1.Tumour Characteristics and Real-world Treatment Patterns Among Patients Diagnosed With Metastatic Prostate Cancer in Sarawak - A 8-year Review

Introduction: Numerous novel systemic treatments have emerged for patients with primary metastatic prostate cancer (mPC), addressing both hormone-sensitive (mHSPC) and castration-resistant prostate cancer (mCRPC). However,
limited understanding exists in integrating these therapies into clinical practice. This study provides an overview of
mPC, presenting baseline disease characteristics, treatment profiles, and outcomes of mPC specifically in Sarawak.
Materials and methods: Our study focused on registered male patients diagnosed with metastatic prostate cancer at
Sarawak General Hospital between 2016 and 2023, aged over 18. Surveys were carried out during routine clinical
practice, covering patient demographics, clinical parameters, primary treatments, follow-up, and outcomes. The
study described treatment patterns following diagnosis. Results: Demographic and tumor profiles of 212 patients
with metastatic prostate cancer in Sarawak General Hospital from 2016- 2023 were retrospectively analyzed. Patients with mPC was notably prevalent among individuals of Chinese ethnicity, accounting for 43.4% of cases, with
over 50% of patients presenting with high-volume disease irrespective of ethnicity. The primary treatment modality
for the majority of mPC patients was androgen deprivation therapy (ADT) alone. Among the cohort, 19.3% (n=41)
experienced disease progression to metastatic castration-resistant prostate cancer (mCRPC) since 2016. Novel hormonal therapy (NHT) emerged as the predominant first-line treatment for mCRPC, administered to 53.7% of patients.
Conclusion: The majority of prostate cancer patients in Sarawak are diagnosed in the metastatic stage and were of
high volume at diagnosis. Aggressive treatment was initiated early in an attempt to improve treatment outcomes and
overall survival.
2.Key node design and optimization of internet hospital service processes

Against the backdrop of deep integration between digital technology and healthcare,internet hospitals are tran-sitioning from"channel supplementation"to"service reconstruction."Based on conventional internet hospital service processes,this paper proposes construction suggestions and optimization strategies for four key nodes:the development of intelligent triage systems,the improvement of appointment systems,the integration of online and offline services,and the expansion of post-consul-tation services.By focusing on patient experience,medical services can become more humanized and personalized,thereby en-hancing patient efficiency and satisfaction with internet-based healthcare services and promoting the overall development of the medical service industry.
3.Implementation of integrated collaborative emergency management among multiple hospital districts from the perspective of disaster vulnerability

Objective This study focuses on the analysis of disaster vulnerability among multiple hospital districts to ex-plore the core of integrated emergency management among these districts.Methods Taking the First Affiliated Hospital of Guan-gzhou Medical University as an example,the Kaiser model analysis tool was used to identify risk events in the three hospital dis-tricts.A 42-item questionnaire was developed based on four dimensions,and surveys were conducted among administrative de-partment heads and some frontline staff in the three districts.After selecting valid data,the arithmetic mean of single-item scores from multidimensional scorers was calculated as the final single-item score for risk evaluation.Results The analysis revealed significant differences in the ranking of the top 10 risk events among the three hospital districts.The top risk event in the riverside district was typhoon,with a risk coefficient of 60.2%,which was higher than the top risk events in the Tan Sha district(55.2%)and Hai Yin district(52.8%).This indicates that the overall risk coefficient in the riverside district is higher,with a higher probability of different types of risk events occurring.Conclusion The study found differences in risk events among multiple hospital districts and identified conflicts in the implementation of integrated collaborative emergency management.To ad-dress these differences and conflicts,it is necessary to establish a unified command system,enhance information sharing,in-tegrate resources,strengthen risk assessment,and optimize management culture,thereby achieving efficient operation of compre-hensive emergency management in multiple hospital districts.

8.Safety and efficacy of domestically produced novel bioabsorbable vascular scaff old in the treatment of complex coronary artery lesions for 3 years

