BACKGROUND:Studies have shown that mitochondrial oxidative stress has an important role in the development of knee osteoarthritis, and Hemin can regulate the expression of mitochondria-related proteins. OBJECTIVE:To study the regulatory effect of Hemin on oxidative stress in mouse chondrocytes and its interventional effect and mechanism in knee osteoarthritis. METHODS:(1)In vitro cell experiment:Primary chondrocytes from C57BL/6 mice were extracted and induced with 10 ng/mL interleukin-1β to construct an in vitro chondrocyte model of osteoarthritis. The optimal concentration of Hemin(0,1,10,20,40,80,and 160 μmol/L)for the intervention in mouse chondrocytes was determined by cell counting kit-8 method.Chondrocytes were randomly divided into control group, model group(interleukin-1β)and Hemin group(interleukin-1β+Hemin).Reactive oxygen species, mitochondrial membrane potential and apoptosis of chondrocytes in each group were detected.(2)In vivo experiment:Adult C57BL/6 mice were randomly divided into normal group, model group(osteoarthritis)and Hemin group(osteoarthritis+Hemin),with eight mice in each group. After 4 weeks of Hemin treatment, the behavioral test and histopathological observation of the knee joint were performed in each group.Changes in extracellular matrix-related protein expression and apoptosis in chondrocytes and the expression level of Nrf2/HO-1 protein in cartilage tissue were detected. RESULTS AND CONCLUSION:In vitro experiment:the optimal concentration of Hemin on primary chondrocytes was 40 μmol/L.Compared with the model group, the level of reactive oxygen species was significantly reduced, the mitochondrial membrane potential was significantly improved, and the apoptosis of chondrocytes was reduced in the hemin-treated interleukin-1β-induced chondrocytes. In vivo experiment:After 4 weeks of treatment, compared with the model group, the lower limb function of mice in the Hemin group was significantly improved, the histopathological score was significantly improved, and the apoptosis of knee chondrocytes was significantly reduced. All these findings indicate that Hemin can alleviate oxidative stress, restore mitochondrial function and reduce apoptosis in mouse chondrocytes induced by interleukin-1β.Hemin can improve extracellular matrix degradation, promote chondrocyte anabolism, reduce catabolism and reduce chondrocyte apoptosis in knee osteoarthritis. It may act by activating the chondrocyte Nrf2/HO-1 signaling pathway in the inflammatory environment.

The aim of this study was to investigate the trend in testicular volume changes after orchiopexy in children with cryptorchidism. The clinical data of 854 children with cryptorchidism who underwent orchiopexy between January 2013 and December 2016 in Shenzhen Children's Hospital (Shenzhen, China) were retrospectively analyzed. The mean (standard deviation) age of the patients was 2.8 (2.5) years, and the duration of follow-up ranged from 1 year to 5 years. Ultrasonography was conducted preoperatively and postoperatively. The variables analyzed included age at the time of surgery, type of surgical procedure, laterality, preoperative testicular position, preoperative and postoperative testicular volumes, and the testicular volume ratio of them. The average testicular volumes preoperatively and at 1 year, 2 years, 3 years, and 5 years postoperatively were 0.27 ml, 0.38 ml, 0.53 ml, 0.87 ml, and 1.00 ml, respectively ( P < 0.001). The corresponding testicular volume ratios were 0.67, 0.76, 0.80, 0.83, and 0.84 ( P < 0.001). The mean volume of the undescended testes was significantly smaller than the mean normative value ( P < 0.001, lower than the 10 th percentile). The postoperative testicular volumes in children with cryptorchidism were generally lower than those in healthy boys but were still greater than the 10 th percentile and exhibited an increasing trend. The older the child is at the time of surgery, the larger the gap in volume between the affected and normal testes. Although testicular volume tends to gradually increase after orchiopexy for cryptorchidism, it could not normalizes. Earlier surgery results in affected testicular volumes closer to those of healthy boys.

