1.Effect of Yiqi Yangyin Huoxue Formula(益气养阴活血方)on Oxidative Stress and Inflammatory Injury in Membranous Nephropathy Model Rats:Based on the NOX4-mediated SDF-1α/CXCR4 Signaling Pathway

ObjectiveTo investigate the potential mechanism of Yiqi Yangyin Huoxue Formula (益气养阴活血方, YYHF) in the treatment of membranous nephropathy (MN) based on the NOX4-mediated stromal cell-derived factor-1α(SDF-1α)/CXC chemokine receptor 4 (CXCR4) signaling pathway. MethodsA total of 42 male SD rats were randomly divided into blank group (n=6) and modeling group (n=36). The modeling rats were given a single injection of sheep antirat Fx1A (6 ml/kg) serum into the tail vein to establish MN model. Thirty-six rats with successful modeling were further divided into model group (normal saline, 10 ml/kg), benazepril group (10 mg·kg-1·d-1), the medium-dose YYHF group (11.0 g·kg-1·d-1) and high-dose YYHF group (22.0 g·kg-1·d-1), with 9 rats in each group. All groups received daily intragastric administration for 6 consecutive weeks. During the experimental period, the general condition of rats in each group was observed. After gavage administration finished, a fully automated biochemical analyzer was used to measure the 24-hour urine total protein (24 h-UTP) and serum biochemical parameters, including albumin (ALB), aspartate aminotransferase (AST), blood urea nitrogen (BUN), serum creatinine (SCr), total cholesterol (TC), and triglyceride (TG). Hematoxylin-eosin (HE) staining and periodic acid-silver metheramine (PASM) staining were used to observe the histopathological changes of renal tissues in each group of rats. The levels of superoxide dismutase (SOD) and malondialdehyde (MDA) in renal tissues were detected by ELISA. The protein expression levels of NOX4, SDF-1α, CXCR4, Nrf2, nuclear factor kappa B subunit p65 (NF-κB p65) and podocyte slit diaphragm protein nephrin in rat renal tissues were detected by Western Blotting. The mRNA expressions of NOX4, SDF-1α, CXCR4, Nrf2, NF-κB p65 and Nephrin in renal tissues were detected by real-time fluorescence quantitative polymerase chain reaction (RT-PCR). ResultsCompared to the blank group, rats in the model group exhibited typical features of MN, including lethargy, reduced activity, dull and coarse fur, marked edema, and massive proteinuria. Compared to the model group, these abnormal conditions were improved to varying degrees in all treatment groups. The high-dose YYHF group showed a level of improvement comparable to that of the benazepril group, with overall recovery superior to that of the medium-dose YYHF group. Compared to the blank group, the model group showed increased levels of 24 h-UTP, BUN, SCr, TC and TG, decreased level of ALB, increased levels of MDA and protein and mRNA expressions of NOX4, SDF-1α, CXCR4 and NF-κB p65 in renal tissue, along with decreased levels of SOD and protein and mRNA expressions of Nrf2, Nephrin (P<0.01), accompanied by severe renal pathological damage. Compared to the model group, the benazepril and the medium- and high-dose YYHF groups showed significant improvements in the above indicators (P<0.01), along with alleviation of renal pathological injury. The 24 h-UTP level in the high-dose YYHF group was lower than that in the medium-dose group (P<0.01), while no statistically significant difference was observed among the other treatment groups (P>0.01). ConclusionYYHF may alleviate oxidative stress and inflammatory responses by down-regulating the expression of NOX4 and inhibiting the SDF-1α/CXCR4-Nrf2 signaling pathway, thereby improving podocyte damage and protecting renal function.
2.Effects of volatile oil from Acorus tatarinowii on CX3CL1/CX3CR1 signal axis and neuroinflammation in a rat model of tic disorders

