1.A Case of Honeycomb Corneal Epitheliopathy Induced by Netarsudil 0.02% Ophthalmic Solution

Purpose:
To document the clinical presentation and progression of corneal epitheliopathy, with its characteristic honeycomb pattern, associated with topical Netarsudil 0.02% use in a patient with neovascular glaucoma (NVG).Case summary: A 60-year-old man presented with left ocular pain and headache, with an intraocular pressure (IOP) of 37 mmHg in the left eye. After a diagnosis of NVG, he was treated with intravitreal anti-VEGF injection and topical anti-glaucoma medications. On the fifth day after adding Netarsudil, central corneal epithelial edema was noted. By day 7, a diffuse epithelial haze with a honeycomb-like appearance had developed. IOP measured by rebound tonometry was 47 mmHg, while Goldmann applanation tonometry underestimated the value at 22 mmHg, likely due to irregularities at the corneal surface. Netarsudil was discontinued and replaced with latanoprost, leading to rapid improvement. Due to persistent elevated IOP, Ahmed glaucoma valve implantation was performed, resulting in stable IOP and complete normalization of the corneal surface.
Conclusions
While Netarsudil is effective for lowering IOP, it can infrequently lead to reversible corneal epithelial edema characterized by a honeycomb-like pattern, particularly in patients with compromised corneal integrity. Therefore, close monitoring of corneal status is necessary when using Netarsudil in high-risk patients.
2.Age group-related characteristics of pediatric drowning patients treated at an emergency medical center in northern Yeongseo, Gangwon Province

Purpose:
We aimed to analyze pediatric drowning cases to study age group-related clinical characteristics, such as causes, location, and outcomes, in an under-researched area in South Korea.
Methods:
From January 1, 2020 through July 20, 2025, we retrospectively analyzed medical records of patients aged 19 years or younger who had experienced drowning and visited Kangwon National University Hospital located in the northern Yeongseo region of Gangwon Province. Their clinical characteristics were compared between those with worse outcomes, defined as hospitalization to the intensive care unit or in-hospital mortality, and those with better outcomes.
Results:
During the period, 27 patients having undergone drowning accidents visited the hospital. The most frequent location was the bathtub at home in the patients younger than 1 year (all 4 patients), commercial swimming pools in those aged 1-14 years (12 of the 16 patients), and rivers in those aged 15-19 years (all 7 patients). Of the 27 cases, 11 had worse outcomes including 4 mortalities. Patients with worse outcomes had higher percentages of guardian’s negligence, altered mental status, pool or river as the location, implementation of oxygen therapy or positive pressure ventilation, and lower mean initial Korean Triage and Acuity Scale (i.e., higher acuity), as well as higher mean concentrations of glucose, urea nitrogen, aspartate aminotransferase, and base deficit.
Conclusion
This study on pediatric drowning patients in the under-researched area showed age-related differences in the primary causes of drowning and clinical features related to the worse outcomes. These findings need to be considered for prompting increased parental vigilance and more comprehensive societal preventive measures.
3.Repeated Health Screening Measures and Incident Ischemic Stroke: Evidence From a Korean Population Study

Objectives:
Ischemic stroke is influenced by long-term metabolic and renal deterioration; however, many risk prediction frameworks rely on single time-point measurements. We examined whether multi-period patterns in national health screening indicators are associated with incident ischemic stroke in Korea.
Methods:
Using customized National Health Insurance Service data with 3 biennial screenings (P1: 2013–2014; P2: 2015–2016; P3: 2017–2018), we identified incident ischemic stroke during 2019–2023 (Korean Standard Classification of Diseases-7 I63). After applying eligibility criteria and excluding individuals with missing screening values, we performed 1:1 propensity score matching on sex, 1-year age strata, and insurance type (97 454 matched pairs; n=194 908). Multi-period indicators included waist circumference increase ≥10%, sustained blood pressure ≥130/80 mmHg, sustained fasting glucose ≥126 mg/dL, proteinuria progression, and creatinine elevation in ≥2 periods (sex-specific thresholds). Associations were evaluated using conditional logistic regression; a comparator model used P3-only indicators.
Results:
In the multi-period model, stroke was associated with waist circumference increase ≥10% (odds ratio [OR], 1.05; 95% confidence interval [CI], 1.01 to 1.08), sustained blood pressure ≥130/80 mmHg (OR, 1.34; 95% CI, 1.31 to 1.37), sustained fasting glucose ≥126 mg/dL (OR, 1.66; 95% CI, 1.60 to 1.73), creatinine elevation in ≥2 periods (OR, 1.08; 95% CI, 1.06 to 1.10), and proteinuria progression (OR, 1.36; 95% CI, 1.32 to 1.39). In the P3-only model, all single-time-point indicators were associated with incident stroke (ORs, 1.08 to 1.47).
Conclusions
Multi-year patterns in metabolic screening indicators were associated with incident ischemic stroke. Repeated health screening measurements may complement single time-point assessments and support continuous risk-factor monitoring and patient-centered prevention.
4.A Critical Reconsideration of the Adoption of “German Medicine” in Early Meiji Japan: Sagara Chian’s Conception of Medical Authority and His Project for the Professionalization of Physicians

