1.Prospective Associations between Self-perceived Voice Disorders and Psychological, Social, and Physical Well-being among Community-Dwelling Older Adults

Background:
Self-perceived voice disorders (PVD) are common in later life and may affect psychological, social, and physical well-being. However, longitudinal evidence in community settings remains limited. This study sought to examine whether PVD predicted changes in well-being among community-dwelling older adults.
Methods:
Data were obtained from a community cohort in Kasama City, Japan. Older adults (n=273; aged 76.5 years; 57.1% female) were classified based on baseline Voice Handicap Index-10 scores (0, no; 1–4, mild; and ≥5, PVD). Outcomes were assessed annually through 2025 using the Geriatric Depression Scale (GDS), Lubben Social Network Scale (LSNS), Physical Activity Scale for the Elderly, and physical performance measures (handgrip strength, single-leg balance with eyes open, sit-and-reach, and 5-m habitual walk). Linear mixed models analyzed the fixed effects of group, time, and their interaction while adjusting for selected covariates.
Results:
Significant group effects were observed for the GDS, total LSNS scores, and the LSNS family scale, indicating that participants with PVD had weakened psychological and social well-being and smaller family networks. Additionally, a notable group effect was observed in sit-and-reach, with PVD participants showing less flexibility than those without it in the physical performance measures.
Conclusion
Among community-dwelling older adults, PVD was prospectively associated with increased depressive symptoms, diminished social networks, particularly smaller family networks, and reduced physical flexibility. These findings suggest that PVD preceding medical diagnosis may signal broader declines in psychological, social, and physical well-being, highlighting the importance of early identification and support.
2.Dose-Response Association between Hand Dexterity and Functional Disability: A Longitudinal Study from the Kasama Study

Background:
Poor hand dexterity may increase the risk of functional disability; however, few studies have examined the relationship between hand dexterity and incident functional disability. The aim of this study was to prospectively investigate the dose-response association of hand dexterity with incident functional disability in community-dwelling older adults.
Methods:
This study included 1,069 older adults aged ≥65 years in Kasama City, Japan. Peg-moving and circle-drawing tasks were used to evaluate hand dexterity. Functional disability was identified using the Japanese Long-Term Care Insurance System database. Restricted cubic spline analysis was performed to investigate the dose-response association between hand dexterity and incident functional disability.
Results:
During a mean follow-up of 8.5 years (maximum 14.0 years), 248 participants (23.2%) developed functional disability. The lowest performance group in each hand dexterity test had a significantly higher risk of functional disability than the highest performance group—peg-moving (hazard ratio [HR]=1.92, 95% confidence interval [CI] 1.29–2.87) and circle drawing (HR=1.66, 95% CI 1.15–2.41). Spline analysis confirmed curvilinear dose-response associations between hand dexterity and incident functional disability. Increased risk was observed when participants performed worse than the cut points (peg-moving, 37.9/38.0 seconds; circle drawing, 21/20 points), and no decreased risk was observed for those who performed better than these cut points.
Conclusion
Easily evaluated hand dexterity tests may be valuable for predicting functional disability in older adults. Curvilinear dose-response associations suggest that maintaining adequate hand dexterity could be a key strategy to support functional independence.

Behavioral and psychological symptoms were examined by psychiatric and cognitive functional evaluation in cognitive dysfunction patients who received weak ultrasound stimulation and drugs. We found great improving effects on motor symptoms for the first time in this study.

This case report describes our first experience performing percutaneous epicardial catheter ablation for Burugada syndrome in our hospital. We describe the good results achieved in this case. The patient was a man in his 30s with no remarkable medical history. However, his family history was notable for the sudden death of his grandfather at age 37 years and his father at age 27 years. While asleep, the patient experienced convulsions and lost consciousness. During emergency transportation, defibrillation was performed 7 times by the ambulance crew. When the patient arrived at our hospital, sinus rhythm was observed on ECG. During resuscitation, Burugada syndrome was diagnosed based on ECG findings. On hospital day 6, an internal cardioverter defibrillator was implanted. After discharge, the defibrillator operated 10 times, so we opted for ablation treatment. Fractionated potential of over 150 ms was confirmed in the right ventricular outflow tract. A low voltage zone of <1 mV could be mapped, and the same site was cauterized a total of 46 times. As a result, ST segment amplitude decreased significantly in lead V1 on ECG. Percutaneous epicardial catheter ablation performed with reference to Nademanee’s report achieved good results in this case of Burugada syndrome.
5.Long-Term Intake of Ubiquinol May Improve Cognitive Performance in Community Residents

