1.Comparative Study on the Effects of Different Dosage Forms of Yinqiao Powder (银翘散) on Inflammation and Immunity in Acute Pharyngitis Model Rats

ObjectiveTo compare the effects of different formulations of Yinqiao Powder (银翘散, YP) on inflammation and immune function in a rat model of acute pharyngitis induced by ammonia water. MethodsA rat model of acute pharyngitis was established by spraying 15% ammonia water into the pharynx of male SD rats using a laryngeal atomizer twice daily (three sprays each time) for three consecutive days. Successfully modeled rats were randomly divided into 5 groups, including a model group (normal saline, 10 ml/kg), YP decoction group (10.73 g/kg), YP granule group (10.73 g/kg), cydiodine group (0.39 mg/kg) and blank control group (normal saline, 10 ml/kg), with 12 rats in each group. All groups received intragastric administration once daily for 10 consecutive days. After the final gavage administration, frequency of back arching per minute, frequency of vibrissae grooming per minute, and grooming episodes per five minutes were recorded in rats. The cumulative body weight change was calculated as the difference in body weight between day 10 and day 1 of gavage administration. Behavioral observations were performed using the open field test, and the total distance traveled was recorded. After sample collection, the lung index and thymus index were calculated. Histopathological changes in the pharyngeal and tracheal tissues were observed using HE staining. Serum inflammatory markers including eosinophil cationic protein (ECP), angiotensin-converting enzyme 2 (ACE2), and C-reactive protein (CRP), as well as immune parameters including immunoglobulin M (IgM), immunoglobulin A (IgA), immunoglobulin G (IgG), CD4, CD4/CD8 ratio, and CD8 were measured by ELISA. In addition, ELISA was used to determine the levels of inflammatory mediators in pharyngeal tissues, including phospholipase A2 (PLA2), cyclooxygenase-1 (COX-1), cyclooxygenase-2 (COX-2), prostaglandin E2 (PGE2), vascular cell adhesion molecule-1 (VCAM-1), C-X-C motif chemokine ligand 1 (CXCL1), vascular endothelial growth factor (VEGF), and matrix metalloproteinase-9 (MMP-9). ResultsCompared to the blank control group, the model group exhibited significantly increased frequencies of back arching per minute, vibrissae grooming per minute, and grooming episodes per five minutes, and lung index, and decreased cumulative body weight change, thymus index, and total distance traveled, as well as elevated serum levels of ECP, ACE2, and CD8, reduced levels of IgM, IgA, IgG, CD4, and CD4/CD8 ratio, and increased levels of PLA, COX-1, COX-2, PGE2, VCAM-1, CXCL1, VEGF, and MMP-9 in pharyngeal tissues (P<0.05). Histopathological examination revealed keratinization of the superficial epithelial layer of the pharyngeal mucosa, accompanied by congestion and edema in the lamina propria and infiltration of inflammatory cells; in the tracheal mucosa, extensive inflammatory cell infiltration, ciliary disruption, and goblet cell damage were observed. Compared to the model group, except for CRP, IgG, and CD4, all above mentioned indicators were improved in the YP granule group, YP decoction group, and cydiodine group (P<0.05). Histopathological lesions in the pharyngeal and tracheal tissues were also alleviated. Compared to the YP decoction group, rats in the YP granule group exhibited decreased frequencies of back arching per minute, vibrissae grooming per minute, and grooming episodes per five minutes, as well as reduced serum ECP levels. Meanwhile, the cumulative body weight gain and serum levels of IgM, IgA, IgG, and CD4/CD8 increased, whereas CD8 levels decreased (P<0.05). ConclusionYP can effectively alleviate symptoms of acute pharyngitis in a rat model, improve inflammatory responses and immune-related indicators, demonstrating superior therapeutic effects in the dosage form of granule.
2.Clinical manifestations and genetic analysis of two patients with familial hypercholesterolemia caused by complex heterozygous variants