Objective To investigate the safety and efficacy of novel bioabsorbable vascular scaffold(BVS)in the treatment of patients with complex coronary artery disease.Methods This was a retrospective,matched,single-center observational study.45 patients with coronary atherosclerotic cardiopathy received BVS treatment in the cardiovascular medicine department Department of the Affiliated Hospital of Guangdong Medical University from June 2020 to June 2021(BVS),and 45 patients treated with drug-eluting stents(DES)group were selected according to matching study requirements during the same period.Baseline,surgical,and follow-up data were compared between the two groups to evaluate safety and efficacy.The main measures of safety were:surgical time,intraoperative adverse events,etc.,and the end point of efficacy was target lesion failure(TLF),including cardiac death,target vessel myocardial infarction,and ischa-driven target lesion revascularization.Results A total of 90 patients were enrolled in this study,all of whom were followed up for at least 3 years.There were 20 cases of bifurcation lesions and 25 cases of diffuse long lesions in the two groups,and 50 cases of imaging were reviewed among the 90 patients.The proportion of stable coronary heart disease,history of diabetes,history of hypertension,history of smoking,pre-dilated balloon pressure and postoperative diastolic blood pressure in BVS group was higher than that in DES group,and the proportion of family history was lower than that in DES group(all P<0.05).There were no statistically significant differences in the rates of cardiac death,target vessel myocardial infarction,and ischemia-driven revascularization of target lesions between the two groups(all P>0.05).Binary Logistic regression model analysis showed that the diameter stenosis ratio of target lesions was an independent risk factor for intrastent restenosis(OR 2.786,95%CI 1.096-7.081,P=0.031).Conclusions Compared with traditional DES,BVS implantation has consistent safety and efficacy in the treatment of complex coronary artery disease within 3 years.The diameter stenosis ratio of target lesions was an independent risk factor for intrastent restenosis.
9.Association of BMI with acute exacerbation in mild to moderate chronic obstructive pulmonary disease patients in community: a prospective study.

Objective: BMI may play a protective role in reducing the mortality rate of patients with chronic obstructive pulmonary disease (COPD), but its effect on acute exacerbation of COPD remain unclear. Methods: Subjects were selected from the COPD patients registration system established in 2014 in Pudong new district, Shanghai. COPD patients from 8 communities were selected by cluster sampling and follow up was conducted prospectively for 18 months. Basic information and BMI were obtained from baseline survey, and acute exacerbations were collected during follow-up. The association between BMI and risk of acute exacerbation was evaluated by using multiple negative binomial regression. Results: Among 328 community COPD patients, 295 who completed the follow up were included in the analysis, in whom 96.3% (284/295) were mild COPD patients. During the follow-up, 11.1% (33/295) of the patients reported acute exacerbation. The results of multiple negative binomial regression suggested that, the risk for acute exacerbation decreased with the increase of BMI (IRR=0.85, 95%CI:0.73-0.98), overweight patients with BMI ≥25.0 kg/m2 (IRR=0.36, 95%CI:0.13-0.91) or moderate BMI (T2 vs. T1, IRR=0.31, 95%CI:0.11-0.77) had lower risk for acute exacerbation compared with the patients with normal or low BMI. BMI had a linear correlation with the risk of acute exacerbation. Conclusion: The risk for acute exacerbation in patients with mild or moderate COPD in communities decreased with the increase of BMI, and being overweight might be a protective factor for the acute exacerbation of COPD.

10.Clinical Characterization of Patients with Ovarian Mass Combined with Dysplasia of Secondary Sexual Characteristics.

Objective To investigate the clinical characteristics of preadolescent and adolescent female patients with ovarian mass combined with dysplasia of secondary sexual characteristics. Methods This study retrospectively analyzed 18 cases of ovarian mass combined with dysplasia of secondary sexual characteristics aged 0-19 years admitted to Peking Union Medical College Hospital from January 2012 to November 2019.By analyzing the clinical manifestations,surgical methods,postoperative pathology,therapies and prognosis of the cases,we summarized the diagnosis and treatment ideas. Results Among the 18 cases,7(7/18,38.9%)developed secondary sex signs before puberty,including 5 cases showing precocity(including 2 cases of juvenile granulosa cell tumor,1 case of gonadoblastoma,1 case of ovarian follicular cyst,and 1 case of 46,XY simple gonadal dysplasia combined with dysgerminoma)and 2 cases presenting masculine manifestations(1 case of steroid cell tumor and 1 case of sclerosing stromal tumor).The rest 11(11/18,61.1%)cases showed abnormal development of secondary sexual characteristics during puberty,including 8 cases with masculine manifestations or abnormal menstruation after menarche(7 cases with sex cord stromal cell tumor and 1 case with cystic granulosa cell tumor),2 cases with primary amenorrhea(1 case with androgen insensitivity syndrome combined with testicular sertoli cell tumor and 1 case with endometriosis cyst combined with reproductive tract malformation),and 1 case diagnosed as 46,XX gonadal dysplasia with serous cystadenoma and no secondary sexual development during puberty. Conclusions Sex hormone levels should be actively tested in the case of prepubertal secondary sexual characteristics appearing early,pubertal secondary sexual characteristics being abnormal(underdevelopment),and/or menstrual abnormalities.Imaging examination should be performed to exclude ovarian organic lesions,and chromosome karyotype analysis should be performed if necessary.The diagnosis of ovarian mass in preadolescent and adolescent females with related symptoms should first be alerted to cord stromal cell tumor.It is recommended to rule out the possibility of combined reproductive tract malformation in the adolescent patients with primary amenorrhea.Chromosome examination should be conducted to rule out the possibility of gonadal dysplasia in the adolescent patients with primary amenorrhea and/or no development of secondary sexual characteristics.
