1.Regulation of white adipose tissue in mice by immunization with recombinant Bacillus Calmette-Gue?rin with c-di-AMP adjuvant

This study assessed the role and mechanism of the recombinant Bacillus Calmette-Gue?rin vaccine(rBCG)with c-di-AMP adjuvant in regulating metabolism and immunity in epididymal white adipose(eWAT)in mice.Male C57BL/6 mice were intravenously immunized with BCG and rBCG,and their body weights were monitored.eWAT was isolated from the mice,and the stromal vascular fractions(SVFs)cell number was counted with a hemocytometer.Sections of mouse adipose tissue were prepared,and the size,number,and morphology of eWAT adipocytes and crown-like structure(CLS)formation were compared under a microscope after HE staining.The transcription levels of lipid metabolism-associated factors,cytokines and aging-associated genes in each group were determined with qRT-PCR.The body weights of mice gradually increased after immunization with BCG and rBCG.The proportions of eWAT increased,and the SVFs cell number decreased,in rBCG immunized mice.HE staining indicated that BCG immunization promoted hyperplasia,whereas rBCG immunization promoted hypertrophy of eWAT adipocytes;moreover,both BCG and rBCG immunization induced CLS formation in eWAT.The qRT-PCR results indicated that rBCG immunization inhibited the expression of genes associated with lipolysis and energy expenditure in eWAT.BCG immunization had little effect on cytokine transcription,whereas rBCG significantly induced the transcription of IFN-γ and IL-1Ra,and inhibited that of IL-15 and IL-2,but did not induce the expression of aging-associated genes.Thus,rBCG immunization induced eWAT adipocyte hypertrophy,which was associated with the inhibition of eWAT lipolysis and the regulation of cytokine expression.
2.Correlation between D-dimer, antithrombin Ⅲ levels and multiple organ dysfunction syndrome in patients with severe multiple trauma

Objective:To explore the correlation between D-dimer (D-D), antithrombin Ⅲ (AT-Ⅲ) at admission, and multiple organ dysfunction syndrome (MODS) in patients with severe multiple trauma.Methods:A retrospective study was conducted to collect data from 80 patients with severe multiple trauma admitted to Jinhua Central Hospital from April 2021 to April 2024. According to the occurrence of MODS within 1 week of treatment, the patients were divided into an occurrence group with 25 cases and a non-occurrence group with 55 cases. The general data, plasma D-D, serum AT-Ⅲ levels and other laboratory indexes at admission were compared between the two groups. The correlation between D-D, AT-Ⅲ at admission and MODS in patients with severe multiple trauma was analyzed.Results:The injury severity rating score (ISS) in the occurrence group was higher than that in the non-occurrence group: (29.12 ± 3.84) points vs. (26.18 ± 3.12) points, the proportion of damage control surgery was lower than that in the non-occurrence group: 32.00% (8/25) vs. 65.45% (36/55), the platelet, AT-Ⅲ and albumin at admission were lower than those in the non-occurrence group: (112.28 ± 20.12) ×10 9/L vs. (123.56 ± 23.47) ×10 9/L, (133.79 ± 18.21) mg/L vs. (148.26 ± 19.57) mg/L, (35.12 ± 6.35) g/L vs. (38.47 ± 5.88) g/L, the D-D at admission was higher than that in the non-occurrence group: (11.36 ± 3.35) mg/L vs. (8.18 ± 2.83) mg/L, and there were statistical differences ( P<0.05). Multivariate Logistic regression analysis result showed that the occurrence of MODS in patients with severe multiple trauma may be related to the ISS at admission, the status of receiving damage control surgery, and the D-D, AT-Ⅲ, albumin at admission ( OR = 1.29, 0.14, 1.34, 0.95 and 0.87; P<0.05). Restrictive cubic spline analysis showed that the risk of MODS in patients with severe multiple trauma showed a non-linear dose-response relationship with D-D and AT-Ⅲ at admission ( P<0.05). When D-D at admission ≥ 9.25 mg/L, the risk of MODS increased with increasing levels, while when AT-Ⅲ at admission ≤ 143.49 mg/L, the risk of MODS increased with decreasing levels. The D-D and AT-Ⅲ at admission had a negative interactive effect on the occurrence of MODS in patients with severe multiple trauma. By drawing decision curves and column charts, it was found that D-D and AT-Ⅲ at admission, as well as other major indicators, had high predictive power for the occurrence of MODS in patients with severe multiple trauma. Conclusions:The occurrence of MODS in patients with severe multiple trauma may be related to the levels of D-D and AT-Ⅲ, and the risk of MODS increases with the increase of D-D and the decrease of AT-Ⅲ at admission. Both can effectively assist other indicators in predicting the risk of MODS in patients.
3.Analysis of Risk Factors for Uremic Encephalopathy in Maintenance Hemodialysis Patients

