2.Comparison of Injection Sclerotherapy Between 5% Phenol in Almond Oil and Aluminum Potassium Sulfate and Tannic Acid for Grade 3 Hemorrhoids.

PURPOSE: Injection sclerotherapy for hemorrhoids has been performed for many years. Currently, 5% phenol in almond oil (PAO) and aluminum potassium sulfate and tannic acid (ALTA) are used as the agents. The purpose of this study was to compare the efficacy of the two agents. METHODS: A retrospective study was conducted involving 135 patients who underwent injection therapy for grade 3 hemorrhoids for the first time between 2013 and 2014 (PAO, 55 patients; ALTA, 80 patients). The efficacy was established as the proportion (%) of patients without symptoms such as hemorrhage and prolapse one year after treatment. We investigated four factors-sex, age, number of hemorrhoids, and agent-that might have an influence on the efficacy. RESULTS: The efficacies of ALTA and PAO one year after treatment were 75% and 20%, respectively. Only the agent was a significant independent factor (P < 0.01). CONCLUSION: The results suggest that ALTA is markedly more useful than PAO for injection sclerotherapy for grade 3 hemorrhoids.

3.A Study on the Association Between Skin Color and Health Condition

In Japanese traditional medicine, “ganmenshin” (facial diagnosis), which means assessment of a patient's health based on the color of his/her face, is frequently used in a clinical setting. However, little scientific evidence is available to substantiate such diagnoses. In the present study, we investigated the relationship between skin color and patient's health status to ascertain the clinical usefulness of the “ganmenshin” technique. We evaluated the color of the cheeks and forearms of 30 healthy study participants and also administered questionnaires to determine their health. A correlation was observed between skin color and work efficiency and the inkyo (yin-fluid deficiency) scores in all subjects. In male subjects, skin color was correlated with their bodymass index (BMI), self-rated health status, work efficiency, and suitai (fluid stagnation) scores. In female subjects, skin color was correlated with the patient's age, work efficiency, and inkyo scores. These results suggest that “ganmenshin” may be clinically useful because of the correlation between skin color and the health of patients.
4.Influence of Signal Intensity Non-Uniformity on Brain Volumetry Using an Atlas-Based Method.

OBJECTIVE: Many studies have reported pre-processing effects for brain volumetry; however, no study has investigated whether non-parametric non-uniform intensity normalization (N3) correction processing results in reduced system dependency when using an atlas-based method. To address this shortcoming, the present study assessed whether N3 correction processing provides reduced system dependency in atlas-based volumetry. MATERIALS AND METHODS: Contiguous sagittal T1-weighted images of the brain were obtained from 21 healthy participants, by using five magnetic resonance protocols. After image preprocessing using the Statistical Parametric Mapping 5 software, we measured the structural volume of the segmented images with the WFU-PickAtlas software. We applied six different bias-correction levels (Regularization 10, Regularization 0.0001, Regularization 0, Regularization 10 with N3, Regularization 0.0001 with N3, and Regularization 0 with N3) to each set of images. The structural volume change ratio (%) was defined as the change ratio (%) = (100 x [measured volume - mean volume of five magnetic resonance protocols] / mean volume of five magnetic resonance protocols) for each bias-correction level. RESULTS: A low change ratio was synonymous with lower system dependency. The results showed that the images with the N3 correction had a lower change ratio compared with those without the N3 correction. CONCLUSION: The present study is the first atlas-based volumetry study to show that the precision of atlas-based volumetry improves when using N3-corrected images. Therefore, correction for signal intensity non-uniformity is strongly advised for multi-scanner or multi-site imaging trials.

5.Effects of electroacupuncture stimulation on repetitive exercise-induced oxidative stress