AIM To investigate the effects of volatile oil from Acorus tatarinowii Schott(A.tatarinowii)on neuroinflammation in a rat model of tic disorders.METHODS The SD rats were randomly divided into the blank group(8 rats)and the model group(40 rats).The rat models of tic disorders established successfully by intraperitoneal injection of iminodiapropionitrile(IDPN)were further divided into the model group,the tiapride group and the high-dose,moderate-dose and low-dose A.tatarinowii volatile oil groups,with 8 rats in each group.The 4-week intragastric treatment of respective drug was initiated the next day after the completion of modeling,and normal saline was dosed upon the blank group and the model group,during which the rats' behavioral changes were assessed by stereotyped behavior and motor behavior score every week.After the administration,the rats had their morphological changes of striatal neurons observed by Nissl staining;their levels of TGF-β,IL-10,TNF-αand IL-1β in serum and striatum detected by ELISA;their striatal protein expressions of CX3CL1 and CX3CR1 detected by Western blot and immunohistochemistry;and their striatal expressions of M1,M2 microglia marker proteins CD86,CD206,SYN and PSD-95 detected by immunofluorescence co-staining.RESULTS Compared with the model group,the A.tatarinowii volatile oil groups demonstrated improved twitch-like behavior;decreased scores of motor behavior and rigid behavior(P<0.01);alleviated damage of Nissl bodies in neurons;increased serum and striatum levels of TGF-β and IL-10(P<0.05,P<0.01);decreased levels of TNF-α and IL-1β(P<0.01);decreased striatal protein expressions of CX3CL1 and CX3CR1(P<0.01);increased protein expressions of PSD95 and SYN(P<0.05,P<0.01);and decreased CD86/Iba1(P<0.01)and increased CD206/Iba1(P<0.01)in terms of the fluorescence intensity.CONCLUSION A.tatarinowii volatile oil contributes an anti-tic effect and improves the neuroinflammation in the brain of the rat model of tic disorders by promoting the transformation of microglia into M2 type via CX3CL1/CX3CR1 signal axis.
3.Correlation between overactive bladder and neuropsychological characteristics in patients with cerebral small-vessel disease

Objective To explore the relationship between overactive bladder(OAB)and neuropsychological characteristics in patients with cerebral small-vessel disease(CSVD).Methods A total of 208 patients with CSVD admitted to the First Affiliated Hospital of Shihezi University from December 2023 to November 2024 were selected as research subjects.They were divided into OAB group(44 cases)and non-OAB group(164 cases)according to overactive bladder symptom score.The general information and imaging data of two groups of patients were compared.The cognitive function,anxiety and depression of patients were evaluated by using Montreal cognitive assessment(MoCA),digit span test(DST),auditory verbal learning test(AVLT),verbal fluency test(VFT),clock drawing test(CDT),Hamilton anxiety scale(HAMA)and Hamilton depression scale(HAMD).Logistic regression analysis was used to analyze the influencing factors of OAB in patients with CSVD.Pearson correlation analysis was used to analyze the correlation between imaging indicators and anxiety symptoms of patients in OAB group.Results The HAMA and HAMD scores of patients in OAB group were higher than those in non-OAB group(P<0.05),but there were no statistically significant differences in MoCA,DST,AVLT,VFT,and CDT scores between two groups(P>0.05).There was a statistically significant difference in the incidence of anxiety among patients with different degrees of OAB(P<0.05).Multivariate Logistic regression analysis after adjusting for confounding factors showed that anxiety was an independent influencing factor for OAB in patients with CSVD(P<0.05).The periventricular white matter hyperintensity of patients in OAB group was negatively correlated with the HAMA score(P<0.05),while basal ganglia lacunar infarction,centrum semiovale lacunar infarction,basal ganglia enlarged perivascular space and centrum semiovale enlarged perivascular space were all positively correlated with the HAMA score(P<0.05).Conclusion The anxiety and depression symptoms of patients with CSVD accompanied by OAB were more severe than those of non-OAB patients.Anxiety may be an independent influencing factor for the occurrence of OAB in patients with CSVD.
4.Expression Levels of Plasma Lp-PLA2 and SIRT1 in Patients with Sepsis Complicated with Acute Kidney Injury and Their Relationship with Short-Term Prognosis