Objective:To explore the independent risk factors for uremic encephalopathy(UE)in maintenance hemodialysis(MHD)patients and provide evidence for early clinical warning and intervention.Methods:A case-control study was conducted,enrolling 67 MHD patients diagnosed with UE(UE group)at Laibin People's Hospital from January 2010 to December 2024,and 67 non-UE patients during the same period(control group).Demographic characteristics,dialysis parameters,laboratory indicators,and infection events were collected.Univariate and multivariate logistic regression analyses were used to identify independent risk factors for UE.Results:The UE group had significantly higher rates of infection(58.2%vs.29.9%),serum creatinine(789 vs.702 μmol/L),and iPTH levels(568 vs.385 pg/mL)compared to the control group(P<0.05).Multivariate analysis revealed that concurrent infection(OR=3.022,95%CI:1.312-6.958),elevated serum creatinine(OR=1.004,95%CI:1.000-1.008),and elevated iPTH(OR=1.002,95%CI:1.001-1.003)were independent risk factors for UE(P<0.05).The combined prediction model achieved an AUC of 0.878(95%CI:0.822-0.934),with 82.1%sensitivity and 80.6%specificity.Conclusion:Infection,elevated serum creatinine,and elevated iPTH significantly increase the risk of UE in MHD patients.Clinical management should emphasize infection prevention,toxin clearance optimization,and parathyroid function regulation to reduce UE incidence.
4.Design and application of novel protective ventilator circuit component

Objective To develop a novel protective ventilator circuit component and to verify its performance by water seal and anti-splash experiments.Methods A novel protective ventilator circuit component had a design scheme with the multifunctional joint,and consisted of a tee connection tube,an isolation sleeve and a stop sleeve,of which,the tee connection tube was made of polyethylene polymer material and the others were made of silicone material.The tee connection tube had a T-shaped structure with two standard connection ports,which was composed of an adapter,a sealing cap,a plug and a sealing ring;the isolation sleeve was in the shape of a cylinder with a raised bottom,which was inserted into the adapter;the stop sleeve was located in the isolation sleeve,with an inverted frustum of a cone at the bottom and a rounded hole in the middle of the inverted frustum.An open ventilator circuit tube was involved in the performance verification of the circuit component developed.In the water seal experiment,sputum aspiration was simulated and the heights of the liquid level drop in the L-shaped tubes were compared after sputum aspiration.In the anti-splash experiment,the infection rates on the surfaces of the sterile hole towels and gloves were calculated.Results Water seal experiment showed after sputum aspiration the open ventilator circuit tube had the liquid level at the L-shaped tube higher significantly than that of the circuit component;the anti-splash experiment indicated sputum aspiration resulted in the occurance of the splashing out of the secretion and 77.5%infection rate by the open ventilator circuit tube,while no splashing out and 0%infection rate by the circuit component developed.Conclusion The novel protective ventilator circuit component behaves well in sealing and anti-splashing,and thus is worthy of clinical application for sputum aspiration.[Chinese Medical Equipment Journal,2025,46(4):113-117]

6.Primary central nervous system lymphoma with clonal bone marrow B cells:16 cases and literature review

