1.Application progress of digital health technology in nutrition management of gestational diabetes mellitus patients

In recent years,using digital health technology can improve the quality and effect of nutrition management for patients with gestational diabetes mellitus(GDM).This article provides an overview of digital health technology,reviews the application form and effect of digital health technology in nutrition manage-ment of pregnant women with GDM,puts forward suggestions,in order to provide references and bases for promoting the more scientific,effective and standardized application of digital health technology in nutrition management of GDM patients.
2.Effects of Polygonatum sibiricum polysaccharides on high-sugar diet-in-duced neuroinflammation in mice

AIM:To elucidate the mechanism by which Polygonatum sibiricum polysaccharides(PSP)miti-gate high-sugar diet-induced neuroinflammation through the gut microbiota-serum metabolome axis.METHODS:Fifty male ICR mice were divided into 5 groups:control group,model group,low-dose(250 mg/kg)PSP group,high-dose(500 mg/kg)PSP group,and donepezil hydrochloride(3 mg/kg)group.The neuroinflammation model was established through administration of high-sugar chow and 10%sucrose water for 12 weeks.Cognitive function assessment was per-formed utilizing the Morris water maze.Hippocampal histopathology was examined by hematoxylin-eosin(HE)staining,activated microglia were assessed via immunofluorescence,and neuronal apoptosis was evaluated by TUNEL assay.Serum and hippocampal levels of interleukin-6(IL-6),IL-1β,tumor necrosis factor-α(TNF-α)and nitric oxide(NO)were quantified using ELISA and Western blot.Gut microbiota diversity and serum untargeted metabolomics analyses were car-ried out employing 16S rRNA sequencing and LC-MS/GC-MS,respectively.Differential metabolites were screened,and key metabolic pathways were enriched using MetaboAnalyst 6.0.Spearman correlation analysis established relationships between gut microbiota,metabolites,and inflammatory factors.RESULTS:Model mice demonstrated increased escape latency(P<0.05 or P<0.01)and decreased platform crossings(P<0.01)compared with controls,which were reversed by PSP treatment(P<0.05 or P<0.01).Treatment with PSP substantially reduced IL-6,IL-1β,TNF-α and NO levels in se-rum and hippocampus(P<0.05 or P<0.01),diminished inflammatory infiltration,inhibited microglial activation,and re-duced neuronal damage.Gut microbiota analysis demonstrated that PSP increased Lactobacillus and Bacteroides abun-dance while reducing Alistipes(P<0.05).Metabolomics identified 15 differential metabolites(including betaine,kyotor-phin and ε-caprolactam)and highlighted the significance of alanine,aspartate and glutamate metabolism pathways.Spear-man analysis revealed that abundance of Alistipes and Bacteroides were positively correlated with IL-1β(P<0.05),while abundance of Lactobacillus was negatively correlated with inflammatory factors(P<0.05 or P<0.01).Key metabolites(be-taine,kyotorphin,ε-caprolactam,trans-cinnamate,cis-zeatin and galactitol)showed strong associations with inflamma-tion factors(P<0.05 or P<0.01).CONCLUSION:Treatment with PSP attenuates neuroinflammation through modula-tion of gut microbiota(Lactobacillus,Bacteroides and Alistipes),regulation of metabolites(betaine,kyotorphin and so on),and targeting amino acid metabolism pathways.
3.Consistency analysis of UBM and ArcScan Insight 100 measurements in the anterior segment of the myopic eye

