Objective To explore the application value of CT-based radiomics in differentiating pneumonia-type mucinous adenocarcinoma(PTMA)from organizing pneumonia(OP).Methods A total of 52 PTMA patients and 102 OP patients were retrospectively included and randomly divided into training set(n=124)and test set(n=30)in an 8∶2 ratio.Eight PTMA patients and 22 OP patients from another hospital during the same period were included as external validation set(n=30).Clinical characteristics and CT signs of the patients were selected to construct the clinical model.Radiomics features were extracted and dimensionality reduction was performed through the least absolute shrinkage and selection operator(LASSO)algorithm.A radiomics model was constructed and the Radiomics score(Radscore)was calculated.The Radscore was combined with clinical factors to establish the combined model and a nomogram was illustrated.The models' fitting degree was analyzed by the calibration curve,while their efficacy was evaluated by the receiver operating characteristic(ROC)curve and decision curve analysis(DCA).Results The clinical model,established based on the border,cystic space and bronchial leafless tree sign,achieved area under the curve(AUC)of 0.850,0.782,and 0.759 in the training set,test set,and external validation set,respectively.Thirteen features were obtained to construct the radiomics model,with AUC of 0.925,0.865,and 0.830,respectively.The AUC of the combined model were 0.970,0.905,and 0.864,respectively,in which the calibration curve demonstrated good model fitting.DCA indicated that the combined model had the greatest clinical net benefit.Conclusion The combined model based on CT radiomics can effectively distinguish PTMA from OP.

Objective:To evaluate the role and molecular mechanism of laminin β1(LAMB1)in cortical neurons in regulation of glutamate receptors.Methods:Recombinant lentivirus(LV-shLamb1)-mediated knockdown of LAMB1 expression in mouse primary cortical neurons was performed,followed by immunofluorescence staining and Western blot to detect changes in F-actin,glutamate receptor subtypes(AMPA receptors GluR1/GluR2,NMDA receptors NR1/NR2A),and ERK-related protein expression in cortical neurons.Results:LV-shLamb1 significantly inhibited LAMB1 expression in mouse cortical neurons.Concurrently,LV-shLamb1 markedly increased F-actin polymerization,as well as the expression of AMPA receptor subunits GluR1 and GluR2,and NMDA receptor subunits NR1 and NR2A.Further,Western blot detection showed that the phosphorylation level of ERK was significantly increased after LV-shLamb1 infec-tion.Conclusion:LAMB1 is expressed in cortical neurons.Suppression of LAMB1 expression in mouse cortical neu-rons activates the ERK pathway,which in turn promotes the polymerization of the cytoskeletal protein F-actin and the expression of glutamate receptors.This suggests that LAMB1 may regulate F-actin homeostasis and glutamate receptor levels through the ERK pathway,thereby playing a potentially important role in neuronal function.

Objective To analyze the important abnormal results of physical examination of the staff in our hospital in recent 2 years,and to provide evidence for the health management of the staff in our hospital.Methods The physical examina-tion data of hospital staff in recent 2 years were retrospectively analyzed,and the important abnormal results were statistically ana-lyzed.Results A total of 3 584 employees participated in physical examination,including753 males and 2831 females.There were 92 cases with significant abnormal results,and the total detection rate was 2.57%.The detection rate of significant abnor-mal results was slightly lower in males(17cases,2.26%)than in females(75cases,2.65%),and the age of females[(48.89±19.53)years]was lower than that of males[(55.68±17.43)years].The age group with the most significant detec-tion rate of important abnormal results was 50-59 years old(3.07%),and the diseases detected by important abnormal results were as follows:There were 26 thyroid tumors,19 lung tumors,13 breast tumors,12 space occupying cases(3 liver,4 kidney,2 adrenal glands,1 adnexa,1 pancreas,1 mediastinum),9 vascular lesions,6 abnormal tumor markers,5 cervical lesions,1 tuberculosis,and 1 subdural hematoma.Among them,thyroid tumors,breast tumors,lung tumors and cervical lesions were the main cases in women,and vascular diseases,lung tumors,space-occupying lesions(liver and kidney)and thyroid tumors were the main cases in men.Conclusion Physical examination is of great significance in detecting important abnormal diseases.We should attach great importance to physical examination,detect serious diseases as early as possible,and strengthen health man-agement accordingly.

The formation of learning and memory is regulated by synaptic plasticity in hippocampal neurons. Here we explored how gestational exposure to dexamethasone, a synthetic glucocorticoid commonly used in clinical practice, has lasting effects on offspring's learning and memory. Adult offspring rats of prenatal dexamethasone exposure (PDE) displayed significant impairments in novelty recognition and spatial learning memory, with some phenotypes maintained transgenerationally. PDE impaired synaptic transmission of hippocampal excitatory neurons in offspring of F1 to F3 generations, and abnormalities of neurotransmitters and receptors would impair synaptic plasticity and lead to impaired learning and memory, but these changes failed to carry over to offspring of F5 and F7 generations. Mechanistically, altered hippocampal miR-133a-3p-SIRT1-CDK5-NR2B signaling axis in PDE multigeneration caused inhibition of excitatory synaptic transmission, which might be related to oocyte-specific high expression and transmission of miR-133a-3p. Together, PDE affects hippocampal excitatory synaptic transmission, with lasting consequences across generations, and CDK5 in offspring's peripheral blood might be used as an early-warning marker for fetal-originated learning and memory impairment.