Dementia is one of the most serious problems in aging societies. A recent study has reported that higher serum coenzyme Q10 levels could reduce the risk of incident dementia. In this community-based study, we evaluated changes in serum ubiquinol level and cognitive performance following long-term intake of ubiquinol. Sixty-one residents (26 men, 35 women; aged 33-87 years) of Kamijima town in Ehime Prefecture agreed to participate in this study. The participants consumed 100-150 mg ubiquinol per day for 6 months to 2 years. We measured serum ubiquinol level and assessed cognitive performance using the 1-min Digit Symbol Substitution Test (DSST) both at baseline and after long-term ubiquinol intake. We also examined the subjects after a 3-month washout period. At baseline, a strong negative correlation was found between age and DSST score. There was no significant correlation between serum ubiquinol level and DSST score. Following long-term ubiquinol intake, both serum ubiquinol level and DSST score increased significantly. After the 3-month washout period, serum ubiquinol level decreased to almost the baseline level, whereas DSST score did not change significantly. Because this study is a single-arm trial these results should be considered carefully; nonetheless, our findings indicate the probable effects that long-term ubiquinol supplementation improves cognitive performance among community residents.
6.Oxidized human serum albumin as a possible correlation factor for atherosclerosis in a rural Japanese population: the results of the Yakumo Study.

BACKGROUND:
The effect of the redox state of human serum albumin (HSA) on the antioxidant properties of the entire body has been a focus of recent research. The usefulness of HSA redox state as a biomarker for reducing oxidative stress has been investigated in clinical settings; however, evidence for its significance as a health index in non-clinical settings is yet to be established. This study aimed to examine the associations between HSA redox state and the atherosclerotic indices of carotid intima-media thickness (IMT) and plaque formation in a rural Japanese population.
METHODS:
We conducted a cross-sectional study as part of a health check-up program in the rural area of Hokkaido, Japan, at the end of August 2013. A total of 281 residents (124 men and 157 women) were included in the final analysis. Lifestyle-related data were obtained through a self-reported questionnaire, and ultrasound examinations were performed to measure IMT and determine plaque formation. The high-performance liquid chromatography postcolumn bromocresol green method was used to separate HSA into human nonmercaptalbumin and human mercaptalbumin (HMA).
RESULTS:
We found a significant negative relationship between the fraction of HMA [f(HMA)] and IMT (standardized β = - 0.132, p = 0.03). Moreover, f(HMA) was significantly associated with plaque formation (p < 0.01) with an odds ratio of 0.89 (95% confidence interval, 0.81-0.97) for every 10% increment in f(HMA).
CONCLUSIONS
We found that the HSA redox state, as determined by f(HMA), was associated with atherosclerotic indices in Japanese subjects. These results suggest that the HSA redox state indicates the risk of developing atherosclerosis.

7.Noninvasive Assessment of Advanced Fibrosis Based on Hepatic Volume in Patients with Nonalcoholic Fatty Liver Disease.

BACKGROUND/AIMS: Noninvasive liver fibrosis evaluation was performed in patients with nonalcoholic fatty liver disease (NAFLD). We used a quantitative method based on the hepatic volume acquired from gadoxetate disodium-enhanced (Gd-EOB-DTPA-enhanced) magnetic resonance imaging (MRI) for diagnosing advanced fibrosis in patients with NAFLD. METHODS: A total of 130 patients who were diagnosed with NAFLD and underwent Gd-EOB-DTPA-enhanced MRI were retrospectively included. Histological data were available for 118 patients. Hepatic volumetric parameters, including the left hepatic lobe to right hepatic lobe volume ratio (L/R ratio), were measured. The usefulness of the L/R ratio for diagnosing fibrosis ≥F3–4 and F4 was assessed using the area under the receiver operating characteristic (AUROC) curve. Multiple regression analysis was performed to identify variables (age, body mass index, serum fibrosis markers, and histological features) that were associated with the L/R ratio. RESULTS: The L/R ratio demonstrated good performance in differentiating advanced fibrosis (AUROC, 0.80; 95% confidence interval, 0.72 to 0.88) from cirrhosis (AUROC, 0.87; 95% confidence interval, 0.75 to 0.99). Multiple regression analysis showed that only fibrosis was significantly associated with the L/R ratio (coefficient, 0.121; p<0.0001). CONCLUSIONS: The L/R ratio, which is not influenced by pathological parameters other than fibrosis, is useful for diagnosing cirrhosis in patients with NAFLD.