Objective:To investigate the gene detection results of 2 patients with familial hypercholesterolemia (FH) caused by complex heterozygous variation, and to clarify the relationship between clinical manifestations and gene variation.Methods:Two patients (patient 1 and 2) with FH who visited Beijing Anzhen Hospital Affiliated to Capital Medical University in 2018 were selected as research subjects. A retrospective study method was used to collect clinical and family history data of the two patients. And 2 mL of peripheral venous blood from each of the two patients was collected, and genomic DNA extraction was performed on the blood samples. Sanger sequencing was used to validate the variant sites of the two patients detected by whole-exome sequencing (WES). Pathogenicity of variants was classified based on the American College of Medical Genetics and Genomics (ACMG) Standards and Guidelines for the Classification of Genetic Variants (hereinafter referred to as the " ACMG Guidelines" ), and the impact of variant was analyzed using multiple bioinformatics tools including SIFT, PolyPhen-2, and SWISS-MODEL. This study has been approved by Beijing Anzhen Hospital Affiliated to Capital Medical University (Ethics No. 2024215X).Results:Patient 1 initially presented with early-onset coronary heart disease, with initial lipid levels of serum total cholesterol (TC) 9.86 mmol/L (normal reference value: 3.10~5.20 mmol/L) and serum low-density lipoprotein cholesterol (LDL-C) 8.37 mmol/L (normal reference value: 1.27~3.12 mmol/L) on admission. Patient 1 initially underwent treatment with rosuvastatin combined with ezetimibe for one month, but the lipid-lowering effect was not significant. The lipid-lowering therapy was then adjusted to atorvastatin combined with ezetimibe and probucol. After one year of treatment, the patient developed paroxysmal chest pain symptoms. A follow-up lipid profile showed a serum TC level of 4.50 mmol/L and a LDL-C level of 3.55 mmol/L. The lipid-lowering regimen was continued, and the serum LDL-C levels were maintained between 2.65 and 3.66 mmol/L. Patient 2 was found to have an abnormally high blood lipid level and carotid artery hardening during physical examination, with an initial blood lipid level of serum TC 11.82 mmol/L and serum LDL-C 9.63 mmol/L. After receiving rosuvastatain therapy, the lipid-lowering effect was significant. WES revealed that patient 1 carried the heterozygous variants c. 1871_1873del(p.Ile624del) and c. 1747C>T(p.His583Tyr) in the LDLR gene (NM_000527.4), while patient 2 carried the heterozygous variants c. 1747C>T(p.His583Tyr) in the LDLR gene and c. 6936_6937inv(p.Ile2313Val) in the APOB gene (NM_000384). According to the ACMG Guidelines, the LDLR gene c. 1747C>T(p.His583Tyr) was classified as a pathogenic variant (PS3+ PM1+ PM2_supporting+ PM5+ PP2+ PP3), and c. 1871_1873del(p.Ile624del) was classified as a pathogenic variant (PS3+ PS4+ PM2_supporting+ PM1+ PM4); the APOB gene c. 6936_6937inv(p.Ile2313Val) was classified as a variant of uncertain clinical significance (PM2_supporting BP4). Conclusions:Patients 1 and 2 in this study were patients with complex heterozygous variant FH, and their genotypic differences may be related to the differences in clinical serum LDL-C levels and the efficacy of hypolipidemic agents.

Objective:To analyze the clinical characteristics and cytokines expression characteristics in infants with mild and severe respiratory syncytial virus (RSV) infection.Methods:From May 2023 to December 2023, plasma samples and clinical information were collected from 16 infants with RSV infection and 14 control infants. Cytek Aurora flow cytometry (Cytek, America) and Enzyme linked immunosorbent assay (ELISA) were used to detect the expression levels of 25 cytokines after mild and severe RSV infection.Results:Cough and nasal obstruction were the main clinical manifestations in infants with mild RSV infection, accompanied by polypnea, wheezing and other symptoms. The main symptoms of severe RSV infection were cough and rales, accompanied by fever and polypnea. In comparison with the control group, the expression levels of IL-2, IL-4, IL-5, IL-6, IL-9, IL-13, IL-22, TNF-α, IFN-α, IFN-β, MIP-1β, I-TAC, ENA-78, GROα, Eotaxin, and MCP-1 in the RSV infection group all exhibited an upregulation trend. Both IP-10 and MIP-3α demonstrated a downward trend in the RSV infection group; however, there was no statistically significant difference ( P>0.05). The levels of IL-10, IFN-γ, MIP-1α, and IL-8 in the RSV infection group were significantly higher than those in the control group, whereas the levels of MIG, TARC, and RANTES in the RSV infection group were significantly lower than those in the control group ( P<0.05). The levels of IL-2, IL-4, IL-5, IL-6, IL-9, IL-10, IL-13, IL-22, IFN-β, IFN-γ, TNF-α, IL-8, I-TAC, MIP-1β, Eotaxin, and MCP-1 in the mild RSV infection group were significantly higher than those in the severe RSV infection group ( P>0.05). Among these, the levels of MIG, RANTES, TARC, MIP-3α, and ENA-78 in the mild infection group were all lower than those in the severe infection group. The expressions of ENA-78 and MIP-1α in the severe infection group were significantly higher than those in the mild infection group and also higher than those in the control group. There was no significant difference in IP-10 and GROα between the mild and severe RSV infection groups ( P>0.05). Conclusions:The differences in clinical features and cytokines between infants with mild and severe RSV infection provide important data support for the prevention and treatment of RSV infection in infants.
5.Identification of Medical Surge Risk Influencing Factors and Analysis of Causal Coupling Relationships Based on DEMATEL-ISM

Objective To identify the key factors affecting the risk of medical surges and their coupling relation5 ships,providing strategic support for medical institutions to optimize risk management and emergency governance.Methods 17 influencing factors were determined based on WSR theory,and an expert scoring method was employed to assess the impact strength among the factors.The DEMATEL method was applied to calculate the centrality,cau5 sality,influence,and being influenced degrees of the influencing factors.The ISM method was used to construct a hierarchical structure of the influencing factors related to medical surge risks,thereby revealing the connections and interaction mechanisms among these factors.Results Seven critical influencing factors were identified,including the crisis decision-making capacity and leadership effectiveness of emergency managers,the completeness of the emer5 gency system and dynamic execution capabilities,and the cross-departmental coordination mechanism and com5 mand collaboration efficiency.Deep driving factors and coupling pathways were also revealed.Conclusion The risk of medical surges exhibits multi-factorial coupling cascade effects;attention should be directed towards the construc5 tion of mid-to-deep level mechanisms such as information systems,institutional frameworks,and organizational management,to enhance targeted capabilities and systemic resilience in risk governance.
