1.Computer-simulated repositioning combined with pelvic reduction frame for treatment of anteroposterior compression-III pelvic fractures

BACKGROUND:Pelvic fractures encompass a range of types,and the utilization of a pelvic reduction frame for restoration often lacks a systematic repositioning method.Instead,it relies on the operator's experience in conjunction with fluoroscopic findings,which can lead to uncertainty and non-reproducibility. OBJECTIVE:To investigate the clinical efficacy of combining computer-simulated repositioning techniques with a pelvic reduction frame for the treatment of anteroposterior compression-III pelvic fractures. METHODS:A retrospective analysis was conducted on 19 patients with anteroposterior compression-III pelvic fractures who underwent preoperative repositioning via computer simulation and intraoperative repositioning with the assistance of a pelvic reduction frame between January 2018 and December 2021.Among them,7 cases were fixed with double plate in anterior ring and 12 cases were fixed with single plate combined with anterior subcutaneous internal fixation(INFIX).All patients received posterior ring fixation with two sacroiliac screws.Operative duration,intraoperative reduction time,the frequency of intraoperative fluoroscopy use,blood loss,and follow-up duration were documented.These data were utilized to monitor fracture healing time and postoperative complications.Fracture reduction quality was evaluated according to the Matta scale,and the Majeed Pelvic Function Score was employed to assess patient function during the final follow-up. RESULTS AND CONCLUSION:(1)Surgery was successfully completed in all 19 patients.The anterior ring was secured with double plates in 7 cases,while a single plate combined with INFIX was utilized in 12 cases.The posterior ring was stabilized with two sacroiliac screws,specifically targeting the S1 and S2 cones.(2)The operation duration ranged from 74 to 147 minutes,with a mean of(101.63±19.55)minutes.Intraoperative repositioning took place over a period of 26 to 41 minutes,with a mean of(38.11±3.31)minutes.The number of intraoperative fluoroscopies conducted ranged from 35 to 81,with a mean of(62.68±13.11)times.Intraoperative bleeding volumes varied from 60 to 130 mL,with a mean of(85.37±20.57)mL.(3)All the patients were diligently monitored for a duration of 12 to 26 months.Fracture healing was observed within a time frame of 12 to 20 weeks,with a mean of(16.37±2.50)weeks.(4)The evaluation according to Matta's criteria one day post-surgery revealed excellent outcomes in 14 cases and good outcomes in 5 cases.At the final follow-up,the Majeed function score indicated excellent results in 16 cases and good results in 3 cases.(5)Two patients experienced localized fat liquefaction phenomena,characterized by redness,swelling,and oozing at the incision site,which gradually resolved with proactive dressing changes.None of the patients encountered complications such as internal fixation loosening,loss of fracture reduction,or nerve injuries post-surgery.It is concluded that the combined approach of using computer-simulated repositioning techniques in conjunction with pelvic reduction frames for the treatment of anteroposterior compression-III pelvic fractures has advantages in enhancing repositioning efficiency and improving pelvic function.
2.Application of Gas Chromatography Ion Mobility Spectrometry Technology Combined with Chemometric Methods in Identification of Foeniculi Fructus from Haiyuan Region

ObjectiveTo establish a geographical origin identification model for Foeniculi Fructus from Haiyuan, providing a new technical reference for the protection of Haiyuan's geo-authentic medicinal materials and its designation as a national geographical indication agricultural product. MethodsSamples of Foeniculi Fructus were collected from eight producing areas, including Minqin (Gansu), Bozhou (Anhui), Qingdao (Shandong), Dezhou (Shandong), Urumqi (Xinjiang), Nujiang (Yunnan), Gutuo (Inner Mongolia), and Haiyuan (Ningxia). Gas chromatography-ion mobility spectrometry (GC-IMS) was used to detect the volatile organic compounds (VOCs) in samples from these geographic origins. VOCs were qualitatively analyzed through dual matching with the National Institute of Standards and Technology (NIST) mass spectral database and the IMS drift time database. Using the Reporter module and Gallery Plot visualization tools within the LAV analytical platform, VOC fingerprint profiles characterizing geographic origins were constructed. A non-targeted analytical strategy was adopted, and 97 VOCs detected via GC-IMS were subjected to principal component analysis (PCA) and partial least squares discriminant analysis (PLS-DA) based on their differential distribution patterns to construct an origin identification model for Foeniculi Fructus from Haiyuan region. Key discriminative markers were screened using variable importance in projection (VIP) values greater than 1. ResultsA total of 97 VOCs were identified, including alcohols, aldehydes, ketones, esters, organic acids, terpenoids, ethers, alkenes, and benzenes. The PLS-DA model, based on VOCs data obtained by GC-IMS, effectively distinguished Foeniculi Fructus in Haiyuan region from those of other origins. During cross-validation, the model achieved a prediction parameter (Q2) of 0.976 and a goodness-of-fit parameter (R2) of 0.936, with no overfitting observed in permutation testing. Twelve key flavor markers with VIP > 1 were identified as characteristic indicators of Haiyuan origin. ConclusionA stable and highly predictive origin identification model for Foeniculi Fructus from Haiyuan was successfully established using GC-IMS technology, PLS-DA, and VIP-based marker screening. This model provides a novel technical strategy for accurately distinguishing Foeniculi Fructus in Haiyuan region from other regional varieties and offers new technical support for its protection as a geo-authentic medicinal material and a nationally designated geographical indication agricultural product in China.