Paget disease of the vulva is the most common type of extramammary Paget disease and is a rare clinical malignancy occurring in the vulva. At present, the specific pathogenesis is not clear, and the lesions mainly involve areas rich in apocrine sweat glands, such as the external genitalia. The clinical manifestations show no obvious heterogeneity, and may be characterized by erythema, pruritus, scurf and tingling in the lesion area, with erosion and exudation in severe cases. Diagnosis requires histopathological examination and immunohistochemistry. Paget disease of the vulva is often misdiagnosed as eczema or dermatitis due to the lack of knowledge about this disease in clinicians. This article reviews the current research progress of Paget disease of the vulva, in order to deepen clinicians' understanding of this disease and effectively improve clinical diagnosis and treatment abilities.

Objective:To assess the feasibility and effects of the mobile device-based video teaching in plastic surgery education.Methods:In the study, 22 participants were assigned into two groups according to their standardized training area. The intervention group ( n=11) which was outside Beijing had an unlimited access to video-based education; the control group ( n=11) which was in Beijing received traditional teaching. After the end of the first semester of teaching, the two groups of students were tested on theory and clinical operational theory, and the evaluation results were compared. SPSS 18.0 software was applied for t test and chi-square test. Results:After the first semester, the effect of teaching method was evaluated by the scores of written examination and clinical examination. The average scores of the written test in the intervention group were (38.82±3.22) points, while that in the control group were (38.36±2.98) points, without significant differences between the two groups ( P=0.74); the average scores of the clinical examination theory test in the intervention group and the control group were (46.36±3.44) points and (41.00±5.24) points, respectively, with significant differences between the two groups ( P=0.01). In term of total scores, the average scores of the intervention group were (85.18±4.83) points, and those of the control group were (79.36±5.52) points, with statistical differences between the two groups ( P=0.016). Conclusion:Mobile device-based video teaching in plastic surgery education can not only improve students' performance but also facilitate their clinic skills, which is worth popularizing.

Objective:To investigate the method of the complex labia minora and clitoris prepuce hypertrophy and summarize the experience.Methods:From January 2016 to October 2019, a total of 432 patients with complex labia minora and clitoris prepuce hypertrophy in the 10th Department of Plastic Surgery Hospital, Chinese Academy of Medical Sciences and Peking Union Medical College, were selected by classification. The patients were 18-46 years old (average 28 years old). The redundant prepuce of clitoris was repaired to transform the complex labia minora and clitoris prepuce hypertrophy into simple labia minora hypertrophy, and then labiaplasty was performed by edge or wedge resection.Results:A total of 346 cases were followed up for 3-12 months, 86 cases were lost. Hematoma occurred in 2 cases, bilateral asymmetry in 9 cases, irregular edge in 12 cases, and scar hyperplasia in 1 case. After respective treatment and secondary repair, the patients were satisfied. The overall satisfaction rate was 93.6%(324/346).Conclusions:Our method, through the decomposition operation, will change the complex shape into simple labia minora hyperplasia, reduce the difficulty of operation at the same time, which can achieve more satisfactory appearance.

Objective:To investigate the motivation of patients seeking labiaplasty and the external influencing factors.Methods:From January 2018 to December 2019, 268 patients who sought for operative labiaplasty at the Gynecological Plastic Surgery Center of the Plastic Surgery Hospital of the Chinese Academy of Medical Sciences were investigated for their motivation and influencing factors in the questionnaire. The motivation includes four options: aesthetic reasons, functional impairment, both aesthetic and functional reasons, and psychological factors. External influencing factors include two options: a self-requirement, was influenced by others. After collating the data, statistical analysis of the survey result was performed.Results:A total of 216 valid questionnaires were received. Motivation for pursuing operative labiaplasty: 22.2% (48/216) of the patients complained about cosmetic reasons, 38.4% (83/216) of dysfunction, and 35.2% (76/216) had both aesthetic and functional reasons. Psychological factors accounted for 4.2% (9/216). In terms of external influencing factors: all patients motivated by dysfunction were not influenced by others, while 6.3% (3/48) of patients of cosmetic reasons were affected by male partners. In the other two motivations, 7.9% (6/76) and 6/9 respectively indicated that they were affected by male spouse. In addition, there were 2.6% of patients with these 2 motivations (2/76) and 3/9 were affected by the media.Conclusions:Most patients seek labia minora reduction surgery because of functional reasons, and a small number of people are affected by sexual partners and media.
