1.Accuracy of Intraocular Lens Power Prediction Using 9 Formula

Purpose:
To compare the accuracy of nine intraocular lens (IOL) power calculation formulas.
Methods:
This study is retrospective consecutive case series. A total of 228 eyes of 228 patients who underwent uncomplicated cataract surgery between October 2015 and March 2021 were included. The accuracy of nine IOL calculation formulas (Kane, Emmetropia verifying optical version, Hill-radial basis function, Olsen, Barrett Universal II [Barrett II], Haigis, Holladay, Hoffer, and SRK/T) was compared and analyzed using differences between the predicted refractive power and actual refractive power at 2 months after cataract surgery. Mean prediction error (ME), mean absolute error (MAE), median absolute error (MedAE), and the percentage of eyes that were within ± 0.50 diopters (D), ± 0.75 D, and ± 1.00 D of the target refraction were calculated for each formula.
Results:
For all eyes, Haigis formula was the most accurate in MAE, followed by Barrett II formula. Comparing the probability of MAE within 0.50 D, Haigis formula showed the highest accuracy. A total of 20 eyes of 20 patients had more than 25 mm axial length. Regarding MAE, Hoffer Q was the most accurate, followed by the Olsen formula. Comparing the probability of MAE within 0.50 D, the Hoffer Q formula was the most accurate. A total of 24 eyes of 24 patients had more than 46 D keratometry. Regarding MAE and MedAE, Haigis was the most accurate, followed by the Barrett II formula. Comparing the probability of MAE within 0.5 D, Holladay formula was the most accurate.
Conclusions
Barrett II and Haigis formulas showed greater accuracy for all patients when we determined IOL power before cataract surgery. Hoffer Q, Olsen, and Barrett II formulas showed higher accuracy for long axial length. And Haigis and Barrett II were the most accurate for steep keratometry.
2.NOX4 and its association with Anatomy/Histology/ Embryology myeloperoxidase and osteopontin in regulating endochondral ossification

Objective:
This study investigates NOX4, an enzyme producing hydrogen peroxide, in endochondral ossification and bone remodeling. NOX4’s role in osteoblast formation and osteogenic signaling pathways is explored.
Methods:
Using NOX4-deficient (NOX4−/− ) and ovariectomized (OVX) mice, we identify NOX4’s potential mediators in bone maturation.
Results:
NOX4−/− mice displayed significant differences in bone mass and structure.Compared to the normal Control and OVX groups. Hematoxylin and eosin staining showed NOX4−/− mice had the highest trabecular bone volume, while OVX had the lowest. Proteomic analysis revealed significantly elevated MPO and OPN levels in bone marrow-derived cells in NOX4−/− mice. Immunohistochemistry confirmed increased MPO, OPN, and collagen II (COLII) near the epiphyseal plate. Collagen and chondrogenesis analysis supported enhanced bone development in NOX4−/− mice.
Conclusions
and Relevance: Our results emphasize NOX4’s significance in bone morphology, mesenchymal stem cell proteomics, immunohistochemistry, collagen levels, and chondrogenesis. NOX4 deficiency enhances bone development and endochondral ossification, potentially through increased MPO, OPN, and COLII expression. These findings suggest therapeutic implications for skeletal disorders.
3.Atypical Antipsychotics Use and Neuroleptic Malignant Syndrome: A Review With a Clinical Perspective

Neuroleptic malignant syndrome (NMS) is a potentially life-threatening idiosyncratic reaction associated with the use of antipsychotic medications, characterized by hyperthermia, muscle rigidity, and alterations in consciousness. Despite a reduction in its incidence due to the predominance of atypical antipsychotics in contemporary treatment regimens, NMS remains a critical neuropsychiatric emergency that warrants vigilant attention. There is a hypothesis proposing that catatonia and NMS may exist along a continuum of the same disorder, and evidence suggests that lorazepam may offer therapeutic benefits for both conditions. It is potential for the concurrent occurrence of NMS and serotonin syndrome when antipsychotics and antidepressants are administered concomitantly. This paper presents a case of NMS following the reinitiation of antipsychotic treatment over an extended duration, a scenario inadequately addressed in existing reports, and discusses strategies for its management in clinical practice.
4.Assessing Stress and Anxiety in Firefighters During the Coronavirus Disease-2019 Pandemic: A Comparative Adaptation of the Stress and Anxiety in the Viral Epidemic–9 Items and Stress and Anxiety in the Viral Epidemics–6 Items Scales