4.Summary of YIN Kejing's experience in treating lateral epicondylitis with classical acupuncture techniques.

Professor YIN Kejing's academic thoughts and clinical experience in treating lateral epicondylitis using classical acupuncture techniques are introduced. Professor YIN Kejing believes that the onset of lateral epicondylitis is due to external pathogenic factors and internal overstrain, leading to a pathological mechanism of muscular tension, meridian obstruction, and qi-blood stasis, resulting in disharmony between nutritive qi and defensive qi. Treatment should adhere to the principles of disease pathogenesis, with meticulous diagnosis and emphasis on relaxing tendons, relieving muscular tension, dredging meridians, and regulating nutritive qi and defensive qi. Based on the theories of yin-yang balance, nutritive qi and defensive qi regulation, and the collateral connection of meridians, the treatment involves selecting positive muscular reaction points, the five-shu points of the related meridians, and collateral connection points. Classical acupuncture techniques, including floating needling, guan needling, three-needle needling, cross-needling, and through-needling, are applied to stimulate defensive qi, regulate nutritive qi, facilitate the transmission of meridian qi, and adjust tendon qi. This approach effectively relieves muscular tension, alleviates pain, restores function, and rebalances yin and yang. The treatment is characterized by precise acupoint selection, distinctive features, and remarkable efficacy.

The writing of pharmacist-managed clinics documents (hereinafter referred to as “outpatient medication record”) is a necessary part of pharmacist-managed clinics service. Outpatient medication record is an important carrier to reflect the quality of pharmacist-managed clinics service. The Chinese Hospital Association Pharmaceutical Specialized Committee was entrusted by the Pharmaceutical Administration Department of the National Health Commission to lead the formulation of the Guidelines for the Pharmacist-managed Clinics Service and Document Writing and Usage (Reference) (hereinafter referred to as Guidelines) according to the compilation method of group standards and the technical route of “documentation combing→framework establishment→draft writing→opinion collection→Guidelines formation”. The Guidelines standardizes the basic requirements of pharmacist-managed clinics record management and the basic content of record, and provides a general template and two specialized templates including pregnant and lactating pharmacist-managed clinics record template and cough and asthma pharmacist-managed clinics record template, which provides a reference for medical institutions to write pharmacist-managed clinics record. This paper introduces the formulation process of Guidelines and analyzes the key contents of Guidelines, which is helpful for the application practice of Guidelines and further improves the quality of pharmacist-managed clinics work.
6.Attention impairment in patients with temporal lobe epilepsy

Attention impairment in temporal lobe epilepsy (TLE) leads to other advanced cognitive impairments, such as memory and emotional recognition. Moreover, the severity of attention impairment determine the differences in strategies for preventing sudden unexpected death in epilepsy (SUDEP). Factors such as abnormal brain electrical discharges, sleep, and antiepileptic drugs are all associated with attention impairment. Neuroelectrophysiological observations have shown that abnormal electrical discharges interfere with the synchronization of cortical oscillations, and preventing the spread of abnormal epileptic discharges to subcortical activation structures helps reduce attention disorders. Neuroimaging reveals damage to the gray matter and white matter of the brain related to attention and poor connectivity of the whole-brain network. Therefore, early identification of attention impairment in TLE patients has become even more important. Treatment of attention impairment includes supplementing attention-related neurotransmitters, cognitive-behavioral therapy, surgical procedures, and neuromodulation techniques. This article mainly reviews the characteristics of attention impairment in TLE patients from the perspectives of clinical behavior, pathophysiology, and treatment, aiming to provide new insights for the early identification and management of attention impairment in TLE patients.
7.Practice and analysis of implementing drug traceability code management in outpatient pharmacy