Objective To summarize the clinical features,treatment and prognosis of patients with primary central nervous system lymphoma(PCNSL)with clonal bone marrow B cells,and to explore the influence on clinical diagnosis and treatment.Methods PCNSL patients with clonal bone marrow B cells diagnosed by flow cytometry between Jan 2020 and Jul 2023 at Huashan Hospital of Fudan University were enrolled.The auxiliary examination data of these patients were collected,including complete blood count,routine biochemistry,bone marrow aspiration and biopsy,contrast-enhanced brain MRI,and whole-body PET-CT.Kaplan-Meier was used to draw the survival curve,and relevant literature was reviewed.Results A total of 223 newly diagnosed PCNSL patients were included,187 of whom completed bone marrow puncture and biopsy evaluation.We found clonal bone marrow B cells in 16 of 187 cases(8.56%)by flow cytometry.2 patients showed B lymphoma involving the bone marrow.All patients received a high-dose methotrexate based chemotherapy.The median progression free survival(PFS)of 16 patients with clonal bone marrow B cells was 11.1 months,and the median PFS of 171 patients with normal bone marrow was 12.6 months.There was no significant difference in the PFS between the two groups.Conclusion PCNSL with clonal bone marrow B cells had no specific clinical features,but bone marrow flow cytometry showed clonal B cells.High-dose methotrexate treatment regimen is effective.There was no significant difference in PFS for PCNSL patients with clonal B cells and normal findings in bone marrow.Clonal B cells in bone marrow may be caused by monoclonal B-cell lymphocytosis(MBL),lymphoma involves the bone marrow and the presence of common precursor cells.Bone marrow examination should be performed in the initial evaluation of suspected PCNSL.
7.Establishment and validation of a predictive model for increased drainage volume after open transforaminal lumbar interbody fusion

Objective To analyze the risk factors for increased drainage volume after open transforaminal lumbar interbody fusion(TLIF),and to establish a predictive model and then validate it.Methods The clinical data of 680 patients who underwent open TLIF at the General Hospital of Northern Theater Command from January 2016 to December 2019 were collected and the patients were randomly divided into the training group(n=476)and the validation group(n=204).Taking the predictive factors screened out by LASSO regression analysis as independent variables,a multivariate Logistic regression predictive model was constructed.The model was internally validated through the receiver operating characteristic(ROC)curve,Hosmer-Lemeshow goodness-of-fit test,and calibration curve,and its clinical utility was assessed via decision curve analysis(DCA).Results LASSO regression analysis screened out four predictive variables:age,number of surgical segments,operative duration,and intraoperative blood loss.The multivariate Logistic regression predictive model demonstrated that age≥60 years,number of surgical segments≥4,operative duration≥2 hours,and intraoperative blood loss≥200 mL were independent influencing factors for the increased postoperative drainage volume in patients undergoing TLIF(P<0.05).ROC curve analysis revealed an area under the curve(AUC)of 0.816(95%CI:0.798 to 0.867)in the training group and 0.783(95%CI:0.685 to 0.823)in the validation group,indicating that the predictive model had good discriminatory ability.Additionally,the Hosmer-Lemeshow goodness-of-fit test and calibration curve indicated that the predictive model had a good degree of fit,and the predicted probability was basically consistent with the actual probability,demonstrating a good calibration.The DCA results confirmed that this predictive model could be applied in clinical practice.Conclusion The risk factors for increased drainage volume after open TLIF include age,number of surgical segments,operative duration,and intraoperative blood loss.The predictive model established based on these factors demonstrates good performance,and it can be applied in clinical guidance for the selection of drainage tube removal time after TLIF.
8.Analysis of risk factors for noncontiguous spinal fractures in the elderly

Objective To explore the risk factors for noncontiguous spinal fractures(NSFs)in the elderly.Methods The clinical data of 614 elderly patients with spinal fracture from January 2013 to December 2019 were analyzed retrospectively.Patients were divided into the NSFs group and the Non-NSFs group according to whether NSFs occurred or not.Univariate analysis and multivariate Logistic regression analysis were used to screen the risk factors of NSFs.Results Univariate analysis showed that female(P=0.003),high-energy violent injury(P=0.032),osteoporosis(P=0.004),fracture in spring(P=0.020),and previous spinal fracture history(P<0.001)were associated with the occurrence of NSFs.Multivariate Logistic regression analysis showed that fracture in spring(P=0.024),previous spinal fracture history(P<0.001)and high-energy violent injury(P=0.038)were the independent risk factors for the occurrence of NSFs in the elderly.Conclusion High-energy violent injury,fracture in spring and previous spinal fracture history are the independent risk factors for the occurrence of NSFs in the elderly.Therefore,elderly patients with the above risk factors should be examined more carefully and comprehensively to avoid missed diagnosis and delayed diagnosis.In order to reduce the incidence of this disease,corresponding measures should be taken according to the preventable risk factors.
