Cemental tear is a rare and indetectable condition unless obvious clinical signs present with the involvement of surrounding periodontal and periapical tissues. Due to its clinical manifestations similar to common dental issues, such as vertical root fracture, primary endodontic diseases, and periodontal diseases, as well as the low awareness of cemental tear for clinicians, misdiagnosis often occurs. The critical principle for cemental tear treatment is to remove torn fragments, and overlooking fragments leads to futile therapy, which could deteriorate the conditions of the affected teeth. Therefore, accurate diagnosis and subsequent appropriate interventions are vital for managing cemental tear. Novel diagnostic tools, including cone-beam computed tomography (CBCT), microscopes, and enamel matrix derivatives, have improved early detection and management, enhancing tooth retention. The implementation of standardized diagnostic criteria and treatment protocols, combined with improved clinical awareness among dental professionals, serves to mitigate risks of diagnostic errors and suboptimal therapeutic interventions. This expert consensus reviewed the epidemiology, pathogenesis, potential predisposing factors, clinical manifestations, diagnosis, differential diagnosis, treatment, and prognosis of cemental tear, aiming to provide a clinical guideline and facilitate clinicians to have a better understanding of cemental tear.

Journal of Clinical Otorhinolaryngology Head and Neck Surgery 2024;38(12):1091-1099
Objective:Pediatric chronic sinusitis (CRS) is a common disease within the field of otolaryngology-head and neck surgery. Due to the immaturity of sinus development and immune competence in children, its etiology and pathophysiology are complex, and its clinical features and outcomes differ significantly from those in adult patients. Currently, there are issues in the diagnosis and treatment of pediatric CRS, particularly in areas such as antibiotic use and surgical interventions, owing to a lack of sufficient attention. In recognition of this, the Chinese Rhinopathy Research Cooperation Group developed this expert consensus based on a systematic review of the latest literatures from both domestic and international sources, with reference to the latest evidence-based medical evidence worldwide, and in combination with their own clinical experience. The consensus covers various aspects including epidemiology, predisposing factors, pathophysiology, diagnosis and differential diagnosis, as well as treatment strategies such as medical therapy and surgical intervention. It aims to standardize the clinical diagnosis and treatment of pediatric CRS, improve clinical efficacy and patient satisfaction, reduce clinical expenditures, and decrease the occurrence of adverse reactions.

Chemical constituents of n-butanol part of ethanol extract from the leaves of Cyclocarya paliurus (Batalin) Iljinskaja were studied.Eight glycosides were separated and purified by silica gel, MCI, ODS, Sephadex LH-20 column chromatography and semi-preparative high-performance liquid chromatography.Based on the physicochemical properties and spectral data, these compounds were 3-ethyl-4-methyl-pentyl ester-O-β-D-apiofuranosyl-(1→6)-β-D-glucopyranoside (1), juglanoside E (2), (4S)-α-terpineol-8-O-α-L-arabinofuranosyl-(1→6)-β-D-glucopyranoside (3), (4S)-α-terpineol-8-O-β-D-apiofuranosyl-(1→6)-β-D-glucopyranoside (4), eugenyl-O-β-D-apiofuranosyl-(1→6)-O-β-D-glucopyranoside (5), kaempferol-3-O-β-D-glucuronopyranosyl methylester (6), kaempferol-3-O-β-D-glucuronopyranoside (7), and quercetin-3-O-β-D-glucuronopyranoside (8).Among them, compound 1 was a new compound, and compounds 2-6 were isolated from the genus Cyclocarya for the first time.

Objective:To establish a composite functional evaluation system to study the clinical efficacy of single plate and double plate internal fixation in the treatment of C3 distal radius fractures in the elderly.Methods:62 elderly patients (≥ 60 years old) with type C3 distal radius fractures who were admitted to the First Affiliated Hospital of Anhui University of Science and Technology from September 2018 to August 2021 were retrospectively selected and divided into two groups according to the surgical methods: bilateral plate internal fixation treatment group (double plate group) and unilateral plate internal fixation treatment group (single plate group); The preoperative anatomical angle, wrist mobility (flexion and extension) and radius shortening of the two groups were recorded. The anatomical angle, wrist mobility (flexion and extension), clinical Gartland-Werley, DASH Score for Upper Limb Function (DASH) , Wrist Joint Cooney Score and clinical healing time at the 3rd, 6th, 9th and 12th months after operation were followed up. A composite evaluation table was established to comprehensively evaluate the postoperative recovery of patients in the two group. Results:There was no significant difference in anatomical indexes of radius between the two groups before operation (all P>0.05). All anatomical angles and wrist range of motion were within the normal range after operation, and there was no statistical difference between the two groups in each anatomical index of radius within 3 and 6 months after operation (all P>0.05). However, the palmar inclination, ulnar deviation, wrist flexion and dorsiflexion mobility of the double plate group were relatively stable at 9 and 12 months after operation, which were better than those of the single plate group (all P<0.05). The palmar inclination and ulnar deviation of the single plate group increased with time. After operation, the radius shortening of the two groups recovered significantly, and the radius length of the double plate group was maintained better than those of the single plate group for 9 and 12 months (all P<0.05). There was no significant difference in the scores of the two groups within 3 and 6 months after operation (all P>0.05), but the Gartland Werley score, DASH score and Cooney wrist score of the double plate group were significantly better than those of the single plate group at 9 and 12 months after operation (all P<0.05). The clinical healing time of single plate treatment was (14.51±0.88)weeks, the longest was 108 days, while that of double plate treatment was (12.03±1.77)weeks, the longest was 129 days, with statistically significant difference ( P<0.05). There was no significant difference in the incidence of complications between the two groups ( P>0.05). There was no statistically significant difference between the two groups in the final composite evaluation results ( P>0.05), but the total score of the double plate group was smaller than that of the single plate group, with a difference of 39.67 points, indicating that the double plate group still had certain advantages in various scores. Conclusions:According to the composite evaluation system, both single plate and double plate treatment of C3 distal radius fractures in the elderly can achieve satisfactory results. Double plate fixation of distal radius fractures still has a certain significance in maintaining joint stability and improving joint mobility.

Objective To investigate the different expression levels of Na+ - K+ - 2Cl- co- transporter NKCC1 mRNA in the cochlea of rats after sodium salicylate injection and to explore the mechanism underlying the change of outer hair cells, induced by different salicylate administration. Methods Twenty-four normal adult rats were randomly divided into four groups with six rats in each group. Rats in control group, did not recieve sodium sa-licylate injection. The other three groups were acute group, chronic group, and recovered group according to the dif-ferent doses of sodium salicylate. The fluorescence quantitative PCR method was used to detect the expression levels of NKCC1 mRNA in the rat cochleas of the four groups. Results NKCC1 mRNA was expressed in all of the four groups. After sodium salicylate injection, the expressions of NKCC1 mRNA in chronic and recovered group were higher than that in control group(P<0.05). While the expression of NKCC1 mRNA in acute group was lower than that in control group(P(0. 05). Conclusion The expression of NKCC1 mRNA in the normal cochlea indicates that NKCC1 may play an important role in the maintenance of Cl- in the endolymph of the cochlea. The alteration of NKCC1 mRNA expression caused by sodium salicylate injection may lead to the change of the outer hair cell electro-motility.