[Objective]Daily repetitive exercise is known to be necessary for players in sports. In this study, to further explore the potential functions of acupuncture, we studied whether electroacupuncture (EA) stimulation has efficacy for repetitive exercise-induced oxidative stress.
[Methods]Six healthy male volunteers participated in both the non-treatment control group and the EA group in a crossover design. EA stimulation was applied to both the medial vastus muscles at 2 Hz and optimum intensity for each subject during ten min just before each exercise period. The subject performed ergometer exercise for 20 min with 75%of maximal oxygen uptake each day. This exercise period was performed once a day for three consecutive days. Blood lactate level (BLL) and plasma lipid peroxide (LP) concentration were measured as the markers for metabolism and oxidative stress, respectively. Visual Analogue Scale (VAS) was used to evaluate the individual level of fatigue due to exercise.
[Results]During and just after the exercise period, each value of BLL in the EA group was lower than the same points in the control group. Whereas LP concentrations before the exercise period on the first day in the control group was higher than the values in the same points on the second and third days. LP concentrations before the exercise period in the EA group were almost the same levels for three days. Furthermore, after the exercise period for the three consecutive days, LP concentrations and VAS in the EA group were lower than each value of the same points in the control group, and especially, the alteration of VAS showed a significant difference.
[Conclusion]The increased tendency of LP concentrations before the exercise period for three days in the control group indicated that repetitive exercise induced the accumulation of excess lipid peroxide. It might be suggested that EA stimulation suppressed subjective fatigue by an enhanced energy metabolic rate and decreased production of lipid peroxide. Hence the evidence strongly supported that EA stimulation might be useful for sports conditioning.
6.PROGNOSTIC SIGNIFICANCE OF HEART RATE RESPONSE TO EXERCISE FOR 20-YEAR SURVIVAL IN PATIENTS WITH CORONARY HEART DISEASE

The purpose of this study was to investigate the prognostic value of change in heart rate (HR) response to oxygen uptake (VO2) during exercise in patients with coronary heart disease (CHD).We retrospectively studied 204 patients with CHD who were sent to our exercise testing laboratory between August 1983 and February 1985. The following equation was used to determine the relation between VO2 and HR during a graded treadmill exercise test: HR= A・exp B・VO2, where the constant b was defined as the inclination of the exponential curve-fitting model for oxygen uptake and heart rate during graded treadmill exercise (I-ECOH). Data on mortality were determined in May 2006 by examining medical records from the outpatient clinic and/or conducting telephone interviews with the patients or their families. Among the 204 study subjects, there were 54 cardiac deaths during the entire 20-year follow-up period. Kaplan-Meier survival curves for 20 years of follow-up demonstrated a survival rate of 81.3% for patients with a lower I-ECOH (<35) and 56.0% for those with a higher I-ECOH (≧35), showing a significant difference in survival (p<0.001). Multivariate Cox proportional hazards analysis revealed left ventricular ejection fraction (LVEF), peak oxygen uptake (VO2 peak) and I-ECOH as independent predictors of survival. In this analysis, the prognostic power of minutes ventilation/carbon dioxide output (VE/VCO2)slope was insignificant. The patients were divided into two groups with (<45%) and without (≧60%) LV dysfunction using LVEF for more detailed analysis. The VO2 peak and VE/VCO2 slope were significant independent predictors of survival in patients with LV dysfunction. However, the I-ECOH was a significant independent predictor of survival in patients with and without LV dysfunction. Our data indicate that I-ECOH provides independent prognostic information on CHD patients with and without LV dysfunction.
7.Effect of acupuncture stimulus on the deep pain threshold and deep tissue temperature of the favorite site of shin-splints

[Objective]As fundamental research on the acupuncture treatment to shin-splints, we studied what kind of acupuncture stimulus raises the pain threshold and deep tissue temperature of the periosteal part of the tibia.
[Methods]Twelve healthy subjects (Age: 22 ± 2years old) participated in this study. We recorded how the pain threshold of the periosteum would change under the influence of the kind of stimulus of leaving a needle, sparrow pecking a needle and no stimulation. Simultaneously, we measured the deep tissue temperature near the measured part of pain threshold with a deep tissue thermometer.
[Results]The pain threshold of leaving a needle group rose significantly (p < 0.05) as compared with the control group. The sparrow pecking group showed an upward tendency. There was no significant difference in the deep tissue temperature between each group at all times.
[Conclusion]The leaving the needle technique is suitable for needle stimulus to shin-splints. It was suggested that pain control was expected with a leaving needle stimulus. The needle stimulus did not show a remarkable effect on the reaction of regional circulation.
8.Effect of electroacupuncture on exercise-induced oxidative stress

