1.Impact of problematic support on self-management of elderly patients with chronic heart failure

Objective:To explore the impact of problematic support on self-management of elderly patients with chronic heart failure (CHF), so as to provide a theoretical basis for optimizing social support strategies.Methods:From February 2022 to December 2024, patients with CHF in Taizhou Hospital of Zhejiang Province were selected by convenience sampling method as study subjects. Patients were surveyed using the General Information Questionnaire, the Self-management Rating Scale for Patients with Heart Failure, and the Problematic Support Scale (PSS) .Results:A total of 509 questionnaires were distributed and 502 valid questionnaires were recovered, with a valid recovery rate of 98.62%. Among 502 CHF patients, the total score of the Self-management Rating Scale for Patients with Heart Failure was (85.69±12.66) and the mean of the items was (2.68±0.40), and the total score of the PSS was (32.52±4.40) and the mean of the items was (3.25±0.44). Hierarchical regression analysis showed that age, education level, personal monthly income, cardiac function classification, and problematic support were influencing factors of self-management in CHF patients ( P<0.05) . Conclusions:The self-management of elderly patients with CHF needs to be improved, and there is a moderate level of problematic support. Problematic support undermines the self-management of elderly CHF patients, and it is recommended that support strategies be optimized to provide support that precisely matches patients' needs in order to enhance their self-management.

3.Effect of continuous nursing intervention on the ability of self-management and cancer pain of patients with non-small cell lung cancer during the interval of chemotherapy

Objective:To explore and analyze the effect of continuous nursing intervention on the self-management ability and cancer pain of patients with non-small cell lung cancer during the interval of chemotherapy.Methods:Totally 86 patients with non-small cell lung cancer admitted to our hospital from March 2016 to December 2018 were selected as the study objects. According to the random number table method, they were divided into two groups. The control group received routine discharge guidance, and the observation group received continuous nursing intervention under multi-disciplinary cooperation. The self-management ability, cancer pain and quality of life of the two groups were compared before and after nursing.Results:Compared with before nursing, the scores of self-management ability of the two groups were improved, and the observation group was (128.88±8.88) points higher than (88.15±3.69) points in the control group ( t value was 27.775, P<0.01); compared with that before nursing, the Visual Analogue Scale (VAS) scores of the two groups after nursing decreased, the VAS scores of the observation group were (1.02±0.28) points, lower than that of the control group (3.15±0.15) points, the difference was statistically significant ( t value was 43.971, P<0.01); compared with that before nursing, the quality of life in the two groups improved after nursing, the scores in the observation group were (85.28±5.88), (88.02±6.88), (89.08±6.18), (90.02±2.57), (89.96±3.68) points, higher than (63.15±3.85), (65.05±4.28), (66.28±5.16), (67.15±3.25), (62.12±2.58) points in the control group, the difference was statistically significant ( t values were 9.637-12.632, P<0.01) . Conclusion:the continuous nursing intervention under multi-disciplinary cooperation can improve the self-management ability and quality of life of patients with non-small cell lung cancer during the chemotherapy interval, and reduce cancer pain.
4.Hyperbilirubinemia models caused by hemolysis in newborn rhesus monkeys