Objective:
This study assessed the reliability and validity of the Stress and Anxiety to Viral Epidemics–9 items (SAVE-9) and Stress and Anxiety to Viral Epidemics–6 items (SAVE-6) scales for measuring viral anxiety among firefighters during the coronavirus disease-2019 pandemic.
Methods:
An online survey was conducted among 304 firefighters assigned in Gyeonggi-do. The SAVE-9 scale, initially developed for healthcare workers, was adapted for firefighters. We compared it with the SAVE-6 scale designed for the general population among the firefighters sample. The confirmatory factor analysis (CFA) was conducted to explore the factor structure of both scales. Internal consistency reliability was checked using Cronbach’s alpha and McDonald’s omega. Convergent validity was assessed in accordance with the Patient Health Questionnaire–9 and Generalized Anxiety Disorder–7 scales.
Results:
The SAVE-9 scale demonstrated a Cronbach alpha of 0.880, while the SAVE-6 scale yielded an alpha of 0.874. CFA indicated good model fits for both SAVE-9 and SAVE-6 scales among firefighters sample. The SAVE-9 and SAVE-6 comparably measures viral anxiety of firefighters.
Conclusion
Both of the SAVE-9 and SAVE-6 scales are reliable and valid instruments for assessing viral anxiety among firefighters during the pandemic.
5.Validation of the Measures for Activities of Daily Living Function: the Korean Version of the University of California San Diego Performance-Based Skills Assessment

Objective:
The study’s aim was to evaluate the validity of the Korean version of the University of California San Diego Performance-based Skills Assessment, Validation of Intermediate Measures (K-UPSA-2-VIM) in patients with dementia (D), mild cognitive impairment (MCI), and cognitive normal control group (CN).
Methods:
Study participants were 25 patients with D, 43 patients with MCI, and 111 controls with CN group, respectively. For cognitive assessment, Mini Mental State Examination, Consortium to Establish a Registry for Alzheimer’s Disease neuropsychological battery, and Clinical Dementia Rating were used. For functional assessment, Barthel-Activities of Daily Living, Instrumental Activities of Daily Living, Dementia Screening questionnaire, and K-UPSA-2-VIM were used.
Results:
Statistically significant differences were observed in all subdomains and total score of the K-UPSA-2-VIM among three cognitive groups. K-UPSA-2-VIM demonstrated 75.7% of sensitivity and 65.1% of specificity, with an area under the curve (AUC) of 0.731 (95% confidence interval [CI]: 0.641-0.821, p<0.001) in discriminating between CN and MCI groups. In discriminating between MCI and D groups, 76.7% of sensitivity and 64.0% of specificity, with an AUC of 0.706 (95% CI: 0.580-0.833, p=0.005) were demonstrated.
Conclusion
The K-UPSA-2-VIM is useful to evaluate activities of daily living function in Korean patients with D and MCI.
6.Analysis of Telephone Counseling of Patients in Chemotherapy Using Text Mining Technique

Purpose:
The purpose of this study was to analyze the frequency and relevance of frequent keywords using text mining analysis for symptom-related telephone counseling of patients undergoing chemotherapy, and to understand the current status and characteristics of the nursing intervention.
Methods:
442 cases of telephone counseling of patients undergoing chemotherapy were collected. The symptoms were classified and separated according to the contents of the consultation between the nurse and the counseling participants. Using the python library, frequency words were extracted, and the generation of word co-occurrence matrices was analyzed through social network analysis.
Results:
For the four cancers to be analyzed (breast, colorectum, stomach, lung), the common frequent words of nurse and counseling participants were ‘medical staff (uilyojin)’, ‘medical treatment (jinlyo)’, ‘treatment (chilyo)’, ‘other hospital (tabyeongwon)’, ‘prescription (cheobang)’. In the analysis of social networks, words with highly betweenness centrality, which appear in common, almost matched those of frequent words.
Conclusion
In this study, it was possible to extract the most frequent words by cancer type from the contents of telephone counseling with cancer patients and to understand the current status and context of the actual telephone counseling focusing on each keyword.
7.Psychiatric Inpatient Care During the COVID-19 Pandemic: A Review