Objective:It is very important that oxidative stress is estimated for us to understand a player's condition in the field of sports. The purpose of this study was to investigate whether electroacupuncture has an effect on exercise-induced oxidative stress.
Methods:Ten healthy male volunteers participated in both the electroacupuncture (EA) group and the control group in a crossover design. EA at a frequency of 2 Hz and optimum intensity was performed in the subjects for 10 minutes in both the medial vastus muscles. During ergometer exercise by ramp load, respiratory metabolism including the RC point was recorded as the indication of energy metabolism. Blood was collected from the fingertips of the subjects, and then their levels of oxidative stress (d-ROMs test) and antioxidative (BAP test) were determined by using a Free Radical Analytical System (FRAS4, Wismell Instruments). Six measurements were taken;at rest, immediately after EA and following exercise, and at 20, 40, and 60 minutes following exercise.
Results:The RC point was significantly prolonged in the EA group compared with the control group. The level of oxidative stress (d-ROMs test) in the control group increased significantly at 20 minutes following exercise compared with rest (before exercise) and this increase was sustained until 60 minutes after exercise. In contrast, it did not significantly change in the EA group. Whereas the antioxidative level (BAP test) in the EA group significantly increased immediately following exercise compared with rest (before exercise), it did not increase in the control group.
Conclusion:These results suggest that electroacupuncture might enhance the antioxidative level (BAP test) and inhibit the level of oxidative stress (d-ROMs test) by effecting a change in respiratory metabolism. We propose that electroacupuncture might be useful for sports conditioning.
9.A Case of Successful Acupuncture Treatment for Diffuse Pan-Bronchiolitis

[Aim] Diffuse pan-bronchiolitis (DPB) causes severe respiratory dysfunction and severely limits a patient's daily activities. Case: This paper reports a case of a DPB patient whose respiratory symptoms were successfully improved by acupuncture. In a case report in January 2002, a 62-year-old male visited the Department of Respiratory Internal Medicine at Gifu University Hospital, complaining of cough, pumlent sputum and dyspnea on exercise. His general condition had worsened despite ordinary medical treatment. Thus acupuncture was added to the treatment in August, 2001. Intervention: The patient received acupuncture treatments once a week for 50 weeks. The acupuncture treatment was based on the Chinese medicine theory. Measurements: Outcome measures were respiratory symptoms using the Fletcher-Hugh-Jones (F-H-J) classification, 6-minute walking distance (6 MWD), blood test, arterial blood gas and pulmonary function tests. Computed tomography (CT) was assessed. Each evaluation was done after 15 weeks and 50 weeks later.
[Results] After 15 weeks of acupuncture treatment, cough, pumlent sputum, walking distance, the Borg scale and respiratory function were significantly improved compared with the baseline. Moreover, the effect of the acupuncture treatment continued for 50 weeks.
[Conclusion] This suggests that acupuncture treatment was effective in treating the advanced case of DPB.
10.INCLINATION OF EXPONENTIAL CURVE-FITTING MODEL FOR HEART RATE AND OXYGEN UPTAKE DURING INCREMENTAL EXERCISE AS INDEX OF CARDIO-PULMONARY FUNCTIONAL IN PATIENTS WITH ISCHEMIC HEART DISEASE

This study was conducted to clarify the validity and availability of inclination of exponential curve-fitting model for oxygen uptake (VO2) and heart rate (HR) during incremental exercise (I-ECOH) as an index of cardio-pulmonary functional reserve in patients with ischemic heart disease (IHD). A treadmill exercise test was used to measure the VO2(L/kg/min) and HR (beat/min) during incremental exercise of all subjects. I-ECOH was derived from the following equation : HR=A·expB·VO2. The constant "B" represents I-ECOH. The following two identifications were made : 1) the relation between peak oxygen uptake (VO2peak) and I-ECOH in IHD patients with normal left ventricular function and with chronic heart failure (CHF); 2) the relation between I-ECOH and the New York Heart Association (NYHA) functional classification of IHD patients with CHF.There were significant differences among IHD patients with normal left ventricular function, CHF patients, normal controls and long distance runners in I-ECOH and VO2peak, respectively (p<0.001). There were inverse correlations between I-ECOH and VO2peak in IHD patients with normal left ventricular function (r=-0.64, p<0.001) and CHF (r=-0.63, p<0.001). I-ECOH could be used to discriminate effectively between NYHA functional classes (p<0.001).In conclusion, these results suggest that I-ECOH is adequate and useful as an index of cardio-pulmonary functional reserve in patients with ischemic heart disease.
