengthen medication education and adherence management, foster multidisciplinary collaboration, and develop medication strategies for specific patient populations to reduce the incidence of falls and alleviate the burden of disease.
3.Effect and mechanism of circular RNA SEC24A on proliferation and apoptosis of synovial fibroblasts in osteoarthritis

BACKGROUND:Synovitis is involved in all stages of osteoarthritis and is a key factor contributing to the development of osteoarthritis.Studies have shown that circular RNA(circRNA)plays an important role in the proliferation,apoptosis and extracellular matrix degradation of synovial cells and chondrocytes.OBJECTIVE:To observe the effects of circRNA SEC24A on the interleukin-1β-induced proliferation,apoptosis,and expression of inflammatory factors in human synovial fibroblasts.METHODS:Human synovial fibroblasts were divided into four groups,including control group,interleukin-1β group,empty vector group,and sh-circSEC24A group.Except for the control group,the other three groups were induced with 10 ng/mL interleukin-1β for 24 hours to establish inflammatory cell models;the empty vector group and sh-circSEC24A group were infected with empty vector virus and lentiviral vector knocking down circSEC24A.CCK-8 assay was used to detect cell proliferation.Flow cytometry was used to detect cell apoptosis.ELISA was used to detect the levels of matrix metalloproteinase-9,matrix metalloproteinase-13,interleukin-6,and tumor necrosis factor-α in cell supernatant.Western blot assay was used to detect the relative expression levels of Bax,Bcl-2,matrix metalloproteinase-9,matrix metalloproteinase-13,casepase3,cleaved-casepase3,casepase8,and cleaved-casepase8 proteins in cells.RESULTS AND CONCLUSION:(1)qRT-PCR results showed that compared with the normal group,the expression of circSEC24A in human synovial fibroblasts induced by interleukin 1β was significantly up-regulated.(2)The absorbance value of cells in the sh-circSEC24A group detected by CCK-8 assay was significantly higher than that of interleukin 1β group and empty vector group(P<0.05).The apoptosis rate of sh-circSEC24A group detected by flow cytometry was significantly lower than that of interleukin 1β group and empty vector group(P<0.05).(3)The levels of tumor necrosis factor α and interleukin 6 in the supernatant of human synovial fibroblasts in the sh-circSEC24A group detected by ELISA were significantly lower than those in the interleukin 1β group and the empty vector group(P<0.01,P<0.001).(4)Western blot assay results showed that compared with the interleukin 1β group and the empty vector group,the expression of the pro-apoptotic factor Bax protein in the sh-circSEC24A group significantly decreased,and the expression of the anti-apoptotic factor Bcl-2 protein increased significantly(P<0.05);apoptosis and related activating factors cleaved-casepase3 and cleaved-casepase8 protein expressions were both reduced(P<0.05).(5)ELISA and western blot assay results showed that compared with the interleukin 1β group and the empty vector group,the sh-circSEC24A group had lower levels of matrix metalloproteinase 9 and matrix metalloproteinase 13 protein(P<0.05).These findings indicated that the expression of circSEC24A was abnormally increased in human synovial fibroblasts induced by interleukin 1β.Knocking down circSEC24A expression could promote the proliferation of human synovial fibroblasts and inhibit apoptosis,inflammatory factor release,and extracellular matrix degradation,suggesting that circSEC24A may be an important intervention target for early osteoarthritis.

5.Research progress in chemical compositions and pharmacological effects of Dachengqi Decoction and predictive analysis on its quality marker

Dachengqi Decoction is a classic prescription attacked by Yangming excessive syndromes in clinic, which has the effects of relieving heat, softening and dispersing knots, etc., and is often used in the treatment of gastrointestinal dysfunction caused by various diseases. This article reviewed the recent studies on the chemical compositions and pharmacological effects of Dachengqi Decoction in recent years. On this basis, combined with the "five principles" of TCM quality markers, the quality markers of Dachengqi Decoction were predicted and analyzed. It is suggested that emodin, Rhein, chrysophanol, aloe-emodin, synephrine, hesperidin, naringin, magnolol and magnolol can be used as quality markers of Dachengqi Decoction.
6.Circular RNAs are involved in the pathogenesis of osteoarthritis through intracellular mechanisms

BACKGROUND:Currently,there is no drug that can completely cure osteoarthritis and its pathogenesis is still unclear.Circular RNAs(circRNAs)are differentially expressed in patients with osteoarthritis and are closely associated with various pathological processes in osteoarthritis.circRNAs play an important role in various physiological and pathological processes,such as chondrocyte homeostasis,extracellular matrix formation,and inflammatory response. OBJECTIVE:To mainly review the effects of circRNAs on pathological factors related to osteoarthritis,as well as the types and expression levels of circRNAs in osteoarthritis. METHODS:Related articles published from 1976 to August 2023 were retrieved from CNKI,WanFang,VIP,PubMed,Medline,Web of Science and Elsevier databases.The keywords were"osteoarthritis,circular RNA,non-coding RNA,synovial tissue,chondrocytes"in Chinese and English,respectively.All the relevant articles were screened,summarized,analyzed,and finally 69 papers were included in the review. RESULTS AND CONCLUSION:circRNAs are non-coding RNAs widely found in eukaryotic cells,with covalently closed continuous loop structure,but with no 5'hat structure and 3'poly A tail,which are involved in multi-gene and multi-target regulatory networks and cannot be degraded by nucleic acid exonucleases(RNase R).circRNAs have a high abundance,high conservativeness and stability,and cell and tissue specificity.circRNAs have biological functions such as acting as molecular sponges for miRNAs,regulating linear RNA transcription and RNA shearing,interacting with RNA-bound proteins,and translating proteins.circRNAs regulate chondrocyte apoptosis and proliferation,degradation of cartilage extracellular matrix,and inflammation and other physiopathologic processes.circRNAs are expected to become biomarkers and potential therapeutic targets for clinical diagnosis and prognosis of osteoarthritis,and may become a new strategy for clinical treatment of osteoarthritis in the future.