OBJECTIVE To explore optimization pathways for the drug traceability code management model in outpatient pharmacy workflows, providing practical evidence for enhancing the efficiency of pharmaceutical service. METHODS Taking the outpatient pharmacy of the First Affiliated Hospital of Sun Yat-sen University as the research subject, a comprehensive drug traceability system was established through three key interventions: upgrading the information system architecture [including integration of the hospital information system (HIS) with the traceability platform], workflow optimization (reorganizing the inventory-dispensing-verification tripartite process), and designing a dual-mode traceability data collection mechanism (primary data capture at dispensing stations and supplementary capture at verification stations). Operational efficiency differences before and after implementation were analyzed using the medical insurance data and service timeliness metrics in September 2024. RESULTS After the implementation of drug traceability code management, in terms of data collection: Mode Ⅰ (verification-stage capture) uploaded 26 144 records, while Mode Ⅲ (inventory-as-sales capture) uploaded 443 061 records, totaling 469 205 entries; in terms of time efficiency: average drug dispensing time increased from 28.74 s to 43.37 s (enhanced by 51%). Through dynamic staffing adjustments, patient wait time only extended from 8.04 min to 8.67 min (enhanced by 8%). CONCLUSIONS Drug traceability code management can be effectively implemented via a “system reconstruction-process reengineering-human-machine collaboration” trinity strategy, leveraging informatization (e.g., dual-mode data capture) to offset manual operation delays, which validates the feasibility of balancing national traceability demands with service efficiency in outpatient pharmacies.
8.Role of FAP-mediated cancer-associated fibroblast function in the immune microenvironment and therapeutic response of triple-negative breast cancer

Background and Aims:Triple-negative breast cancer(TNBC)is a clinically aggressive subtype of breast cancer with limited treatment options.Immune checkpoint blockade(ICB)combined with chemotherapy has emerged as a key neoadjuvant therapeutic strategy for TNBC.However,significant variability in ICB efficacy exists among patients,and the underlying mechanisms related to the tumor immune microenvironment(TME)remain unclear.Cancer-associated fibroblasts(CAFs),as major stromal components,regulate TME and influence immunotherapy responses.Fibroblast activation protein(FAP),a key marker of CAFs,has been associated with poor prognosis in multiple solid tumors,yet its immunological role in TNBC has not been systematically investigated.This study aims to elucidate the expression pattern of FAP in TNBC,its impact on the immune microenvironment and ICB efficacy,and to explore its potential immunosuppressive mechanisms and clinical implications.Methods:The data from TCGA and the I-SPY2 clinical trial were integrated to assess the association of FAP expression with prognosis,immune cell infiltration,and immune checkpoint molecule expression in TNBC.Immune landscape profiling was conducted using CIBERSORT,GSEA enrichment analysis,and differential gene expression analysis(DESeq2)to characterize the immune features associated with FAP expression and to identify downstream genes at the transcriptomic level.CAF models with FAP overexpression or knockdown were constructed and co-cultured with CD8? T cells to evaluate FAP's regulatory effects on CD8? T cell activity and apoptosis.The expression of COL1A1,a potential FAP-regulated gene identified from transcriptomic analysis,was validated using qPCR and Western blot.Finally,clinical data and multiplex immunofluorescence pathology samples from TNBC patients at Xiangya Hospital were analyzed alongside I-SPY2 data to determine the predictive value of FAP expression for pathological complete response(pCR)following neoadjuvant immunotherapy.Results:FAP was significantly upregulated in TNBC tumor tissues compared to normal tissues and associated with shorter overall survival.Multivariate Cox regression analysis identified FAP as an independent adverse prognostic factor.High FAP expression was correlated with reduced infiltration of CD8? T cells,NK cells,and Tfh cells,as well as upregulation of immune checkpoints including CD276,TIM-3,and PD-L2.In CAF models,FAP overexpression suppressed CD8? T cell activity and promoted apoptosis,while FAP knockdown had the opposite effect.Transcriptomic analysis showed that COL1A1 and other collagen-related genes were significantly upregulated in the FAP-high group and positively correlated with FAP expression;qPCR and Western blot confirmed that FAP positively regulates COL1A1 expression.Analysis of I-SPY2 data revealed that FAP-low patients receiving pembrolizumab plus chemotherapy had significantly higher pCR rates compared to FAP-high patients.Consistently,clinical data from the Xiangya cohort showed reduced CD8? T cell infiltration and lower pCR rates in FAP-high patients,with a ROC AUC of 0.857 for predicting treatment response.Conclusion:FAP-high CAFs contribute to the formation of an immunosuppressive TME in TNBC by promoting COL1A1 secretion,inhibiting CD8? T cell function,and upregulating immune checkpoint molecules.High FAP expression is associated with poor prognosis and reduced response to immunotherapy,highlighting FAP as both a prognostic biomarker and a potential therapeutic target for stratified and combination treatment strategies in TNBC.

Therefore, it is crucial to explore the relationship between epilepsy, VPA, metabolic syndrome and reproduction, the underlying pathogenesis, risk factors, and therapeutic measures. This exploration is essential for preventing adverse drug reactions and for guiding safe and rational use of drugs in clinical practice. This article summarizes the incidence of drug-induced metabolic syndrome and infertility in men with epilepsy who are treated with oral VPA, aiming to provide new research insights for optimizing treatment and reducing drug side effects.