Objective:To explore the changes in plasma lipoprotein associated phospholipase A2(Lp-PLA2)and silencing information regulatory protein 1(SIRT1)in patients with sepsis complicated with acute kidney injury(AKI)and their relationship with short-term prognosis.Methods:243 sepsis patients who received treatment in our hospital from May 2022 to May 2024 were prospective selected,including 80 sepsis patients with AKI(AKI group)and 163 sepsis patients without AKI(non AKI group),the plasma Lp-PLA2 and SIRT1 levels between the two groups were compared.They were divided into good prognosis group and poor prognosis group according to 28 d prognosis after admission in AKI group.The influencing factors of short-term prognosis in sepsis patients complicated with AKI were analyzed by multiple logistic regression model.The short-term prognostic value of plasma Lp-PLA2 and SIRT1 alone and in combination for sepsis complicated with AKI patients was analyzed using receiver operating characteristic(ROC)curve.Results:Compared with non AKI group,AKI group had higher Lp-PLA2 and lower SIRT1(P<0.05).39 deaths within 28 d after admission in AKI group(poor prognosis group),41 cases survived(good prognosis group),with poor prognosis rate of 48.75%(39/80).Sequential organ failure assessment(SOFA)score,acutephysiology and chronic health evaluationⅡ(APACHEⅡ)score,creatinine(Scr),lactate dehydrogenase albumin ratio(LAR)in poor prognosis group were higher than those in good prognosis group,while procalcitonin(PCT)was lower than that in good prognosis group(P<0.05).Compared with the good prognosis group,poor prognosis group had higher Lp-PLA2 and lower SIRT1 at admission(P<0.05).Elevated SOFA score,elevated Scr,elevated APACHEⅡ score,and elevated plasma Lp-PLA2 were risk factors for poor prognosis in sepsis patients complicated with AKI(P<0.05),while elevated plasma SIRT1 was a protective factor(P<0.05).ROC curve analysis results showed that,the combined detection of plasma Lp-PLA2 and SIRT1 predicted a poor prognosis for sepsis patients with AKI with an area under the curve(AUC)of 0.935,which was better than the prediction of 0.813 and 0.858 for plasma Lp-PLA2 and SIRT1 alone.Conclusion:Sepsis complicated with AKI patients have elevated plasma Lp-PLA2 and decreased SIRT1,combined detection of the two can assist in predicting the risk of poor prognosis.
5.Clinical efficacy of TIPS combined with main splenic artery embolization in the treatment of portal hypertension upper gastrointestinal bleeding complicated with extensive portal vein thrombosis

Objective:To analyze the efficacy of transjugular intrahepatic portosystemic shunt (TIPS) combined with main splenic artery embolization in the treatment of patients with portal hypertension upper gastrointestinal bleeding complicated with extensive portal vein thrombosis (PVT).Methods:This study was a prospective, single-center, open-label, single-arm clinical trial. In the first phase, 81 patients with portal hypertension upper gastrointestinal bleeding who were admitted to Beijing Shijitan Hospital, Capital Medical University from January 2018 to December 2018 were consecutively enrolled, including 57 males and 24 females, with the age of (51.3±10.4) years. During TIPS surgery, the pressure of the portal vein before and after the balloon blocking the splenic artery was measured to clarify the contribution of the splenic artery to portal hypertension. In the second stage, from January 2019 to December 2022, 104 patients with portal hypertension upper gastrointestinal bleeding complicated with extensive PVT were re-enrolled, including 71 males and 33 females, with the age of (50.9±12.5) years. TIPS combined with main splenic artery embolization was performed, and portal vein pressure was measured before and after embolization. Follow up on the postoperative esophageal and gastric varices of the patients in the second stage.Results:The portal vein pressures before and after the first stage of balloon occlusion of the splenic artery were (35.2±8.4) mmHg (1 mmHg=0.133 kPa) and (24.2±6.3) mmHg, respectively. The pressure after occlusion was lower than that before occlusion, and the difference was statistically significant ( t=10.54, P<0.001). The portal vein pressures before and after the second stage embolization were (36.1±9.5) mmHg and (21.1±4.7) mmHg respectively. The pressure after embolization was lower than that before embolization, and the difference was statistically significant ( t=13.47, P<0.001). In the second stage, among the 104 patients, the proportion of those whose varicose veins disappeared or improved 6 months after the operation was 43.3%(45/104) and 51.0%(53/104), respectively. There were no patients with aggravation or rebleeding due to rupture. One year later, 8 patients (7.7%) had aggravated or ruptured esophageal and gastric varices with bleeding. Two years later, 12 patients (11.5%) had aggravated or bleeding. Conclusion:TIPS combined with main splenic artery embolization can effectively reduce the portal vein pressure in patients with portal hypertension upper gastrointestinal bleeding complicated with extensive PVT, improve the degree of esophageal and gastric varices, and reduce the risk of gastrointestinal bleeding.
6.Extracellular vesicles secreted by adriamycin-resistant breast cancer cells promote the proliferation and migration of HUVEC and tumor angiogenesis