BACKGROUND:At present, lumbar interbody fusion is widely used in the treatment of a variety of lumbar diseases. However, how to reduce postoperative complications such as pseudarthrosis formation, screw loosening and fracture, and cage failure remains a serious challenge. OBJECTIVE:To analyze the effect of total lumbar range of motion on the stress or strain of bone grafts, cage, and screw-rod system,so as to better guide patients to carry out lumbar activities to reduce the risks of pseudarthrosis formation and instrumentation failure. METHODS:An intact human L1-S1 finite element model was constructed using Mimics,3-Matic,HyperMesh,and Abaqus software and the transforaminal lumbar interbody fusion was simulated. The average strain of the interbody bone grafts and the peak stresses of the cage and screw-rod system were compared before and after applying the bending moment, and the changing trend with the total range of motion was analyzed. The stress nephogram was drawn to observe the stress distribution. RESULTS AND CONCLUSION:(1)Compared with applying the vertical compression load alone, the average strain of the interbody bone grafts, peak stresses of the cage and screw-rod system after applying bending moment increased by 2.6%-55.3%,65.6%-166.8%,and 36.0%-353.4%,respectively.(2)With the increase of total range of motion, the average strain of the interbody bone grafts increased nonlinearly and produced the maximum value under left and right axial rotation, while the peak stresses of the cage and screw-rod system increased linearly and produced the maximum value under left and right lateral bending.(3)The stress distribution of the interbody bone grafts and cage was related to the loading condition. The stress of the screw-rod system was mainly concentrated in the interfaces of the screw-bone and screw-rod.(4)Therefore,increasing axial rotation activity after operation may reduce the risk of pseudarthrosis formation, while reducing lateral bending activity may reduce the failure of the cage and screw-rod system.

Objective:To analyze the epidemiological and clinical characteristics of respiratory syncytial virus (RSV) infection in children in Jiangsu Province from 2014 to 2023.Methods:The acute respiratory infection cases in children aged 0-14 years were selected from outpatient/emergency or inpatient departments in 2 surveillance sentinel hospitals, respectively, in Nanjing, Suzhou and Taizhou of Jiangsu from 1 July 2014 to 31 December 2023, and RSV nucleic acid test was conducted and the intensity of the RSV infection was accessed by WHO influenza epidemiological threshold method, and case information and clinical data were collected. χ2 test was used to compare the differences between groups, and the Bonferroni method was used for pairwise comparisons between groups. Results:In 4 946 cases of acute respiratory infections, the RSV positive rate was 8.21% (406/4 946), and the age M( Q1, Q3) of the cases was 1 (0, 3) years. The RSV positive rate was 10.92% (258/2 362) during 2014-2019 and 6.06% (118/1 948) during 2019-2023, the difference was significant ( χ2=31.74, P<0.001). RSV infection mainly occurred from October to March during 2014-2019, with the incidence peak in December and moderate or higher intensity. The seasonality of RSV infection was not obvious during 2019-2023, with low intensity. The RSV positive rate was highest in children in age group 0- years (17.85%, 151/846), and the positive rate declined gradually with age ( χ2=184.51, P<0.001). The RSV positive rate was higher in inpatient cases (9.84%, 244/2 480) than in outpatient/emergency cases (6.57%, 162/2 466) ( χ2=17.54, P<0.001). In the 155 RSV infection cases with complete clinical data, the clinical symptoms mainly included cough (99.35%, 154/155), fever (55.48%, 86/155), and shortness of breath (45.16%, 70/155). In the cases aged <6 months, the proportion of those with fever was low, but the proportion of those with shortness of breath, transferred to intensive care units, and receiving oxygen therapy were higher (all P<0.05). Children aged <6 months and those with underlying diseases were more likely to have severe RSV infection (all P<0.05). Conclusions:RSV infection in children in Jiangsu Province showed seasonal prevalence in winter from 2014 to 2019. Since 2020, the seasonal characteristics of the epidemic have changed, the epidemic period has been dispersed and the epidemic intensity has decreased. Infants <1 year old were at high risk for RSV infection, and those <6 months old and with underlying diseases might have severe infection.
