5.Stylet-driven lead fracture in left bundle branch area pacing: a case report

Background:
Left bundle branch area pacing (LBBAP) represents an advanced physiologic conduction system pacing technique. However, the long-term stability of stylet-driven leads (SDLs) in LBBAP remains uncertain, with limited documented reports on lead fractures.Case presentation: An 81-year-old male with a history of chronic kidney disease, heart failure, and paroxysmal atrial fibrillation (AF) was admitted for medical management.The patient experienced persistent dyspnea and dizziness attributable to bradycardia and required ongoing rhythm control medication for paroxysmal AF. Given the presence of an underlying left bundle branch block with a mildly reduced left ventricular ejection fraction, a dual-chamber pacemaker (Enitra 8 DR-T; Biotronik, DDDR) with LBBAP using SDL (Solia S50) was implanted. As the patient underwent dialysis via the left arm, pacemaker implantation was performed through the right axillary vein. At 30 months post-implantation, elevated lead impedance exceeding 2,000Ω and complete ventricular lead dysfunction were highly suggestive of lead fracture despite fluoroscopic imaging showing no definitive evidence of fracture. During ventricular lead extraction, severe adhesions and right axillary vein obstruction were encountered, requiring an 11Fr TightRail device for successful removal.The extracted lead displayed a distinct bend near the interelectrode segment, confirming a typical SDL fracture in LBBAP.
Conclusions
This case highlights the typical characteristics of lead fracture and precautions associated with SDL-LBBAP and emphasizes that such fractures can occur beyond previously reported timeframes. To prevent SDL-LBBAP fracture, careful attention to lead manipulation during rotational adjustments is essential during the procedure.
6.A case of oral rehabilitation in a patient with severe tooth wear and occlusal plane collapse, utilizing maxillary fixed prosthesis and mandibular implant-assisted removable partial denture

In patients with multiple missing molars, wear patterns or displacement of anterior teeth, collapsed occlusal plane, and reduction in vertical dimension may occur. Particularly, in case of a few remaining teeth on one side, a removable partial denture has biomechanical disadvantage. For this reason, an implant-assisted removable partial denture with a few implant surveyed crowns can be an alternative. In this case, due to the right mandibular posterior teeth loss, the anterior teeth were severely worn and the occlusal plane was collapsed. With minimal increasing vertical dimension, oral rehabilitation was performed using a maxillary fixed prosthesis and mandibular implant-assisted removable partial denture. As a result, functional and aesthetic clinical outcomes were obtained.
7.Nationwide Incidence Trends of Pediatric Parotid Malignancy in Korea and a Retrospective Analysis of Single-Institution Surgical Experience of Parotidectomy

Background/Objectives:
Pediatric parotid malignancies are rare but represent a critical subset of head and neck cancers. This study integrates nationwide incidence trends in Korea with detailed surgical outcomes from a single tertiary institution to better understand the characteristics and management of these tumors.Materials & Methods: Nationwide data from the Korea Central Cancer Registry (1999-2019) were analyzed to assess trends in pediatric parotid malignancy incidence. A retrospective review was conducted on 31 pediatric parotidectomy cases at a tertiary hospital from 2011 to 2024. Clinical data, surgical methods, pathology results, and follow-up outcomes were examined.
Results:
Nationwide analysis revealed an annual mean of 9.0 ± 4.0 cases of pediatric parotid malignancies, with a significant rise in incidence among patients aged 10-19 years (APC 5.4%, 95% CI 1.1-9.8, p=0.016). Institutional data showed that the median age of patients underwent parotidectomy was 15.0 years, with males comprising 67.7%. Among 31 cases, 19 (61.3%) were benign, primarily pleomorphic adenomas (68.4%), and 12 (38.7%) were malignant, predominantly mucoepidermoid carcinoma (87.5%). Superficial or partial parotidectomy was the most common surgical approach (71.0%), and no unexpected complications or recurrences were observed.
Conclusion
Pediatric parotid malignancies in Korea exhibit rising incidence rates, particularly among teenagers. Surgical outcomes confirm the predominance of pleomorphic adenomas and mucoepidermoid carcinomas. Pediatric parotid surgery is a procedure with minimized complications and a promising prognosis. Tailored treatment strategies adapted to the unique characteristics of pediatric patients are essential to optimize outcomes.
8.Epidemiological and Clinical Characteristics of Women Living with HIV in Korea

Background:
While Korea maintains a low prevalence of human immunodeficiency virus (HIV), the number of newly diagnosed cases has been steadily rising, reaching approximately 1,000 annually in recent years. The 2022 annual report from the Korea Disease Control and Prevention Agency revealed that women living with HIV (WLWH) constitute 6.4% of the total confirmed people living with the HIV population, totaling 1,219 individuals. Despite this, only a few studies have focused on WLWH in Korea. This study aims to analyze the epidemiological and clinical characteristics of WLWH in Korea.
Materials and Methods:
We retrospectively collected data by reviewing the medical records of all WLWH who visited 10 urban referral hospitals across Korea between January 2005 and May 2023.
Results:
A total of 443 WLWH were enrolled in this study. The predominant risk exposure was heterosexual contact, with 235 (53%) participants either married or cohabiting with a male partner at their initial clinic visit. Among the participants, 334 (77.7%) were Korean, 27 (6.1%) were Southeast Asian, and 19 (4.3%) were African. Antiretroviral therapy was initiated by 404 WLWH (91.2%). We observed 118 pregnancies in WLWH following their HIV diagnosis, resulting in 78 live births (66.1%), 18 induced abortions (15.2%), 10 pre-viable fetal losses (8.5%), and four stillbirths (3.4%). Over a cumulative follow-up duration of 3,202.1 years, the incidence rates of breast and cervical cancers were both 2.18 per 1,000 person-years. Additionally, the incidence rates of pelvic inflammatory disease, cervical intraepithelial neoplasm (above grade II), and osteoporosis were 4.67, 11.21, and 13.39 per 1,000 patient-years, respectively.
Conclusion
This is the first multicenter study to investigate the clinical and epidemiological characteristics of WLWH in Korea. The incidence and prevalence of diseases in women, including breast cancer, cervical cancer, and chronic comorbidities, are high in WLWH in Korea; therefore, further research and efforts are needed to manage these diseases.