Objective To establish the newborn rhesus monkey model of hemolytic hyperbilirnbinemia and provide an experimental basic model for research of hyperbilirubinemia.Methods Sixteen 3-day old newborn rhesus monkeys were divided into experimental group and control group,with 8 newborn rhesus monkeys in each group.Eight newborn rhesus monkeys in experimental group were treated with intravenous injection of l0 g/L phenylhydrazine hydrochloride (50 mg/kg) to establish model of homolytic hyperbilirubinemia.The newborn rhesus monkeys in control group were treated with intravenous injection of 9 g/L saline at the same time.Twenty-four hours and 48 hours after the experimental treatment,the bilirubin in blood was detected to evaluate the models,and the clinical manifestations of newborn rhesus monkeys with hyperbilirubinemia were recorded by using monitoring equipment.The brain slices were made to evaluate the model in 1 dead monkeys of experimental group.Results The newborn rhesus monkey of experimental group showed obvious skin,sclera jaundice and hemoglobinuria.The serum total bilirubin [(252.76 ± 63.42) μmol/L],unconjugated bilirubin[(165.85 ±44.93) pmol/L] and conjugated bilirubin [(87.16 ±21.22) μmol/L] in the experimental group were significantly higher than those [(20.62 ± 5.72) μmol/L,(7.93 ± 2.31) μmol/L,(12.51 ± 3.53) μmol/L] in the control group,and the differences were statistically significant (t =14.581,13.881,14.040,all P < 0.01).The level of hemoglobin [(47.18 ± 10.09) μmol/L] in the experimental group was significantly lower than that of the control group [(136.85 ± 13.48) μmol/L],and the difference was statistically significant (t =-21.308,P < 0.01).The results of pathological showed brain edema,rupture and eosinophilic and bilirubin deposition in the basal nuclei,and necrosis appeared in some severe parts.And there were different degrees of retardation and coordination disorders in the experimental group(s) newborn rhesus monkeys,but gradually returned to normal in 4 months later.Conclusion Intravenous injection of phenylhydrazine hydrochloride can be used to produce newborn rhesus monkey models of hemolytic hyperbilirubinemia.
5.Establishment of SD neonatal rat hippocampal neuron models with human cytomegalovirus infection in vitro and preliminary study on calcium metabolism

Objective To investigate the influence of primary cultured neonatal rat hippocampal neurons caused by human cytomegalovirus (HCMV AD169) infection on intracellular calcium and its mechanism.Methods Twenty SPF SD rats born within 24 hours(10 cases of male and 10 cases of female) were assigned to establish the primary rat hippocampal neuronal monolayer cells; After cultured 8 days in vitro,the eligible cells were randomly divided into HCMV infection group,HCMV + MK-801 group,MK-801 group and control group,with 10 wells in each group.The fluorescence intensity values of the intracellular free calcium were detected after 24 hours of treatment with Fluo-3AM fluorescence staining.Results Inoculation of HCMV neurons after 24 h turned to round and swollen gradually,and 4days later,most of the cells disappeared; by immunohistochemistry in cultures of hippocampal neurons in HCMV,visible early proteins,brownish yellow granules,hematoxylin were found after being stained with brown pigment.The fluorescence intensity values of neuronal intracellular calcium (215.5 ± 14.9) in HCMV group was higher than that of control group (116.4 ± 5.9) (t =15.2,P < 0.01),whilerise,that in MK-801 group (88.1 ± 4.5) was significantly lower than that of control group,with decreased rate of (24.0 ± 6.7) % (t =-9.3,P < 0.01).The fluorescence intensity values of neuronal intracellular calcium in HCMV + MK-801 group (135.5 ± 8.6) was significantly decreased compared with that of HCMV group (215.5 ± 14.9),with decreased rate of (37.0 ± 3.4) % (t =11.3,P < 0.01).Conclusions Intracellular calcium overload of cultured rat hippocampal neurons in vitro with HCMV AD16 strains infection can be detected.One of its main mechanisms is the N-methyl-D-aspartic acid receptor channel-mediated calcium influx.
6.Dynamic expressions of interleukin-1 receptor associated kinase-4 and interleukin-1 receptor associated kinase-M in rats with endotoxin tolerance and their significance