8.A Surgical Case of Infective Endocarditis with Annular Abscess into Mitral Annulus Calcification

A 79-year-old man, who had undergone aortic valve replacement due to severe aortic stenosis 2.5 years previously and permanent pacemaker implantation for sick sinus syndrome 2 months after aortic valve replacement, was admitted for congestive heart failure and suspicion of prosthetic valve endocarditis. However, he had a fever in spite of medical therapy, and transthoracic echocardiography revealed a 20 mm vegetation on the posterior mitral valve leaflet. He underwent emergency surgery on a diagnosis of infective endocarditis. The intraoperative examination showed annular abscess on the calcified mitral annulus, and a part of abscess had disintegrated, from which the vegetation arose. We performed maximal possible debridement of the infected tissue and mitral annulus reconstruction with a bovine pericardium. Subsequently, mitral valve replacement and annulus reinforcement with a prosthetic valve collared with a bovine pericardium were performed to prevent perivalvular leakage. The patient showed no recurrence of infection and perivalvular leakage at 1.5 years of follow-up.
9.Hyperbaric Oxygen Treatment of Carbon Monoxide Poisoning in the Past 5 Years

Hyperbaric oxygen (HBO) therapy for acute carbon monoxide (CO) poisoning is performed after oxygen therapy and breathing therapy. The usefulness of HBO therapy in emergency treatment has been reported. In this study, we examined the effectiveness of HBO for CO poisoning that was performed at our hospital over the past 5 years. Subjects were 23 patients who had HBO therapy for CO poisoning in the period January 2008-November 2013. The male to female ratio of the cases was 14:9 and the mean age was 54.6±20.8 years. The cause was suicide in 39.1% of cases and accident in 60.9%. The mean number of treatments was 5.4±6.8. The atmosphere absolute was 2ATA:2.8ATA=12:11. The ratio of direct conveyance of the patient to our hospital to indirect conveyance of the patient from another hospital was 15:8. The effectiveness of HBO therapy at the time of discharge was 73.9%. However, delayed neuropsychiatric sequelae (DNS) was detected in 3 of the 23 cases of CO poisoning. No correlations were found between elapsed time after CO inhalation and various blood parameters. However, time to start of therapy is important for DNS, and our results suggest that early treatment at a hospital with a hyperbaric chamber is necessary.
10.Changes in susceptibility to antibacterial agents of Haemophilus influenzae isolated in central Tokyo for 10 years (2004-2013)

Introduction : The purpose of this study was to clarify the antimicrobial susceptibility of Haemophilus influenzae in central Tokyo over a ten-year period.
Methods : We investigated the susceptibility of Haemophilus influenzae isolated in Tokyo Saiseikai Central Hospital for 10 years from 2004 through 2013.
Results : Significant increases in the proportion of strains resistant to ampicillin, ampicillin / sulbactam and cefditoren pivoxil were noted for some years in comparison to the first year studied. There were no significant changes in the susceptibility to levofloxacin and clarithromycin. The proportion of strains resistant to ampicillin and cefditoren pivoxil showed statistically significant increases during the study period. The proportion of the strains resistant to ampicillin / sulbactam isolated from children was greater than from adults for every year studied. Levofloxacin resistant strains were isolated only from adults. The proportion of strains resistant to clarithromycin were about 1% in Japanese national surveillance data, but the proportions in this study were over 9%.
Conclusion : Over a ten-year period in central Tokyo, Haemophilus influenzae strains resistant to ampicillin and cefditoren pivoxil increased. Strains resistant to clarithromycin occurred at a higher rate than nationally.