Objective To investigate the regulatory effects and molecular mechanisms of extracellular vesicles(EVs)secreted by adri-amycin(ADR)-resistant breast cancer cells on vascular endothelial cells related to tumor microenvironment.Methods The ADR-sen-sitive breast cancer cell line(MCF-7)and its ADR-resistant counterpart(MCF-7-ADR)were established in vitro.Then,the EVs were isolated from the supernatant of cell culture using a commercial kit.The fluorescence labeling and tracking technology using PKH67 was used to validate the internalization of EVs by human umbilical vein endothelial cell(HUVEC).The effects of EVs on the proliferation,migration,and angiogenesis of HUVEC were assessed by the colony formation,scratch wound healing,and tube formation assays,re-spectively.The expression level of CD31 protein was detected by Western blot,and the levels of vascular endothelial growth factor(VEGF)in cell supernatant were determined by ELISA.The microRNA(miRNA)sequencing technology was utilized to identify the differentially expressed miRNAs in EVs secreted by ADR-resistant cells.Results Compared with the ADR-sensitive strain,the EVs secreted by the ADR-resistant strain significantly enhanced the proliferation(colony number increased by 38.7%,P=0.011),migra-tion(scratch healing rate improved by 42.3%,P=0.004),and tube formation ability(tube formation rates increased by 1.8 times and 2.3 times at 4 h and 16 h,respectively,P=0.044 and 0.010)of HUVEC.Western blot analysis showed that the expression level of CD31 in the EVs secreted by the ADR-resistant strain increased by 1.85 times compared with the ADR-sensitive strain(P=0.039).The results of ELISA showed that the expression level of VEGF increased by 2.12 times(P=0.012).The miRNA sequencing identified a differentially expressed miRNA profile.Conclusion The ADR-resistant breast cancer cells significantly enhance the proliferation,migration,and angiogenesis of vascular endothelial cells by secreting EVs rich in specific miRNA,and its mechanism may be related to the up-regulation of CD31/VEGF protein levels mediated by EVs,suggesting that the drug-resistant cells reshaping the tumor vascular microenvironment through EVs may become a new therapeutic target of chemotherapy resistance in breast cancer.
7.Automatic diagnosis method for keratitis based on regional image patches from slit-lamp images

A method that integrates the features of image patches from corneal lesions and conjunctival congestion-like complications is proposed to address the limitations of manual keratitis diagnosis(i.e.,time-consuming,laborious,high subjectivity)and the generally low accuracy of automatic keratitis diagnosis based on original slit-lamp images.Specifically,samples are acquired from the corneal and conjunctival regions.A cost-sensitive convolutional neural network is then used to extract and concatenate the high-level features of these image patches.After dimensionality reduction through principal component analysis,the processed features are input into the fully connected layer for classification.Trained and evaluated on 6414 slit-lamp images collected from Ningbo Eye Hospital,the proposed method achieves accuracies of 97.8%,98.6%,and 97.0%for keratitis,normal cornea,and other abnormal corneas,respectively.This method effectively integrates relevant features and provides a feasible solution for high-accuracy keratitis diagnosis.
8.Bone marrow mesenchymal stem cell-derived exosomes inhibit dexamethasone-induced C2C12 myotube atrophy

Aim To investigate the effect of exosomes derived from bone marrod-derived mesenchymal stem cells(BMSCs)on dexamethasone-induced C2C12 muscular canal atrophy.Methods(1)C57BL/6J mouse bone marrow mesenchymal stem cells were isola-ted and cultured by whole bone marrow adhesion meth-od.(2)Extraction and identification of BMSCs EXOs were performed.(3)Myogenic differentiation of C2C12 cells was carried out.(4)The successfully differentia-ted myotubes were divided into the control group(cul-tured in 2%equine serum medium for 48 h),dexam-ethasone group(dexamethasone,DEX,10 μmol·L-1 concentration of DEX interfered with myotubes for 48 h),and exosomes group(exosomes,EXOs,interfered with myotubes for 48 h),exosome inhibitor group(exo-somes extracted from BMSCs after 10 μm GW4869 in-tervention,interfered with myotubes for 48 h).48 h later,the morphology and diameter of muscle tubes were observed and measured by microscope.Cell via-bility of each group was detected by CCK-8 method.The expression levels of atrogin-1 and MuRF-1,myo-genic differentiation antigen(MYOD)in each group were detected by Western blot.Results BMSCs were long spusiform,and BMSCS-EXOS showed a circular bilayer structure under transmission electron microsco-py,with a diameter of about 200 nm.CD9,CD63 and CD81 were highly expressed.Compared with the con-trol group,cell activity in DEX group decreased(P<0.01),diameter of myotubes decreased(P<0.01),expressions of atrogin-1(P<0.05)and MuRF-1(P<0.01)were significantly up-regulated,and expression of MYOD(P<0.01)was significantly down-regula-ted.Compared with the DEX group,cell activity in the BMSCs-EXOs group increased(P<0.01),diameter of myotubes increased(P<0.01),expressions of atrogin-1(P<0.05)and MuRF-1(P<0.01)were signifi-cantly down-regulated,and expression of MYOD(P<0.01)was up-regulated.Compared with the BMSCs-EXOs group,cell activity of the BMSCs-EXOs(GW4869)group decreased(P<0.05),diameter of myotubes decreased(P<0.01),expressions of atrog-in-1(P<0.05)and MuRF-1(P<0.05)were up-regulated,and expression of MYOD(P<0.01)was down-regulated.Conclusion Bone marrow mesen-chymal stem cell-derived exosomes(BM-MSCs-EXOs)inhibit dexamethasone-induced C2C12 muscle tube at-rophy.
9.1 new case of hepatitis B-induced acute liver failure after receiving heart transplantation from a HBV-positive donor and literature review