Objective To explore the dynamic expressions of interleukin-1 (IL-1)receptor associated kinase (IRAK)-M and IRAK-4 in rats with or without endotoxin tolerance (ETT)in acute liver failure (ALF).Methods Sixty-six male SD rats were divided into three groups:ALF group,ETT group and control group.The rats in ETT group received daily lipopolysaccharide (LPS)intraperitoneal injection for 5 days,while the rats in ALF group received daily injection with same volume of 0.75% NaCl solution.Both ETT group and ALF group received intraperitoneal injection with D-galactosamine (D-GalN)and LPS at 24 hours after the 5th injection of LPS or NaCl solution.At 2,6,12,24 and 48 h after D-GalN and LPS injection,the serum levels of tumor necrosis factor-α (TNF-α)and interleukin-6 (IL-6)were detected by enzyme-linked immunosorbent assay (ELISA).The liver pathologic changes were observed with HE staining by microscope.The mRNA expressions of IRAK-4,IRAK-M and NF-κB (p65)were detected by reverse transcriptase polymerase chain reaction (RT-PCR).The pairwise comparison between groups was done by lease significant difference (LSD)and Dunnet's t test.Results The liver pathologic changes in ETT group were much milder than those of ALF group.In ALF group,IRAK-4 mRNA/β-actin absorbance ratios at 2,6,12,24and 48 h after D-GalN and LPS challenge were 0.711 ±0.074,0.904±0.118,1.012 ±0.098,1.534±0.279 and 1.451±0.290,respectively,while the IRAK-M mRNA/β-actin absorbance ratios were 0.496±0.018,0.516±0.089,0.503±0.023,0.503±0.057 and 0.469±0.142,respectively.In ETT group,the IRAK-4 mRNA/β-actin absorbance ratios at 2,6,12,24 and 48 h after D-GalN and LPS challenge were 0.619±0.083,0.587±0.033,0.623±0.034,0.720±0.044 and 0.654±0.041,respectively,while the IRA'K-M mRNA/β-actin absorbance ratios were 0.929 ± 0.064,1.111±0.138,1.113±0.027,1.891±0.315 and 1.710±0.303,respectively.The IRAK-M mRNA expression level was significantly increased and IRAK-4 mRNA expression level was relatively decreased in ETT group compared to ALF group.The differences were all statistically significant at 2,6,12,24 and 48 h after D-GalN and LPS challenge (F= 17.305,54.921,121.031,67.607,55.279,respectively; F=19.506,43.777,110.823,302.681,202.822,respectively; F=172.003,59.519,987.055,68.463,96.601,respectively; all P<0.05).Conclusion LPS pretreatment may induce ETT in rat model by downregulating the expression of IRAK-4 and upregulating the expression of IRAK-M.
7.Preparation and purification of monoclonal antibodies against human chorionic gonadotrophin

Purpose To prepare monoclonal antibody against hCG. Methods Balb/c mice were immunized with hCG, and spleen cells from the mice were fused with myeloma cells SP2/0 at ratio of 5:1. Positive clones were screened by indirect enzyme-linked-immunoadsorbent assay (ELISA) ,then the clones were subcloned by limiting dilution and amplified further. After intraperitoneal injection of hybridoma cells, mouse ascites antibody was prepared. Titres and specificity of the ascites antibody were identified and determined by indirect ELISA. The ascites were purified by protein A affinity chromatography. Results Two strains of hybridoma cell lines obtained could secrete MAb stably. The titre of MAb of cell culture supernate is more than 10~(-3), and the titre of prepared ascites MAb is more than 10~(-7).The purity of the obtained monoclonal antibody was more than 98% and recovery reached 75 % after purification. Conclusion The obtained two strains of hybridoma cell lines can secrete MAb stably. The monoclonal antibodies can be used in the research of early pregnancy and tumor diagnosis .
8.Isolation,identification and sequence analysis of the strain JL/07/SW with deletion mutations of PRRSV

One strain of PRRSV was isolated from tissue of piglets died of obvious respiratory syndrome and high fever.It was identified as a strain of American type PRRSV by serological test and gene sequencing.According to American type PRRSV VR-2332,we designed 3 pairs of primers to aim directly at N gene,GP5 gene and variation sequence of NSP2.The results of sequence analysis indicated a discontinuous deletion of 30 amino acids in nonstructural protein 2 (NSP2).N and GP5 gene is conservative relatively.