Objective:To analyze the incidence and epidemiological characteristics of traumatic spinal cord injury in China in 2018.Methods:Multi-stage stratified cluster sampling was used to randomly select hospitals capable of treating patients with spinal cord injury from 3 regions,9 provinces and 27 cities in China to retrospectively investigate eligible patients with traumatic spinal cord injury admitted in 2018. National and regional incidence rates were calculated. The data of cause of injury,injury level,severity of injury,segment and type of fracture,complications,death and other data were collected by medical record questionnaire,and analyzed according to geographical region,age and gender.Results:Medical records of 4,134 patients were included in this study,with a male-to-female ratio of 2.99∶1. The incidence of traumatic spinal cord injury in China in 2018 was 50.484 / 1 million (95% CI 50.122-50.846). The highest incidence in the Eastern region was 53.791 / 1 million (95% CI 53.217-54.365). In the whole country,the main causes of injury were high falls (29.58%),as well as in the Western region (40.68%),while the main causes of injury in the Eastern and Central regions were traffic injuries (31.22%,30.10%). The main injury level was cervical spinal cord in the whole country (64.49%),and the proportion of cervical spinal cord injury in the Central region was the highest (74.68%),and the proportion of lumbosacral spinal cord injury in the Western region was the highest (32.30%). The highest proportion of degree of injury was incomplete quadriplegia (55.20%),and the distribution pattern was the same in each region. A total of 65.87% of the patients were complicated with fracture or dislocation,77.95% in the Western region and only 54.77% in the Central region. In the whole country,the head was the main combined injury (37.87%),as well as in the Eastern and Central regions,while the proportion of chest combined injury in the Western region was the highest (38.57%). A total of 32.90% of the patients were complicated with respiratory complications. There were 23 patients (0.56%) died in hospital,of which 17(73.91%) died of respiratory dysfunction. Conclusions:The Eastern region of China has a high incidence of traumatic spinal cord injury. Other epidemiological features include high fall as the main cause of injury cervical spinal cord injury as the main injury level,incomplete quadriplegia as the main degree of injury,head as the main combined injury,and respiratory complications as the main complication.
8. Application of arthroscopic video teaching in clinical practice teaching of sports medicine

Objective:
To investigate the effect of arthroscopic video teaching in clinical practice teaching of sports medicine.
Methods:
The undergraduate students of clinical medicine in Xinjiang Medical University who received practice teaching from April 2018 to July 2018 were selected and randomly divided into experimental group and control group. Students in the experimental group received arthroscopic video teaching, and those in the control group received traditional teaching. After the teaching ended, examination scores and pass rate were analyzed, and the degree of satisfaction with teaching method among students was also evaluated.
Results:
Compared with the control group, the experimental group was better at learning the anatomy of tissue and organs, understanding the pathological features and diagnosis of disease, understanding the surgical procedures, memorizing diseases and had better interests and efficiency in learning (
9.Approaches to improving the teaching quality of orthopedics in digital and network environment

Based on the digital and network environment, this paper analyzes the problems existing in the teaching of traditional orthopedics in medical colleges and universities, including single teaching form, neglecting the students' practical skills and the phenomenon of students' poor learning initiative. It points out that the orthopedics teaching under the digital and network environment should be integrated through interactive teaching, multimedia teaching, WeChat network teaching, simulation teaching, practice teaching and other teaching forms and should pay attention to the reasonable use of teaching methods, make clear teachers and students' subject status, pay attention to students' learning ability training, so as to improve the teaching quality of orthopedics in medical colleges.
10.Bankart repair under arthroscope in treatment of recurrent anterior shoulder joint instability

Objective To assess the effectiveness of Bankart repair under arthroscope in treatment of recurrent anterior shoulder joint instability. Methods From February 2010 to February 2015, 63 patients with recurrent anterior shoulder dislocation under arthroscipic Bankart repair, and 53 patients (28 male, 25 female) were available for follow-up. The mean age at the time of surgery was 27.9 years (range, 16 ~ 51). All of the 63 patients underwent surgical treatments, and the effectiveness was evaluated with University of California at Los Angeles (UCLA), American Shoulder and Elbow Surgeon scores (ASES), Constant-Murley scores, forward elevation, abduction and external rotation and X-ray. Results The ASES scores, UCLA scores were (97.0 ± 6.8), (29.6 ± 2.5) respectively, improved significantly higher after the surgery (P < 0.05). No significant change was found regarding Constant-Murley scores (99.1 ± 2.4), forward elevation (168.7 ± 2.7)°, abduction and external rotation (72.6 ± 8.7)° compared with the pre-operation. Conclusions Significant difference between pre-operation and post-operation suggests that Arthroscopic Bankart repair is a good option for the treatment of recurrent anterior shoulder dislocation without large glenoid bone loss and poor quality Ligament and joint capsule. Risk factors of recurrence should be evaluated before the surgery to reduce the recurrence rate of dislocation.
