Vertebral refracture following percutaneous vertebral augmentation (PVA) is commonly seen in elderly patients with osteoporotic thoracolumbar compression fractures (OTLCF). It can lead to recurrent pain, loss of vertebral height, progression of kyphosis, and even neurological dysfunction, significantly impairing patients′ quality of life. Current diagnosis and treatment face multiple challenges, including high misdiagnosis rate, difficulty in choosing between surgical and non-surgical treatment options, lack of standardized surgical protocols, interference from intralesional bone cement during procedures, inadequate stability of internal fixation in osteoporotic bone, and suboptimal compliance of anti-osteoporotic therapy. Establishing a standardized diagnostic and therapeutic framework is urgently needed. To standardize the management process and improve outcomes for vertebral refractures after PVA in elderly OTLCF patients, Spinal Trauma Group of the Orthopedic Branch of Chinese Medical Doctor Association organized experts in the field to develop Guideline for the diagnosis and treatment of vertebral refracture after percutaneous vertebral augmentation in elderly patients with osteoporotic thoracolumbar compression fractures ( version 2025), based on current literature and clinical experience, and adhering to principles of scientific rigor and clinical applicability. A total of 11 recommendations were proposed, encompassing diagnosis, treatment, and rehabilitation of vertebral refracture after PVA in elderly patients with OTLCF, aiming to provide a foundation for a standardized management.

High-risk carriers were identified based on the genetic screening results. This study was approved by the Medical Ethics Committee of Chongqing Health Center for Women and Children (Ethics No.2021-RGI-02). Results:① Among the 17 926 women of childbearing age, 298 (1.66%) were identified as heterozygous carriers, including 278 (1.55%) with concurrent deletions of E7 and E8, and 20 (0.11%) with isolated deletions of E7. ② Seven high-risk couples were identified, six of whom were prenatal couples. Of the two fetuses from these high-risk pregnancies, both exhibited heterozygous deletions of E7 and E8 in the SMN1 gene, while four fetuses showed no abnormalities.
CONCLUSION
This study provides a comprehensive assessment of the carrier frequency of SMA among women of childbearing age in Chongqing, offering valuable data for the primary and secondary prevention of SMA-related birth defects in the region.

6.Summary of best evidence for screening and evaluating the risk of sarcopenia in gastric cancer patients

Objective:To collect the best evidence for screening and evaluation of sarcopenia risk of gastric cancer patients at home and abroad, and to provide guidance for screening and accurate evaluation of gastric cancer sarcopenia patients.Methods:The computer was used to search literature related to screening and evaluation of sarcopenia risk of gastric cancer patients in UpToDate, BMJ Best Clinical Practice, Australia Joanna Briggs Institute Evidence-based Health Care Centre Database, International Guide Library, China Medlive Guideline network, National Institute for Health and Clinical Excellence, Cochrane Library, Registered Nurses' Association of Ontario, PubMed, Embase, Geriatrics Branch of the Chinese Medical Association, China National Knowledge Infrastructure, Wanfang and China Biology Medicine disc. The search time was from database establishment to September 1, 2023. Literature selection and quality evaluation were conducted independently by two researchers with evidence-based knowledge training, and evidence extraction and summary were carried out for literatures meeting the requirements.Results:A total of 18 literatures were included, including four guidelines, five expert consensus, four systematic reviews and five diagnostic studies. The best evidence summarized included a total of 18 best evidence from 5 aspects, such as risk factors of sarcopenia in gastric cancer patients, screening time, screening content, evaluation content, diagnostic criteria and staging.Conclusions:The summary of evidence provides evidence-based evidence for improving the accuracy of risk screening for gastric cancer sarcopenia patients. Medical staff can refer to the best summary of this study, combine clinical practice with the characteristics of gastric cancer patients and choose evidence reasonably.
7.Basic public health service personnel allocation in five regions of Guizhou province