OBJECTIVE To summarize the follow-up,survival time and treatment of a patients who received heart transplantation from a hepatitis B virus(HBV)-positive donor so as to raise the perception and awareness of pre-vention of hepatitis B after receiving the heart transplantation from a HBV-positive donor and extend the survival time of the patient.METHODS A patient who had new onset of hepatitis B after receiving heart transplantation from a HBV-positive donor and was treated in infectious diseases department of Zhongnan Hospital of Wuhan Uni-versity in Mar.2021 was enrolled in the study.The clinical data,including the main clinical symptoms,medication and treatment history,individual history,major positive signs and treatment outcomes,were retrospectively ana-lyzed.The process of diagnosis and treatment was analyzed and was further discussed in combination with litera-ture search.RESULTS The patient was treated with immunosuppression and anti-HBV therapy for 11 months after the heart transplantation,and the patient had anorexia,fatigue,diarrhea,and skin icteric sclera in the current month of withdrawal of drug therapy.The patient was admitted to the hospital and was finally diagnosed with a-cute hepatic failure,acute hepatitis B,acute renal failure,septic shock,heart transplantation and gastrointestinal hemorrhage.The patient died due to ineffective treatment.CONCLUSIONS The patient with new onset of HBV in-fection who receives organ transplantation and takes a long-term use of immunosuppressors may have severe con-sequences such as new onset of hepatitis B and acute hepatic failure.It is necessary to attach great importance to and take comprehensive measures for the prevention of new onset HBV infection in the donor after the organ transplantation,stick to the long-term reexamination of HBV infection-related indexes and maintain the antiviral therapy as long as possible(at least 1 year),which may facilitate the protection of the patient from HBV infec-tion-related adverse events and help to extend the survival time of the patient after the organ transplantation.
10.The effect of NaClO and EDTA pretreatment on the shear bond strength between the enamel white spot lesions and resin composite:An in vitro study

Objective To compare the impact of NaClO and EDTA pretreatment on the enamel surfaces pore exposure rate,resin in-filtration effectiveness and shear bond strength between the enamel white spot lesions(WSLs)and resin composite.Methods A total of 104 sound premolars were selected for the study.4 mm×4 mm×2 mm enamel blocks from 64 sound premolars were cut in the center of the buccal surfaces of the teeth.All blocks were randomly divided into five groups:group SE(sound enamels as negative control);group DE(enamel blocks demineralised as positive control);group RI(resin infiltration);group N/RI(5.25%NaClO pretreatment and resin infiltration)and group E/RI(17%EDTA pretreatment and resin infiltration).The pore exposure rate of enamel surfaces and resin penetration depths were observed by scanning electron microscopy(SEM)and confocallaser scanning microscopy(CLSM)in each group.The remaining 40 premolars were cut from the cervical roots and the residual crowns were embedded in self-condensing resin ma-terial(buccal surfaces of the enamel exposed only).According to the grouping information,the samples were given the corresponding pretreatments and resin infiltration respectively and were bonded with a cylindrical resin cylinder on the buccal enamel surface for shear bond test.The shear bond strength and fracture modes were recorded and analyzed in each group.Results Compared to group RI,the pore exposure rate of the enamel surface,resin penetration depths,and percentage of resin penetration area in the N/RI and E/RI groups were significantly increased(P<0.05).The shear bond strength of the samples in group SE was the highest,and the lowest in group DE.Compared to group DE,the shear bond strength in groups RI,N/RI,and E/RI was significantly increased(P<0.05).Con-clusion Enamel surface pretreatment with 5.25%NaClO or 17%EDTA has been demonstrated to effectively enhance the enamel pore exposure rate,resin penetration depth and the shear bond strength between WSLs and resin composite.
