1.Analyses of the epidemiological and clinical characteristics of 21 confirmed monkeypox cases in a district of Chengdu City

ObjectiveTo analyze the epidemiological and clinical characteristics of the 21 confirmed monkeypox cases in a district of Chengdu City, and to provide scientific guidance for the prevention and control of subsequent monkeypox epidemics. MethodsData of confirmed monkeypox cases residing in this district were collected from the Disease Control and Prevention Information System of China. A retrospective descriptive epidemiological analysis was used to analyze the demographic, distributional and behavioral characteristics of the cases. ResultsThe first confirmed case of monkeypox was reported on July 5, 2023. Up to April 30, 2025, a total of 21 confirmed cases of monkeypox have been reported. All cases were male, with a mean age of (30.9±6.2) years. The highest proportion of cases(47.62%) was in the 30‒40 years age group. The majority were men who have sex with men (MSM) population (90.48%, 19/21). The results showed that 19.05% of cases were co-infected with HIV, and 19.05% had a history of syphilis infection. Within 21 days prior to symptom onset, 19 cases (90.48%) self-reported engaging in male-to-male sexual contact, among whom 10 cases (52.63%) reported having taken protective measures, while 9 cases (47.37%) did not take safety precautions. Thirteen cases (61.90%) had no travel history to areas with reported monkeypox cases during the 21 days before symptom onset. The predominant manifestation was exanthem (100%, 21/21), followed by fever (57.14%, 12/21) and lymphadenectasis (47.62%, 10/21). Among febrile cases, 50.00% (6/12) had low-grade fever (37.3‒38.0 ℃). All cases were identified through active medical consultation. The median interval from symptom onset to the first medical visit was 3 (2, 6) days, with a maximum interval of 14 days. The median interval from symptom onset to laboratory confirmation was 7 (5, 9) days. Six cases (28.57%) had two or more visits to the hospital, with bacterial infection being the primary initial diagnosis. ConclusionMonkeypox prevention and control efforts in a district of Chengdu City should prioritize MSM population and young and middle-aged adults aged 30 to <40 years. It is recommended to establish an integrated monkeypox epidemic prevention and control network by leveraging existing HIV/AIDS prevention and control network. Concurrently, accelerating the deployment of the national intelligent infectious disease monitoring and early warning front-end software will strengthen early detection capabilities and be beneficial for the overall effectiveness of epidemic prevention and control efforts.
2.Effects of Different Anticipated Conditions on Biomechanical Characteristics of Lower Limbs in Individuals with Chronic Ankle Instability

Objective The differences in biomechanical characteristics of the lower limbs between individuals with chronic ankle instability(CAI)and healthy individuals during unanticipated and anticipated jumping were compared,in order to provide practical references and ideas for the prevention and treatment of recurrent ankle sprains.Methods Thirty subjects were recruited,including 15 patients with CAI and 15 healthy volunteers.All subjects completed unanticipated and anticipated jumping tests in a random order,with a 1-week interval between the two tests.Kinematic and kinetic data of lower limbs were collected synchronously using Vicon infrared high-speed motion capture system and Kistler three-dimensional force platform.Results At the moment of touchdown,knee flexion angle was significantly greater during unanticipated jumping than that during anticipated jumping(P=0.009),while ankle eversion angle was notably lower(P=0.043).During the early landing phase,unanticipated jumping showed significantly greater peak hip flexion and abduction angles,as well as knee flexion(P=0.038,P=0.036,P=0.04),while peak ankle dorsiflexion and eversion angles were significantly lower(P=0.001,P=0.01)compared to anticipated jumping.Additionally,peak hip abduction moment during unanticipated jumping was significantly higher in patients with CAI than that during anticipated jumping(P=0.028).Conclusions Unanticipated jumping reduced ankle dorsiflexion and eversion angles in individuals with CAI,putting the ankle in an open,sprain-prone position.Individuals with CAI compensated proximally by increasing hip flexion,abduction,knee flexion angles,and hip extension moment to stabilize the ankle.
3.Effect of Guiqi Yiyuan Ointment on Lewis Lung Cancer Mice by Increasing Autophagic Flux and Stabilizing PD-L1 Expression Through Regulation of ERK Signaling Pathway