Objective:To investigate basic public health service personnel allocation in five regions of Guizhou province, providing reference and strategies for the construction of grass-roots disease prevention and control system in Guizhou province and the training of grass-roots public health service talents in colleges and universities.Methods:According to the topographical features of Guizhou province, stratified random sampling was completed in five regions including Guiyang, Zunyi, Liupanshui, Qianxinan Buyei and Miao Autonomous Prefecture, Qiannan Buyei and Miao Autonomous Prefecture. Questionnaires and on-the-spot symposiums were conducted among basic public health service personnel from 20 township health centers and 20 community health service centers.Results:Women (82.7%), 25-35 years of age (41.7%), working years < 5 years (65.7%), and junior professional titles (59.7%) accounted for a higher proportion of the staff in the five regions. Basic public health service personnel in Guiyang had the highest percentage of undergraduate education (47.5%) and those in other regions had the highest percentage of a junior college education. 40.3% and 26.4% of basic public health service personnel were devoted to nursing and clinical specialties, and only 3.2% of basic public health service personnel were devoted to general practice and preventive medicine. The number of public health practitioners (assistants) per 10,000 residents was 0.05, and 43.5% of public health practitioners had multiple duties.Conclusion:The professional structure of public health personnel is not reasonable at the grass-roots level in Guizhou province. There is an extreme shortage of public health practitioners (assistants), the professional title is low, staffing is inadequate, and staff loss is serious.
8. Breakthrough of invasive fungal disease with posaconazole as primary prophylaxis after induction chemotherapy for acute myeloid leukemia

Objective:
To investigate the breakthrough incidence of invasive fungal disease(IFD) and side effects of posaconazole as primary prophylaxis during induction chemotherapy for acute myeloid leukemia(AML).
Methods:
A total of 206 newly diagnosed AML patients admitted to our department during January 2016 and December 2018 were enrolled in the study. Exclusive criteria were as followings including patients diagnosed as acute promyelocytic leukemia; those who received intravenous antifungal therapy after admission or had history of IFD one month before induction chemotherapy, or those with functional insufficiency of vital organs and those older than 65. Forty-seven patients received posaconazole (posaconazole group), 61 cases received voriconazole (voriconazole group) and 98 cases did not receive any prophylaxis (control group) during induction chemotherapy. Prophylactic efficacy and safety between posaconazole and voriconazole were compared.
Results:
During induction chemotherapy, five possible cases of IFD occurred in posaconazole group (10.6%); while 11 cases (18.0%) were in voriconazole group including 7 possible, 3 probable and 1 proven. Thirty-five cases (35.7%) in control group were diagnosed as IFD including 19 possible, 11 probable and 5 proven ones. The incidences of IFD in posaconazole and voriconazole group were significantly lower than that in control group (

Percutaneous vertebral augmentation is the most commonly used minimally invasive surgery in the treatment of osteoporotic vertebral compression fractures(OVCF).The efficacy and safety have been widely recognized,however,there are still some complications of percutaneous vertebral augmentation.And the most common complication is bone cement leakage,which can lead to nerve root and spinal cord injury,increase the risk of secondary fractures of adjacent vertebral,and even cause serious complications such as pulmonary embolism.The diagnosis,classification,hazard,influence factors and preventive measures of bone cement leakage in vertebral augmentation for OCVF are reviewed in this paper,which will provide reference for prevention and treatment of bone cement leakage in clinical practice.
10. Effect of FLT3-ITD with DNMT3A R882 double-mutation on the prognosis of acute myeloid leukemia after allogeneic hematopoietic stem cell transplantation

Objective:
To investigate the impact of FLT3-ITD and DNMT3A R882 double mutations to the prognosis of acute myeloid leukemia after allogeneic hematopoietic stem cell transplantation (allo-HSCT).
Methods:
FLT3-ITD, DNMT3A, C-kit, CEBPA, FLT3-TKD and NPM1 mutations were detected in 206 newly diagnosed AML patients by Sanger sequencing (M3 and those received FLT3 inhibitor were excluded). Clinical data of AML patients were retrospectively analyzed to compare the prognosis of each gene mutation group.
Results:
①Of 206 patients, 104 were male and 102 female with a median age of 38 (3-63) years, including 6 cases of M0, 24 cases of M1, 56 cases of M2, 39 cases of M4, 63 cases of M5, 6 cases of M6 and 12 unclassified cases. ②All 206 patients were divided into four groups according to the mutation gene at the time of diagnosis: FLT3-ITD+ DNMT3A R882+ group (group A), FLT3-ITD+ DNMT3A R882- group (group B), FLT3-ITD- DNMT3A R882+ group (group C) and FLT3-ITD- DNMT3A R882- groups (group D). Gender, leukocyte count at diagnosis, chromosome karyotype, the median age, FAB classification, disease status prior to transplantation, type of donor, conditioning regimen and GVHD were not significantly different between four groups (