This article critically reconsiders existing explanations for the adoption of “German medicine” in early Meiji Japan and reconstructs the institutional and political context of that decision by focusing on Sagara Chian’s conception of medical authority (iken) and his project of medical professionalization. Previous studies have explained the shift largely in terms of the academic superiority of German medicine, the German provenance of Dutch-learning medical texts, the advice of foreign advisers such as Verbeck and Bauduin, political or ideological affinity between Japan and Prussia, or factional conflict surrounding William Willis. Yet these explanations do not sufficiently clarify why the final outcome took the form of a German-style institutional model combining university-based medical education, state examinations and licensure, a hierarchical professorial order, and state regulation of medical practice. This article first shows that, in the aftermath of the Boshin War, the British model represented by William Willis enjoyed strong political legitimacy within the new Meiji government. It then traces the appointments of Sagara Chian and Iwasa Jun, the proposal and collapse of the dual-employment proposal, and the subsequent politics of separation and compromise, arguing that the crucial issue was not a simple confrontation between pro-British and pro-German camps but a contest over who would control medical education, institutional design, and the authority to certify physicians. It further demonstrates that Sagara understood “Germany” not as a repository of superior medical techniques but as an institutional model linking the university, state licensure, hierarchical academic authority, military medicine, and hygienic administration. In this light, the adoption of “German medicine” in early Meiji Japan is best understood less as passive reception of a purportedly superior foreign medicine than as an institutional choice through which the modern Japanese state sought to reorganize physicians into a state-managed and state-certified profession. This reinterpretation sheds light not only on the formation of the Meiji state and its professional order but also on the broader genealogy of modern medical education in East Asia, including colonial Korea.
5.Navigating the New Therapeutic Landscape: Innovative Strategies for Overcoming Resistance and Degeneration

Recent technological advancements and environmental shifts have reshaped the therapeutic landscape of human diseases, driving a transition from merely understanding pathogenesis to developing precise and targeted therapeutic solutions. While the 2025 Special Issue focused on identifying emerging risk factors, the 2026 Special Issue (Vol. 34, No. 1) pivots toward concrete methodological innovations and advanced therapeutic interventions. This issue presents a curated collection of ten distinguished articles organized around three core themes. First, in the field of oncology and drug resistance, studies investigate transglutaminase 2 (TG2)-mediated autophagy and ferroptosis as strategies to overcome therapeutic resistance, alongside advances in CAR-T cell engineering and the integration of artificial intelligence (AI) with robotic surgery to enable precision medicine. Second, addressing degenerative and metabolic diseases, contributions elucidate the role of Wnt/β-catenin signaling in osteoporosis, recent therapeutic advances in knee osteoarthritis, mechanisms underlying drug-induced senescence, and the application of gene-editing technologies in iPSC-derived hepatic models. 6.Health status of poor, older urban adults compared with key health indicators from the 2023 Korea National Health and Nutrition Examination Survey in the Republic of Korea: a cross-sectional comparative study

This study compared key health indicators of poor, older urban adults attending a free clinic with those of the general older population, using data from the 2023 Korea National Health and Nutrition Examination Survey (KNHANES). Methods: This cross-sectional comparative study included 60 adults aged ≥60 years who attended the Raphael Nanum Homeless Clinic in Seoul. Participants completed a questionnaire, underwent anthropometric assessment, and provided fasting blood samples for measurement of total cholesterol, low density lipoprotein (LDL)-cholesterol, and triglycerides (TG). Obesity, current smoking, monthly alcohol use, poor self-rated health, and strength exercise (≥ 2 days/week) were defined according to 2023 KNHANES criteria and compared with age-matched 2023 KNHANES estimates for adults aged ≥ 60 years using independent t-tests and two-proportion z-tests. Results: Participants were predominantly men (80.0%) with a mean age of 79.9 years; 70.0% reported no regular income, and 46.7% rated their health as poor. Compared with their 2023 KNHANES counterparts, the clinic group had a higher prevalence of obesity (50.0% vs. 35.0%), particularly among men, and a more atherogenic lipid profile characterized by higher LDL-cholesterol despite similar total cholesterol levels and lower TG. The prevalence of current smoking (3.3% vs. 10.6%) and monthly alcohol use (31.7% vs. 53.0%) was significantly lower, whereas participation in strength exercise was low in both groups, with no significant differences observed. Conclusion: Poor, older urban adults exhibited multidimensional health disparities, including obesity, adverse lipid profiles, and markedly poorer self-rated health, despite lower levels of smoking and alcohol consumption. Community-based interventions targeting nutrition, physical activity, and chronic disease management are needed to reduce health inequalities in this vulnerable population.
7.Exosomes from Human Embryonic Stem Cell-Derived Mesenchymal Stem Cells Protect Lung Epithelium and Attenuate Fibrosis