Objective:To evaluate the repeatability of anterior segment biometry measurements obtained using ultrasound biomicroscopy (UBM) and the ArcScan Insight 100, and to compare the agreement between the two devices.Methods:A cross-sectional study was conducted.Seventy myopic patients (70 eyes) who underwent V4c implantable collamer lens implantation at Henan Eye Hospital from March to May 2023 were included.The ArcScan Insight 100 and UBM were used to measure the following parameters three times: angle-to-angle distance (ATA), sulcus-to-sulcus distance (STS), anterior chamber depth (ACD), crystalline lens rise (CLR), anterior chamber width (ACW), ciliary body inner diameter (CBID), anterior chamber angle (ACA), trabecular-ciliary angle (TCA) and maximum ciliary body thickness (CBTmax). The repeatability of anterior segment biological measurements obtained using the two devices was assessed by intraclass correlation coefficient (ICC). The consistency between the two instruments was evaluated by Bland-Altman consistency test.This study complied with the Declaration of Helsinki and was approved by the Ethics Committee of Henan Eye Hospital (No.HNEECKY-2021[13]). All patients understood the purpose and significance of this study and signed the informed consent form.Results:The repeatability of ATA, STS, ACD, CLR, ACW, CBID, ACA, TCA and CBTmax measured by UBM and ArcScan Insight 100 was good (all ICC>0.9). There was no significant difference in ACD, ACW, and ACA between the two instruments ( t=0.696, -1.025, -1.447; all P>0.05). ATA, STS, CLR and CBID measured by UBM were lower and TCA and CBTmax were higher than those measured by UBM ArcScan Insight 100, and the differences were statistically significant ( t=-8.586, -12.551, -4.481, -4.420, 4.535, 7.812; all P<0.05). The differences of ATA, STS, ACD, CLR, ACW, CBID and CBTmax between UBM and ArcScan Insight 100 were 0.38, 0.47, -0.01, 0.07, 0.3, 0.26 and -0.21 mm, respectively, with the 95% limits of agreement (LoA) of (-0.34, 1.10), (-0.15, 1.09), (-0.28, 0.26), (-0.20, 0.35), (-0.33, 0.93), (-0.71, 1.23) and (-0.64, 0.23)mm, respectively, which showed good coherence.The differences in ACA and TCA measurements were 2.26° and -7.81°, respectively, and the 95%LoA values were (-23.36°, 27.89°) and (-36.05°, 20.43°), respectively, with poor coherence.There was a strong positive correlation in ACD measurements measured by UBM and ArcScan Insight 100 ( r=0.827, P<0.05). There were moderate positive correlations in ATA, STS, CLR, ACW and CBID ( r=0.678, 0.749, 0.617, 0.765, 0.519; all P<0.05). There was no significant correlation in ACA, TCA and CBTmax ( r=0.270, 0.032, 0.178; all P>0.05). Conclusions:The repeatability of ArcScan Insight 100 and UBM in measuring anterior segment biological parameters is good.However, the consistency of ACA, TCA and CBTmax measured by the two instruments is poor and may be affected by self-regulation of the body.ATA, STS, ACD, CLR, ACW and CBID have good consistency and can be used interchangeably.

7.ROS-removing nano-medicine for navigating inflammatory microenvironment to enhance anti-epileptic therapy.

As a neurological disorder in the brain, epilepsy is not only associated with abnormal synchronized discharging of neurons, but also inseparable from non-neuronal elements in the altered microenvironment. Anti-epileptic drugs (AEDs) merely focusing on neuronal circuits frequently turn out deficient, which is necessitating comprehensive strategies of medications to cover over-exciting neurons, activated glial cells, oxidative stress and chronic inflammation synchronously. Therefore, we would report the design of a polymeric micelle drug delivery system that was functioned with brain targeting and cerebral microenvironment modulation. In brief, reactive oxygen species (ROS)-sensitive phenylboronic ester was conjugated with poly-ethylene glycol (PEG) to form amphiphilic copolymers. Additionally, dehydroascorbic acid (DHAA), an analogue of glucose, was applied to target glucose transporter 1 (GLUT1) and facilitate micelle penetration across the blood‒brain barrier (BBB). A classic hydrophobic AED, lamotrigine (LTG), was encapsulated in the micelles via self-assembly. When administrated and transferred across the BBB, ROS-scavenging polymers were expected to integrate anti-oxidation, anti-inflammation and neuro-electric modulation into one strategy. Moreover, micelles would alter LTG distribution in vivo with improved efficacy. Overall, the combined anti-epileptic therapy might provide effective opinions on how to maximize neuroprotection during early epileptogenesis.
8.Analytical performance of a candidate reference measurement procedure for serum 17α-hydroxyprogesterone based on liquid chromatography tandem mass spectrometry