ObjectiveTo investigate the antitumor effect and mechanism of Guiqi Yiyuan ointment on Lewis lung cancer mice based on the extracellular regulatory protein kinase (ERK) signaling pathway. MethodsA Lewis lung cancer mouse model was established. Except for the blank group, the model mice were randomly divided into the model group, Guiqi Yiyuan ointment low, medium, and high dose groups, and the extracellular ERK1/2 inhibitor group, with 10 mice per group. The Guiqi Yiyuan ointment was administered by gavage at doses of 1.75, 3.5, 7.0 g·kg-1·d-1 for the low, medium, and high dose groups, respectively. The ERK1/2 inhibitor group was given the ERK1/2 inhibitor LY3214996 (100 mg·kg-1·d-1) by gavage. The treatment was administered for 14 consecutive days, after which samples were collected. Tumor histopathological changes were observed using hematoxylin-eosin (HE) staining. Transmission electron microscopy was used to observe ultrastructural changes in tumor cells. Immunofluorescence was performed to measure the phosphorylation of ERK1/2 (p-ERK1/2) and the expression of programmed cell death ligand-1 (PD-L1) in tumor tissues. Western blot and real-time quantitative polymerase chain reaction (Real-time PCR) were used to detect the expression of p-ERK1/2, PD-L1, the autophagy marker Beclin-1, the autophagic protein p62, and the microtubule-associated protein light chains LC3Ⅰ and LC3Ⅱ at both the protein and gene levels. ResultsCompared with the model group, the average tumor weight was significantly reduced in the low and medium dose groups of Guiqi Yiyuan ointment (P<0.05), and markedly reduced in the high dose and inhibitor groups (P<0.01). Tumor cells in all treatment groups became progressively irregular, with ruptured nuclei and expanded areas of cell disintegration and necrosis. The number of organellar ablations in tumor tissues increased, and the number of autophagic vesicles also increased in all groups. The mean fluorescence intensity of p-ERK1/2 and PD-L1 was reduced in the low and medium dose groups of Guiqi Yiyuan ointment (P<0.05), and significantly reduced in the high dose and inhibitor groups (P<0.01). The mRNA expression of ERK1/2, PD-L1, Beclin-1, and p62 was reduced in the medium dose group (P<0.05), while LC3Ⅰ/Ⅱ mRNA expression was elevated (P<0.05). In the high dose and inhibitor groups, mRNA expression of ERK1/2, PD-L1, Beclin-1, and p62 was significantly reduced (P<0.01), while LC3Ⅰ/Ⅱ mRNA expression was significantly increased (P<0.01). Protein expression of p-ERK1/2, PD-L1, Beclin-1, and p62 was reduced in the medium dose group (P<0.05), and LC3Ⅰ/Ⅱ protein expression was elevated (P<0.05). In the high dose and inhibitor groups, protein expression of p-ERK1/2, PD-L1, Beclin-1, and p62 was significantly reduced (P<0.01), while LC3Ⅰ/Ⅱ protein expression was significantly elevated (P<0.01). ConclusionGuiqi Yiyuan ointment may inhibit the activation of the ERK signaling pathway, downregulate the expression of p-ERK1/2, promote autophagic flux in tumor cells, and regulate the expression of PD-L1, thereby exerting an inhibitory effect on tumor growth in Lewis lung cancer mice.
4.Research progress of internal limiting membrane peeling in the treatment of rhegmatogenous retinal detachment

Rhegmatogenous retinal detachment(RRD)is a vision-threatening ocular emergency with the potential risk of blindness. Pars plana vitrectomy(PPV)is the treatment of choice for RRD, especially for complex retinal detachments such as posterior pole retinal tears, grade B or higher proliferative vitreoretinopathy, and concomitant choroidal detachment. Although most patients achieve good anatomical restoration after surgery, some patients may still experience postoperative symptoms such as distorted vision, abnormal color perception, and lack of improvement in corrected visual acuity. Epiretinal membranes(ERM), as one of the most common complications after RRD, occur in 4% to 13% of cases, which not only affect the recovery of patients' postoperative vision, but also are frequently associated with visual distortion. In severe cases, a second surgical intervention may be required. In recent years, internal limiting membrane peeling(ILMP)has been introduced as an adjunctive procedure in the treatment of RRD, to facilitate retinal reorientation, reduce postoperative complications, and improve patients' postoperative visual quality and quality of life. However, previous studies on the effects of combined intraoperative ILMP on retinal anatomy and visual function recovery are limited. Therefore, this article reviews the mechanism, clinical application, challenges and research progress of ILMP in RRD, providing a reference for surgical decision making, disease assessment, and prognosis evaluation.
5.Design for assisted management system based on disease DRG for medical consumables