There are multitudinous obstacles in psychiatric inpatient treatment in the face of the coronavirus disease (COVID-19) pandemic. In addition, there are few guidelines or recommendations for the operation of psychiatric wards in Korea. This study reviewed the impact and changed patterns of psychiatric hospitalization due to the COVID-19 pandemic and recommendations and cases for psychiatric hospitalization. From keyword search of electronic databases, a total of 72 studies was eligible to review. It was reported that the diagnostic distribution of the patient population requiring psychiatric hospitalization before and after COVID-19 might change, and the severity of hospitalized patients might become higher after COVID-19. Although a causal relationship could not be established at that time, a confirmed case of COVID-19 appeared to impact psychiatric symptoms. 1) Consistent screening tests and symptom monitoring, 2) proper wearing of masks and personal protective equipment by patients and treatment staff, 3) distancing and limiting the number of inpatients, 4) limiting unnecessary visits and visits by outsiders, 5) close cooperation and linkage between treatment staff, and 6) referral, were consistently recommended. In addition, several works recommended non-face-to-face visits during hospitalization for patient recovery, and some attempts were made to provide virtual psychotherapy or prescriptions. It is necessary to flexibly operate the details by accommodating the needs of the mental health system for psychiatric inpatient treatment in the pandemic period. Future attempts are necessary to search through systematic reviews by integrating the reported studies.
8.Factors associated with anticholinergic burden among older patients in long-term care hospitals in Korea

Background/Aims:
Drugs with anticholinergic properties (DAPs) are associated with adverse health outcomes in older patients. The objective of this study was to evaluate the factors that determine the prescribing of more DAPs in long-term care hospitals (LTCHs) in Korea. In addition, the current patterns of DAP prescription were explored using a novel platform, which can collect data from LTCHs.
Methods:
This was a Health-RESPECT (integrated caRE Systems for elderly PatiEnts using iCT) sub-study, which is a pragmatic, cluster-randomized, controlled trial. The Health-RESPECT platform was used to collect prescribed medication data of 466 patients (aged ≥ 65 years) from seven LTCHs. DAPs were identified using the Korean Anticholinergic Burden Scale (KABS). Physical frailty, cognitive function, functional status, and quality of life were evaluated.
Results:
Among 466 LTCH patients, 88.8% (n = 414) were prescribed DAPs, and the prevalence of high KABS (≥ 3) was 70.4% (n = 328). The drugs that contributed most to the total KABS were quetiapine (20.7%), chlorpheniramine (19.5%), tramadol (9.8%), cimetidine (5.8%), and furosemide (3.6%). Polypharmacy, higher body mass index, less dependence, better communication and cognitive functions, and poorer quality of life were associated with high KABS.
Conclusions
Although the patients with a high burden of DAPs were less dependent and had better cognitive and communication functions, they had poorer quality of life. DAP use in LTCH patients should be monitored carefully, and the risk/ benefit relationship for their use should be considered.
9.Comparison of the Accuracies of Intraocular Lens Power Formulae by the Type of Implanted Lens

Purpose:
We compared the accuracies of formulae used to calculate intraocular lens (IOL) powers when predicting postoperative refraction, by the type of lens.
Methods:
We retrospectively reviewed the medical records of 445 eyes (332 patients) that had undergone phacoemulsification and posterior chamber lens implantation between January 2017 and December 2019. Axial length was measured via optical low-coherence reflectometry (Lenstar LS900; Haag-Streit, Bern, Switzerland). The IOLs implanted were all one-piece acrylic IOLs, thus, the ALCON SN60WF (215 eyes), the TECNIS ZCB00 (139 eyes), and the MBI SAL302AC (91 eyes). The mean absolute errors (MAEs) and mean numerical errors (MNEs) were calculated using the SRK-2, SRK/T, Hoffer Q, Holladay, Olsen, and Barrett formulae 1-2 weeks after surgery.
Results:
For the ALCON SN60WF, the Hagis formula yielded the lowest power and thus tended to be myopic, but we found no significant difference in the MAE. For the TECNIS ZCB00, the Olsen formula yielded the lowest power and thus tended to be myopic, and the Barrett formula yielded the lowest MAE. For the MBI SAL302AC, all MAEs and MNEs were similar.
Conclusions
Depending on the IOL type, the IOL power calculation formulae differed in terms of predicting postoperative refraction.
10.The Role of Mobility of Cognition and Emotion in Elderly : A Conceptual Review

Considering that the aged population increases and the mobility problem is pointed out as a factor that indisposes the quality of life, cognition, and mood, it is important to understand and evaluate the elderly’s mobility. Factors that deteriorate mobility in the elderly include physical senility, various health changes including chronic diseases, polypharmacy as well as anticholinergics. Common mobility problems in old age are reduced gait speed, increased gait variability in walking length, careless walking, and frequent falls. Several studies have reported that decreased mobility and deterioration of gait can predict cognitive decline and emotional problems. Aerobic exercise, resistance exercise, and balance exercise are suggested as therapeutic interventions for mobility problems. Active correction for factors that reduce mobility in the elderly and prescribing physical activity can conserve the elderly’s quality of life and help improve cognition and mood. There is a need for related research in the future.