Idiopathic pulmonary fibrosis (IPF) is characterized by maladaptive epithelial–mesenchymal crosstalk and progressive extracellular matrix accumulation, whereas currently available antifibrotic agents merely decelerate functional decline.This study investigated whether exosomes derived from human mesenchymal stem cells derived from embryonic stem cells (ESC-MSCs) restore epithelial stress responses and attenuate fibrotic remodeling. Human IPF lung transcriptomes were integrated with a bleomycin-induced murine model analyzed by RNA sequencing and protein signaling, together with cigarette smoke extract-induced injury in A549 epithelial cells. ESC-MSCs-derived exosomes exhibited typical morphology and size distribution, enrichment of tetraspanins, and absence of endoplasmic reticulum contamination, consistent with high-purity preparations. Across human IPF and bleomycin-injured lungs, transcriptomic profiling revealed prominent enrichment of extracellular matrix and cytoskeletal gene programs, whereas mitogen-activated protein kinase (MAPK) and Smad families displayed only modest alterations at the mRNA level. In vivo administration of exosomes during the fibrotic remodeling phase, via either intravenous or intratracheal delivery, resulted in improved body weight, reduced lung weight-to-body weight ratios, and decreased collagen deposition and Ashcroft scores. These structural and functional improvements were accompanied by suppression of profibrotic and mesenchymal markers and selective attenuation of activator protein-1 (AP-1) activity. In epithelial injury models, ESC-MSCs-derived exosomes enhanced cell viability, restored redox homeostasis, and constrained stress-induced mesenchymal gene expression and MAPK phosphorylation in both co-treatment and post-treatment settings. Collectively, these data support an epithelial-centered mechanism in which ESC-MSCs-derived exosomes re-establish oxidative balance and selectively restrict AP-1-driven stress signaling, thereby secondarily limiting extracellular matrix accumulation and fibrotic remodeling.
9.Nutrition Status and Comorbidities Are Important Factors Associated With Mortality During Anti-Tuberculosis Treatment

Background:
The increasing incidence and mortality rates of tuberculosis among older individuals who suffer from multiple morbidities and are vulnerable to malnutrition are major obstacles to efforts to eradicate tuberculosis in the Republic of Korea. Herein, we identified the factors associated with mortality during anti-tuberculosis treatment in patients with pulmonary tuberculosis.
Methods:
We conducted a case-control study and extracted data from the database of a multi-center prospective observational cohort study in Korea. Among the participants with rifampicin-susceptible pulmonary tuberculosis, the survival group was defined as those who successfully completed treatment within one year, whereas the mortality group was defined as those who died during treatment. Univariable and multivariable logistic regression analyses were performed to identify factors associated with TB mortality.
Results:
Among 1,119 participants with pulmonary TB registered between 2019 and 2021, 799 and 59 were grouped in the survival and mortality groups, respectively. Age, positive smear results, alarming symptoms, nutrition risk score, Charlson comorbidity index score, and initial standard treatment regimen were significant based on univariable analysis and were selected for the multivariable logistic regression model. Nutrition risk score (adjusted odds ratio, 2.44; 95% confidence interval, 1.72–3.48) and Charlson comorbidity index score (adjusted odds ratio, 1.62; 95% confidence interval, 1.35–1.94) remained statistically significant in the multivariate analysis.
Conclusion
Nutritional status and comorbidities at baseline were identified as important factors associated with mortality in patients with pulmonary tuberculosis.
10.Thoracic spinal cord damage in rat following cardiac arrest: neuronal loss, blood-spinal cord barrier leakage, and astrocyte endfeet disruption

Objective:
Cardiac arrest and cardiopulmonary resuscitation (CA/R) lead to whole-body ischemia and reperfusion (IR) injury, causing multiple organ dysfunction, including ischemic spinal cord injury. The thoracic spinal cord levels are crucial for maintaining the sympathetic functions vital for life. This study examined blood-spinal cord barrier (BSCB) leakage and astrocyte endfeet (AEF) disruption and their effects on survival, physiological variables, and neuronal damage/death in the intermediate zone (IMZ) at the seventh thoracic spinal cord level after asphyxial CA/R in rats.
Methods:
The rats underwent whole-body IR injury by asphyxial CA/R. Kaplan-Meier analysis was conducted to assess the cumulative survival post-CA/R. The histological changes post-CA/R were evaluated using immunohistochemistry, histofluorescence, and double histofluorescence.
Results:
No significant differences in body weight, mean arterial pressure, and heart rate were found between the sham and CA/R groups post-CA/R. The survival rates in the CA/R group at 12, 24, and 48 hours were 62.58%, 36.37%, and 7.8%, respectively. Neuronal loss and BSCB leakage began 12 hours post-CA/R, increasing with time. Reactive astrogliosis appeared at 12 hours and increased, while AEF disruption around blood vessels was evident at 48 hours.
Conclusion
The survival rate declined significantly by 48 hours post-CA/R. Neuronal loss and BSCB leakage in the thoracic spinal cord IMZ was evident at 12 hours and significant by 48 hours, aligning with AEF disruption. Neuronal loss in the thoracic spinal cord IMZ post-CA/R may be related to BSCB leakage and AEF disruption.