Objective:To design an assisted management system for medical consumables that integrated management logic of diagnosis related groups(DRG)for disease,so as to standardize the use for medical consumables and to strengthen regulatory efficiency.Methods:The system was designed by a microservices architecture,and the data fusion of business data,data of diagnosis and treatment,and DRG data was realized through constructed data lake.A knowledge graph of associated rule for medical consumables was established,and the intelligent analysis function included prediction for demand,usage optimization,and anomaly detection was provided to assist management decision-making on the basis of inference for historical data and knowledge,and multi-dimensional data queries.A rule engine was deployed at key clinical nodes for real-time compliance review and intelligent alerts.A DRG-based business management process was constructed to cover full lifecycle of consumables,including admission,procurement,warehousing,storage,requisition,usage billing,and traceability,so as to realize real-time monitoring and early warning for consumable costs at the DRG level.Six clinical doctors with 2-3 years of experience in using medical consumable were selected as test users from Tangdu Hospital of the Air Force Medical University,and they were randomly assigned into a system group and a control group,with 3 subjects in each group.The system group utilized the auxiliary management system for medical consumable to perform prediction for the demand of monthly procurement,and rationality verification of consumable.The control group relied on individual experience of doctors.The predicted monthly procurement demand for medical consumables,the accuracy of verifying rationality of using medical consumables between two groups were compared,and the efficiencies of two kinds of management modes also were compared.Results:The accuracy rate of system group was 89.17%in predicting the monthly procurement demand for medical consumables,and the rate of checking accuracy and the rate of checking comprehensiveness of system group were respectively 87.50%and 91.67%in verifying rationality of using medical consumables,which were significantly higher than those of control group,and the differences were significant(x2=6.62,1.96,16.73,P<0.05).The durations of predicting the demand for procurement,and verifying and testing the rationality of consumables in system group were significantly shorter than these in control group[(3.54±0.45)s,(2.23±0.15)s],and the differences were statistically significant(t=1.97,1.65,P<0.05).Conclusion:The medical consumables auxiliary management system integrated with DRG management logic can significantly enhance the accuracy of monthly demand prediction for consumables and the recall rate and precision rate of rationality verification for consumables usage,while greatly improving work efficiency.It is conducive to enhancing the standardized and refined supervision level of consumables usage.
6.Clinical efficacy evaluation of unilateral and bilateral puncture vertebral body shaping in the treatment of osteoporotic vertebral compression fractures

Objective:To compare the clinical efficacy of unilateral puncture and bilateral puncture vertebroplasty in osteoporotic compression fracture patients.Methods:A regression comparative analysis was performed using case-control study to collect clinical data of 372 patients with osteoporosis compression fractures admitted to Juxian People′s Hospital from January 2022 to July 2023, including 213 males and 159 females, and the age was (59.8±6.2) years old, aged range of 50 to 70 years old. 194 patients treated with unilateral puncture vertebroplasty were included in the unilateral group, and 178 patients treated with bilateral puncture vertebroplasty were included in the bilateral group. The intraoperative indicators, vertebral changes before surgery and 6 months after surgery, and the occurrence of adverse reactions after surgery of the two groups of patients were compared. The visual analogue scale (VAS), activities of daily living (ADL) score, oswestry disability index (ODI), and quality of life (QOL) scale were used to compare the pain degree, activity ability, improvement of functional disability, and changes in quality of life and clinical efficacy of the two groups of patients before the operation and at 3 and 6 months after the operation. SPSS 21.0 software was conducted to analyze data. Measurement data with normal distribution were represented as mean±standard deviation( ± s), and the comparison between groups was conducted using the t-test. The comparison of count data were represented as n(%) and was conducted by χ2 test or Fisher exact probability. Between-group comparisons of different time intervals were performed with repeated-measures analysis of variance test. Results:The surgical time, cement dosage, bleeding amount, and fluoroscopy of patients in the unilateral group were (29.86±5.87) min, (2.58±0.37) mL, (22.65±4.83) mL, and 12 times, respectively, and (42.63±4.55) min, (7.12±0.61) mL, (50.31±5.84) mL, 25 times in the bilateral group, respectively. The difference between the two groups was statistically significant ( P<0.05). Preoperatively, the anterior height ratio of the vertebral body, sagittal concavity angle, and kyphosis angle of the affected vertebra in the unilateral group were (59.83±5.55)%, 13.04°±1.14°, and 18.93°±2.56°, respectively, while in the bilateral group, there were (60.28±5.79)%, 13.08°±1.13°, and 18.67°±2.69°, respectively. After the operation, the unilateral group were (90.20±5.52), 3.95°±0.57°, and 11.03°±1.21°, respectively. The bilateral group were (90.58±6.00), 3.99°±0.59°, and 10.91°±1.14°, respectively. Comparison of data before and after surgery between the two groups, the difference was statistically significant ( P<0.05). The overall complication rate of unilateral group was 13 cases (6.70%), and the overall complication rate of bilateral group was 26 cases (14.61%). The difference between the two groups was statistically significant ( P=0.013). The VAS, ADL, ODI, and QOL of the unilateral group were (3.49±1.10), (66.10±13.02), (18.22±4.81) and (62.10±10.00) points, the two-sided groups were (5.49±1.13), (56.19±10.27), (24.76±5.81) and (52.13±9.56) points, respectively. The differences between the two groups were statistical significance ( P<0.05), the overall treatment efficiency of patients in the unilateral group was 90.72%, and the overall treatment efficiency of patients in the bilateral group was 83.15%, and the difference between the two groups was statistically significant( P<0.05). Conclusions:Unilateral puncture vertebroplasty has the advantages of short operation time, less bone cement usage, low blood loss, fewer fluoroscopy times, low complication rate, better postoperative pain relief and functional recovery, and high overall treatment efficiency in the treatment of patients with osteoporotic compression fractures. It has obvious advantages over bilateral puncture vertebroplasty.