Objective:To determine the analytical performance of a candidate reference measurement procedure for 17α-hydroxyprogesterone in human serum by liquid chromatography-tandem mass spectrometry (LC-MS/MS).Methods:The serum spiked with a deuterium-labeled internal standard was extracted from serum from individual undergoing physical examination by liquid-liquid extraction with n-hexane/ethyl acetate (3∶2, v/v), separated by C18 reversed-phase chromatography and detected by positive electrospray ionization mass spectrometry. According to the Clinical and Laboratory Standards Institute (CLSI) C62-A documents, the analytical performance including linearity, limit of detection,limit of quantitation,relative matrix effect,precision and trueness,carry-over and specificity was evaluated.Results:The linear range of 17α-hydroxyprogesterone by LC-MS/MS was 0.21-119.67 μg/L. The limit of detection and limit of quantitation were 5.218 ng/L and 0.116 μg/L. The relative matrix effects were -0.02%, -0.40% and -0.90% for sera and solution mixtures in 3 different ratios (50∶50, 80:20 and 20∶80). The coefficients of variation ( CVs) of intra-assay were 1.73%-2.11%, 0.98%-1.71%, 0.47%-0.87% at 0.164 μg/L, 14.81 μg/L, 81.63 μg/L and the CVs of inter-assay were 1.82%, 1.03%, 0.80% at above three concentrations. The average recovery rates of 3 levels (0.5, 20 and 100 μg/L) were 100.4%, 101.7%, 102.8%, respectively. The measured values of GBW09829 of National Institute of Metrology were within the specified uncertainty range. Conclusion:The candidate reference measurement procedure for 17α-hydroxyprogesterone in human by LC-MS/MS is established with good accuracy and precision, which can be clinically used for measurement traceability.
9.Application of ultrasound in diagnosis of thyroid nodular disease

Objective To explore the diagnostic value of ultrasound in thyroid nodular disease.Methods Eighty-seven patients with thyroid nodule in hospital from 2010-2011 were taken ultrasound examination,including 23 cases with benign nodules and 64 cases with malignant nodules.Univariate analysis and mutivariate Logistic regression analysis were taken to test the ultrasonic index (internal echo,boundary,size,shape,microcalcifications,and aspect ratio) for discovery of the best diagnostic ultrasonic index to identify benign and malignant nodules.Results The ultrasound in the patients with malignant thyroid nodules showed the hypoecho,unclear border,irregular shape,microcalcifications,low blood supply,the aspect ratio > 1.The ultrasound in the patients with benign thyroid nodules showed high or equal echo,clear border,regular shape,high blood supply,and the aspect ratio≤ 1.The ultrasonic indices between two groups had statistically significant difference (P < 0.05).The shape and microcalcifications were the best ultrasonic diagnostic index to indentify the benign and malignant nodules with an EXP(B) 20.12 and 19.14,respectively.Conclusions The ultrasound is helpful in the diagnosis of thyroid nodular disease and identification of benign and malignant thyroid nodules.The ultrasonic images are the basic data for the clinical diagnosis of thyroid nodules.
10.Clinical research on low molecular heparin for preventing perioperative lower limb deep vein thrombosis in elderly hip peripheral fracture

Objective Toobservetheeffectofperioperativeuseoflowmolecularheparin(LMH)forpreventinglowerlimbdeep vein thrombosis(DVT ) in elderly hip peripheral fracture .Methods 105 cases of elderly hip peripheral fracture from July 2007 to July 2011 were divided into the LMH group(65 cases) and the conventional treatment group(40 cases) .The two groups were per-formed the physical therapy for preventing lower limb DVT .The LM H group was added with LM H for preventing DVT .The sta-tuses of DVT occurrence and the coagulation related indicators were compared between the two groups .Results In the LM H group ,the thrombus events happened in 4 cases ,accounting for 6 .15% .In the conventional treatment group ,thrombosis occurred in 13 cases ,accounting for 32 .5% .The difference between the two groups had statistical significance (P<0 .05) .Hemoglobin on post-operative 1 ,10 d was obviously improved ,the difference had statistical significance(P<0 .05) .Conclusion Using LMH has rela-tively ideal effect for preventing lower limb DVT in elderly hip peripheral fracture .