9.Research progress on the pathological mechanism of meibomian gland dysfunction in diabetic patients

Meibomian gland dysfunction is a chronic and diffuse disease of the meibomian glands, characterized by obstruction and(or)abnormal secretion of the terminal ducts. Clinically, it can lead to tear film abnormalities and inflammation of the ocular surface, resulting in symptoms of ocular irritation and potential corneal damage that may impact visual function. Meibomian gland dysfunction can be classified into two types based on meibomian gland secretion: low secretion type and high secretion type. The low secretion type further includes acinar atrophy type and obstruction type. In recent years, research has revealed that patients with diabetes experience chronic damage to their meibomian gland tissue in the early stages of the disease, leading to structural and functional changes. The incidence and severity of meibomian gland dysfunction are higher in diabetic patients. However, there are numerous complex factors contributing to this condition in diabetes patients, and mechanisms remain unclear at present. This article reviews both domestic and international research progress on the pathological mechanism underlying meibomian gland dysfunction in diabetes.
10.Astigmatism inclusivity of regional refractive intraocular lens

AIM: To investigate the effect of residual astigmatism on visual quality after phacoemulsification combined with regional refractive intraocular lens(IOL)implantation in patients with age-related cataract, and to evaluate the astigmatism inclusivity of regional refractive IOL.METHODS: Retrospective cohort study. The clinical data of 62 cases(73 eyes)of age-related cataract patients who underwent phacoemulsification combined with regional refractive IOL(Lentis Comfort LS-313 MF15)implantation from July 2020 to March 2022 at the ophthalmology department of our hospital were collected. They were grouped according to residual astigmatism at 6 mo postoperatively, taking 35 cases(40 eyes)with residual astigmatism of 0.75 D to 1.50 D as the experimental group, and 27 cases(33 eyes)with residual astigmatism ≤0.75 D as the control group. Visual acuity, defocus curves, objective visual acuity [wavefront aberrations, Strehl ratio(SR), modulation transfer functions(MTF)], subjective visual acuity(national eye institute visual function questionnaire-25), patients' satisfaction, and spectacle independence were compared between the two groups at 6 mo postoperatively.RESULTS:There was a difference in the preoperative astigmatism and the number of postoperative 6 mo residual astigmatism between the two groups(P<0.01). At 6 mo postoperatively, there was no difference in uncorrected distance, intermediate, and near visual acuity, objective visual quality, subjective visual quality, satisfaction, and spectacle independence between the two groups(P>0.05). The defocus curves showed that there was no difference in visual acuity between the two groups at all points in the +2.00 to -4.00 D defocus range of the additional spherical equivalent(P>0.05).CONCLUSION:Lentis Comfort LS-313 MF15 IOL was able to accommodate regular astigmatism of 1.50 D.
